Dairy worker dies under fallen hay bale

SUNNYSIDE, WA -- The dairy worker killed when a hay bale fell on him Sunday night died of suffocation, Yakima County Coroner Maury Rice said Friday. Oner Villa, a 28-year-old employee of the Tony Veiga Dairy, died when one of the 1,500-pound hay bales he was unloading from a truck fell on him just after 8 p.m. Thursday. Co-workers found him after noticing the forklift he'd been using had not moved for some time, according to a Yakima County Sheriff's Office news release. According to the release, Villa had been cutting twine from bales stacked one on top of the other. One of the bales on the bottom loosened and lost its shape, causing the bale on top to fall on Villa.


Ford worker killed at Brook Park plant

Brook Park, OH- A Ford Motor Co. employee was killed Friday evening when he was caught in machinery. Fransisco Fraticelli, 59, was dead at the accident scene in the casting plant of the Ford complex on Brookpark Road and Henry Ford Boulevard, according to Brook Park police. Emergency crews were called at 5:28 p.m. Representatives of the Cuyahoga County coroner's office, who removed the body, said employees told them Fraticelli was caught in a core casting machine.


Decatur tree trimmer who was hit by falling limb has died

DECATUR, Ill. -- A falling tree limb has killed a 32-year-old tree trimmer in Decatur. John Martin of Decatur was trimming trees damaged in the November 30th ice storm when a limb struck him in the head. He was injured on Monday and died early yesterday. On January Ninth, another man with the same tree service was injured while working.


Two workers dead in sewage line accident near Orleans

Las Vegas, NV -- Two workers died Friday afternoon in an accident near The Orleans. It happened near a parking lot on the Arville side of The Orleans. Apparently a worker was cutting pipe above ground with a saw when the pipe burst and water rushed out. The worker tried to recover his saw and fell in a sewage line. Two men jumped in after him to save him. Clark County Fire was able to resuscitate one of the men but the other two died. The men apparently died from methane gas inhalation.


Driver dies of heart attack

GULFPORT, MS - A driver who pulled over on the roadside today and died shortly afterwards had a heart attack, said Harrison County Coroner Gary Hargrove. Woodland Weaver II, 53, an employee of American Medical Response, was driving a company vehicle when the incident occurred this morning, Hargrove said. Weaver was northbound on Cowan-Lorraine Road, just south of Interstate 10.


Stacey Stogner dies Wednesday Following mill accident

BOGALUSA, LA - Stacey Stogner, 34, of Angie, who was injured in an industrial accident at Temple-Inland early Saturday morning, died about 10 p.m. Wednesday night. A service operator on the No. 8 paper machine, Stogner was injured when he was pinned between his machine and a roll of paper in the accident, which occurred about 3:45 a.m.


Flora Vista man dies after industrial accident

FARMINGTON, NM — Friends and family are mourning the passing of a 47-year-old Flora Vista man who died of injuries sustained during an industrial accident last week.
Joel Cordell died around 9:15 p.m. Wednesday at San Juan Regional Medical Center. Cordell was pinned to the engine of his Bearcat Drilling forklift boom on Jan. 23, according to his brother, Farmington Police Sgt. Pat Cordell. Joel Cordell, who worked at Bearcat Drilling since its opening in April 1998, was finishing a day of work at the company yard when his leg accidentally hit a lever that controls the forklift's boom. "He wasn't able to recover from it," Pat said. "He had some pretty severe brain injuries from the pressure and he just wasn't able to survive."


Woman found dead in truck was pinned between pallets

Yuma, AZ -- A woman whose body was found in a truck delivering Arizona lettuce to a grocery store warehouse in Iowa was accidentally pushed into the trailer by a forklift after she walked into a loading zone at a Yuma packing plant, Yuma police said. The body of Sheila Kay Ross, 47, of McLoud, Okla., was found Tuesday morning in the back of a semitrailer delivering lettuce to the Hy-Vee warehouse in Chariton, southeast of Des Moines. An autopsy showed she died of compressional asphyxiation, and her death was ruled an accident. Ross' body was found by a dock worker who was unloading the truck at the Hy-Vee warehouse.


Youngest Old Fort officer is first-ever killed in line of duty

OLD FORT, NC – The youngest of the town's six police officers was killed Thursday night when his car ran off a curve in the road on his way to a burglary in progress. Shawn Joshua Dean "Josh" Williams, 23, leaves behind a wife, an 18-month-old daughter and a tight-knit family of law enforcement in McDowell County.


Worker dies after clothing catches in lathe

INDIANA, PA -- A worker was killed when his clothes became caught in machinery at a factory that makes truck-mounted cement mixers, a coroner said. William Lloyd Downs, 56, of Cowanshannock, died in the accident Wednesday afternoon at Elkin Manufacturing Co. in White Township, Indiana County. Downs was killed when his clothes got caught in a rotary lathe, Indiana County Coroner Michael Baker said.


Explosion in Little General Convenience Store Kills 4 in Ghent; Propane Gas Leak Blamed

Ghent, WV (BNN) -- Just before 11 o’clock Tuesday morning, Jan. 30, 2007 an explosion at the Little General convenience store near Winter Place resulted in the deaths of 4 individuals.

At 10:43 a.m., the Ghent Volunteer Fire Department responded to a call about a gas leak at the Little General Store. Shortly after firefighters arrived, an explosion, likely resulting from a propane gas leak, killed a volunteer firefighter, an emergency medical services worker and a building inspector along with one other person.

Five people were seriously injured in the blast and remain hospitalized. Two of the injured were transported to the Charleston Area Medical Center.

The Little General service station was completely destroyed in the blast which covered an area 100 yards in diameter. Despite that, the West Virginia state Fire Marshal said the blast could have been much worse as neither of the two 500 pound propane tanks on site exploded.

State Fire Marshal Sterling Lewis has identified the dead and injured in the Tuesday, Jan. 30, 2007 explosion at the Little General Store on U.S. 19 in Ghent, in the extreme southern tip of Raleigh County.

The dead are:

Frederick Allen Burroughs, 51, of Cool Ridge, a county building code inspector and volunteer firefighter.

Jeffrey Lee Treadway, 21, of Beckley, a propane tank worker.

Craig Lawrence Dorsey, 24, of MacArthur, a volunteer firefighter and EMT.

Glenn Ray Bennett, 44, of Beckley, a propane tank worker.



Worker dies after fall from roof

Omaha, NE -- A construction worker died today after he fell from the roof of a three-story home in an upscale neighborhood under construction near 192nd Street and West Dodge Road. Eleazar Jimenez-Morales, 24, landed headfirst onto the frozen ground about 9:45 a.m., said Douglas County Sheriffs Chief Deputy Marty Bilek. Jimenez-Morales was flown to Creighton University Medical Center, where he died late this morning. He was a roofer, Bilek said. Jimenez-Morales was with a crew working on a home in the Five Fountains subdivision. It is part of the Metro Omaha Builders Associations 2007 Street of Dreams Tour, with homes starting at $500,000.


VILLAGE WORKER DIES IN FALL

Medina, NY -- A Department of Public Works employee from the Village of Medina died Wednesday morning after falling from the bucket truck he was working in. “Something caused the bucket to turn upside down and he fell off the bucket,” Medina Police Chief Jose Avila said. “We are trying to determine what took place.”


Construction worker dies at Jamestown

Jamestown, MO -- A 27-year-old Bois D'Arc man was killed Friday just after noon, while working at Jamestown. The man was a member of a crew taking core samples for an engineering firm. The man apparently bent over to clear some debris from an auger, when a piece of material from his clothing became ensnared and pulled him into the machinery, according to Rogersville Police Chief Leland McMasters.


Worker Dies in Brooklyn After Falling Into Open Elevator Shaft

Brooklyn, NY -- A day laborer is dead after falling through an open elevator shaft in Brooklyn. Officials say the unidentified man was pronounced dead at Woodhull Medical and Mental Health Center at 11:18 a.m. Tuesday, an hour after the accident. The city's Buildings Department is investigating and issued a stop order against the construction site. A spokeswoman said that the open elevator shaft did not have proper guardrails.


Construction Worker Killed In Traffic Crash

AVENTURA, Fla. -- A construction worker was killed Wednesday morning when he was thrown from the bed of a pickup truck that was struck by a car and sent crashing into a pole, police said. According to traffic investigators, the Ford pickup was carrying five construction workers northbound on Biscayne Boulevard when a Mitsubishi Montero whose driver was trying to make a left turn from the southbound lanes struck it. One of the construction workers died and four others were taken to Aventura Hospital with non-life-threatening injuries.


Fall from scaffold kills worker

A Blue Springs construction worker died after he fell from scaffolding Monday morning near 17th and Vesper streets, his employer said. David D. Hamar, 40, was working on the Blue Springs School District’s administrative offices when he fell 12 to 15 feet and apparently landed on his head, said John Sheehy, Summit Masonry’s owner and president.


Truck driver killed in fiery crash

Firefighters climb a steep ravine after extinguishing a fire on a coal truck that crashed Monday at the intersection of U.S. Highway 11 and Exit 97 of Interstate 20/59. State troopers have not released information about a accident that left one man dead Monday morning. The man was driving a coal truck, and was killed at the intersection of U.S. Highway 11 and Exit 97 of Interstate 20/59, according to police scanner traffic. Witness accounts indicate that the driver of the coal truck didn’t stop at the sign at the end of the exit before hitting two other vehicles. Tonya Koll was sitting at a stop sign opposite the one that the hauler was passing through. The hauler struck her after it collided with a Dodge pickup truck. “I don’t know how he didn’t hit me head on," she said. “He was running through the stop sign like he couldn’t brake."


Man dies after Brooklyn shipyard accident

Brooklyn, NY -- A worker at the Brooklyn Navy Yard died Tuesday after falling from a ladder in the hold of a tanker ship in dry dock for repairs, the second accidental death this month at the GMD Shipyard, authorities said. Arturo Medina, 36, of Brooklyn, was ascending a ladder and stepped onto a 5-inch-wide ledge to let another worker pass when he lost his balance, falling backward about 25 feet, said Martin Fernandez, who has worked at the company for five years. Fernandez was one of about five workers welding in the compartment before the accident. Another worker at the GMD Shipyard, Christopher Thomas, 51, of the Bronx, was killed at the shipyard Jan. 6 after being crushed by a 6,000-pound metal plate that fell from a crane.


Stillwater Guard's Death Ruled Suicide

Bayport, Minn. -- The weekend death of a guard at the Stillwater state prison was a suicide, corrections officials said Monday. He was identified as corrections officer Steven J. Jensen, 41, who had worked at Stillwater since 1989. He died of a single self-inflicted shotgun wound, said Shari Burt, a spokeswoman for the state Department of Corrections. Jensen died shortly before his shift was to begin at 6 a.m. Sunday. No inmates were involved and no foul play was suspected, officials said.


Police Search For Masked Gunmen

FORT WASHINGTON, Md. -- Police are asking for help solving a fatal double shooting at a liquor store in Fort Washington. Two masked men shot two employees during a robbery at Fort Washington Liquors on Old Fort Road Saturday afternoon. One of the employees, Seong-Hoon No, died. The other, No's brother, is struggling with life-threatening injuries. Police said the two employees were shot while fighting back.



Father of six killed on job

Brooklyn, NY -- For his six children, Miguel Marquez left his field job cutting sugar cane in Ecuador 19 years ago and came to America, settling into a white rowhouse in Brooklyn. Marquez, 59, was a line picker at Omni Recycling of Babylon Inc. in West Babylon, a transfer station for waste and debris en route to landfills. There, Marquez worked the overnight shift six days a week, where he hand-sorted pieces of metal and concrete from the giant piles of debris. It was a grueling but solid living with union benefits, and two of his sons worked there with him. Early Friday, just a couple of hours after his shift started, Marquez was run over and killed by a Payloader driven by one of his sons, Luis Marquez.


Cement company worker dies in mishap


Bloomingdale, IL - An employee at a Bloomingdale cement company died after being trapped in a sand hopper Wednesday evening, authorities said. The employee, who has not been identified as of Wednesday evening, was engulfed in the sand in the funnel-like equipment on the roof of the Graber Concrete Pipe Company at 24 W 121 Army Trail Rd., according to authorities and the company. The Bloomingdale Fire Department was called to the scene at around 3 p.m., and immediately called for special equipment and teams to try to rescue the employee, moving sand from around the person's waist. After trying for several hours, the agencies moved from rescue to recovery of the employee's body, authorities said.



Race car driver killed in Charlotte


Charlotte, County, FL -Race car driver Hugh Murphy crashed his #5 car into a wall on a turn Sunday and was killed in a race at the Charlotte County Speedway. His son, Todd, also was racing in the V-8 Enduro Feature event and was the first family member to get to the car, literally before the safety crew, according to the Charlotte County Motorsports Park website. Hugh Murphy, 65, of New Port Richey, was pronounced dead at Charlotte Regional Medical Center and an autopsy is being conducted, the Charlotte County Sheriff's Department said in a press release. The accident occurred on the 40th lap of the V-8 Enduro Feature event when Murphy made contact with the turn-4 wall, according to speedway spokesman Bill Green. Immediately afterward, racing was halted.



Cow Kills Farmer


Stark County, OH - A community in Ohio is in disbelief after a strange and deadly farming accident. A Stark County, Ohio grandfather was working his farm with his grandchildren, when he was attacked by a cow and killed. "All we saw was the cow running after him." Zach and his cousin were helping grandpa feed his herd Sunday, when the mundane became inconceivable. "He was gonna separate the mom from the little calf." But one of these Angus, became defensive and caused the injury that took the life of this grandfather of four, Steven Walker. "The mother cow thought that he was going to harm her baby." Wilma Walker says this farm has been in their family for 70-years and Steven was raised around cattle, which is why the death of her son is so hard to understand. "It’s just such a freak accident."



Oklahoma EMT Dies After Ambulance Crash


Clayton, OK - An Oklahoma EMT died in the line of duty Thursday following a Wednesday night crash on an icy road. The Oklahoma Highway Patrol told Firehouse.com that 51-year-old Janez Case ambulance she was riding in slid off the road and overturned. was seriously injured when the Case was an employee of Clayton, Okla. EMS. Case was transported to Sparks Regional Medical Center in Fort Smith, Ariz. where she later died.



FALLEN WORKER IDENTIFIED: Investigation continuing after public works employee falls from bucket truck


Medina, NY - Medina village officials released the name of the Department of Public Works employee who died Wednesday morning after a 25-foot fall in Boxwood Cemetery on North Gravel Road. Michael Russo, 47, of Medina was pronounced dead at the scene after plunging from the bucket truck he was working from. Police Chief Jose Avila said an investigation by his department in conjunction with the Department of Labor will continue.



Heating contractor killed in Missouri metal factory fire


COLUMBIA, Mo. - A fire at an Audrain County metal factory killed one worker and injured 11 more. Charles Martin, 59, an employee of Mexico Heating and Cooling, was welding on an overhead ventilation duct at the Spartan Light Metals factory on Thursday when sparks fell into a container of scrap magnesium and aluminum and caught fire, officials said. Martin was pronounced dead at the scene. Nine employees were treated and released for smoke inhalation at Audrain Medical Center, and two employees required hospitalization. Spartan Light Metals, which specializes in aluminum and magnesium custom die castings, has launched an internal investigation, the company said in a prepared statement. Officials at both companies declined to comment Friday.



Wisconsin Officer Dies On Duty


Grand Ledge, MI - A Wisconsin police officer suffered a massive heart attack at his desk Jan. 26. Grand Ledge Patrolman David Ash, 57, was pronounced dead a short time later at St. Lawrence Hospital. Ash, who had been with the department for nearly 30 years, had not complained of feeling ill before collapsing in front of the records clerk, said Detective Dan Spagnuolo. CPR was immediately initiated by his co-workers, including Spagnuolo. However, they were unable to resuscitate him. A Wisconsin native, Ash was a U.S. Navy veteran. He is survived by his wife of 32 years and a daughter. It was the first on-duty death for the Grand Ledge Police Department that consists of 15 full-time and two part-time officers and a records clerk.



Officer dies in highway crash


Casper, WY - An off-duty Casper police officer died Wednesday night in a car accident north of Wheatland, Police Chief Tom Pagel said Thursday morning. Alcohol is being investigated as a contributing factor in the crash, according to the Wyoming Highway Patrol. Officer Kelly B. Pebbles, 39, was driving southbound in an F-250 pickup on Interstate 25 when the crash occurred at 4:59 p.m. Pebbles was 6.5 miles north of Wheatland when he tried to pass a tractor-trailer. His pickup struck the truck's front left corner and went off the highway in a broadside slide, the highway patrol said.



GameStop offers $10,000 for employee's killer


San Antonio, TX - Two days after a clerk was found murdered at a North Side San Antonio electronics store, her employer offered a reward to help catch her killer. GameStop Corp., the parent company of EB Games, put up $10,000 for the arrest and prosecution of Amber Belken's killer. She was suffocated during a robbery at the EB Games on San Pedro Avenue Monday. If you can help crack the case, call police. A viewing for Belken will from 1 p.m. to 9 p.m. Thursday at Chapel Hill Funeral Home and Memorial Park. Her funeral is Friday at 2 p.m.



Man Killed In Industrial Accident Near Ogden


Ogden, UT - One man was killed in an industrial accident on the I-15 widening project near Ogden. Police say the man, who has not been identified, was working in the belly of a dirt-moving rig. They say he was making sure the the dirt-mover was clean to insure proper operation. Officials say the belly doors closed on him as he was cleaning the surfaces. He was discovered a short time later by his son, who also works as a contractor on the project. A spokesperson for the Utah Department of Transportation says it’s not uncommon for drivers of the dirt-movers to clean out the belly of the truck before starting a day’s work. The UDOT spokesperson says there are safety measures in the trucks that keep the doors from closing. Officials say those devices were not engaged when the accident happened.



Worker crushed to death at NE Portland construction site


Portland, OR - A worker at a construction sit was killed Monday after he was pinned between a truck and a building in Northeast Portland. According to Portland Fire Lieutenant Allen Oswalt, it happened at Canron Construction around 1:45 when the 55-year-old employee was pinned to an outbuilding by a semi cab. Rescue crews tried to free the man using heavy equipment, but he was dead at the scene, Oswalt said. OSHA is investigating. The man’s identity was not released.



Towboat employee presumed dead in Ohio River accident


PADUCAH, OH - Searchers are presuming a towboat employee did not survive after the boat he and two others were on Friday night sank in the Ohio River near Paducah. Brent Stringer, director of emergency management for Livingston County, said a team of eight to 10 people took boats out on the river about 7 a.m. CST yesterday to search for the missing man, who was a crew member on the towboat. They called off the search about 3:30 p.m. CST yesterday. The U.S. Coast Guard said two men had been pulled from the water Saturday. They were Steve Fuller, 46, of Benton, and Raymond Lang, 33, of Metropolis, Ill. The boat flipped near Owens Island, a small area on the river just off Paducah. The missing man's identity was not released.



Man Found Dead At Marathon Refinery


Robinson, IL - A man was found dead at the Marathon Refinery in Robinson, Illinois. Saturday, Dan Gunraj, a refinery employee was found unconscious on the refinery floor and although paramedics responded, he died at Crawford Memorial Hospital. A Marathon Refinery spokesperson told us there is an in-depth investigation going on to discover the cause of death. Monday, the Crawford County Coroner performed an autopsy. The results should be released in 3 to 4 weeks. Marathon tells us this is the first death of an employee at the plant since the 1970`s.



Ejected, struck by own vehicle, driver killed in Watchung wreck


WATCHUNG, NJ -- A Perth Amboy man was killed Monday morning on Route 22 after the truck he was driving hit the center divider and ejected and ran over him, police said. It still isn't clear to investigators how Thomas C. Melvin, 67, ended up under the truck. He was pronounced dead at the scene of the accident, which occurred at 10:32 a.m. on Route 22 West near Gray Street, police said. "I've been a police officer for 28 years, and I've seen some strange things, but I think this is the first time I've seen someone get run over by their own vehicle," Sgt. Tim Faust said. Few other details about the accident were available, but more information will be released as the investigation progresses, Faust said Monday evening. Melvin's vehicle was the only one involved in the crash, Faust said.




Store worker shot to death


Surprise, AZ - One man is dead and another injured after unknown persons fired on employees outside Cindy's Market, 15444 N. Greasewood St., Surprise, on Sunday night. The Surprise Police Department reported that two men parked their older-model blue Ford F-150 in the market parking lot around 10:30 p.m. Employees Frank Khosho, 22, and Ramsin Hirmis, 24, went outside to ask the occupants to move their truck. For an unknown reason, someone in the truck started shooting, killing Khosho. Hirmis received non-life-threatening injuries.



Son of 9/11 cop seeks meeting with Bush


New York, NY -- A day after his father's death, the grieving son of a Sept. 11 police officer said Wednesday he wants to meet with President Bush to describe his father's sacrifice and the health needs of other sick ground zero workers. Ceasar Borja Jr. attended the president's State of the Union address on Tuesday night just hours after learning his father had died from lung problems. "I want a meeting with the president to make the case directly about how important these health programs are," Borja told The Associated Press. "I want him to hear from me how my father died a hero last night, and there are many heroes that will and are continuing to die because they're not given the proper medical attention or not given enough help from the federal government," said the 21-year-old college student, his voice breaking with emotion. After getting the awful news that his father had died, the son had insisted on going ahead and attending the president's speech to honor his father and draw attention to the issue of Sept. 11-related health problems. He had been invited by Sen. Hillary Rodham Clinton, D-N.Y., a longtime advocate for Sept. 11 health issues. "I need to be strong, and I am just doing my best," Borja said. The family was beginning to prepare for a weekend funeral. His father, 52-year-old Cesar Borja, was a 20-year veteran of the New York police who died Tuesday at Mount Sinai Medical Center in New York while awaiting a lung transplant.



Road Employee Killed by Vehicle Near Colby


Wausau,WI - A road department employee for the town of Hull in Marathon County was killed this morning after he was hit by a vehicle. The Marathon County Sheriff's Department says it happened outside of Colby at about 7 a.m. on Huckleberry Road near Maple Road as the man got out of his vehicle. While he was bent over to look at something on the ground, he was struck by a vehicle driven by an 18-year-old woman. Authorities have not released his name or age, but they say they believe he's approximately 36. The crash remains under investigation.



Worker dies after fall from scaffolding in Medfield


Boston, MA - A construction worker died today when scaffolding collapsed in Medfield and he fell at least 10 feet, authorities said. Raymond Fenoff, 38, of Douglas, was working as a roofer on a new home on Old Village Square when the scaffolding beneath him appears to have failed, officials said. Fenoff was working for his family's company. A number of law enforcement officials responded to the scene, including Medfield Police Chief Robert Meaney, Jr. and State Police detectives working for the office of Norfolk District Attorney William R. Keating.



Limo driver crushed between vehicles at airport


Middletown, NY - Andrew Catroppo, 58, was unloading the trunk of his limo at Stewart Airport when he was struck from behind by another limo that had been parked behind him, New York State Police said. That car had been driven by James Reed, 70, an employee of Marshall and Sterling, of Poughkeepsie. He apparently failed to properly put his car in "park" mode when he got out to let off a passenger, police said. When the car started rolling forward, Reed got back in but accidentally pushed the gas pedal instead of the brake. Catroppo, who works for L&B Limo Service in Monroe, was crushed between the two cars and suffered massive leg injuries. He was flown to Westchester for treatment, where he was listed in critical condition last night. No one else was hurt. Reed was cited for several traffic violations.



Michigan Officer Dies while on Patrol, Funeral information below


MI - Livonia Police Officer Ervin Johnston was one of those guys his fellow officers liked to see on the beat. A man's sized man, Johnston once helped fellow officers box in a fleeing felon with his car on the freeway before the suspect got out, intent on doing some harm. "Ervin took this guy to the ground," said Sgt. Pat Moug. "Everyone watched that video over and over again. It was like a sigh of relief when Ervin Johnston showed up." Johnston, an eight-year veteran of the Livonia police department and a husband and father of three, died Friday night while on patrol, apparently of natural causes. Johnston was 45 years old and family members told police he had just had a physical several weeks earlier. The U.S. Army veteran was a resident of Southfield. Before coming to the Livonia department, he worked eight years with the Homewood Police Department in Alabama. Police say he moved here to be near family when his mother was ill.



Employee at Orange Park vet clinic killed in apparent robbery


ORANGE PARK, FL - Investigators are trying to determine who killed an employee at an Orange Park veterinary clinic in an apparent robbery. Clay County authorities believe 25-year-old Andrea Boyer was killed shortly after arriving at work about 5:30 a-m yesterday. Sheriff officials say money was missing from the business. A passer-by reported seeing a gray or silver 1990s model full-size pickup truck leaving the clinic about 5:45 a-m. Boyer was found by a co-worker arriving at the clinic just before 7 a-m. Details of the crime weren't released.



65-year-old Irmo man found dead at his business


COLUMBIA, S.C. - A 65-year-old Irmo man was found shot to death Wednesday morning in front of the business he owns, Lexington County Sheriff James Metts said. An employee of custom homebuilder Lexington Partners found Anthony Francis Ippolito lying facedown under shrubbery in the parking lot around 8:50 a.m., Metts said. Ippolito had been shot several times in the chest, Metts said. Ippolito's wife told deputies he often went to work early in the morning without waking her up, the sheriff said. Deputies aren't sure when or where Ippolito was killed, Metts said. Ippolito was dressed casually and his vehicle was parked behind the business, Metts said. Investigators don't have any suspects or a motive for the killing.



Proposed safety law follows worker's death


Gary,IN - On the day of his funeral, a friend of union organizer Craig Harlacker proposed a new law to promote worker safety. Harlacker, 48, of Lake Station, died Jan. 17 when he was struck by a payloader at a job site in Chicago. After an initial delay, the death has been officially ruled an accident, according to the Cook County Medical Examiners office. Harlacker had been a shop steward for Teamsters Local 142 during a contentious 22-week strike and lockout in 2006 at Dietrich Industries in Hammond. Like several other union workers, Harlacker did not return to Dietrich after the strike was settled. But dozens of workers appeared at his funeral Tuesday at Ridgelawn Funeral Home in Gary, said Charles Everett Burns Jr.



Fort Polk contractor dies in vehicle accident


Shreveport, LA - A Fort Polk contract employee died today when his car collided with a military vehicle, a release from the post says. Jonathan T. Dunmire died less than an hour after the 12:17 p.m. collision at the intersection of H Avenue and Entrance Road on North Fort at the sprawling training base near Leesville. Dunmire was taken by post ambulance to the Bayne-Jones Army Community Hospital where he was pronounced dead at 1:09 p.m. "The entire Fort Polk community mourns the loss of Dunmire and our thoughts and prayers go out to his family at this difficult time," said Col. David Sage, garrison commander of the Joint Readiness Training Center and Fort Polk. Further details were not released by the post.



East Peoria worker dies in accident


EAST PEORIA IL-- A man died after he was hit with a backhoe at a construction site. 30-year-old Josh Jones was a 10-year employee of the City of East Peoria. The accident happened at a residential subdivision a little after 11 a.m. Police say Jones was working inside a water service trench -- when a bucket on a backhoe accidentally hit him. Jones was pronounced dead at the scene. The city says it's like losing a member of the family.


Woman Dies After Shooting at Credit Union in Greeley

Colorado Springs, CO - The woman accused of shooting a Greeley credit union employee to death is due in court for a bond hearing Wednesday afternoon. The shooting happened Tuesday night outside the Colorado State Credit Union. Police say the victim was a credit union employee who was shot in the parking lot as she was leaving work. She was pronounced dead at a nearby hospital. The names of the victim and suspect have NOT been released, but Greeley City Manager Roy Otto says the suspect is a former police dispatcher and wife of a Weld County sheriff's office worker, and that the victim was the wife of a Greeley police officer. A motive is unknown.


Two arrested in pizza killing


HAMILTON, OH – Two Hamilton men have been charged with murder in the death of a Donatos Pizza delivery driver, Butler County Sheriff Richard K. Jones said Wednesday. Jeff Wilson, a 47-year-old Donatos Pizza delivery driver, was found dead about 11 p.m. Tuesday on a street in St. Clair Township. Jones said he was beaten after being lured into a trap to rob him. An autopsy showed only minor injuries on Wilson’s face. Wilson was suffering from hardening of the arteries and a heart-valve problem, Butler County Coroner Richard Burkhardt said. But Wilson’s death could still be ruled a homicide if investigators determine stress of the attack played a significant role, Burkhardt said.


Victim ID'd in workplace death

BLACKSTONE, IL - A Pontiac man died at a manufacturing plant in northwest Livingston County, but few details about the incident have been made public. Robert L. Fanchi, 40, died around 10:30 a.m. Tuesday at Prestress Engineering Corp. in rural Blackstone. His name was the only new information released Wednesday. Livingston County Coroner Mike Burke said he is not releasing any details about the incident until an inquest. A date for the inquest has not been scheduled. A telephone operator at the plant said the company would not comment on the incident, other than to say the situation is being investigated. The business was operating Wednesday.


Man dies after he was found in tank

ONTARIO, CA - A Montclair man died Saturday, three days after he was found unresponsive by co-workers in a water tank where he had been painting. Wilson Leon, 27, of Montclair was found at 11 a.m. Wednesday in the 800 block of East California Street in Ontario, San Bernardino County coroner's officials said. The Ontario Fire Department and American Medical Response Ambulance responded and Leon was transported to San Antonio Community Hospital. He was pronounced dead at 2:01 a.m. Saturday.


Driver killed in truck crash

BRIGHTON, NY — A local man died Friday morning after the dump truck he was driving overturned and crashed down a 50-foot embankment near interstates 390 and 590 in Brighton. Dusty Q. Dusty, 65, of Rochester, was driving on the southbound ramp of Interstate 590, said state trooper Mark O'Donnell. The man lost control of the vehicle, which went over a concrete guardrail and traveled down a steep embankment toward the shoulder of the northbound Interstate 590 about 7:50 a.m., he said. The man was ejected from the vehicle and died at the scene, O'Donnell said. He was reportedly alone in the truck. No other vehicles were involved in the crash, although several drivers witnessed the accident, he said.


Truck backing up runs over, kills worker

GRIMES, CA -- The driver of a gravel truck who was backing up at the site of a natural gas well failed to see a man working in the same area and ran over him late Thursday afternoon. John Kenneth "Ken" Stegall, 63, of Colusa, who apparently took his eyes off the vehcile as it was backing up, was killed immediately, according to information released by Colusa County sheriff's Lt. Doug Turner. The truck is owned by North Gate Rock Co. of Paradise. Officials, who are withholding the driver's name pending further investigation of the incident, said he was not cited at the time.


Palo man killed in construction accident in eastern Iowa

ATKINS, Iowa -- A man was killed in an accident involving an earth-moving machine at a construction site, officials said. Paul Herbert Thompson, 48, of Palo, was struck Wednesday by an excavating machine driven by his employee, who was backing up at the time. Thompson was the owner of Thompson Excavating of Atkins, Benton County Sheriff Randall L. Forsyth said.


Truck driver killed in crash

A 41-year-old truck driver died in a crash about 4 p.m. Thursday on northbound Interstate 71 near Ohio 132 in Warren County. Police said witnesses told them that the semi-truck veered off the side of the road, hit a sign and some trees, then overturned. The victim, Perry Hane of North Canton, Ohio, was thrown from the truck and pinned under it. Investigators suspect Hane feel asleep or suffered a medical problem.



2 Dead in Sewer Line Accident at The Orleans

Las Vegas, NV - Trying to save their co-worker, two men risked their own lives and jumped into a sewer hole after him. An engineer was trying to fix a sewer pipe, but as he finished sawing, a torrent of water swept the tool from his hands and he fell in after it. Scott Allison, Clark County Fire Dept. said, "The water pressure once that pipe broke through hit the saw, knocked it out of his hand, he leaned forward and that's when he fell in." Then his co-workers didn't hesitate to go in after him in an attempt to save him. The only survivor of this tragic accident was brought to University Medical Center where doctors have been working to save his life. He, just like the two men who died, has been exposed to toxic fumes during what was supposed to be a normal day at work.


Bouncer Killed Outside Hollywood Nightclub

HOLLYWOOD, Calif. -- A bouncer was fatally shot and an employee was wounded Saturday after an argument outside a Hollywood club police said. Police responded at about 2:05 a.m. Saturday to a report of shots fired outside The Vanguard, located in the 6000 block of Hollywood Boulevard, police said. Upon arriving, police found two men suffering from gunshot wounds, Los Angeles police spokesperson Sandra Escalante said. Ron Emerick, 29, of Redondo Beach, was taken to Cedars-Sinai Medical Center where he died, Coroner's Lt. Cheryl MacWillie said. The other victim was sent to a hospital, where he was treated for a gunshot wound to the leg and released, Escalante said.

Wendy's employee found shot to death

GREENVILLE, SC-- Authorities say they have no suspects in the shooting death of a 22-year-old, fast-food restaurant employee. He was killed in the restaurant's parking lot. Police say Marvelin Isaac Bass was shot once in the neck as he walked toward his car around 1 AM Saturday. Owner Jim Furman says Bass had worked at the Wendy's restaurant for about three years, and was closing up. The coroner's office says Bass' body was found across the street from where he was shot. He later died at a hospital.



Fast-Food Restaurant Manager Shot, Killed

HOUSTON, TX -- The night manager of a northwest Houston fast-food restaurant was shot and killed when he took a break outside late Thursday, KPRC Local 2 reported. Houston police said John Marx, 27, was killed at a Whataburger in the 2400 block of Gessner Driver near Emnora Lane at about 11:30 p.m. "An employee stepped out and a few minutes later the other employees heard gunshots," Officer E.R. Carstens said.

Worker killed, 1 injured in collapse of Nashville building"

Nashville, TN - A one-story building undergoing renovations collapsed Friday, killing one worker and injuring another, fire department officials said. Firefighters who responded to the scene found the two victims in the rubble and then used a search dog to confirm no one else was injured in the morning collapse. Police identified Richard Whiteshield, 57, of Clewiston, Fla., as the utility subcontractor who was killed. "Our initial search determined that we had one person that was critical," Assistant Fire Chief Charles Shannon told WTVF-TV. "The second person unfortunately had expired on the scene.

Film crew member killed by falling debris

Washington,DC - A crew member working on the coming Samuel L. Jackson movie "Jumper" was killed by frozen debris that fell on him as he tried to tear down part of an exterior set. Workers were outside under frigid conditions Thursday when dislodged debris rained down on them. "Set dresser David Ritchie and three of his co-workers were striking a portion of a wall when a large piece of frozen sand and ice broke free from the top of the set," company publicist Karen Pidgurski said. "Work was stopped and paramedics and police were called to the scene immediately. Crew members from all over the site immediately went to his rescue. Ritchie was pronounced dead at the scene." One of the people working with him at the time of the accident required medical attention, but his family said his injuries are not life-threatening, Pidgurski said. Ritchie, 56, was a veteran of the movie industry, with credits on films as diverse as "New York Minute," "The In-Laws," "X-Men," "The Family Man," "The Corruptor" and "Simon Birch."
