Culture of Life

Ok before I get bombarded by those who know me. No I am not a basher however this man gets under my skin. I am an independent voter, I take the voting process very serious by taking much time to research whom I will vote for. It just so happens that Bush is a Republican that I chose not to vote for in 1 or 2 and lost. Nope I'm not a sore looser I just feel Bush as a person is not one that I would chose to know and that from the get go he gave me the motherly warning signs...stay away!
So take it for what you will. As a little cartoon once said, "Whatever I'll do what I want". and another once said, "I'm the decider and I decide what's best"
It's a joke! Just have a good laugh or get made whatever motivates you.

The Pump

In the few years I have been doing this there have been many ideas as to how we can improve standards and the way families are treated during injury, illness and death investigations. Besides the obvious awareness and involvement of organizations and government the key is to get individuals who are truly concerned about safety and who better than those affected, families who have already experienced the processes and who have nothing more to gain than justice.

Families are mad as hell and prepared to take action. Our Family Bill of Rights and Resource Guide or the “Hand Book for Change” is a good foundation that not only outlines basic human rights but also contains information for families during and after the investigation.

We will be sharing the completed version with you all soon. For now Celeste has posted a little about it and some other related information you will not want to miss on the Pump Handle.

Man Killed When Forklift Collides With Train

INDIANTOWN, Fla. -- Authorities said a man died after a New York-bound Amtrak train collided with a forklift in Palm Beach County. According to police, a worker operating a Bobcat forklift near the intersection of Beeline Highway and Indiantown Road was struck by the train around 11:15 a.m., launching the equipment and operator into the air. The forklift operator was found dead a few minutes after the impact, WPBF News 25 reported. Police said none of the 192 passengers or crew on the train, which was headed from Miami to New York City, were injured.


Muleshoe farmer killed in tractor crash

IN - A 41-year-old Muleshoe farmer was killed Sunday morning in Bailey County when a tractor he was driving overturned on U.S. 70/84 after being struck by a pickup truck, according to the Texas Department of Public Safety. Paul Fulcher was driving eastbound around 11 a.m. (CDT) when the tractor was struck by a Dodge Ram driven by Eduardo Rodriguez, 33, of Muleshoe, the DPS reported. The accident occurred about three-tenths of a mile from Progress, Mark Ellison, police communications operator, said Monday. Rodriguez’ vehicle clipped the axle of the tractor, causing his truck to spin into the ditch and the tractor to overturn, Ellison said. Fulcher was ejected from the tractor and crushed beneath it, Ellison said. He died at the scene.


Man dies after being pinned between trucks on Route 108

NEWMARKET — A Sandown man died after being pinned between a trash truck and a pickup truck on Route 108 Tuesday morning, police said. Joseph Tesoro, 26, of Sweetfarm Road was standing behind a Bestway garbage truck in front of 147 Exeter Road when it was struck by a 2004 Ford Ranger pickup truck driven by Louis Gielar, 86, of 111 Grant Road, according to police. Tesoro was an employee of Bestway Disposal Services of Epping. The large vehicle was stopped in the northbound lane when the Ranger, driven by Gielar, struck it from behind, pinning Tesoro between the two vehicles, according to police.


"Authorities Identify Driver Killed In Tanker Explosion"

TN - Authorities have identified the truck driver killed in Wednesday’s fiery tanker explosion on Briley Parkway near County Hospital Road as 29-year-old Daniel Moss of Wallace Road. Police said Moss appeared to drift to the right before overcorrecting, causing the tanker to roll over and explode. The tanker was hauling gasoline and was quickly engulfed in flames. The flames quickly caught the surrounding field on fire, creating a plume of black smoke visible from as far away as LaVergne. Fire department officials on the scene determined it would be unsafe to try and extinguish the blaze. Three hours later, the extremely hot fire burned out. There were no other vehicles involved in the crash and Moss, who worked for American Highway Transport based in Knoxville, was the only person killed. video


Capstar employee dies at drill site from electrocution

TX - A Capstar Drilling employee was pronounced dead Tuesday after he was electrocuted at a drilling operation in Cochran County. "From looking at the statements, a co-worker took him to a mud pump and was showing him how it worked," Cochran County Sheriff R.W. Stalcup said. "The man then put his hand on an electrical cord that gave power to the machine and the area he touched, some copper was exposed." Paramedics worked on the unidentified man, who was electrocuted around 3:45 p.m., for at least 30 minutes, Stalcup said. They pronounced the man dead after a doctor in Lubbock, via telephone, told emergency medical technicians he couldn't be revived, Stalcup said. It took at least 15 minutes for the emergency personnel to get to the man. "The area where the man was electrocuted is far out," Stalcup said. Police believe the man is in his 30s and from the Odessa area. The Cochran County Sheriff's Office is investigating.


Industrial accident fatal

KS - Another man injured when scaffolding tips over One man died and another was seriously injured Wednesday morning when a motorized scaffolding capable of extending 30 feet into the air tipped over in an accident at a south Topeka industrial building. Both victims were employed by Larry's Electric, authorities said. Their names weren't being released pending notification of family members. Shawnee County sheriff's spokeswoman Lt. Martha Lutz said the man who died was 46. The injured man was 29. The accident occurred at about 10:40 a.m. inside a building at 7215 S.W. Topeka Blvd., where Topeka Trailer Repair had been located until a July fire moved the business to a nearby warehouse.


County highway worker killed on the job

Collins, NY – Erie County Sheriff Timothy Howard Wednesday said his department is investigating an accident that took the life of a Town of Collins highway worker. Edward Wittmeyer, 58, of Springville, was pronounced dead at the scene. Sheriff’s investigators report that a highway crew was working on South Quaker Road in Collins installing support posts for guard rails. They were using a wheeled excavator to drive the posts into the ground. A part of the machine came off striking two workings supporting the post. Peter Waterman, 49, of Collins, was taken to Erie County Medical Center with a head injury. Wittmeyer suffered head and internal injuries and was pronounced dead at the scene. The investigation is continuing.


Worker dies after Madison accident

MADISON - A flagger who was struck by a falling utility pole at the Somerset County Jail construction site died after being taken to a Bangor hospital. Officials say 65-year-old Wayne Steward of Skowhegan was struck in the head when an excavator accidentally hit a guy wire and tore the top of the pole off at the Madison construction site. Steward was taken by ambulance to a local hospital and then by a LifeFlight helicopter to Eastern Maine Medical Center, where he died. Steward was wearing a hard hat at the time of the accident. Federal workplace safety officials are investigating.



5th Mine Worker Dies in W.V.

WV - Mining officials continue to investigate the weekend death of a worker at an underground coal mine in southern West Virginia. A spokesman for the Office of Miners' Health, Safety and Training identified the victim as 53-year-old Robert D. Fraley of Dorothy. Fraley's body was found Sunday at the bottom of a 300-foot mine shaft at the Mountain Laurel Complex near Sharples, which is close to the Logan-Boone county line. The mine is owned by the Mingo Logan Coal Company, a wholly owned subsidiary of Arch Coal of St. Louis. State Police Trooper J.E. Williams says a co-worker noticed Fraley wasn't in the area he was supposed to be in and a search was started. The state Division of Homeland Security and Emergency Management was notified of the death at 8:31 a.m. Sunday.


Man dies in accident at Wyoming mine

GREEN RIVER _ An accident involving a mine transport vehicle at a trona mine near Green River has left one worker dead. The accident occurred shortly before noon Wednesday. FMC spokesman Jim Fitzwater said two mine operators were en route to their designated work area when their mine transport overturned. Fitzwater said a crew that was getting off work discovered the accident. He said 41-year-old Carder Largent Junior, of Mountain View, later died of injuries. Largent had been an employee of the company since 1990. The other worker was treated and released at Memorial Hospital of Sweetwater County. Fitzwater said the mine was temporarily shut down pending an investigation. It was scheduled to reopen at 4 p.m. Thursday.


Man dies in conveyor belt accident

AK - A Palmer man died Thursday afternoon after becoming entangled in a conveyor belt at a gravel pit in the Matanuska-Susitna Borough, according to Alaska State Troopers. Rick Meshew, 49, suffered fatal injuries shortly before 5 p.m. at Mile 38.5 Parks Highway, said troopers spokeswoman Megan Peters. An investigation by the federal Occupational Health and Safety Administration and the state Mining Safety and Health Administration is under way, Peters said. She said the state medical examiner would perform a routine toxicology screen but no autopsy. "Right now it appears to be an industrial accident," she said. "We'll know more after OSHA gives us their report. There's nothing to indicate foul play."



Rural Plymouth farmer killed in truck accident

PLYMOUTH, Neb. - A southeast Nebraska farmer is dead after being crushed beneath a truck this week. Authorities say 79-year-old Glenn Ebbers died Wednesday night following the accident on his farm seven miles west of Plymouth. His wife says he was working under the truck, but had forgotten to take it out of gear and became trapped beneath it. His son was with him at the time of the accident. Emergency workers freed Ebbers, but he died on the way to the hospital. Ebbers' death is believed to be the 13th fatal farming accident this year in Nebraska.


Tow truck driver killed in strange accident

PHOENIX, AZ - Phoenix police say a tow truck driver was killed this afternoon when a gun owner was cleaning a rifle when it went off. The incident occurred near 29th Avenue and Deer Valley Road. Investigators say the gun owner was manipulating the firearm and thought it was unloaded. The rifle discharged sending a bullet through a mobile home window covering, a window and across 29th Avenue where it struck the windshield of a tow truck that was traveling east. Police say the bullet went through the windshield striking the driver identified as 59-year-old Cecil Pugh. After the rifle discharged the gun owner ran outside where he found Pugh in the street with a gunshot wound. The gun owner ran to the truck and jumped into the cab stopping the truck. He then ran back to the victim and called police and fire. Phoenix P.D. says the preliminary investigation indicates the shooting was accidental.


MDOT Worker Killed In Logging Truck Crash

OSSUTH TOWNSHIP, ME - A State Department of Transportation worker was killed in a fatal crash on Route 6 in Kossuth Township Monday morning. Jarod Damon of Grand Lake Stream was pronounced dead at the scene. Officials say Damon was driving an MDOT truck when a logging truck hit it head on. Damon's passenger, Gregory Scott, suffered serious injuries and was transported to a local hospital. State police say it looks like the driver of the logging truck lost control of his rig. "The truck was hauling tree length logs. It appears he lost control around a sharp corner and the trailer of that truck crossed the center line and struck an MDOT truck head on," said Lt. Chris Coleman from the Maine State Police.


Worker killed at Millennium Pipeline project

TUXEDO, NY - A New Jersey man was killed yesterday morning after a piece of pipe-laying equipment rolled over him and down treacherous terrain at the site of the Millennium Pipeline project in Harriman State Park. Pat McCaffrey, 67, of Lebanon, N.J., was an employee of Precision Pipeline, a Wisconsin-based contractor hired by the Millennium Pipeline Co. LLC of Pearl River, a spokesman for the company said. The Millennium Pipeline Co. is building a massive underground natural gas pipeline there. It stretches 182 miles, from Montebello to Corning in the New York's Southern Tier. The incident occurred about 2 miles east of the New York State Thruway near the border of Rockland and Orange counties.


Worker Falls From Clear Lake High-Rise Building, Dies

TX - The Pasadena Police Department tells FOX 26 News that a construction worker was killed after falling from a Clear Lake high-rise building. The name of the building the worker fell from was the Endeavor, a condominium community under construction located on the 3800 block of NASA Pkwy. The man was pronounced dead by ambulance workers who responded to the site. The name of the victim has yet to be identified.


City Worker Dies In Tree Limb Accident

SAN ANTONIO, CA -- A city worker died after a tree limb fell on him near Brackenridge Park on Tuesday morning. The worker had been helping his crew cut down old limbs from a tree when the limb fell from above and hit him in the chest around 10 a.m., officers said. He was pronounced dead at the scene. The high limb had been resting on top of another limb, Sgt. Alfred Trinidad said. When a worker tried to trim it to alleviate pressure, it fell off and landed on the worker below, Trinidad said. The identity of the worker has not been released.


Autopsy ties death to chlorine exposure

TX - A preliminary autopsy report released this week attributes the 2006 death of a Dow employee to pulmonary toxicity after being “acutely exposed to chlorine gas on the job” at the Dow Chemical Co. plant in Freeport. The Galveston County Medical Examiner’s Office turned over the results to a local justice of the peace Friday, about 10 months after the death of Gerald Hamilton, 36, of Galveston. Family members said the results might help them come to terms with his death by knowing why he fell asleep one night after work and never woke up. “It helps to finally know at least how he died,” said Hamilton’s mother, Betty Thies, as she broke into tears. Hamilton, a union process operator, was clearing a pipeline at the plant in November 2006 when less than a third of a gallon of chlorine was released into the air.


Church worker dies in accident

Editor Three members of Southern Oaks Missionary Baptist Church in Tyler, Texas, were involved in an accident on Thursday night while travelling to Alicia to work on the Alicia Baptist Church. Amanda K. Barfield, 32 of Whitehouse, Texas, was killed when the vehicle she was riding in was hit head-on by a vehicle that was travelling the wrong direction on U.S. Hwy. 67, near Cabot. According to Cpl. Stacy Sims of the Arkansas State Police, the accident occurred when a 2005 Chrysler 300, traveling southbound in the northbound lane, struck a northbound 1995 Toyota Camry. The Camry was then struck in the rear by a 2006 Dodge pickup truck, which was also traveling north. The driver of the Chrysler, Candace J. Hazlett, 58, of Jacksonville, was also killed in the accident. Jacqueline Harris, 47, of Searcy, driver of the Dodge, and Bradley D. Jordan, 37, and William "Trey" H. Barfield III, 32, both of Whitehouse, Texas, were all seriously injured in the accident.


El Paso CBP officer dies in crash near Moriarty

WASHINGTON - A U.S. Customs and Border Protection (CBP) Air Interdiction Agent (Pilot Trainee) died while performing training as a result of an aircraft crash that occurred Monday at approximately 12:40 p.m. EDT at Moriarty Airport, near Albuquerque, N.M. The Air Interdiction Officer who died in the crash is Julio E. Baray. He is survived by his wife and two children. "Today, the Customs and Border Protection family mourns the loss of Officer Julio Baray," said Michael Kostlenik, Assistant Commissioner, CBP Office of Air and Marine. "Our thoughts and prayers are with his family and friends."


Man Falls From Radio Tower

AL - Allegheny County detectives and Deer Township police are investigating after a man plunged an estimated 100 feet to his death late Monday afternoon (09/24). Since this was an on the job accident, it's likely that OSHA, the Occupational Safety and Health Administration, will be looking into the mishap as well. A girl walking her dog in a field near the WGBN Radio tower saw the man lying on the ground. She ran home to tell her parents, who immediately notified authorities. A short time earlier, a neighbor said she saw the victim working alone on the tower. Linda Hobaugh said she had also seen him earlier, using a torch to dismantle the tower piece-by-piece. East Deer Fire Chief Tim McGorty said it appeared the man was wearing some sort of a belt or safety harness at the time of the fall. Whether it was attached or how it was attached to the tower is unclear. The top section of a ladder appeared to have buckled somehow. A hook and ladder truck from O'Hara Township was used so firefighters could retrieve the victim's ladder, and duffle bag, with some safety equipment still inside. The tower was one of two used by the full-time gospel radio station, WGBN. The victim and his son had painted the larger, 450-foot tower nearby several weeks ago. Neighbors said recently, however, they thought he was working alone on the shorter tower.


Caltrans worker struck by car on the job dies

CA - A Caltrans employee who was struck by a vehicle Tuesday while performing shoulder work at the Interstate 580-680 interchange has died, authorities said Wednesday. Djuan Bush, a 24-year-old San Leandro man, was pronounced dead at 11:35 p.m. Tuesday at Eden Medical Center in Castro Valley, the Alameda County Coroner's Office said. Bush was injured when the driver of a Porsche reportedly lost control, veered into a Caltrans truck, bounced off and hit the worker before striking another truck, CHP Officer Steve Creel said. Bush was taken by ambulance to Eden Medical Center where he succumbed to his injuries, which included several broken bones, the coroner's office said.


Port of Oakland workers stay off job after longshoreman killed

OAKLAND, Calif. - Workers at the Port of Oakland are staying off the job today after a longshoreman was crushed to death by a shipping container. The decision by nearly 15 hundred workers to stay away has brought the loading and unloading of ships at 1 of the nation's busiest ports to a halt. A union spokesman says the 15-ton container was being locked onto the top of another container on the deck of a cargo ship when it slipped and fell on the worker yesterday afternoon. The spokesman said the man died of internal bleeding after suffering a crushed pelvis and broken ribs. A port spokeswoman said much of the operation at the country's fourth largest port could remain shut down until tomorrow morning. An investigation is continuing into how the accident occurred.


Dump truck jackknifes in Brooklyn, killing two

Two men were killed when a dump truck filled with debris turned over, crushing their car at a Brooklyn intersection early Wednesday, police said. Witnesses told police it appears that the driver of a Toyota Camry tried to cut in front of the truck, causing it to jacknife and flip onto its side, landing on the car at Atlantic and Washington avenues in Prospect Heights. One of the men in the Camry appears to have died instantly, while witnesses heard the other moving around before falling silent.


Wauconda pilot killed on volunteer flight

IL - Paul Harris of Wauconda knew he was fortunate to own a plane, so he felt he had to give something back. He became a volunteer for Angel Flight Central, ferrying sick people to treatment centers around the Midwest. On Wednesday, while on a mission to pick up a patient in Findlay, Ohio, he died when his single-engine plane crashed. Harris, 57, who has two adult children, began flying 13 years ago, said his wife, Rebecca. "It was a childhood dream," she said Wednesday night. "As we got older and our kids were grown up he decided it was something he wanted to do. And that's what he did."


Accident at Tube Mill claims life of Commerce man, 26

MI - A Commerce Township man died Friday after being trapped inside a machine he was trying to fix at the Michigan Seamless Tube plant in South Lyon. The incident happened around 7:15 p.m. when Kevin Michael Wilson, 26, and a co-worker were working on a machine that encountered a problem and stopped, according to a South Lyon Police report. Wilson entered the working part of the machine, and once inside, the machine malfunctioned and started up again, according to the 21-year-old Belleville man who was working with Wilson. Wilson was trapped inside and suffered severe fatal head injuries. Police and emergency personnel arrived at 7:23 p.m. and Wilson was pronounced dead at the scene.


Construction Worker Killed in Polk City

POLK CITY, FL - A 39-year-old construction worker was killed today after he was crushed by a trailer, the Polk County Sheriff’s Office said. Charles Vanduyn was working for L-J Construction Company at 15720 N. Commonwealth Ave. in Polk City when the trailer he was working with became detached and crushed him shortly before noon, sheriff’s spokeswoman Donna Wood said. Vanduyn’s death has been ruled an accident, Wood said. Wood said Vanduyn was a relative who worked for the family business. The Occupational Safety and Health Administration has been notified, but it’s not known whether they will investigate the accident, Wood said. OSHA could not be reached late Thursday afternoon.


Grain elevator owner dies in soybean bin

MAZON, Ill. -- A grain-elevator owner is dead after being buried under a massive pile of soybeans in a storage bin. Grundy County Chief Deputy Coroner Phyliss Dralle says Sean Garkey of Ottawa died Tuesday. The accident happened at Access Ag in Mazon, about 35 miles west of Kankakee. Garkey was 33. Dralle says Garkey's death is being investigated by the Grundy County Sheriff's Department, the coroner's office and the federal Occupational Safety and Health Administration.


Fatal W. Knox County Industrial Accident

TN - A Knoxville man is dead following an industrial accident in West Knox County on Thursday afternoon. It happened around 2:00 PM at Ready-Mix Concrete on Lovell Road. Authorities tell us the man was an employee but they are not releasing his name right now. Keep it with Volunteer TV News and VolunteerTV.com for the latest on this developing.


Custodial worker found dead at high school

FL - On Thursday, Sept. 20, Union County EMS responded to a reported call of a man passed out in the bus loading area in front of the gym at Union County High School. According to Captain Garry Seay of the Union County Sheriff's Office, Dennis Weeks, 69, was found slumped over in the seat of his vehicle, and was pronounced dead at the scene. According to Seay, Weeks left his home around 5 a.m. to report to his custodial job at the high school and apparently died of a massive heart attack upon arriving at work. Weeks' body was taken from the scene by ambulance prior to the arrival of students at the high school. In a statement from principal Alex Nelson, he said that the Union County High School family mourns the loss of a dear friend and co-worker, Dennis Weeks. "We will miss his friendly smile and joyful presence. He was a friend to all and did his work with great pride, keeping the campus of Union County High spotlessly clean," Nelson said.


CONSTRUCTION WORKER FOUND DEAD ON SITE IDENTIFIED

CA - A construction worker who died early Monday night in San Francisco's Sunset District has been identified as 35-year-old Genaro Magana, according to the San Francisco Medical Examiner. Magana, of San Pablo, reportedly collapsed atop a scissor lift at the site of a Sunset Reservoir construction project at 1949 24th Ave., said San Francisco fire Lt. Ken Smith. Rescue crews were called to the site at about 5:05 p.m., but attempts to revive the man were unsuccessful, Smith said. The cause of Magana's death is under investigation. The death does not appear to be suspicious, according to Smith.


Pa. worker dies in construction accident

PA - A worker for a Western Pennsylvania company died Wednesday when a 6,500-pound metal form with a construction worker strapped to it fell from a crane at an Interstate 64 bridge construction site in Dunbar, W.Va. The victim was identified by police and fire officials as Jon Beatty, 47, of Punxsutawney. Mr. Beatty, a married father of three, worked for Brayman Construction Corp. of Saxonburg. The company said yesterday he was a carpenter foreman. Today would have been his 48th birthday, said Mike Welsh, lead council representative for Carpenters Local 2274 in Robinson. Mr. Beatty was initiated to the union in 1990. The local represents nearly 1,000 carpenters in 60 counties.


Search Underway For Deli Shooting Suspect

MT. OLIVER PA - Police are now looking for a suspect following a deadly shooting in Mt. Oliver. It happened just after 9am in a local deli on Brownsville Road near Arlington Avenue. According to investigators, the clerk in the A & E Deli Food Mart was shot. Authorities say the victim was taken to Mercy Hospital where he was pronounced dead just before 10am. Officials say an autopsy is scheduled for tomorrow. Investigators say the suspect was wearing a black hooded shirt and black pants. Authorities are urging anyone with information on this case to contact them.


Funeral Services For 2 Men Killed In Robbery

OH - Friends and family gathered Saturday afternoon to remember two men killed during a robbery at the Covault Market in Dayton Tuesday. Funeral services for Robert Harris started at 10 a.m. Saturday at the Schlientz and Moore Funeral home. Harris was a long time employee of Covault Market and was killed along with owner, Roger Covault, Tuesday afternoon. The funeral for Roger Covault was at 2 p.m. Saturday at Westbrock Funeral Home.


Oilfield worker killed in accident

PIEDMONT, Okla. - Authorities say an oil-field worker was killed in an accident at a well site near Piedmont in Canadian County. Piedmont police Sergeant Jerry Goodwin says three employees of Southwest Oilfield Construction were working to pull piping out of the ground when a pipe strand snagged a piece of the oil rig about 7:15 last night. Goodwin says the large object -- possibly weighing as much as 1,800 pounds -- came up from the ground with the pipe and struck the platform where the men were working. He says the men were launched into the air and falling debris crushed 1 of the workers after he fell to the ground. Goodwin says that man died at the scene. Another worker was flown by medical helicopter to OU Medical Center in Oklahoma City, where he's listed in critical condition. The third man was taken to Integris Baptist Medical Center in Oklahoma City and is listed in good condition.


Man faces felony DUI charge in death of Island Packet employee

SC - A Hilton Head Island man who was at fault in a Saturday car crash that killed a 36-year-old Bluffton woman will face a charge related to felony driving under the influence, according to police. Alicia Orozco, an Island Packet employee, was killed when Alexander Zavalla Palacios, 25, was driving his Chrysler Town and Country minivan north on John Smith Road at about 3:30 a.m. Saturday when he went through the stop sign at U.S. 278 and hit the passenger's side of a Nissan Armada SUV going east on the highway -- killing passenger Alicia Orozco, 36, of Bluffton -- Hardeeville police Lt. Dan Martin said.


Driver Charged After One Is Killed and Three Are Hurt in Crash in the Village

NY - A delivery worker was killed and three others were injured early yesterday when a drunken driver crashed a sport utility vehicle into their parked van at an intersection in Greenwich Village, the police said. One of the deliverymen, Antonio Barranco Hernandez, 36, was dead when the authorities arrived, according to the police. Witnesses said that Mr. Hernandez and two other men were unloading fruit from the rear of their gray Ford Econoline van on Seventh Avenue South just south of Grove Street about 7:20 a.m. when a cream-colored Lincoln Aviator, hurtling down Seventh Avenue South, apparently lost control after swerving to avoid another vehicle and rammed into the parked van.

Man Electrocuted At Local Plant

NEWELL, W.Va. -- Police said a 23-year-old man died in an industrial accident in Hancock County early Tuesday. Ethan Caleb Hart is believed to have been electrocuted at the C.E. Mineral Plant on Kennedy Park Road near state Route 2 around 12:45 a.m. The Hancock County Sheriff's Department is investigating, and Jones said the plant will be working to with Occupational Safety Hazard Association representatives to determine the cause of the accident. Ken Jones, vice president of the plant, said Hart had worked as a forklift driver at the plant for less than six months. He was found on the ground by his supervisor and another worker in front of an electrical panel out of his regular work area, Jones said. He said police and paramedics believe Hart was electrocuted, but officials won't know for sure until an autopsy is performed. About 35 people work at the plant, which produces different types of minerals that are used in steel mills. Jones said this is the first fatal accident at the plant that he can recall.


Anglen, Truck Driver Killed in Kansas

IA - An Iowa tractor-trailer driver died Wednesday in a crash on northbound Interstate 435 in western Wyandotte County. The accident happened about 11:30 a.m. when a car struck the truck over the Metropolitan Avenue bridge, according to a Kansas Highway Patrol report. The collision sent the car and the tractor-trailer into the interstate bridge rail. The truck cab flipped over the rail and plunged into the concrete interstate embankment, igniting an intense fire that destroyed the vehicle. The rig's trailer flipped and skidded about 100 feet and landed in the north lanes of I-435. The truck driver, Richard Anglen, 40, of Silver City, Iowa, was ejected during the crash. Anglen was airlifted to a hospital, where he was pronounced dead. The car, driven by John K. Nichols, 17, of Johnson County, bounced off Anglen's vehicle and came to rest in a ditch. Troopers said Nichols suffered minor injuries, but a passenger in his car was not injured.


Sanitation worker killed in fatal crash in Newmarket

NEWMARKET, N.H. --One person was killed and two others injured in two unrelated crashes on the same road in Newmarket on Tuesday. Police said sanitation worker Joseph Tesoro, 26, of Sandown, was killed when he was pinned between the trash hauler he was riding on and a pickup truck. The crash occurred at about 11 a.m. on Route 108, and the road was closed for hours while police investigated. The truck was being driven by an 86-year-old man; police said it's too early to tell what caused the crash.


Man, 35, killed in welding explosion

GA - A 35-year-old contractor was killed this afternoon in an explosion at a south Augusta business, according to the Richmond County coroner. The accident took place about 1:30 p.m. at Farmers & Truckers Biodiesel Fuel Supply at 4207 Mike Padgett Highway, the coroner said. The identity of the victim is expected to be released later this afternoon. Richmond County Coroner Grover Tuten said the victim was welding a stainless steel transfer tank when it exploded. No one else was injured. He is an employee for Advanced Sheet Metal, officials said.


Ferguson Unit warden found dead in pool

TX - A Texas Department of Criminal Justice warden was found dead in a Madison County unit’s pool only two weeks after taking the prison’s top spot. A TDCJ spokesperson confirmed Monday that Clarence Mosley, 45, was found dead in the Ferguson Unit’s employee pool a little after 1 p.m. Sunday. He was pronounced dead on the scene at 1:50 p.m. The Ferguson Unit is located about 5 miles south of Midway. Jason Clark, a spokesperson for TDCJ, said the death appeared to be accidental.


Lorain worker crushed to death

LORAIN, OH — An employee of Brush Wellman Engineered Products was crushed to death Sunday night in a casting machine. Matthew Salisbury, 30, of Day Drive in Lorain, was pronounced dead at the scene following the accident, which occurred at about 10:50 p.m., said county Coroner Paul Matus. “He died as a result of acute trauma,” Matus said. “His injuries were obvious and fatal.” The U.S. Occupational Safety and Health Administration’s Toledo office is investigating the accident. “He was inside a machine and got pinned,” said Dick Tracy, the agency’s assistant area director. He said it was too early to say how the accident occurred. Salisbury’s wife, Jennifer Salisbury, said a co-worker told her he saw the whole thing. “The machine was broke down — it was clogged — and he was trying to unclog it,” she said.


State worker dies after injury in L&I

WA - Bill Harmon, an office assistant in the Department of Labor and Industries, died last night after suffering an injury in a stairwell at the agency's Tumwater headquarters. He was 87. Department Director Judy Schurke said in a message today: Although there were no witnesses to this unfortunate situation, L&I employees heard the sounds of a fall and responded immediately. They called 911, notified building security, and took action to block the area. In addition, a number of L&I staff went to the hospital to show their support for the family. I’m proud of the way our employees responded.


Worker killed at Brush Wellman

LORAIN, OH -- A Lorain man died at the Brush Wellman facility late Sunday night while trying to fix a piece of machinery that had malfunctioned, according to a Lorain police report. Employees called 911 at 11:18 p.m. after they heard Matt Salisbury, 30, yelling for help by a machine he was operating. Salisbury was attempting to fix a malfunction with a saw and once he cleared the problem, the saw, which was under pressure, rose and crushed his head against a steel beam, according to the report, Co-worker David Sullivan, 43, heard a banging followed by Salisbury yelling and he ran to his aid. Sullivan lowered the saw and pulled Salisbury to the floor and attempted CPR while another employee called 911.


Tietex forklift operator killed on the job

NC - A Spartanburg County forklift operator died while on the job at Tietex International Monday morning. Francis Ernest Littlejohn, 57, of 2046 Little Africa Road, Chesnee, died about 11:30 a.m. Monday, said Joseph Stewart, an investigator with the Spartanburg County Coroner's Office. Littlejohn was operating a forklift in reverse when he backed into another, unoccupied forklift and was thrown from his machine, Stewart said. An autopsy is scheduled for today, which should help determine whether the cause of death was medical or a result of the crash, he said.


Two Shot, One Dead, Inside Business

ATLANTA, GA -- A gunman walked into an Atlanta business Monday morning and shot his wife in front of stunned co-workers. The man then shot and killed himself. The husband was pronounced dead inside the business -- Staveley Services Fluids Analysis -- on Presidential Drive in DeKalb County. The woman was taken to the hospital in critical condition. The shooting was reported at 8:30 a.m. as employees were starting their day. Among those left shaken by the events was the woman's mother, who also works at Staveley Services. Witnesses said the husband crawled through a service window and went directly for his wife. He shot her and then turned the gun on himself.



Cave-in kills construction worker in Gresham

OR - Edward Joseph Manley of Boring was working at subdisvision site. Edward Joseph Manley, 29, of Boring, died Friday morning, Aug. 17, after a 30-foot deep hole caved in and the 1,600-pound pipe he was helping position crushed him Thursday, Aug. 16. The man was working on a subdivision construction site at 425 N.E. Williams Road just south of Division Street at 4:40 p.m. when the 30-foot-deep, 15-foot-by-15-foot hole he was in caved in, said Jim Klum, deputy chief of emergency operations for Gresham Fire & Emergency Services. The force of the soil hitting him knocked him over, hundreds of pounds of clay pinned his legs and the soil’s momentum moved the 4-foot diameter pipe, crushing him, Klum said.


Mill Worker Killed By Fall

WILLIAMSTON, S.C. -- A contractor working at the Cushman Milliken Mill in Williamston was killed Monday afternoon. According to a release from the company, 39-year-old Jerry Lee Reid was a subcontractor who worked with Top Level Management Services Company. Reid was using a scissor lift while painting a support column just before 1 p.m. Mill officials said that someone on a monorail system hit the lift, causing it to topple over. Reid fell about 15 feet and was killed. Kevin Cox, Cushman's manufacturing manager, said, “Our thoughts and prayers are with the family of this individual, as well as his co-workers at Top Level. We reach out in sympathy to each one of them and share in their devastation over this tragedy."


State investigating after man crushed at plant

BESSEMER CITY, N.C. -- Investigators with the North Carolina Department of Labor are trying to figure out what caused a deadly accident a Gaston County metal fabrications plant. Gastonia police say Cudiel Ambrocio VelazQuez, 23, was crushed by a machine just before 6 a.m. at Porter’s Fabrications, a company that makes steel parts. Investigators say he was crushed by a machine raises and lowers material and was pronounced dead at the scene. According to Charles Saleh, the company’s owner, VelazQuez didn't run that machine, and was more of a helper, so it's not clear how the accident happened. Saleh also said VelazQuez worked third shift and the accident happened as crews were changing shifts. Officers say there is no indication of foul play.


MGM-Mirage Cites Human Error in Construction Worker's Death at CityCenter

NV - Eyewitness News has learned the victim of Thursday's deadly accident at the MGM-Mirage CityCenter project was 65-year-old Harvey Englander. His widow says he was close to retiring. Meanwhile, she waits for information about how he died. MGM-Mirage representatives say it was human error. However, one Perini construction worker who knew the victim told Eyewitness News safety is not the top priority. It was back to work Friday for thousands of CityCenter project construction workers who just lost one of their own. Officials say Englander never turned off this manlift while he was greasing it, but it was the counterweight system that came down on top of him. One worker, who wishes to remain anonymous, works on the same building and was the victim's friend. "It hits home. It hits home real bad. I've gotta live with it, deal with it and go back today and do it again," he said.

But through his grief there lies anger. He says most accidents on job sites could be prevented if safety is a priority. "Every contractor in the world is supposed to send us home with ten fingers and ten toes. That's it. That's the bottom line," he continued. But this worker feels safety has taken a back seat in Las Vegas because of pressure to get these buildings up quickly. He described, "When you get out in the field it's just go, go, go, go, go, and just do and hope for the best." In February, two workers were killed on the CityCenter project after retaining walls fell on top of them.


Worker killed in fall from Driscoll Bridge

NJ - A construction worker fell to his death this morning from the Alfred E. Driscoll Bridge as he was walking along a temporary wooden platform, officials said. Thomas Oset, 45, of Manville was carrying lumber about 8:30 a.m. on the Sayreville side of the bridge when he fell through the boards of the platform and tumbled 96 feet, said Sgt. Stephen Jones of the State Police. He was pronounced dead at the scene. Oset was wearing a safety harness but was not hooked to a safety line at the time of the accident, Jones said. Workers have been resurfacing the older span of the Driscoll Bridge, which carries the Garden State Parkway over the Raritan River.


Worker killed at Harriman State Park didn’t wear seat belt: cops

Tuxedo, WI - The man who was killed in an accident while working on the Millennium Pipeline in Harriman State Park last weekend was thrown from his vehicle because he wasn’t wearing his seat belt, State Park Police said yesterday. Pat McCaffrey, 67, of Lebanon, N.J., was in the enclosed cab of a sideboom, putting segments of the new pipeline in place when the sideboom tumbled backward down a hill and threw McCaffrey, an employee of Precision Pipeline Co. in Wisconsin. “Witnesses said he was tossed from the vehicle on the first roll,” Detective Sgt. Steven Huff of the State Park Police said. “Examining the vehicle, you could tell the seat belt wasn’t worn.” Work has resumed at the site, but investigators with the police and the Occupational Safety & Health Administration have not determined what caused the sideboom to roll over. Huff said the accident might have been caused by the steep, gravelly terrain. Millennium spokesman Michael Armiak said the vehicle is designed to work on terrain of that type.


Slain SAPD officer loved job and life

TX - A day after the phone call, Officer Ray Gallegos still feels the thumping pain in his chest. Mario Moreno, Gallegos' close friend and colleague, had been shot while on duty, said the voice on the other end. "My heart started beating 100 miles per hour," Gallegos remembers. "The first thing I thought was 'I hope he's alright.'" Then came the second call. Moreno was dead. Moreno was slain outside The Villas at St. Moritz on the Northwest Side on Friday afternoon, becoming the first San Antonio police officer killed by gunfire since 2001. Oncor service worker dies

A 39-year-old Oncor worker in Forest Hill who may have been electrocuted as he worked died Saturday, authorities said. Police say the man, who has yet to be identified, was working in a service truck bucket in the 6000 block of Guilford Street. Emergency workers were called shortly after noon, but the worker was pronounced dead at John Peter Smith Hospital in Fort Worth.


Hallettsville man killed in fiery wreck

TX - At 12:18 a.m. Saturday, a 20-year-old Hallettsville man died after his truck went off the road and struck a tree and burst into flames, a Department of Public Safety official said. Russell Grahmann, 20, an employee at Pioneer Drilling Company, was pronounced dead at 1:08 a.m. by Fred Nieto, justice of the peace for Lavaca County, said senior operator Anna Mendez of DPS in Corpus Christi. Grahmann was taken to the Kubena Funeral Home in Hallettsville. Two other passengers were taken to Lavaca Medical Center in Hallettsville. Both were not wearing seat belts, Mendez said. Christopher Rainosek, 23, laborer, of Hallettsville, was in serious condition with incapacitating injuries, Mendez said. Rainosek was taken by ambulance from the hospital to Brooke Army Medical Center in San Antonio, said Richard Garcia, on-call supervisor for Lavaca County Rescue Service. Jeffery Allen Tobola, 23, of Hallettsville, was in stable condition with no incapacitating injuries, Mendez said.


Officials Say Barn Floor Gives Way, Kills Farmer

LANCASTER COUNTY, Pa. -- A Lancaster County farm worker died Saturday morning. Fire officials said part of a barn collapsed on the farmer. It happened at the Shellenberger Farm on the 1500 block of Prospect Road near Columbia. Fire officials said the worker was driving a tractor and hauling a tobacco wagon into the barn, when the floor gave way, sending the tractor and wagon crashing into the basement. The worker was pinned beneath the tractor and died at the scene, officials said. Rescuers spent much of the day trying to get to the man's body, according to Washington Borough Fire Chief Jimmy Kapp. Officials did not release the victim’s name, but they said he was 63-years-old and from Mount Joy.


Worker Dies at Chrysler Casting Plant

KOKOMO, Ind. — A 48-year-old man was crushed to death by a machine at a Chrysler LLC casting plant in Indiana, authorities say. The Howard County coroner said John Kelly Wright of Logansport died instantly Saturday morning. Chrysler spokeswoman Michelle Tinson said the company could not comment on specifics of the death because it is being investigated. "It's very unfortunate," she said. "We express our condolences to the employee's family." Local authorities will work with the Indiana Occupational Safety and Health Administration to investigate, Tinson said. The casting plant halted production for 24 hours during the investigation, she said. Howard County Coroner Brad Bray said it appears that Wright was between two side panels of a die cast machine when he was crushed to death. An autopsy was planned to confirm the cause of death.


Man Killed In Brooke County Industrial Accident

BETHANY, W.Va. - The owner of a Brooke County welding shop was crushed to death in an accident on Monday. Sam Mazella, 76, was killed while using a front end loader at Mazella Welding & Fabrication, the company he owned on state Route 67. Officials said Mazella was operating the loader at around 2:30 p.m. when he hit a patch of loose dirt that gave way and caused the machine to topple on top of him. He died at the scene. The sheriff's department is investigating the incident. Vincent Mazella, the victim's son, said OSHA is expected to be at the scene on Tuesday.


Young worker dies in sulfuric acid vat

CA - San Mateo County coroner's officials have identified 18-year-old Fernando Jimenez Gonzalez of Redwood City as the man who drowned in a vat of sulfuric acid at a Redwood City factory Sunday. Gonzalez's body was discovered at Coastal Circuits Factory on Tacoma Way around 1:45 a.m. by his father after Gonzalez didn't arrive home, according to a Redwood City police spokesman. Both men are employees of Coastal Circuits, which manufactures circuit boards. The incident is being investigated, but police believe Gonzalez's death was accidental. Gonzalez is believed to have fallen into the waist-high vat of sulfuric acid after being overcome by chemical fumes, police said. He did not appear to be wearing facial protection, and he was one of two employees working the early-morning shift, police said. Occupational Safety and Health Administration officials are investigating the incident, and the factory has been temporarily closed, police said. Laura Boozer, chief operating officer for Coastal Circuits Factory, told The Chronicle the company is withholding comment until the investigation and autopsy are completed.


Worker killed at plant identified

CAMDEN, NJ - A worker died after being struck by a front-end loader at the city's trash-to-steam plant in the Waterfront South area on Friday. The accident occurred shortly after 5 p.m. at the facility in the 600 block of Morgan Boulevard, said police spokeswoman Teresa Sicard Archambeault. Anthony White, 51, of the 200 block of Rand Street in Camden, was killed after the driver of the truck backed up and struck him. White was a temporary worker at the plant. The unidentified driver of the truck is a longtime employee. OSHA has been notified of the incident.


Third person dies at CityCenter site

LAS VEGAS, Nevada -- A construction worker was killed Thursday at a Strip construction site when he was cut in half by a counter-weight system used to operate an elevator, authorities said. The accident occurred just after 6 p.m. at the Pelli Tower at MGM Mirage's CityCenter project, as the worker leaned into the frame of the counter-weight system to grease it, said Scott Allison, spokesman for the Clark County Fire Department. While the worker was bending over, he was struck by the counter-weight system as it came down at about 300 feet per minute, Allison said. Part of the worker's body ended up on the second floor, and the other half landed in the basement, Allison said. The worker was most likely killed instantly. "He probably had no idea what hit him," Allison said. The body was removed from the site about 9:30 p.m.


11 Family, friends say goodbye to man killed in farming accident

TN - Family and friends said farewell Thursday to a Gibson County farmer who was known for being a "good fellow." Funeral services and a burial were held Thursday afternoon for Morris W. Moore, 80, of Humboldt. Moore died Tuesday morning from injuries he sustained when a tricycle tractor fell on him, according to Jerry Privitt, chief of the Humboldt Area Rescue Squad. Privitt, whose squad worked the accident, said that Moore was last seen out with a chainsaw on one of his farms. "He was doing a little tree work," Privitt said. "I don't know what he was doing to get into the predicament he was in."


Local man dies after falling from grain silo

IL - A local man died Monday after falling 100 feet from a concrete grain silo in Patoka. Justin Edward Carroll, 19, of Salem, was pronounced dead Monday at 2 p.m. at St. Mary's Hospital in Centralia by emergency room physician David Greengart, according to the Marion County Coroner's Office. The coroner's office reported Carroll was an employee of Wedekemper Inc. doing work at ADM in Patoka on a concrete grain silo. The crew had returned from lunch just before the fall occurred. Other workers reported he had not complained of any illness during lunch and appeared to be just fine when returning to work. He also was reported to have had his safety harness on, thus increasing the uncertainty of the cause of the fall. No one witnessed the fall, as his co-worker was around the other side of the silo and out of sight range. No one realized early on that the man had even fallen, as workers were in different areas away from Carroll. No evidence indicates any reason he may have fallen from the 100 foot catwalk.


Worker at laundromat killed during robbery

MI - The slaying of an employee at a Stadium Drive coin-operated laundry occurred during an apparent robbery and more than one suspect is being sought, authorities said today. Daniel McNeill, 59, an 18-year employee of Stadium Coin Laundry and Cleaners, 3313 Stadium Drive, was critically wounded when Kalamazoo Department of Public Safety officers found him at the business at about 3:55 a.m. Tuesday. He was pronounced dead a short time later at Bronson Methodist Hospital. His death is being treated as a homicide. An undisclosed amount of money was taken from the business and investigators believe more than one person participated in the incident, a news release said.

LAS VEGAS, CA - State safety authorities are investigating the death of a 65-year-old worker who was killed in a mishap at a Las Vegas Strip resort construction site. Harvey Englander of Las Vegas died yesterday (Thursday) when a heavy counterweight hit him while he was performing maintenance on a construction lift system at MGM Mirage's CityCenter project. A Nevada Occupational Safety and Health Enforcement official says an investigation could take several weeks. It was the third death at the CityCenter site this year -- where some 3,600 workers are engaged in round-the-clock construction of a $7.4 billion mixed-use hotel, casino, condominium and retail project on 66 acres.


Minnesota Vikings Security Staffer Found Dead Of Apparent Heart Attack

Mankato, MN (AHN) - A member of the Minnesota Vikings' security staff was found dead of an apparent heart attack at the team's training camp facility on Wednesday morning. Frank Foster, 66 who was a retired St. Paul police officer, served as a part-time security guard for the Vikings for the past 14 years, was found sitting on a chair in the RV outside the Gage Hall dormitories at Minnesota State, Mankato. Team officials looked for Foster when he did not show up for breakfast, unusual for the prompt and professional employee he was said to be.


Man Killed In Gaston County Industrial Accident

GASTON COUNTY, N.C. -- Police say a man was crushed to death early Thursday by a piece of machinery at his job. The accident happened just before 6 a.m. at the Porter’s Fabrications plant on Oates Road in Gaston County. Investigators say the man was cleaning under the machine when a piece of it fell on him, killing him. The Occupational Safety and Health Administration is looking into the accident.


Suspect arrested in fatal shooting at Ohio trucking company

OH - A man who fled a shooting that left two people dead at a northwest Ohio trucking company had been living at the motel in Michigan where he was arrested several hours later, authorities said Thursday. Calvin Neyland Jr., 43, of Findlay, Ohio, was driving a semitrailer cab in the hotel parking lot, headed toward an exit, when police closed in Wednesday evening and arrested him without incident about three hours after the shooting, said Sgt. Geoffrey Kovenich of the Monroe County sheriff's office in Michigan. Killed were Douglas Smith, 44, of Toledo and Thomas Lazar, 58, of Pennsylvania, who both were employed by Liberty Transportation, according to the police department in Perrysburg Township, which is about seven miles southeast of Toledo.


Prisoner run over by machinery, dies

A state prisoner in a workrelease program was killed Tuesday when he jumped or fell off industrial equipment used to move cars and was crushed by the machine, a state Department of Correction spokesman said. Cecil Buckner, 28, of Tyronza was riding on the machine that an employee of U-Pull-It Auto Parts was driving at the business in southwest Little Rock on Baseline Road. Buckner jumped or fell as the driver was turning the machine, spokesman Dina Tyler said. Buckner would have been eligible for parole in December, Tyler said. His death was being categorized as an industrial accident.


Worker falls from scaffold, dies in La Villa

LA VILLA, Texas -- A 23-year-old sugar mill worker was killed in an industrial accident, Hidalgo County authorities said. Jose Hernandez was working at the mill in La Villa Monday night when he fell from a scaffold. Officials believe Hernandez struck his head on a pipe as he fell. The injured worker managed to call 911, but he later died. La Villa is 20 miles northeast of McAllen in South Texas.


Man struck, killed at his fruit stand

MILL CREEK, IN - A Mill Creek man who regularly sold fruit from a stand in front of his home died Monday night when a vehicle hit him. A Chevy Trailblazer driven by Richard Russ, 62, struck Kerry Snyder, 71, in Snyder’s front yard in the 6500 block of Ind. 4 at 10:22 p.m., according to a La Porte County Sheriff’s Department report. Snyder was dead when police arrived. He was airlifted from the scene by helicopter, according to a police report. Russ, who lives on South Taylor Road, told police he was eastbound on Ind. 4 when he noticed a vehicle in the 6500 block traveling west with its headlights on. Russ said the vehicle appeared to be traveling in the eastbound lane. Another vehicle was westbound in its proper lane at the same time, Russ told police.


Youth leader, killed in church van crash, was devoted to job, Seems driver of pickup fell asleep, police say

BOONVILLE, NC - They had just left the Charity Baptist Church parking lot early yesterday morning in a van crowded with duffel bags and blankets. The teenagers and their adult chaperones were in high spirits, having returned three weeks ago from a mission in Puerto Rico and now looking forward to a restful week at a Bible camp on the North Carolina coast. Norma Shore, known as “Momma Shore,” was sitting in the front passenger seat where she could talk and keep an eye on everyone. The teenagers loved her, and she had brought along her own mother’s fried pies as a treat. Only minutes later the trip went horribly wrong. As the Charity Baptist Church van crossed the county line on U.S. 601 at 7:50 a.m. it collided with a pickup driven by Allen Groce, 23, who had just got off work in Salisbury. The front of the van was crushed, killing Norma Shore.

MN - Authorities in Minneapolis have identified the driver of a bakery truck who was killed Wednesday, Aug. 1, when the Interstate 35W bridge collapsed. He was Paul Eickstadt, 51, of Mounds View, MN. He was driving for Sara Lee, which owns the former Taystee bakery in Roseville, MN. The Taystee logo was visible on Eickstadt’s trailer, but the eyes of television viewers everywhere were focused on the compelling images of the truck’s cab engulfed in flames. Eickstadt had just started his shift, setting out to deliver a load of bakery items to Mason City, IA, when the bridge collapsed, authorities stated. The cause of the collapse is still under investigation. Authorities did not confirm Eickstadt’s identity until Friday, Aug. 3, two days after the bridge tragedy, when they confirmed he was the fifth person to have been killed.


Two men killed in murder-suicide

TX - McKinney Police Department investigators believe the Saturday shooting deaths of two men were the result of a murder and a suicide. City spokesman Steve Hill said police believe Christopher Ryan Byrd, 20, of McKinney, shot and killed Brad Evans, 26, of Van Alstyne, shortly before 6:30 a.m. Saturday at the Shell gas station in the 4900 block of Eldorado Parkway then turned the gun on himself. Byrd died after a Careflight helicopter flew him to a Dallas hospital. McKinney police Capt. Randy Roland said investigators had not determined a motive for the shooting as of Sunday. Becky Lagro, area supervisor for Quickway Foods of Dallas, said Evans, the store’s manager, was the only employee in the store at the time of the shooting. She said he had arrived early to do some financial paperwork. Some customers heard the shooting then made their grisly discovery inside the gas station’s convenience store. “He was coming in to do some paperwork and do some banking and all that and his relief was coming at 10 a.m.,” Lagro said. “From what I understand, what I’ve been told is they (the customers) called 911 after they heard shots being fired. The customers were pumping gas when it happened, even before they were entering the location and they immediately came back out and got on their cell phones.”


Worker killed while removing rubber from DIA runway

DENVER, CO - Police and federal safety officials are investigating the death of a worker removing aircraft tire marks at Denver International Airport this morning. The worker was hit by a large runway sweeper after he apparently bent over to pick up an object. Police say the driver of the sweeper couldn't see the man. The sweeper machine has a 20-foot broom out front that spins. It's used to both remove light snow during the winter and small debris the rest of the year. Herb Gibson, Denver area director for the Occupational Safety and Health Administration, said he couldn't recall any other runway deaths at the airport. He said the last worker death happened in January 2005 when a United Airlines worker died after device that extinguishes potential fires in jet engines exploded. video


STC officer dies from apparent crash injuries

ST. CHARLES, CA - A St. Charles police officer died this morning nearly three weeks after a crash put him in the hospital, officials said. Officer Vaughn Olson, 41, was found unresponsive in his room at Marianjoy Physical Rehabilitation Hospital in Wheaton, where he was sent following a hospital stay at Delnor-Community Hospital in Geneva, police said. Olson, who was a 15-year department veteran, and fellow officer Michael Griesbaum were involved in the July 14 crash near Route 38 and Meredith Road in Virgil Township. Griesbaum was driving a minivan, with Olson as his passenger, when it collided with a Ford Taurus at 12:24 a.m., according to police. The driver of the Taurus, Mitchell A. Westerlin of Elburn and his passenger, Melanie K. Carlson of Elburn were hospitalized following their crash, along with Griesbaum. Their conditions were unavailable this afternoon. As the investigation continues, Gengler said a third vehicle could have been involved that could have caused Griesbaum to swerve and cause the crash. No charges are expected, but Gengler said investigators planned to meet with prosecutors.


Injured Phillies stadium worker dies

PHILADELPHIA, NJ - One of the two Aramark food service workers who was injured when a cart plunged down an elevator shaft at Citizens Bank Park died Thursday, police said Police did not immediately release the name of the worker who died. Officials said the cart, driven by one of the Aramark food service workers, fell down the shaft onto an elevator on Saturday after hitting and somehow opening the elevator doors on the third level of the stadium. Aramark spokeswoman Kathleen Keenan said the company is offering counseling to the worker's family and co-workers. Officials from the state and the Occupational Safety and Health Administration are investigating.


Worker dies in crane accident

GILLETTE - A 59-year-old railroad worker died when the rail-mounted crane he was operating fell off the tracks. Crook County Coroner Mike Frolander said Charles Willey, of Chappell, Neb., died of "crush injuries." Federal and BNSF Railway officials were investigating why the crane tipped over Thursday as it worked near the Campbell-Crook county line, about 20 miles east of Gillette, officials from several agencies said. Campbell County Undersheriff Scott Matheny said deputies who responded to the scene believe Willey was swinging the 60-foot boom at an angle that caused the crane to lose its balance and fall over.


Man who died after being trapped in trench identified

DENVER, CO – The coroner's office has released the name of a plumbing contractor who died after being buried under 12 feet of dirt Saturday afternoon. According to the fire department, 26-year-old Erick Hernandez of Aurora and a co-worker were trying to find a sewer line at a construction site at 770 Grant. Hernandez dropped his shovel and went to retrieve it. The trench collapsed around 2 p.m. Rescuers estimate he was buried under 12 feet of dirt. "When we arrived on scene, 20 construction workers were fighting against time, time certainly is of the essence," said Lt. Phil Champagne, Denver Fire Department spokesperson. "They did, in fact, use a backhoe, the construction workers, which we never try to do in a scene like this. You can certainly create more harm."

Man who delayed robber dies in bulldozer accident, Was clearing land in Adger, working alone

AL - A man who thwarted an armed robber's escape in May from a Bessemer bank robbery where two employees were killed, died Saturday morning in an accident in western Jefferson County. Jefferson County Sheriff's spokesman Sgt. Randy Christian said Christopher Lynn Chappell, 41, was accidentally killed at 8:15 a.m. by the bulldozer he was operating on Cargile Drive in the Adger community. He said Chappell was clearing land and had been working since 6 a.m. It is believed he became entangled in the track of the bulldozer and was run over while working alone.


Oakland bakery worker, 19, held in death of editor

OAKLAND, Calif. - A 19-year-old handyman at Your Black Muslim Bakery admitted to police on Friday night that he ambushed and killed Oakland Post editor Chauncey Bailey, investigators said. Police officials said Devaughndre Broussard told them he killed Bailey because he was angry over stories the journalist had written about the bakery, its employees and leaders in the past. Investigators said Broussard also was concerned about stories that he thought Bailey might be working on. Bailey apparently had been working on a story about the group and its finances, authorities said. Broussard made the admission to Sgts. Derwin Longmire and Lou Cruz. Broussard had been arrested at his home Friday morning, where police recovered the shotgun that they believe was used to kill Bailey the day before.


2 Shot In Botched Prince George's County Store Robbery, 1 Killed In Double Shooting

PRINCE GEORGE'S COUNTY, Md. -- Police have identified the man who was killed in a double shooting in Prince George's County Saturday night. The shooting happened at Murray's Steak House on the 6100 block of Livingston Road in Oxon Hill. Police said a man tried to rob the store and then shot two people, killing one of them. Police said Andrew Jacobi Davis, 22, of the 1200 block of Devonshire Drive, died after being shot. A second employee suffered non-life-threatening injuries. Police said the shooter is between 15 and 19 years old.


Indianapolis man dies in fall at construction site

Carmel, IN - A 25-year-old Indianapolis man died Saturday after he fell from a building under construction at 336 West Main Street in Carmel. Police and fire units were called to the scene just before noon and found a man on the ground unconscious. He was pronounced dead at the scene. Police found that the victim apparently fell about 40 feet to the ground from some construction scaffolding at a building under construction. Carmel police investigators, along with the Hamilton County Coroner's office are continuing the investigation. Authorities also notified that Occupational Safety and Health Administration. The name of the victim is being with held pending notification to family.


CHP officer dies in car chase, Officer struck attempting to deploy spike strip

PLACERVILLE, CA - A California Highway Patrol officer who was setting a spike strip to stop a driver involved in a chase was killed Tuesday after the driver swerved and struck him on the highway. Douglas Scott Russell, 46, was hit on Highway 50 about 45 miles east of Sacramento and airlifted to the University of California, Davis Medical Center. The 22-year CHP veteran was pronounced dead at the hospital. "Our officer was attempting to deploy the strip, and the suspect aimed his car at the officer and murdered him," CHP assistant commissioner Kevin Green said. The chase began in the parking lot of an auto parts store in the Sacramento suburb of Rancho Cordova. Police were called to the shop to check on a man sleeping in his car, said Tim Curran, a spokesman for the Sacramento County Sheriff's Department. The suspect, identified as David Zanon of Sacramento, tried to hit a Rancho Cordova officer head-on during the pursuit, as well as another CHP officer deploying a spike strip, Curran said. Russell was the third officer Zanon tried to strike.

He loved NASCAR and wolves, but his biggest love was his family, his wife Marcia Deal said Deal, a Mishawaka Utilities worker, died Tuesday morning after falling into a transformer box at an unfinished home in the Reverewood subdivision. An autopsy Tuesday afternoon showed Deal died accidentally by electrocution."I just don't know what I'm going to do without him," Marcia Deal told The Tribune Wednesday. Tom Deal leaves behind a 23-year-old daughter, an 11-year-old son and a 30-year-old stepson. "He was a very meticulous family man who loved his children," Marcia Deal said. "Tom has been actively involved in our son Chase's life," she said. "Our son has been wrestling since first grade. He (Tom) was down at the high school three times a week practicing with him."



Railroad worker dies in a crane accident

WY - A 59-year-old railroad worker was killed instantly when the crane he was operating fell off the tracks Thursday morning east of Gillette. Federal investigators and BNSF Railway officials on Friday continued their investigation into the death of Charles Willey, a Chappell, Neb., man who fellow workers told law enforcement officials had been living with his wife in a mobile home in Newcastle. It was still unclear this morning what caused the crane to tip over as it worked on and around a bridge, officials from several agencies said.


VING RHAMES' DOGS SUSPECTED IN MAULING DEATH: Employee of actor found dead in front yard.

CA - Tragic news for actor Ving Rhames. A caretaker Though Adams, 40, was fatally mauled by at least two dogs at the Brentwood home of the actor. The man was not immediately identified, but according to LAPD, the man is in his early 40s and had worked for Rhames for two years. Police said they believed that the dogs belonged to the actor. The victim's body was found lying in the front yard of Rhames' home on the 12900 block of San Vicente Boulevard, covered in bite wounds. The actor is out of the country filming a movie, according to police. Animal control officers took four dogs into custody, including two bull mastiffs believed to be responsible for the attacks, police said. LAPD investigators said they would not rule out other possible causes of death, including a heart attack or other medical condition. An autopsy is pending.


Seabrook officer dies in car crash

SEABROOK, MA - Sgt. Dana Bedell, 43, an 18-year veteran of the Seabrook Police Department, died Sunday of injuries suffered in a single-car accident on Route 1 in Hampton Falls. At the Seabrook police station yesterday morning, the flag flew at half-staff and black bunting surrounded its entrance as the men and women who worked side-by-side with Bedell for almost two decades mourned his loss. "Dana was promoted to sergeant in 2005," said Seabrook police Chief David Currier. "When you promote someone, you never really know exactly how he's going to perform as a supervisor. I was very pleased to see that he was a strict supervisor. Dana was strict and he supported his officers. I was looking forward to him returning to duty in about a month."



Truck crash causes structure fire; driver killed

RICHFIELD, OH — A Newcomerstown man was found dead after the truck cab he was driving crashed into a building early in the morning of July 31. The Summit County Medical Examiner’s Office (SCMEO) has tentatively identified the driver as 48-year-old Randy St. John. Official information is pending positive identification. An autopsy was scheduled for Aug. 1. According to Richfield Police Department Lt. Joe Davis, the man was driving southbound on Brecksville Road when the truck cab swerved across the northbound lane, through the property of a Richfield service department annex, through a fence and into the southwest corner of Superior Erection Co., 3901 Brecksville Road, which caught fire.


Pa. man killed by falling beam in Dryden barn project mishap

DRYDEN, NY — New York State Police and U.S. Occupational Safety and Health Administration investigators are probing the death of a worker which occurred during a barn-salvaging operation in the town of Dryden Wednesday morning. Gerald A. Hadden, 29, of Susquehanna, Pa., was working at the site at 1890 Dryden Road when a beam, being supported by a crane, dislodged from the barn and struck him in the head, according to police. Members of the state police, New York State Department of Environmental Conservation, the Dryden Fire Department and Dryden ambulance responded to the scene. Hadden was transported to Cayuga Medical Center in Ithaca, where he was pronounced dead at 11:37 a.m.


Man who died in Rodeo Wash had large family

AZ - One of the tragedies of Adalberto Padilla's (60)death in the flooded Rodeo Wash was that he won't see his beloved grandson, Ben, start kindergarten next week, family members say. The 5-year-old "was his life," said Francisca "Panchita" Padilla, 59. She and Mr. Padilla raised eight children in Tucson and enjoyed their family - 24 grandchildren, one great grandchild - and many friends, she said. "He was alive. He was with us. He did not want to die," she said. "He could have had more years." Ben - Benjamin Moreno Jr. - is the son of one of Mr. Padilla's seven surviving children, Benjamin Moreno, 21. Moreno said he and his wife, April, haven't had the heart to tell their son his grandpa is dead. "They were so close. (My father) gave him all his love. He loved him more than life," Moreno said.


Worker Killed In Ballpark Elevator Accident Identified

Philadelphia, PA - One of the men involved in a freak elevator mishap Saturday at Citzens Bank Park had his life support disconnected on Tuesday and he died soon afterward. The victim was identified as Christopher Green, 27, from the 2700 block of South Sheridan Street in Philadelphia. Green and another worker drove a golf cart into a service elevator shaft, but the elevator wasn't there. The vehicle plunged down the shaft, hitting the top of the elevator car. A second employee -- a brother of the one who landed atop the elevator -- was inside.


Hospital employee dies after being hit by car

AL - A Huntsville Hospital employee died Wednesday after she was hit by a vehicle. The accident happened Wednesday in front of the emergency room at Madison Street and Rands Avenue. Police say the driver of a minivan looked away to stop her child from climbing out of his child restraint seat, when she hit 54-year-old Janet Underwood. Underwood was in the crosswalk at the time. Huntsville police say she died from her injuries.


BGE Contractor Electrocuted While Repairing Lines

BALTIMORE, MD An utility worker was electrocuted in Southeast Baltimore Wednesday afternoon. Peggy Lee reports on the accident in the 6400 block of Holabird Avenue. Officials say a contract employee for BGE was working near high voltage lines when he accidentally came in contact with one. A spokesman for the subcontractor, Pike Electric of Mount Airy, N.C. identified the man as 33-year-old Michael Toney, of Forest City, N.C., an electrician for about 15 years. Roy Fullen, a general foreman for the company says Toney was in a work bucket above a truck when his back touched wires carrying 4,000-8,000 volts. The bucket was quickly lowered to the ground by fellow workers and medics arrived moments later and pronounced Toney dead. Chief Kevin Cartwright says Toney's body was taken to the state medical examiner's office for an autopsy. video


Wall tumbles at construction site, killing two

BROWNSVILLE — Two construction workers with Texas Steel Erectors Inc. died Tuesday afternoon and a third one remains in stable condition at a local hospital after a wall panel collapsed at the construction site of the future T-Mobile Customer Service Center in Northeast Brownsville. Brownsville police spokesman Sgt. Jimmy Manrrique said a 9-1-1 call came into dispatch at 3:20 p.m. about the incident at the future call center site at 635 Paredes Line Road. Killed in the accident were a 52-year-old man from Austin and 49-year-old man from Toluca, Mexico, Manrrique said. Justice of the Peace Tony Torres pronounced the men dead at the scene. A third man, a 22-year-old from Austin, was rushed via ambulance to Valley Regional Medical Center, where he was listed in critical but stable condition Tuesday night, Manrrique said.


Owner confesses to killing two employees who asked for a raise

ATLANTA, GA - Two underpaid employees at an East Point car dealership had been hounding their boss for a raise. Instead, the boss gave them each a bullet to the head, police said. Fed up with the two workers' frequent salary demands, owner Rolandas Milinavicius — who was in dire financial straits — apparently snapped Thursday morning and allegedly shot both of them to death inside the business. "He told us that he was under a lot of stress," said East Point police Capt. Russell Popham. "Unfortunately, he decided to take his anger out with violence." Milinavicius, 38, of Alpharetta started RM Auto International on Willingham Drive two years ago amid great hopes that he could feed the demands for American cars in his native Lithuania. He began shipping cars through ports in Charleston, S.C., and elsewhere, and at some point hired the two victims – fellow immigrants from his north European country – as his only employees. Little is known about the first victim, Inga Contreras, 25. She lived in Atlanta but did not have any other family members in the state. The second, Martynas Simokaitis, 28, moonlighted at night as a staff photographer for Lunar, a magazine about metro Atlanta's electronic dance music culture. He had been in Atlanta only a few years, and was working toward a career in his chosen field: music.


Death of diver confirmed by coroner

BILOXI, WA - Harrison County Coroner Gary Hargrove confirmed Friday that a commercial diver died after a dredging accident at a casino that was reported earlier this week. The coroner said Robert J. Harris, 22, of Biloxi, died Thursday night at a local hospital. Harris had been on life support since the accident at the IP Casino Resort Spa on Tuesday. An autopsy performed Friday afternoon concluded drowning was the cause of death, Hargrove said. Harris was working on a project for J.E. Borries Marine when he radioed a co-worker and said he was having problems with his mask, and he allegedly took his equipment off. When he was pulled from the water, Harris was unresponsive and not breathing, investigators told the Sun Herald earlier this week. A Borries company official, after learning about the accident, told the Sun Herald that she was "shaken."


Two state transportation workers killed

PURCELL, Okla. - Two state Department of Transportation workers were killed after they were struck by a tire while working along interstate 35 in McClain County. The Oklahoma Highway Patrol says 29-year-old Jose Milan of Detroit, Mich., and a Mexican national, 18-year-old Avila Bustomonte (ah-VEE'-lah bust-oh-MONT'-a) were killed in yesterday's accident near Purcell. Troopers say the transportation workers were repairing a center cable barrier along the interstate when the left rear tire and wheel of a 16-foot box trailer being towed by a pickup truck detached as it was pulled along the busy interstate highway. The tire struck the two men. The patrol says Bustomonte was pronounced dead at the scene from massive injuries. Milan was transported to Norman regional Hospital, where he was pronounced dead.


Mine worker killed in accident was from Morristown

TN - The contract worker killed in an east Knox County mine accident was from Morristown. Twenty-eight-year-old Daniel E. Frederick fell to his death in the Immel mine near Mascot shortly after midnight Sunday morning. Frederick was working for Cowin & Company Mining Engineers and Contractors, replacing structural steel inside the main shaft of the Immel mine. The work was part of the process of reactivating the mine, which has been closed for five years. An order by the Mine Safety and Health Administration to suspend work in the mine remained in place Tuesday as MSHA, Cowin, and mine-owner East Tennessee Zinc Company continues.


Airport Employee Dies In RDU Accident

MORRISVILLE, NC - An investigation is underway into how an American Eagle Airlines employee was killed in an accident at Raleigh Durham International Airport. Lufuluabo Kalonji, A fleet service clerk, died after a baggage tug he was driving collided with an unused airplane Tuesday night at Gate C23. The accident happened just before 9 p.m. at terminal C. The airplane was not in service at the time. An RDU spokesperson says no flights were affected


Davdison County farmer killed in tractor rollover

WELCOME, NC - A 66-year-old man has died after his tractor slid down an embankment and rolled on top of him. Authorities say Larry Thomas Allen of Davidson County died accidentally. The Franklin County Sheriff's Office says Allen was mowing an area near a ditch. It happened yesterday. Allen was pronounced dead at Lexington Memorial Hospital. He was a retired employee of the state Division of Forest Resources.


Two dead, one injured in accident, One-vehicle wreck occurred on Oklahoma 2 south of Warner

OK - Two people were killed and one critically injured about 7 p.m. Monday night in a one-vehicle accident about 3 1/2 miles south of Warner on Oklahoma 2. Orlando D. Noel, 15, of Porum was pronounced dead on arrival at Muskogee Regional Medical Center with head and trunk internal injuries, according to the Oklahoma Highway Patrol. Amber N. Mann, 33, of Porum also was pronounced dead upon arrival at MRMC with head and trunk internal injuries. Teresa E. Milligan, 51, of Muskogee was transported to MRMC and later taken by helicopter to St. Francis Hospital in Tulsa, were she was listed in critical condition Monday night with trunk and internal injuries, the OHP said.


ID's on 2 workers who fell to their deaths in Round Rock

ROUND ROCK, Texas - ID's have been released on two workers who died following a scaffold collapse at a hospital construction site in Round Rock. The accident happened Saturday morning when the two men fell about 50 feet after the scaffold came down outside Seton Medical Center Williamson. Authorities say the victims were 37-year-old Eloy Luna and 38-year Mario Mojica. Both were believed to be from Mexico but had been living in Austin. The Occupational Safety and Health Administration is investigating.


Newark man killed in Columbus accident

NEWARK, OH — A Newark newlywed known for his big heart and strong work ethic was killed Monday after being pinned between the bucket and the cab of a Bobcat steel loader he was working to repair. Adam Holman, 23, of Newark, was working alone Monday in a maintenance garage at Franklin Tractor Sales in Columbus and might have been trapped for as long as an hour before co-workers discovered him at about 1:30 p.m., Columbus police said. He was taken to Mount Carmel West hospital, where he was pronounced dead. Adam’s father, Bethel Apostolic Church Pastor Jeff Holman, said his son went to Licking Valley schools and the Joint Vocational school before graduating from Northwestern Diesel Auto Mechanic School.


Doylestown man killed by tractor-trailer

PA - Everyone liked David L. Keiser. A kind man who never spoke badly of anyone, he combined the values of his native Midwest with a broad smile and a happy sense of humor, those close to him remembered. “He was the kind of person where if you didn't like him, something was wrong with you,” said his wife of 27 years, Jeanne Scalamoni-Keiser. Keiser, 61, of Christopher Lane in Doylestown Township, was killed Friday in a car accident in New Jersey. Police said a tractor-trailer loaded with batteries struck Keiser's 2007 Dodge Charger about 9:30 a.m. at an intersection in Clinton Township in Hunterdon County.


Man Crushed By Piece Of Equipment

COLUMBUS, Ohio - A worker repairing piece of construction equipment was crushed to death Monday afternoon inside a garage on the city's south side. According to Columbus police, the man was working alone inside the garage at Franklin Tractor Sales when he was killed, 10TV News reported. Investigators said the man was working on a front loader when its front arms unlocked and came down on top of him. His name was not immediately released, but police said he was in his 20s. Construction worker dies on Williamson Co. job site

Austin, TX - A construction worker was killed on a Williamson County job site Tuesday the second incident in less than a week. According to Meritage Homes, a worker for Ace Landscapes died in an accident at a Legacy Homes job site in the Teravista development near Round Rock. Legacy Homes is part of Meritage Homes. John Foster, with the Williamson County Sheriff's office, said the worker was apparently drilling a hole when he hit an electrical current. Foster said it will take an autopsy to determine if the worker, Alberto Vargas, 29, died from the current or from complications relating to the current.


Vernon County man dies after falling off tank

BENSON, Minn. — A Nevada, Mo. man died after falling from a tank under construction at an ethanol plant in western Minnesota. It happened Sunday at the Chippewa Valley Ethanol Company plant in Benson. Swift County Sheriff Scott Mattison said James Pippen, 26, was an employee of USA Tank Sales of Seneca, Mo. The sheriff’s office got a call around 2:15 p.m. that a construction worker had fallen about 60 feet inside a tank under construction at the plant. Rescuers took Pippen to the Swift County Hospital in Benson, where he died. The accident is under investigation by the state OSHA division, the Swift County coroner and the sheriff’s office.


Town worker who died in accident is mourned

NY - Hundreds of people poured into Sts. Cyril and Methodius Church in Deer Park Monday morning to mourn the loss of a popular 20-year-old, who died last week from injuries sustained in a accident while working at Overlook Beach. At one point during the nearly two-hour ceremony, Ryan Horodnicki's father Tim kept both arms around his wife Helene and daughter Heather as he eulogized his son as a confident young man who "understood how to get the most out of every day" and inspired others to be comfortable in their abilities. "He crammed a lifetime of living into 20 years," Tim Horodnicki said, which drew a standing ovation from the near-capacity audience.

BENSON, MO — A 26-year-old Missouri man died Sunday from injuries he suffered in a 60-foot fall at the Chippewa Valley Ethanol Company plant. According to the Swift County Sheriff’s Office, James Pippen of Nevada, Mo., was an employee of USA Tank Sales of Seneca, Mo. The Sheriff’s Office received the call around 2:13 p.m


Deadly restaurant robbery in NN

NM - A deadly restaurant robbery over the weekend has police looking for a killer. It was early Saturday morning, just moments after the Hardees on Denbigh Boulevard opened for business something went wrong. Early Saturday morning, police say two masked men walked into the Hardees and demanded cash. One of the suspects shot and killed 47-year-old Diane Green. That same bullet also hit another employee in the arm.


Riviera worker crushed while changing truck tire

RIVIERA BEACH, FL - A worker at a Michelin tire shop off Bee Line Highway and Garden Road was killed when an 18-wheel truck he was working on crushed him Tuesday morning, police said. The accident occurred about 7:40 a.m. in the Interstate Industrial Park, police Cmdr. Leonard Mitchell said. The worker, whose name was not released, was changing a tire on the truck when the jack fell and the vehicle crushed him, said Darlene Fossum, a spokeswoman with the federal Occupational Safety and Health Administration.


Rookie Seattle police officer killed in crash

WA - A Seattle police officer just weeks away from starting his career as a new patrolman was killed Tuesday morning while driving to work. Officer Michael E. Kelley, a 26-year-old Army veteran, was on his way to the department's South Seattle training complex when his Honda Civic struck a shoulder barrier on northbound Interstate 5 near Boeing Field. Kelley's car spun before being struck on the driver's-side door by a Ford pickup, according to the State Patrol.


Postal Worker Killed in Accident

NORTHEAST PHILADELPHIA - September 11, 2007 - Flowers mark the spots on a tree where a public servant lost his life.He was on the clock doing his job, taking mail downtown when it happened. Stunned postal workers visited the scene where their colleague, Matt Zebin, lost control of his truck and crashed into a tree. The accident happened in the 68-hundred block of Oxford Avenue, near Unruh, around 3:00 Tuesday afternoon. While the cause remains unclear, it is believed Zebin may have suffered a heart attack. He worked out of the Fox Chase Post Office where his co-workers said he was very dependable. Zebin delivered mail for 18 years. The mail in his truck was gathered after accident and delivered as scheduled.


Female worker shot to death at Rockline plant in Sheboygan

SHEBOYGAN, Wis. — April Heinecke huddled with a dozen others in the drizzle outside the manufacturing plant where her sister was shot and killed Monday by a former co-worker. Her sister, Christina Wollenzien, 28, was outspoken and fun-loving, and she had no relationship beyond work with the man who shot her before killing himself, Heinecke said. She couldn’t imagine what the shooter, Shadow Yang, 40, could have had against her sister. “That’s why everybody is baffled,” she said. Police and officials at Rockline Industries, where Yang once worked with Wollenzien, also said they knew of no relationship between the two beyond work. But an expert on workplace violence said it would have been “extremely, extremely rare” for Yang to have chosen Wollenzien at random.


Two Chewelah women arrested in deadly Kansas hit and run crash

SPOKANE, WA -- Two Chewelah women have been arrested in connection with a hit-and-run crash in Douglas County, Kansas that killed two Kansas highway workers. The women, identified as 26-year-old Sabrina Morgan and 48-year-old Ramona Morgan, both of Chewelah, were arrested after leading police on a 10-mile chase. The chase ended when police laid down stop sticks which punctured the tires of their pickup truck. The construction workers, according to Wichita, Kansas ABC affiliate KAKE, were in a work zone along Highway 59 in eastern Kansas when they were hit around 10 a.m. local time Tuesday. Ramona Morgan has been identified by authorities as the driver of the pickup truck that struck and killed Kansas Department of Transportation worker Tyrone Kote, 30, of Seneca, Kansas and contract employee Rolland Griffith, 24, from El Dorado, Kansas. Sabrina and Ramona Morgan are both being held at the Osage County Jail. Charges are pending.


Road Worker Run Over, Killed on I-70

CO - A Colorado Department of Transportation subcontractor was run over and killed on westbound Interstate 70 just west of Denver Tuesday as he was picking up orange cones in a construction zone, authorities said. The victim, whose name has not been released, was riding on the back of a construction truck near the Morrison Road exit when he fell off and was run over by the truck around 4:00 p.m., according to the Jefferson County Sheriff's Office. The man worked for American Barricade Rental and was performing work authorized by C-DOT. Two lanes of westbound I-70 were closed as authorities investigated the accident.


Michigan truck driver killed after apparently falling asleep at wheel

STAUNTON TWP., OH — A Michigan man was killed early Monday when a tanker truck veered off southbound Interstate 75, went across the northbound lanes, drove off the road again and hit trees after the driver apparently fell asleep, Miami County sheriff's deputies said. James Swindell, 35, of Livonia died at the scene of the 12:40 a.m. crash, shortly after being cut from the wreckage. Sheriff's Deputy Bryce Adams said several witnesses saw the truck go out of control while traveling on I-75 south. The truck crossed the median then crossed the northbound lanes before leaving the road and going into trees. The crash report listed driver fatigue/falling asleep as contributing factors.


DAYTON, Ohio — A biplane performing stunts for an airshow crashed into a runway Saturday in front of thousands of spectators, killing the pilot, officials said. Jim LeRoy, 46, was in one of two planes making loop-to-loops with smoke trailing as part of the annual airshow at Dayton International Airport. LeRoy was a frequent performer in the annual Visiting Nurse Association Air Show conducted at Witham Field in Stuart. In a 2005 article for Scripps Treasure Coast Newspapers, LeRoy told columnist Geoff Oldfather: “It’s not my job to impress the other pilots. It’s not my job to do anything other than entertain that guy or gal, and if they leave that airshow thinking ‘Holy cow, I didn’t know an airplane could do that, that was unbelievable,’ then I’ve done my job.”


Man shot to death at downtown barbershop

TUSCALOOSA, AL - A man was shot and killed during a fight that occurred at a downtown barbershop at about 4:15 p.m. Saturday. Otis Brown, owner of Brown’s Barbershop in the 2200 block of Seventh Street, said the two men involved in the fight were both barbers who worked there. Capt. Loyd Baker, commander of the Tuscaloosa County Metro Homicide Unit, declined to identify the men until their families were notified. He said a 32-year-old man was killed and a 26-year-old man was injured. The 32-year-old died at the DCH Regional Medical Center, and the 26-year-old man was treated there for a head injury that was not life-threatening, Baker said. “It appears there had been some altercation during an argument," Baker said. “During the altercation, there were guns drawn, and shots were fired, and this 32-year-old was struck during the altercation." The customers and employees who were at the barbershop when the shooting occurred weren’t injured, Baker said.


Worker dies after fall at E. Knox zinc mine

TN - Federal officials are investigating the death of a worker early Sunday morning at a zinc mine in East Knox County, according to a spokesman for the East Tennessee Zinc Company. The zinc company reported that an employee of Cowin & Company Inc., mining engineers and contractors, fell while working on a project to reactivate the Immel mine, which is located in Mascot. The victim’s name is being withheld pending notification of family. East Tennessee Zinc Company spokesman George Korda said the employee fell “approximately several minutes after midnight Sunday morning” and that he was working as part of a three-man team replacing structural steel in a main shaft.


Man Dies In Car Crash In Kittery

KITTERY, NH - 22-year-old Justin Harding of Dover, NH was killed early Saturday morning when he lost control of the car he was driving and crashed in Kittery. Police say Harding was driving a 2007 Mercedes Benz he had taken from Dreher-Holloway Mercedes Benz in Greenland, NH. They say he was an employee there, but not authorized to drive the vehicle. A New Hampshire State Liquor Enforcement Officer saw Harding in the car speeding through downtown Portsmouth, NH. The officer tried to stop the car, but it sped across the Memorial bridge into Kittery. The officer reportedly turned off his siren and emergency lights, but continued to follow the car.


2 News Helicopters Collide; 4 Dead, Debris Scattered Across Several Acres, Police Say

PHOENIX, AZ -- Two television station helicopters tracking a high-speed police pursuit in central Phoenix collided in midair and crashed Friday, killing everyone on both aircraft. # UNEDITED VIDEO: Moment Of Crash The helicopters collided over Steele Indian School Park while filming the pursuit for Channels 3 and 15. Three other news helicopters were also over the scene. "Wait, wait, wait, wait," Capt. Connie Sullivan reported from KPHO's News Hawk 5 around 12:40 p.m. "We've got a problem here. I've got a helicopter down."KNXV-TV identified the people aboard its chopper as reporter-pilot Craig Smith and photographer Rick Krolak. KTVK said pilot-reporter Scott Bowerbank was aboard its helicopter, along with photographer Jim Cox. # Images: Victims Of Helicopter Crash Sgt. Joel Tranter of the Phoenix Police Department told CBS 5 News no one on the ground was injured or killed. Television viewers did not see the crash since cameras aboard both aircraft were pointed at the ground. Video from the scene showed flames and two columns of thick black smoke rise from where both helicopters crashed. Firefighters and rescue personnel rushed to the scene.


Worker Struck And Killed On Parkway

OWENSBORO, Ky. (AP) - A road worker was killed after being stuck by sport utility vehicle while working along a western Kentucky parkway, officials said. Julio Cesar Hernandez, 22, of Hopkinsville was working with a mowing crew in the grassy median between the northbound and southbound lanes of the William H. Natcher Parkway in Daviess County when he was struck Tuesday morning, Deputy Coroner Darrell Day said. Hernandez died at the scene, Day said. Daviess County Sheriff's Department reports said Hernandez was hit by a red Chevrolet Blazer driven by Jessica Reyes, 19, of Troy, Mo. The reports said Reyes was driving north in the left-hand lane when she drifted into the median and struck Hernandez. Reyes was taken to Owensboro Medical Health System with minor injuries. Capt. Jeff Jones, chief deputy for the sheriff's department, said investigators were still trying to determine the cause of the accident. Keith Todd, public information officer for the state highway department's Madisonville office, said about 20 people have been killed in Kentucky during the past two years in accidents at road work sites.


Search crews find body of second missing sewer worker

St. Paul, Minnesota - The body of Joe Harlow, 34, of Plainview, was found about 4:15 p.m. near a spillway where a storm sewer line drains into the river, Ramsey County Sheriff's Cmdr. Joe Paget said. On Friday evening, crews found the body of Dave Yasis, 23, of Maplewood, in the river near the same spot. Harlow was married with four children. Paget said he hoped the discovery "helps this family put closure to a tragic incident." State workplace safety investigators are checking the training and evacuation procedures of the men's employer, the Lametti & Sons construction firm. Eight people were working as contractors on the city's $4 million sewer restoration project Thursday. They were about 150 feet below ground when they were told to evacuate as a line of thunderstorms approached.


Employee at contracting firm dies in accident

SCITUATE, RI - A worker from a Fall River-based contracting firm dies in a Rhode Island industrial accident. It happened this morning at a treatment plant in Scituate as the employee returned to the work site to dismantle equipment that was used to complete a project for the Providence Water Supply Board. Officials say he was killed when a crane-like device called a gantry collapsed on him. The man's name has not been released, but he worked for Eastern Piping and Engineering of Fall River. Representatives from the federal Occupational Safety and Health Administration are on scene investigating.


LCPD officer dies in motorcycle crash

LAS CRUCES, NM. - David Andrew Cordova, a 24 year old off duty Las Cruces Police officer, died after crashing his personal motorcycle into a four door car at the intersection of Lohman and Indian Hollow Road in Las Cruces. It happened at about 9:30 Thursday night. Officials say Cordova was traveling in a 2005 Yamaha east on Lohman Ave. when it struck a 2003 maroon Mercury that was exiting Indian Hollow toward westbound Lohman. Officials say Cordova attempted to break, but struck the side rear portion of the car. He died on the scene. He was not wearing a helmet. Joseph T. Muldrow, 64 year old driver of the car, was not injured. Las Cruces Police continue their investigation. No charges have been filed.


Man killed in accident at Nestle frozen foods plant

GAFFNEY, SC (AP) - A maintenance mechanic working on a crane in the freezer tower of the Nestle food plant in Gaffney has died in an accident at the plant. Cherokee County Coroner Dennis Fowler says James Redmon died Friday from head injuries when his body became trapped. A spokeswoman for the Nestle Prepared Foods Company says Redmon was repairing a piece of equipment when the accident occurred. Roz O'Hearn says the accident was an isolated incident. On July 16th, an ammonia leak at the plant led to the hospitalization of another employee. O'Hearn says the two accidents were unrelated. Fowler says his office and the Occupational Safety and Health Administration are investigating the death.


Worker killed in industrial accident at R.I. treatment plant

SCITUATE, R.I. - An employee at a contracting firm died Wednesday morning when a piece of heavy equipment collapsed on him at a water treatment plant. The worker was dismantling equipment that had been used to finish a project for the Providence Water Supply Board when the accident occurred. The utility said a gantry, a device used to help raise or lower objects, collapsed on him.

September 6, 2007: First Case of 'Popcorn Lung' in Consumer
For a number of years, workers at microwave popcorn manufacturing plants have been at known risk of developing the lung disease bronchiolitis obliterans (called "popcorn lung") due to on-the-job exposure to diacetyl, a chemical found in buttery flavoring additive. Today, ABC News reports the first known case of "popcorn lung" in a consumer who ate large quantities of microwave popcorn. According to ABC, Wayne Watson of Centennial, Colorado "ate about two bags daily, but now he has quit because of the havoc it reeked on his lungs."

September 5, 2007: Popcorn Makers to Remove Flavoring Chemical
A number of microwave popcorn manufacturers are making efforts to remove the chemical food additive diacetyl from their products. Diacetyl, which is found in butter flavoring mixtures used in microwave popcorn, may cause bronchiolitis obliterans, a lung disease. The New York Times reports that "the three companies that sell Orville Redenbacher, Act II, Pop Secret and Jolly Time microwave popcorn said they planned to change the recipes for their butter-flavored microwave popcorn to remove diacetyl."

October 28, 2005: Federal Judge Dismisses Popcorn Worker's Fraud Claims
A popcorn factory worker filed fraud and civil-conspiracy claims against several butter-flavor manufacturers, alleging that exposure to a chemical in the flavorings (diacetyl) caused respiratory injuries. The plaintiff also contended that the defendants concealed government research about the dangers of these flavorings. A federal judge dismissed the fraud claims because the plaintiff failed to allege facts that could rise to the level of fraud under the law.

November 23, 2004: Defendants Prevail in Popcorn Butter Case
Four employees at a popcorn processing plant in Jasper, Missouri filed suit against the manufacturers of an artificial buttering flavor used in popcorn. The plaintiffs claimed that they developed lung disease due to exposure to diacetyl, the chemical used in butter flavorings. The jury found in favor of the defendants, who argued that they could not have known about (or warned of the potential dangers caused by) diacetyl exposure.

GALVESTON, Texas -The United Steel Workers (USW) have partnered with IBCTV to recreate the BP Texas City explosion trial courtroom experience online and available to anyone 24/7 at www.ibctvlive.com.

For the first time in history the media and general public will be able to watch interactive trial highlights and review the documents and exhibits that will be woven directly into the video. To create this realistic experience, IBCTV will review the video stream daily and receive transcript and evidence directly from the courtroom at the end of each day. This will include video of the explosion overlaid with the actual 911 calls and BP radio transmissions from within the refinery.
The trial centers around four injury lawsuits alleging that on March 23, 2005, BP operated its Texas City, Texas refinery with gross negligence and violated federal, state, industry and even its own safety standards that led to the explosion. The explosion caused hundreds of injuries and 15 deaths. Though a large percentage of the seven million pages of documents obtained and released to the public by the plaintiffs’ attorney Brent Coon of Brent Coon & Associates, additional documents not approved for public viewing will be presented during the course of the trial.

Jay Jackson, president of IBCTV stated, “This is how people want to watch trials. Candidly, trial proceedings put people to sleep. It is our goal to make this experience as intriguing and fast paced as possible.”

Support The Laborstart Initiative

The campaign we launched a little more than a week ago in support of low-paid workers in care homes in north London has generated more support than any other campaign we have ever done.

Already, well over 8,100 messages have been sent to the employer. This is even bigger than our 2005 campaign in support of the Gate Gourmet workers, which was a much more widely publicized dispute in the mainstream media.

As we mentioned in this week's message, the employer reacted swiftly and brutally -- first threatening us with a libel action in the English courts and then sacking a union rep.

And then on Thursday, in an unprecedented move, the employer (Fremantle Trust) contacted our internet service provider and demanded that they shut down the campaign or else face a lawsuit themselves.

We were contacted by the legal department of the internet service provider and told that we had until noon on Friday to close down the campaign or else the entire LabourStart site would be shut down.

We worked very hard over those 24 hours to attempt to get our provider to back down, and had the full support of Unison (Britain's giant public sector union, whose members are at the center of the dispute) but were not successful in doing this before the noon deadline on Friday.

As a result, at 11:59 on Friday we were compelled to shut down the campaigns.

But -- we instantly revived the campaign in nine languages on a different server, in a different country, with a new name that reflects our feeling at this time.

The new site is called "We will not be silenced!" and is located, appropriately enough, at http://www.wewillnotbesilenced.org

If you have not yet sent off your message of protest to Fremantle, please do so from the new page.

Remember that you can use our system to send out your own message to the chief executive of Fremantle Trust -- and you can tell her, if you wish, what you think of this attempt to silence our campaign.

Please also inform all your lists and every trade unionist you know to use the new site to send a loud and clear message to this employer.

If Fremantle Trust were unhappy to receive 8,000 emails, how are they going to feel about getting thousands more in the next few days? Because that is exactly what is going to happen.

We will not be intimidated, we will not be bullied, and we will not be silenced. The campaign continues.

Ramsey man dies in accidental death

MN - On Tuesday, Aug. 21, Randall L. Puppe, 56, of Ramsey was cleaning a machine at TCR Engineered Components in Brooklyn Center, when he was caught in the machine and died, Police Lt. Scott Nadeau said. According to the Star Tribune, Nadeau said police and the state Occupational Safety and Health division are investigating. Puppe was pronounced dead at the scene shortly after 10 p.m. His accidental death was caused by mechanical strangulation and from blunt-force injuries, the Hennepin County medical examiner’s office said.


Worker dies in Greenfield plant accident

Greenfield, IN - A worker at an automotive supply plant was crushed to death early Monday when she became wedged inside a bolt-plating machine she apparently reached inside for a piece of paper, the coroner said. Suzanne Wyatt, 47, of Greenfield died in the 4:10 a.m. accident at Indiana Automotive Fasteners Inc., which makes bolts and other types of fasteners for Toyota, Ford, Honda and Takata Corp. Hancock County Coroner John Jester said Wyatt, who operated a zinc coater, apparently reached into the bolt-plating machine and was pinned when another piece of equipment came down on top of her with crushing force. "It's just a freak accident," he said.


Worker dies at fairgrounds

WELLINGTON, OH -- Jill Stafford, 39, of 120 Morton St., Jackson, was found deceased at 11:30 a.m. yesterday in a small trailer at the fairgrounds, according to Lorain County Coroner Dr. Paul Matus. She was an employee of Bates Amusement, the company which provides rides at the fair. Stafford was found by her daughter, who is also an employee of the company. Matus said the death is under investigation. He said there was no sign of foul play at the scene. He is awaiting the results of toxicology tests. Jackson is 70 miles south of Columbus.


City gas station worker fatally shot

MD - A gas station attendant was shot and killed in Baltimore early this morning, but few details are available about the man or his death. The man was killed at a Texaco gas station in the 2600 block of Gwynns Falls Parkway shortly after 2 a.m., police said. The owner of the gas station declined to comment this morning and did not release the name of the man who was killed.


Deli Clerk Shot, Killed During Robbery

PITTSBURGH -- A homicide suspect is still on the run while a $10,000 reward for his capture is offered. Jamal Mouzaffar, 28, was shot in the chest during a robbery at the A and E Deli Food Mart in Mt. Oliver on Sunday morning. A co-worker told Channel 11 Mouzaffar was the store owner's nephew. According to the Post-Gazette, the victim was just weeks away from returning to his native Syria, after undergoing medical treatment in Pittsburgh. One regular customer said she is sad tragedy struck such a kind family. Loretta Vertullo said, "Little kids didn't have enough change, you know, buying candy or gum and they would give them, let them go, say, ‘Don't worry about it.’ They were very nice people in there and for this to happen, it breaks my heart because they were very nice people." The robber fled the store around 9 a.m. and ran down Arlington Avenue. The $10,000 reward is being offered by an anonymous donor. Anyone with information is asked to contact police.


Man Dies From Deltic Timber Fire

Waldo, LA - Darrell Richards of Junction City was among three workers injured in the August 9th blaze. After the fire, Richards was taken to The Burn Center in Memphis. He died Friday. The other two workers, Andy Emerson of Taylor and Billy Pope of Springhill, Louisiana remain in serious condition at Louisiana State University Hospital in Shreveport. The fire broke out in the planer section of the mill where boards are cut to uniform sizes. The mill has 175 workers, and opened in 1973. Deltic Timber is based in El Dorado, and owns more than 400,000 acres of timberland in Arkansas and Louisiana.


ODNR officer dies in head on car crash

PORTAGE COUNTY, OH - The Ravenna Post of the Ohio State Patrol is investigating a fatal weekend crash that claimed the life of a Ohio Division of Natural Resources ranger. Troopers say 38-year-old Donavan Svab of Mantua died early Sunday morning after his car was hit head on by a driver who went left of center. Svab worked as a full time ranger for ODNR and also as a part time officer for the Garrettsville Police Department. A police spokesman says Svab was on his way home at the time of the crash. Troopers say Svab was driving north on Diagonal Road when a car going south went across the center line about a quarter mile north of Frost Road. The impact caused Svab's car to roll over and it ended up on its roof in a ditch. The other driver, identified as 22-year-old Christopher Jones of Kent, was trapped in the wreckage for an hour.



Bremerton, WA, public works employee dead

BREMERTON, Wash. - A public works employee in Bremerton has died after being run over by a vehicle on a municipal repaving project. Officials say 58-year-old Dean Westcott was with a street crew preparing for the work when he was struck, apparently by a government vehicle that was backing up. Police say Wednesday morning's accident is under investigation. Westcott was a 10-year municipal employee and a U.S. Navy veteran.


Security Guard Dies After Shot During Robbery

HOUSTON, TX -- A security guard died after he was shot during a robbery in southwest Houston, officials told KPRC Local 2. Houston police said the guard was shot in the leg at a business in the 6500 block of West Bellfort Street at about 5 a.m. Sunday. The gunmen entered the business, assaulted the guard and shot him before an employee was robbed at gunpoint, officials said. The guard was transported to Ben Taub Hospital, where he died. His identity was not released. Police said the gunmen got away with a large amount of cash. They were described as black and in their early 20s.


Auto shop employee dies in workplace accident

SUMMERSVILLE, WV — An auto shop worker has died after being pinned between a customer’s vehicle and a toolbox, police said. Another employee was being evaluated Thursday morning at Charleston Area Medical Center General Hospital. Police said Richard Nelson, 56, of Canvas, was getting ready to have his sport-utility vehicle inspected at Scotty’s Discount Tire and Muffler Shop Wednesday afternoon when his foot slipped off the brake. Nelson apparently hit the accelerator and the vehicle struck Kenneth Rogers, 36, and Harry Rogers, 54. Kenneth Rogers was pinned against the toolbox and killed. Harry Rogers was taken to the hospital.


Gunshot kills Overland Park convenience store clerk

MO - A man working at an Overland Park convenience store to support his family in Mexico was found dead near the business’ open cash register Tuesday morning. The 46-year-old clerk, whom police have not yet named, opened Latin-O Market at Metcalf Avenue and 105th Street about 9 a.m. At 11 a.m., a customer found him on the floor of the store and ran to the deli next door. “He said, ‘The guy working next door is on the ground. I think something happened to him,’ ” said Ricardo Calderon, manager of the deli next door. He said, ‘The guy working next door is on the ground. Calderon then went to check on the man he’s come to know in the past year. “There was lots of money all over the floor. He was just lying there. … I couldn’t find a pulse.” When officers responded they found the man on the ground, dead from a gunshot wound. Bar worker shot to death

TRENTON, NJ -- A dispute over a karaoke machine inside a bar in South Trenton led to a shooting yesterday that left a bar employee dead and a Philadelphia teenager in critical condition, police said. The city's 17th homicide of the year -- and third in five days -- occurred at 12:35 a.m. in front of the M&M Bar and Liquor Store at Centre and Federal streets. No arrests had been made as of yesterday. Police say they are looking for an unknown gunman. As city police braced for gang violence late Monday into early yesterday, stemming from a previous night of gang shootings that included the murder of an East Trenton gang leader, the call of "shots fired/man down" in the 400 block of Centre Street caused police on the streets to shudder, officers said. ut by later yesterday morning, city detectives said they believed the murder of Jose Almonte-Duran, 28, in front of the M&M was another senseless killing apparently unrelated to gangs. Almonte-Duran was working inside the bar -- detectives could not explain exactly what his job was -- and confronted then ejected a man who grabbed a karaoke microphone and started a ruckus, police Detective Lt. Stephen Varn said.


Construction worker collapses, dies; heat suspected

PORTLAND, Tenn. - Investigators believe extreme heat is partly to blame for the death of a construction worker who collapsed in a small room where he was hanging drywall. Charles Brown Jr., 29, was working in a small closet area with no ventilation, and the temperature was "horribly warm," Assistant Police Chief Richard Smith told The Tennessean. Temperatures across Tennessee have dipped slightly from weeks of triple digits, and it was not immediately clear how much the outside climate affected the stifling room where Brown was working. Brown's co-workers first thought he was sleeping when they found him on the floor Tuesday afternoon, but then called authorities when they realized he was suffering a health problem. "When we found him, he had both arms clutched over his chest," Smith said. "He probably had a heart attack related to the heat." Smith said Brown had unspecified medical conditions that may have contributed to his death. A toxicology screening was being conducted. Fifteen people in Tennessee, including 14 in Shelby County, have died of heat-related causes in August's scorching heat wave. At least three more, including Brown, are suspected to have died from heat-related causes.


Siemens looks at excessive blade speed in tower fall

Portland, OR - Preliminary findings seem to move suspicion away from a structural problem. Rapidly turning turbine blades might have contributed to the buckling of the massive wind tower that killed one worker and injured another at the Klondike III wind farm Saturday, officials of Siemens Power Generation said Tuesday. "An overspeed operation may have occurred while performing a sequence of procedures," said Melanie Forbrick, a spokeswoman for the German turbine maker. Siemens declined to provide further details about how the blades might have begun turning at excessive speeds or whether inspection procedures may have stressed the system enough to collapse the steel tower. If the preliminary findings hold, they would move attention away from possible flaws in the tower itself. Chadd Mitchell, 35, of Goldendale, Wash., a Siemens employee, was working at the top of the 231-foot tower when it buckled. The rotor and the nacelle, which houses the drive train and generator, crashed to the ground. Mitchell died in the fall.


Identity of construction worker killed at Malmstrom released

MT - The construction worker who was killed in an accident at Malmstrom Air Force Base has been identified as Kelly Carpenter, 24, from Walkerville. He was working as an Apprentice Operator for Garco Construction when he was crushed between the frame and tire of a crane. This is the second work-related fatal accident at a Garco Construction site in the last year. Garco President Tim Welsh says work at the site has stopped, probably for the rest of the week. "We have to be vigilant. We have to watch for pinch points, we have to observe block out, tag out procedures. We have to do all the things and we talk about that every week ."


Driver Killed in I-65 Truck Fire Identified

JOHNSON COUNTY, Ind. - Indiana State Police are investigating a deadly and fiery crash on the interstate. It happened just before 5:00 Wednesday evening on I-65 between Franklin and Edinburgh. According to State Police, the truck struck a guardrail and burst into flames. The driver of the truck was killed. He has been identified as 59-year-old Edward T. Zick of Fountain Inn, South Carolina. Flames could be seen shooting out of the cab and black smoked filled the air at the time of the accident. The fire caused police to close the southbound I-65 lanes between Indiana 44 and Indiana 252.



Marshfield man dies after being run over by truck at logging site

CENTERVILLE, Maine - A 64-year-old Marshfield man died Wednesday afternoon in an accident at a remote logging site. Kenneth Schoppee, an employee of Guptill Logging in East Machias, was run over by a maintenance truck at about 12:42 p.m. and suffered head and chest injuries, according to Sgt. Rodney Merritt of the Washington County Sheriff’s Department. Schoppee died at the scene of the accident, despite the efforts of co-workers to revive him. The accident remains under investigation, and Merritt said investigators are looking into the possibility that some outside issues may have prevented Schoppee from getting out of the way of the truck.


Police hunt robbers who gunned down Sunrise gas station clerk

SUNRISE, FL - Police are seeking clues in Thursday's shooting death of a convenience store clerk who recently started the night shift and was known by friends as a hard worker who regularly sent money home to relatives in Trinidad. Terrance Rakish Thomas, 24, died in a pool of blood behind the counter at a Mobil gas station, at 5990 W. Oakland Park Blvd., after he was shot several times, police said. A customer who walked in saw Thomas lying face down on the floor and called 911 at 12:51 a.m., said police Lt. Robert Voss. Police could not estimate how long Thomas had been on the floor before the customer entered. video


Worker dies in fall from Salem roof

SALEM, MA — A worker died Friday morning after falling from a roof on Lafayette Street. The employee of Olympic Paint and Roofing of Peabody died after he fell off the roof of 292 Lafayette St., the new home of the First Baptist Church of Salem. The accident was reported at 8:53 a.m. "It appears he was a 21-year-old male and he passed away," said Police Lt. Paul Lemelin. "He was one of the workers on the roof." A work crew was removing old slate tiles and replacing them with shingles, according to officials at the scene. A Medflight helicopter arrived to transport the accident victim to a Boston hospital, but it was not needed, police said. "He never made it to the Medflight," said Detective James Page.


Worker accidentally kills himself with power saw

NY — Rochester police are investigating an incident in which a utility worker accidentally killed himself with a power saw. According to Rochester Police Sgt. William Spath, the call to North Clinton Avenue and Hoeltzer Street was reported about 1:30 p.m. today. The man, who was a utility contractor, was cutting through a plastic pipe with a power saw. The saw kicked back and cut him in the neck. He was taken to Strong Memorial Hospital, where he was pronounced dead. Spath said police confirmed it was an accident and the incident has been reported to the Occupational Safety & Health Administration.


Policeman riding with Bush motorcade dies

ALBUQUERQUE, N.M. – President praises service of officer in Albuquerque crash. A Rio Rancho police officer died Monday after crashing his motorcycle while riding in a motorcade as President Bush prepared to leave the city following a fundraiser for Sen. Pete Domenici. Officer Germaine Casey, 40, was rushed to an Albuquerque hospital, where he was pronounced dead. The motorcycle was near the front of the motorcade, and the president saw the wreck as he drove past on the way to the airport, said Officer John Francis, a Rio Rancho police spokesman.


L&I investigates own worker's death

Fl - The Department of Labor and Industries, which investigates the deaths of workers in other companies and agencies, is now looking into the death of one of its own. Bill Harmon of Lakewood, an office assistant in the Employer Services division, was injured in the first-floor stairwell of the agency's headquarters last week, according to the department. He was taken to Providence St. Peter Hospital in Olympia, where he died. Harmon was 87. "Although there were no witnesses to this unfortunate situation, L&I employees heard the sounds of a fall and responded immediately. They called 9-1-1, notified building security, and took action to block the area. In addition, a number of L&I staff went to the hospital to show their support for the family," said department director Judy Schurke in a message to staff.


Simplot Worker Dies After Accident at Pocatello Plant

The worker at the Simplot Plant in Pocatello who was burned in a sulfur accident has died. Frank Rowberry died Wednesday night at 8:00 P.M. at the University of Utah Burn Center. Rowberry was working alone in one of the sulfur unloading areas when somehow, the sulfur ignited his clothes. The 35 year employee was burned over 50 percent of his body. Safety inspectors spent Wednesday on the scene investigating the accident. They are meeting today to discuss their findings. The immediate area where Rowberry was burned is closed off, but it is not affecting the plant's day to day operations.




Truck crash kills driver, shuts down I-65


IN - The driver of a box truck was killed Wednesday when the vehicle crashed on I-65 in Johnson County. The crash was reported at 4:48 p.m. Wednesday at the 85-mile marker on I-65, midway between Franklin and Whiteland. According to Indiana State Police, the southbound truck crashed into a guardrail and burned, killing the driver. The interstate was closed in both directions after the crash and did not completely reopen until nearly 9 p.m. Troopers still were trying to determine the cause of the crash, and the name of the person killed was not immediately released.


Worker dies after falling from Valley Road condo tower

NJ - A 26-year-old roofer working atop 530 Valley Road fell off the seven-story building and died on Thursday morning, Aug. 30, police said. Victor Coelho of Roselle Park was with three other workers finishing the edges of the roof on the Bellaire House condominiums, near the corner of Valley Road and Preston Place, when he fell, according to police. “This is a tragic and unfortunate incident for the Coelho family and loved ones,” stated Montclair Police Lt. James Carlucci. An autopsy was performed later that afternoon, but police did not immediately release the results. Montclair authorities called the Essex County Prosecutor’s Office at 8:50 a.m. to say Coelho’s death appeared to be accidental, and they would not be needing assistance with the investigation, said Paul Loriquet, a spokesman for the prosecutor.


Man arrested in Mich. indicted in Ohio trucking company killings

BOWLING GREEN, Ohio — A man accused of killing two people at a trucking company has been indicted on two counts of aggravated murder. If convicted, 43-year-old Calvin Neyland Jr. could face the death penalty or life in prison with no chance for parole. His indictment came Wednesday from a Wood County grand jury in Bowling Green. Neyland is accused of gunning down Liberty Transportation manager Doug Smith and Thomas Lazar, an employee of the company brought in from Pennsylvania to help fire Neyland Aug. 7. Neyland was arrested about three hours after the shooting in a motel parking lot in Monroe County, Mich., about 15 miles north of the trucking office in Perrysburg Township.


One Dead and Two Wounded in Bronx Shooting

NY - A man who was fired from his job as a janitor in Co-op City in the Bronx and lost a lawsuit to get his job back returned to the housing complex this morning with a .38-caliber revolver and shot three former colleagues, one of them fatally, according to the police.The police said the gunman, Paulino Valenzuela, 50, then boarded a public bus, went to the Bronx Criminal Court building and turned himself in to the authorities. Mr. Valenzuela, of 120 Casals Place in the Bronx, was fired from the RiverBay Corporation, the management company that runs Co-op City, in 2005. Last year, he filed a federal lawsuit to get his job back. The case was dismissed on Friday. Before he was fired, Mr. Valenzuela had been suspended three times since 2000 for threatening co-workers. According to the Police Department, Mr. Valenzuela fatally shot a RiverBay supervisor, Audley Bent, 49, of Rosedale, Queens, in the basement of 2440 Hunter Avenue, at 7:50 a.m.


Man killed by lightning bolt at Motiva dock

PORT ARTHUR, TX — A 25-year-old Mississippi man was killed Wednesday morning after being struck by a bolt of lightning while aboard a vessel at the Motiva Port Arthur dock. The young man is identified as Brandon Jenkins. Walter Blessy, owner of Blessy Marine, a contracting company with Motiva, said the Jenkins was working on the vessel Charles Clark when the lightning struck him around 11 a.m. Jenkins transported by ambulance to Christus St. Mary Hospital where he was pronounced dead. Blessy said Jenkins is divorced and has two small children. He had only been employed with Blessy Marine for a short time as a tankerman trainee.


Crown Point man killed in work accident

EAST CHICAGO, IL A Crown Point man was killed in a rail yard accident early Saturday morning when he was struck by a train. Henry Salinas, 43, of 8172 Ralston Court, Crown Point, a railroad employee, died of blunt force trauma to the torso when he was struck by a moving rail car, according to the Lake County coroner's office. He was pronounced dead at the scene at 7:12 a.m. at 3828 Michigan Ave., East Chicago.


Worker dies in gravel pit fall

UNION TWP. – Police in this Clermont County township are investigating after a worker fell into a gravel pit and died this afternoon. Around 1 p.m., a caller told police the man fell and was unconscious at Evans Gravel Co. on U.S. 50. When officers and firefighters arrived, the man (Pat Kelley, 41) was pronounced dead at the scene. The man, who is either an employee for Evans or a subcontractor, is believed to have been working on pipes above the gravel pit when he fell. No foul play is suspected and the incident is believed to be accidental, police said. His name has not been released pending notification of family.


Longtime resident Bill Araujo killed in industrial accident

LYNWOOD, CA — The city this week lost a longtime resident to an industrial accident. William Araujo, a two-time Planning Commissioner died on Sunday. He was 63. A resident of Lynwood since 1977, Araujo was an advocate for education and almost realized his lifelong dream of opening his own business with his wife’s Tortilleria del Pueblo on Atlantic Boulevard. That business was scheduled to open next month, or very soon, said his daughter, Deborah Garcia. While he has operated a smaller version of his business at Plaza Mexico for the past year, opening up his own business in his own location, was something he’s been working toward all his life, Garcia said in an interview on Wednesday afternoon. “It was something he got from his dad, from my grandfather, his dream was my grandfather’s dream, too,” she said.


Trucker killed in Adair County crash

STILWELL, OK — An Arkansas truck driver was killed when his tractor-trailer rig slammed into the back of another truck and overturned on an Adair County road. David K. Humphrey, 40, of DeQueen, Ark., died at the scene of the crash shortly before 8 p.m. Tuesday, about 14 miles southwest of Stilwell, the Oklahoma Highway Patrol said. Troopers said Humphrey's truck was behind one driven by Robert N. Larue, 47, of Muse as the vehicles were going down a long, steep hill loaded with gravel. As they approached a "T” intersection, Larue slowed to make a right-hand turn and Humphrey's truck struck the back of his vehicle.


Flint police mourn officer killed in crash

FLINT, MI -- Vincent D'Anna spent four years putting lawn mowers together before his dream came true -- the dream of becoming a police officer in a busy city. D'Anna, 26, of Goodrich realized that goal last year when he pinned on a Flint Police Department badge and went to work patrolling the streets. Now, his fellow officers are mourning his death. D'Anna died after his motorcycle was struck from behind by a Camaro on Sashabaw Road outside the DTE Energy Music Theater in Independence Township about 8 p.m. Sunday. The off-duty officer was pinned under the car and dragged several feet before the driver allegedly jumped out and tried to run from the scene.


Worker at Eastern Oregon wind farm killed

OR — A wind turbine tower crashed to the ground at a wind farm east of The Dalles, killing one worker and injuring another, Sherman County authorities said. Sheriff’s Deputy Geremy Shull said the collapse occurred Saturday afternoon. He declined to release the names of the workers, but said the man who died was from Goldendale, Wash. The injured worker was in serious condition at a hospital in The Dalles, Shull said. A third worker was not hurt. Shull said the cause of the collapse had not been determined. Officials from the Occupational Safety and Health Administration were investigating Sunday. Portland-based PPM Energy owns the wind farm, but Siemens manufactures and owns the wind turbine tower that collapsed. Melanie Forbrick, a Siemens spokeswoman based in Orlando, Fla., said the turbine had been in operation for 500 hours and the workers were doing a routine inspection. She said the worker who died was at the top of the tower when it collapsed. She the injured man was inside it. The worker who escaped injury was at the base. Araujo died in an industrial accident at the Firestone plant where he has worked for more than 20 years. Prior to his job there, he was in the food industry, Garcia said.


Cab driver killed in attack

NY - Cabbie may be latest victim in a rash of robberies in city’s North End. A rash of armed robberies in the city’s North End may have left a Falls cab driver dead and sent shockwaves through the Cataract City’s cabbie community. Thomas Zacher, 55, a driver for Blue Taxi company, died Monday afternoon at the Erie County Medical Center after being shot in the back of the head early Monday morning in the 1800 block of Linwood Avenue.


Farmer dies in grain bin accident

BARGERSVILLE, Ind. - An 80-year-old Johnson County farmer died when he apparently fell into a grain bin filled with corn. Fire officials say

fell while working on the bin with his son. He was dead when rescuers reached him after an hour of digging through the grain. Rescuers cut open the walls of the bin so they could reach Henderson. About 8,000 bushels of corn were taken out of the bin. The incident was reported about 9:45 a.m. on the farm near Bargersville, about 5 miles south of Indianapolis.

Second Man Dies From Injuries in Work Site Hit-and-Run

BURTONSVILLE, Md. -- A second construction worker has died from his injuries after a van slammed into five construction workers on Route 29. Montgomery County Police say 37-year-old James Cronin of Glen Burnie died Tuesday. On Monday, Cronin was flown to the University of Maryland Shock Trauma Center in Baltimore, in critical condition with life-threatening injuries. Police say the driver of the van, 31-year-old Manuel De Jesus Gonzalez-Geronimo, turned himself in at around 12:30 a.m. Tuesday. Gonzalez-Geronimo, of Hyattsville, was charged with failure to immediately return and remain at the scene of a collision involving death, failure to immediately stop a vehicle at the scene of a collision involving bodily injury, and driving without a license. He is being held without bond at the Montgomery County Detention Center. Police say Gonzalez-Geronimo could face additional charges. Prosecutors say Gonzalez-Geronimo has also admitted to being in the country illegally for seven years. He is originally from Guatemala. Another construction worker, 30-year-old Martin Ruffin of Baltimore, was killed and three other men injured when the van plowed through a construction site. The accident occurred around 1 p.m. Monday when five construction workers were taking a break and sitting on a guardrail on northbound Route 29 -- north of Route 198 just before the Howard County line.


Police ID contractor killed in accident at Scituate Reservoir

SCITUATE, R.I. -- Police identify a worker killed at a water treatment plant in Scituate. He's 37-year-old Roger G. Coe, of Fall River, Massachusetts. Coe was injured when a piece of heavy equipment collapsed on him. He was taken to Kent Hospital, where he was pronounced dead. Police Captain David Randall says an autopsy is being done today. Scituate police say the man worked for Fall River-based Eastern Piping and Engineering, which was doing contract work for the Providence Water Supply Board. The federal Occupational Safety and Health Administration is investigating the accident.



Worker crushed to death in York, Machinery falls during repairs at Beckenbach USA plant

YORK -- A Spartanburg man was killed Monday when a piece of machinery fell on top of him at a York auto parts factory, authorities said. Around 11:30 a.m., Wade Scharff, 54, was repairing a piece of equipment at Beckenbach USA, in the East York Industrial Park, when a large, metal disc fell on top of him, said Interim York County Coroner Sabrina Gast. She said Scharff was trapped underneath the disc and died from injuries before help arrived. "He was pinned underneath the machine," Gast said. The official cause of death was multiple crush wounds, she said.


Gary officer dies after police cars collide in chase

IN - A 28-year-old Gary, Ind., police officer died Sunday after he crashed into another police car during a chase. Patrolman Benjamin Wilcher was chasing a suspect late Saturday when his vehicle collided with one driven by Patrolman Arthur Lemme at 35th Avenue and Virginia Street in Gary. Wilcher and Lemme were taken to the Methodist Hospitals in Gary before Wilcher was flown to St. James Hospital in Olympia Fields, Ill. Wilcher did not survive the surgery to repair his broken bones and his internal injuries, the Cook County medical examiner's office said. Lemme remained at Methodist on Sunday evening in stable condition. The suspect being chased by Wilcher and Lemme was still at large, said Gary Lt. Del Stout. Wilcher, who lived in Gary with his wife and two children, joined the department two years ago, Stout said.


Accident blamed for mason's death

PA - A mason from Perry County was the victim of "an unfortunate industrial accident" last week when a scaffolding blew over and hit him on the head, killing him, Lower Swatara Twp. police said. Dwaine D. Lesher, 42, of Elliottsburg, was working for Kepler Masonry, a subcontractor working on a 45,000- square-foot expansion of the Phoenix Contact plant on Fulling Mill Road. Kepler masons were erecting scaffolding at the site when the wind picked up, toppling one of the scaffolds about 1 p.m. Wednesday, according to Lower Swatara Twp. police. David Kepler, the owner of the masonry company, told police Lesher wasn't able to get out of the way quickly enough.


Landscaper killed by lightning

FL - The storm clouds looked like they were miles in the distance as four landscapers worked Monday afternoon outside a Parkland home. Then, lightning seemed to come out of nowhere, killing one worker and making him the fifth person to die from a lightning strike in South Florida so far this year. Domingo Andres Francisco, 24, was operating a weed trimmer outside the house, in the 7100 block of Northwest 68th Drive, when the lightning bolt struck him at about 2:15 p.m., according to the Broward Sheriff's Office. Andres, of North Lauderdale, went into cardiac arrest while his co-workers rushed to tell the homeowner, who dialed 911. He was pronounced dead less than an hour later at North Broward Medical Center in Deerfield Beach. ''It probably killed him instantly,'' said Mike Jachles, a BSO spokesman. The investigation continues, while BSO waits for the results of an autopsy. After the accident, one of Andres' friends and co-workers, Francisco Cosdra, called his employer with the news. ''Francisco called me and told me there was a big problem,'' said David Beretsky, a co-owner of the landscaping company, AmeriLawn & Landscaping in Wellington. ``I got off the phone with him and called 911.''


Police find dead farmer and his tractor in river

CROWS LANDING, CA — An 84-year-old man died Monday near Moran and East Marshall roads while driving his tractor alongside the San Joaquin River. Paul J. Bernard told his wife around 10 a.m. that he was going to work on the farm, said deputy Royjindar Singh, a spokesman for the Stanislaus County Sheriff's Department. Bernard was on a dirt road between his corn field and the river when the tractor plunged into the water. Sheriff's deputies received the call about the accident around 12:40 p.m. "He was a good man," Jesús Màrquez of Newman said in Spanish. Màrquez worked for Bernard, helping to irrigate his land, which Màrquez estimated spanned around 105 acres. It was too soon to determine the cause of Bernard's death, said Katie Soto, a Stanislaus County deputy coroner.


Bedford driver killed in wreck, ‘Richie’ Feathers described as avid outdoorsman.

HOBBIEVILLE, IN — A family man who spent a lot of time outdoors has died from injuries he suffered in a traffic accident Sunday. Richard E. “Richie” Feathers, 37, of Kentucky Hollow Road, died after his sport utility vehicle struck a tree in Greene County. “He loved hunting and was an outdoors- man,” said his stepmother, Ina Feathers. “He was a very kind, very polite young fellow.” Feathers graduated from Bedford North Lawrence High School in 1987. He was a laborer with the Laborers’ International Union of North America Local No. 741. “He was a very good worker, and everybody said he was a real hard worker,” said union field representative Bobby Minton. Feathers joined the union in 2005 and had worked for Reynolds Inc. and Midwest Underground. “He worked in the collector well division,” said Reynolds Superintendent Bud Landstrom. “He was very good at it.” Feathers worked with Reynolds about two years.


Lacey worker, 53, killed in accident at construction site

WA - A Lacey worker died Tuesday morning when he was struck with a bucket attached to an excavator at an outdoor construction site on the 4400 block of 163rd Avenue in the Tenino Fire District. Thurston County Coroner Gary Warnock identified the man as Glen Davis, 53. Tenino Fire Capt. James Fowler said the industrial accident occurred about 8:15 a.m. Tuesday. An unidentified worker was operating the excavator, Fowler said. The state Department of Labor & Industries is investigating.


Teen killed in forklift accident

FL - A 16-year-old boy died Friday after being pinned by a forklift, according to West Valley City police. The teen's parents have been notified, but his name was not released as the investigation is ongoing, said Lt. Matt Elson. The boy was killed after the forklift he was operating tipped over, pinning him to the ground at 3:19 p.m., Elson said. He was attempting to move rolled up sheets of aluminum at Target Flexo, at 2203 W. Alexander St. (2420 South). "Apparently, he has worked there for several years," Elson said, noting that several of the boy's family members also work at the business. Police are investigating what caused the accident and whether the teen should have been operating the forklift. The teen died before police arrived on scene, Elson said. His body was taken by the Office of the Medical Examiner.


11-year-old killed in tractor accident

GARNETTA Kansas - Boy, 11, killed in tractor accident. Services were held Tuesday for an 11-year-old rural Garnett boy who died in a tractor accident. Anthony Adams, who lived with his grandparents, was driving an open-cab tractor at their residence about 1:30 p.m. Friday when he apparently jumped out, said Anderson County Sheriff Scot Brownrigg,. He got in the way of the rear tire and suffered massive trauma.


Man Killed While Doing Landscaping Work

OH - A man is killed while doing landscaping work in Ft. Thomas. A car struck Antonio Rodriguez-Ramirez, 54, Monday afternoon on Route 8 near Lester Lane. He was picking up trash in preparation for mowing in the area Tuesday. The driver that struck Rodriguez-Ramirez did stop. No charges have been filed. There was a double fatal accident at the same spot in 1996. Two teenagers were killed. A cross in their memory can be seen at the crash site.


ET Technology Park employee collapses, dies

OAK RIDGE — A 46-year-old employee at the East Tennessee Technology Park died today at the federal work site, officials confirmed. Mike Hughes, the president of Bechtel Jacobs Co., the governments’ cleanup manager at Oak Ridge, said he couldn’t provide details, but he said the man collapsed soon after reporting to work this morning. “It was nothing work-related,” Hughes said. “He worked in an air-conditioned trailer.” The man was an employee of Safety and Ecology Corp., a Knoxville-based company that provides radiological support services to Bechtel Jacobs, Hughes said. Hughes said on-site paramedics responded to the scene within two minutes, but were unable to revive the individual. The man was taken to Methodist Medical Center of Oak Ridge, Hughes said.


OSHA investigating fatal industrial accident

STATESVILLE, N.C. -- A man died Wednesday morning in what police are calling a manufacturing accident. Police were called to R.P. Machine Enterprises Inc. in Statesville shortly after 8 a.m. where Leonard Key, 62, was pronounced dead. Investigators said Key had been working at a lathe which was spinning a metal rod at high speed when that rod came loose and struck Key in the chest.


Worker crushed to death at metal fabrication shop

GASTONIA, N.C. - Authorities say a worker was crushed to death today at a metal fabrication shop in Gastonia. They say 23-year-old Cudiel Ambrocio VelazQuez of Bessemer City died at Porters Fabrication after being crushed by a piece of machinery. Gastonia Police Sergeant Mike Lari says the machine was a stacker that raises and lowers materials. Lari says VelazQuez was under it when it came down. It happened just before 6 a.m. this morning. VelazQuez was originally from Mexico, where his parents still reside.



Hillsborough deputy shot and killed in Brandon

BRANDON, FL – A Hillsborough County deputy is dead after he was shot inside his cruiser early Wednesday morning in Brandon, and investigators say the 22-year-old man suspected of killing him is also dead. Investigators say the shooting happened at about 1:30 a.m. near the intersection of Lumsden Road and Kings Avenue. They say Sergeant Ronald Harrison, 55, was in traffic inside his unmarked cruiser headed eastbound on Lumsden Road. As he made a turn onto north Kings Avenue, witnesses say they heard shots fired.


Construction worker is killed in fall

SAYREVILLE, NJ — A construction worker died when he fell from the Alfred E. Driscoll Bridge on Saturday morning. Thomas W. Oset, 45, of Manville was walking along a temporary walkway made of boards at about 8:30 a.m. when one of the boards broke, causing him to fall about 96 feet to the ground below, State Police said. Oset struck a concrete barrier on the ground below and was pronounced dead at the scene at 8:40 a.m. Workers had been resurfacing the Driscoll Bridge, said Joe Orlando, a spokesman for the New Jersey Turnpike Authority.


Subcontractor Violated Safety Measures

HI - In March, a man fell more than 40 stories to his death. Now the state says subcontractor Permasteelisa will have to pay. Frank Montayre, 52, fell from a construction site on Kapiolani Boulevard. He was caulking a lanai on the 46th floor of the Moana Pacific Condominium, but he was not wearing a safety belt. The state investigated and found several violations. Several were related to fall protection, which covers a variety of measures. "Did it have a mid-railing, did it have side paneling to protect any employee from going over the edge, or through the openings within the lanai, or did they have the fall protection equipment? In this case it was lacking," said James Hardway, Occupational Safety and Health. Permasteelisa is a global design and construction company operating on four continents.


Prosser farmer dies in tractor accident

BENTON CITY, WA -- A longtime Prosser farmer died Wednesday when his tractor rolled on top of him while he was mowing in a vineyard near Benton City. Robert Smith, 57, got stuck in powdery dirt while turning a corner at the end of a row, authorities said. He got off the small tractor to put rocks under the tires for traction, but the tractor slipped and rolled on top of him. It is unknown whether he got back on the tractor before it rolled, or if he was working underneath when it tipped, said Benton County Sheriff's Deputy Joe Lusignan. Smith went out to mow about 10:30 a.m. and said he would return around noon, Lusignan said. Orchard manager Juvencio Garcia found Smith pinned under the tractor about 12:35 p.m. There were no witnesses to the accident, Lusignan said. Smith was pronounced dead at the scene by medical personnel, Lusignan said. The Benton County Coroner's Office will conduct an autopsy to confirm the cause of death. Smith co-owned the 170-acre S & R Smith Bros. orchard with his brother, Steve Smith.


Truck driver killed in accident on I-435 in Wyandotte County

IA - An Iowa tractor-trailer driver died Wednesday in a crash on northbound Interstate 435 in western Wyandotte County. The accident happened about 11:30 a.m. when a car struck the truck over the Metropolitan Avenue bridge, according to a Kansas Highway Patrol report. The collision sent the car and the tractor-trailer into the interstate bridge rail. The truck cab flipped over the rail and plunged into the concrete interstate embankment, igniting an intense fire that destroyed the vehicle. The rig’s trailer flipped and skidded about 100 feet and landed in the north lanes of I-435. The truck driver, Richard Anglen, 40, of Silver City, Iowa, was ejected during the crash. Anglen was airlifted to a hospital, where he was pronounced dead. The car, driven by John K. Nichols, 17, of Johnson County, bounced off Anglen’s vehicle and came to rest in a ditch. Troopers said Nichols suffered minor injuries, but a passenger in his car was not injured.


Two confirmed dead in fiery crash

GRAND JUNCTION, Colo. — Two people were killed and an 11-mile section of Interstate 70 was closed after a semi-truck carrying chemicals crashed and exploded Wednesday, crippling an overpass in this western Colorado city. The Mesa County coroner's office said two bodies were recovered but declined to say whether they were found in the wreckage. The victims' names were not immediately released. One eastbound lane of I-70 was reopened by 12:30 p.m., nearly 10 hours after the crash. Other traffic was being rerouted on side roads. About 19,000 vehicles a day use the interstate in Grand Junction, 200 miles west of Denver. The truck was carrying 55-gallon drums of sodium hypochlorite, a form of bleach, along with commercial cleaning liquids and industrial hardware, police said.


Police find man dead in airport parking lot

CLEVELAND, OH - Police say a 19-year-old man was found dead tonight in a car at an employee parking lot near Cleveland Hopkins International Airport. Cleveland police Sergeant Daniel Fay says the man was slumped in the driver's seat of the car. Two pilots walking through the parking lot called police about 6 p.m. after noticing the man. Police and paramedics later found him dead. Fay says police saw injuries that could have led to his death but the cause of death is unclear. He says the dead man appears to be an airport employee. Police have not released the man's name because his family has not yet been notified.


Worker identified in fatality

KY - The Henderson County coroner's office has identified the man killed in Friday's construction accident as 52-year-old Larry E. Kincade of Smith Mills, Ky. Kincade died in the accident that occurred shortly after 2 p.m. near the Ohio River at the former Riverside Downs harness racetrack. Coroner Bruce Farmer said four men employed by M. Bowling Marine Co. were trying to remove a section of boom from a crane when two of the men were crushed underneath the part being disassembled. The second man was critically injured and transported to St. Mary's Medical Center in Evansville for treatment, Farmer said. Further information about his identity and condition were unavailable Saturday.


Man killed at SPI construction site

TX - A construction site accident on South Padre Island claimed the life of a Mercedes man Friday morning after a steel panel fell on him. Ishmael Garza, 20, was killed after a steel panel fell upon him and another worker at the Sapphire Condominiums high-rise development in the 300 block of Padre Boulevard, Justice of the Peace Bennie Ochoa III said. The accident was reported to South Padre Island police dispatch via a 911 call at 6:59 a.m., said SPI spokesman Jason Moody. Upon arrival, emergency personnel found two men on the ground in front of the construction site. A 20-year-old was beneath a steel panel, police said. They have not identified him as Garza. The other man, Garza’s brother, name unknown, was not injured.


Blaze in vacant New York skyscraper kills two firefighters

NEW YORK -- A multiple-alarm fire broke out Saturday in the vacant Deutsche Bank skyscraper at ground zero, killing two firefighters and injuring at least five others in the relic of the Sept. 11 attacks that was being dismantled. The firefighters that were hospitalized had smoke-related injuries that were not believed to be life threatening. They were among hundreds answering the multiple-alarm, high-rise fire as 87 fire units poured into ground zero all afternoon. The fire was still not under control at least seven hours after it began. The Deutsche Bank building had stood as a ghost since parts of the south tower of the World Trade Center came crashing down into it nearly six years ago, leaving it severely damaged and filled with toxic debris, including asbestos, dioxin, lead and chromium... One was identified as Joseph Graffagnino, 34, of Brooklyn, who had been with the force 8 years. The second firefighter was not identified as Robert Beddia, 53, of Staten Island. An official said he had been with the fire department 23 years and was the senior firefighter on the scene.

Police Investigate Fatal Construction Accident In Queens, State DOT Worker Killed, 3 Others Injured

NEW YORK -- Police are investigating an accident in Queens that killed a State Department of Transportation worker and injured three others. Police said two DOT workers were towing a construction light tower when it dislodged from their pick-up truck at around 9:20 p.m. on the westbound Grand Central Parkway near exit 22 in Hollis Hills. The workers were re-attaching the tower when another car hit them. One worker died at the scene. The other was taken to the hospital in serious condition. The two women in the car that hit the tower were taken to the hospital with minor injuries.


"Fair Employee's Death Likely Due To Heat"

TN - Just a day before it starts, the Wilson County Fair is dealing with a death. Workers believe a fellow employee lost his life while working in the extreme heat. It’s thought to be the first in the Mid-State and the first in the history of the Wilson County Fair. A 48-year-old man died while setting up a ride called the Scooter. “It’s horrible,” said fair employee Rob Vivona. “We are a family. We take care of everyone. It’s unfortunate.” The manager of the amusement company, Hale Moss, said they were bringing ice and water to the employees, but others said it’s just too hard to tell if you are getting overheated. “It just gets up on you, quicker than you expect,” Moss said. Most of the heavy work on the fairgrounds was finished by Wednesday afternoon, but employees were able to work during the night, when it was cooler.


Ramsey man killed in industrial accident

NY - A 56-year-old Ramsey man was killed on the job Tuesday, Aug. 14. Randall L. Puppe, 15500 block of Sodium Way, was caught in a chip spinner while cleaning the industrial machinery at TCR Corporation, 1600 67th Ave., Brooklyn Center.When Brooklyn Center police officers arrived at the scene, Puppe was in cardiac arrest and they were not able to revive him, said Lt. Scott Nadeau. The Hennepin County Medical Examiner’s Office has ruled Puppe’s death an accident, caused by mechanical asphyxia and multiple blunt-force injuries. Brooklyn Center police and Occupational Safety and Health Administration are still investigating the accident. According to its Web site, TCR Corporation manufactures metal components for the automotive, industrial, commercial, hydraulic and recreation industries.


Ohio worker killed in industrial trash compactor

COLUMBUS, OH - A warehouse worker was crushed to death in an industrial trash compactor, authorities said. Police said co-workers heard Gilberto Graza shouting, but no one witnessed Tuesday's accident. It was unclear how Graza, 37, got into the machine, used on an outdoor loading dock to pound down cardboard going into a large recycling container. Graza was pronounced dead a short time after medics arrived at the distribution warehouse operated by Sears Logistics Services, part of retailer Sears Holdings Corp. Police closed their investigation, satisfied the death was accidental, police spokeswoman Amanda Ford said Wednesday. The federal Occupational Safety and Health Administration is interviewing warehouse managers and employees in hopes of determining how it occurred, said Deborah Zubaty, OSHA area director. Sears said it was not able to comment on the investigation. "It was an unfortunate incident, and our thoughts and prayers are with associate's family," said Kim Freely, a company spokeswoman.


St. Louis Police Officer Dies After Being Shot in North St. Louis

MO - A massive manhunt is on as police search for the shooters who killed one of their own. Black bunting hangs over the St. Louis Police Department, marking the loss. It's been three years since a St. Louis police officer was killed in the line of duty. Police sources tell FOX 2 after Officer Norvelle Brown, 22, was shot at least once, he got on his police radio and began calling for help, “officer in need of aid.” Apparently one of the suspects shot Brown again in the face. Officer Brown was shot overnight while on routine patrol on Semple between MLK and Coate Brilliante in North St. Louis. He was alone in the car at the time.


Construction Worker Dies In Accident

GRESHAM, Ore. - A construction worker died Friday after 30-foot deep hole caved in and the 1,600-pound pipe he was helping position crushed him. Officials say 29-year-old Edward Manley was working on a subdivision construction site in Gresham just south of Division Street. Gresham Fire and Emergency Services Jim Klum says Manley was in a 15-foot-by-15-foot hole when it collapsed. The force of the soil hit the man, knocking him over. Hundreds of pounds of clay pinned his legs. Gresham's technical rescue team used picks to chop the clay away to rescue the man. Unstable soil also made the rescue particularly dangerous for firefighters. Klum says Manley sustained serious head injuries. He died Friday morning at Oregon Health and Science University. The Occupational Safety and Health Administration is investigating.


St. Louis Police Officer Dies After Being Shot in North St. Louis

A St. Louis police officer was murdered Wednesday evening after being shot during a shootout in the 16-hundred block of Semple, near Martin Luther King Drive, in North St. Louis. The officer was shot while on routine patrol. He was 22 years old, and had been on the force for a year. St. Louis Police Chief Joe Mokwa confirmed the officer died at Barnes Jewish Hospital. The chief reported the officer was wearing body armor, and was shot in the chest. The bullet may have entered through the side of the bullet proof vest.


OSHA fines Wisconsin company following accident that crushed worker

ROTHSCHILD, Wis. — The company where a 50-year-old worker died after being crushed by a large piece of steel last month has been fined $3,500 by the federal Occupational Safety and Health Administration for a workplace safety violation. Imperial Industries didn't provide a guard mechanism to protect workers from dangerous moving machine parts, the agency said in the citation. The company makes steel tanks. Ken Stankey of Mosinee was killed July 24 when a large steel tank cylinder fell on him.


State investigating workplace death

MN - State workplace safety investigators are trying to determine how an experienced employee got trapped in a machine he regularly cleaned, killing him Tuesday night. Randall Lee Puppe, 56, of Ramsey, was found dead by co-workers at TCR Engineered Components in Brooklyn Center shortly before 10 p.m. "We don't know how this happened. We're very puzzled by it," said Mike Rott, president of the company that makes metal components, particularly specialized fasteners. "It's tragic. I don't know how else to say it." The Minnesota Department of Labor and Industry, which conducts workplace safety inspections in the state, had sent inspectors, said spokesman James Honerman. "We've opened an investigation and we're looking into it," Honerman said State records show the business was last inspected in 2002. The company was cited for 12 safety violations, 11 of which were considered serious. All the problems were quickly resolved.


Second man dies in Clear Lake-store roof collapse

TX - Rain pinged on the roof like falling nails and thunder exploded in a sky that had become almost dark as night. Customers of a Clear Lake strip shopping center Thursday darted for their cars or hunkered beneath overhangs, marveling that a storm that supposedly was losing its punch could be so fierce. Then, without warning, the roof of a supermarket's loading dock/storage area collapsed, crushing one worker to death, a second man died later at Ben Taub General Hospital.


Seasonal employee dies in park wreck

WY - An employee of Yellowstone National Park’s concessions operator died Wednesday when his vehicle rolled over near the Old Faithful interchange. Joshua Spalsbury, 27, of Grand Ledge, Mich., was driving in the Old Faithful area about 7:30 a.m. when his vehicle left the road, rolled and then hit a tree, trapping him inside, according to the park. National Park Service rangers and emergency response personnel found him unresponsive and without a pulse. Extrication equipment and personnel were required to access and recover his body.


Worker killed under mobile home in Yakima

OR - A worker was fatally injured when he was pinned under a mobile home in Yakima. Police say the man was preparing a foundation for the home this morning when it shifted on top of him. He was removed by fire department rescuers and taken to a hospital where he died. The state Department of Labor and Industries is investigating the accident.

It was a devastating turn for the families of the six men trapped in the Aug. 6 collapse at the Crandall Canyon mine and for the relatives of those trying to rescue them. Worker killed at South Padre construction site

TX, - A construction worker was killed on the job in South Padre Island. It happened at a construction site at the Sheraton hotel. We've learned a steel panel used to pour concrete fell on top of the 20-year-old man. Justice of the Peace Bennie Ochoa pronounced the worker dead shortly after 7 a.m. South Padre Island Police Chief Robert Rodriguez says the accident victim was from Mercedes, but they haven't released his name. The victim's brother was also a construction worker at the site. Rodriguez says the brother wasn't hurt. But he tells us the man taken to the hospital, because he was distraught. Construction workers have been sent home for the day. The only people allowed in the Sheraton are hotel guests, workers, and people who have business there. The police chief says OSHA will be doing an investigation, as well as some of the city's building officials.


"Shots are being fired," employee says in 911 call before slaying

OH - A trucking company manager called 911 just before he was shot and killed, saying that one of the company's drivers was trying to take off in a truck after being fired. The manager, Douglas Smith, said the employee had just been let go. Smith's voice was calm until he heard the first shots. "Shots are being fired," he told the 911 dispatcher. "I need to go downstairs and see what's going on." Smith, of Toledo, and another employee were killed Wednesday afternoon at Liberty Transportation in Perrysburg Township near Toledo, police said. Calvin Neyland Jr., 43, of Findlay, was arrested about three hours after the shooting in a motel parking lot in Michigan, where he was driving a red semitrailer cab, authorities said. An extradition hearing was scheduled for Thursday.


Electrician working on field lights killed

TX - Licensed journeyman electrocuted at Liberty Christian football stadium. An electrical company employee was killed Wednesday morning while working on the football field lights at Liberty Christian School. Jacob Dyska, 28, of North Richland Hills, was pronounced dead at the scene. The licensed journeyman electrician was working for the Fort Worth office of Design Electric. Argyle police and fire rescue crews, responding to a 911 call received at 11:39 a.m., found Mr. Dyska lying in the crow's nest at the top of an 85-foot light pole. Because the Argyle Fire Department doesn't have a ladder truck, Fire Chief Mac Hohenberger called for assistance from the Denton Fire Department, which brought a truck and a high-angle rescue crew. Dispatchers told first responders that Mr. Dyska may have received current for 15 to 20 seconds.


Man dies after water treatment plant accident

AR - An industrial accident claimed the life of a 48-year-old employee of a local contractor Wednesday afternoon at the City Corp. Water Treatment Plant in northern Russellville. A steel beam that weighed approximately 500-600 pounds fell off a piece of equipment, struck Roy A. Bradley in the head, and the man died as a result of his injuries, Pope County Deputy Coroner Doug Hottinger said Wednesday night. Hottinger went on to say the accident occurred at 2:30 p.m. Wednesday, and the man was pronounced dead after he was transported by Pope County EMS to Saint Mary’s Regional Medical Center. City Corp. General Manager Craig Noble said Wednesday night the accident — defined as industrial since it took place at a production facility — did not involve any City Corp. personnel, but was “the result of work being performed by a contractor working for City Corporation.”


Contract worker dies at Port Arthur oil refinery

PORT ARTHUR, TEXAS — A contract worker was killed in an industrial accident Thursday on the property of an oil refinery. Emery Bowie, 25, of Port Arthur, died while working on the Alligator Bayou Project at a Motiva Enterprises LLC refinery in an accident that involved heavy equipment, said Jeff Gulledge, project manager for Standard Constructors. Bowie worked Standard Constructors Inc. and was described as an experienced, certified operator of heavy equipment. Additional details about the accident were not released


City worker electrocuted while mowing

MONTEVALLO, AL - A 39-year-old city worker was electrocuted Friday morning after being struck by a fallen power line. Ralph Burke was part of a three-man crew trimming bushes near the intersection of Shoshone Street and Overland Road when he came in contact with a downed line around 9:30 a.m. The three men had just returned from a water break. Burke was pronounced dead at scene. The other two workers were not injured, but have been offered counseling through the chaplain's academy of the Shelby County Sheriff's Office.


Indiana Truck Driver killed In I-26 Accident

IN - Two trucks collide; interstate closed for more than six hours. An Indiana truck driver was killed when two tractor-trailers collided on Interstate 26 in northern Spartanburg County Tuesday night. The accident forced troopers to divert traffic off the interstate for more than six hours. The Spartanburg County Coroner's Office says Richard Groover, Junior, 36, died when another 18-wheeler crossed the median and hit his tractor trailer around 9:00pm Tuesday. The driver of the other truck was flown to the hospital for treatment. The Highway Patrol says a car traveling on west I-26 went off the road and over corrected. That car hit a tractor-trailer and sent that large truck across the median and into the path of the 18-wheeler being driven by Groover.


Worker killed at DIA lived in Pennsylvania

CO - The worker killed Monday at Denver International Airport is a 46-year-old Pennsylvania family man with a son serving in the Army in Iraq. In a freak, predawn accident, Robert L. Genzler, 46, of Beaver Falls, Pa., was hit by a giant sweeping machine as workers scrubbed tire rubber from a DIA runway. Genzler worked for Rampart Hydro Services, a Pennsylvania firm that uses trucks that blast high-pressure water as a way to strip rubber residue left by landing jetliners from runways. Denver police said the DIA worker driving the sweeper lost sight of Genzler, apparently because the man had bent over to pick up an object on the runway, which was closed for cleaning, police said.


2 Killed in Ohio Workplace Shooting

WALBRIDGE, Ohio - A shooting at a trucking company left two people dead Wednesday, and a suspect was arrested in Michigan after fleeing the scene in a semitrailer cab. The 43-year-old man was arrested several hours after the 3 p.m. shooting at Liberty Transportation in Perrysburg Township, township Detective Sgt. Robert Gates said. The cab was found in a hotel parking lot in Monroe County, Mich., just over the state line, police said. Police said they are interviewing witnesses and have not determined a motive. Details on the victims were not released, and no one else was injured, Gates said. The company is in an industrial complex in the township, seven miles southeast of Toledo.


Contracted Austin Energy Tree Trimmer Hits Power Line, Dies

AUSTIN, TX -- A contractor trimming trees for Austin Energy died Wednesday after hitting a power line. Austin Energy spokesman Ed Clark says the man was working for a subcontractor in the Emerald Forest Subdivision around 10:00 a.m. when the incident happened. Clark says this is the first death associated with tree maintenance by Austin Energy. The Austin Police Department is investigating the death, which is routine when a fatality happens at a worksite in Austin. The Occupational Safety and Health Administration, or OSHA is also investigating. Austin Energy officials say they will review the incident its tree trimming program procedures. Officials are not releasing the name of the victim until family members are notified. Austin Energy officials say the man worked for Lynx System Incorporated of Austin. They say it is a subcontractor to Davey Tree Surgery.


Diver who died at BP identified

WA - Police identified the diver who died in Tuesday’s industrial accident at the BP Cherry Point Refinery as Christopher N. Primeau, 35, of Wallula, near Kennewick. Primeau, a freelance diver employed by Spokane-based Associated Underwater Services Inc., was 130 feet under water examining pilings being installed near the refinery when employees on the surface lost radio contact with him. A second diver found Primeau unconscious near the bottom of a piling and brought him to the surface. Paramedics and BP personnel were unable to revive him. The cause of death remains under investigation. Whatcom County Sheriff’s Office Sgt. Doug Chadwick said it appears that one of the pilings fell over, though it’s unclear if that led to Primeau’s death.


Summit man electrocuted on construction site

IL - A 32-year-old construction worker from Summit was killed when a high voltage electrical line fell into the trench where he was working and electrocuted him. Robert Lozano, an employee of Swallow Construction, was electrocuted about 12:30 p.m. Tuesday while working in an Arlington Heights' Lynwood subdivision, the Arlington Heights police said. The workers were connecting fire hydrant pipes when another worker driving an excavating truck snapped a ComEd line, said Cmdr. Nicholas Pecora Jr. of the Arlington Heights Police Department. The power line fell into the trench the men were working in, which contained some water, Pecora said. The second employee declined further medical treatment after being evaluated on the scene, the release said.


Man killed in trench collapse was from Aurora

DENVER, CA - The death of a plumbing contractor buried under an estimated 2,000 pounds of dirt in Denver has been ruled an accident. Coroner's officials identified the man today as 26-year-old Erick Acosta Hernandez of Aurora. Firefighters say he was working on a sewer line yesterday at a construction site when he dropped his shovel into a trench several feet deep. The trench, which did not have required safety supports, collapsed when he went to get the shovel. The man's co-workers tried to dig him out, but Fire Department spokesman Phil Champagne estimates Hernandez was covered for about 20 minutes. Hernandez was pronounced dead at a hospital yesterday afternoon.


Construction Worker Dies After 30 Foot Fall

TEMECULA, Calif. A construction worker plunged 30 feet to his death from a rooftop of an office building under construction in Temecula, authorities reported Tuesday. The fatal fall at the Temecula Corporate Center in the 27000 block of Via Industria was reported at 11:01 a.m., according to Jody Hagemann, with the Riverside County fire information office. The worker, whose name was not immediately released, fell through the roof and onto the concrete below and suffered fatal head injuries, she said.


Airline employee dies in accident at Tunica Airport

TN - The Federal Aviation Administration and the National Transportation Safety Board are looking into the death of a worker at the Tunica Airport. Alan Simpson, a flight mechanic for California-based Sky King Incorporated, died in an accident at the airport on July 10. According to a preliminary NTSB report, Simpson was attempting to close the main cabin door on a flight that was preparing to take off from Tunica when he lost his grip and fell ten feet to the ground. Simpson suffered a skull fracture and broken ribs, and died the next day at The MED.


Front-end loader crushes elderly farmer

ROYAL CENTER, Ind. -- An 86-year-old man was killed when he became trapped between the cab and the hydraulic bucket of a front-end loader on a Cass County farm, Indiana State Police said. Robert L. Phillips, Jr., was attempting to use the loader to move a livestock trailer Monday from his barn on his property in Royal Center, about 55 miles south of South Bend, police said. Phillips apparently attempted to enter the cab of the front-end loader when the hydraulic bucket activated, pinning him to the cab, police said. He was discovered by his wife shortly before noon and pulled from the equipment by members of the Royal Center Volunteer Fire Department. Phillips died of blunt force trauma to the chest, state police said.


Baseball coach hit, killed by line-drive ball, Fatal injury to Rockies' farm team coach raises safety questions

DENVER, CO — The Colorado Rockies are remembering a member of their family--a minor league coach who was killed in a tragic baseball accident. A former big-leaguer himself, Mike Coolbaugh was a coach for a minor league team, which is owned by the Rockies, when he was hit by a foul ball. It happened Sunday night in the ninth inning at a game in Little Rock, Arkansas, between the Tulsa Drillers and the Arkansas Travelers. Coolbaugh was working as the first bse coach last night in Little Rock, when a line drive foul ball hit him in the head. While the Rockies mourn the loss, they're also wondering if something should be done to keep the teams and fans safer. "He was a good man," said Rockies Manager Clint Hurdle. "I spoke with him July 4th for about 20 minutes. We talked about kids, and we talked about the relationship and the demands of a father, as a coach."


Fire-Fighting Pilot Dies in Calif. Crash

CA - Helicopter Crashes, Killing Pilot Fighting Wildfire in California's Klamath National Forest. Fire managers worried that lightning could spark more blazes in the West, while investigators tried to determine what caused a helicopter delivering water to firefighters in the Klamath National Forest to crash, killing the pilot. The chopper went down Monday in "extremely rugged" mountains about 12 miles southeast of Happy Camp in Siskiyou County, said Duane Lyon, a U.S. Forest Service spokesman. The helicopter was carrying a large water container to refill hand-pump backpacks for firefighters on the ground. Some firefighters saw the helicopter crash and reported it to emergency dispatchers. The pilot, who was not identified, was under contract with the Forest Service but was not a government employee, Lyon said. The pilot was the only person on board.



Death reported at St. Ansgar plant

IA - The Iowa Occupational Health and Safety Division is investigating the death of an employee at the Grain Millers Incorporated processing plant in St. Ansgar. OSHA administrator Mary Bryant says the death of a female was reported to her agency last Thursday. The company has issued a press release saying that 54-year-old Melanie Ann Haverhals was found non-responsive by co-workers at about 8 o'clock Thursday night, with emergency personnel immediately being summoned to the plant. Bryant says initial information provided to her by the company says Haverhals was found at the bottom of a belt lift, which transports people to different levels of the plant. Bryant adds that the woman might have fallen nearly eight stories, but the agency's investigation has not been completed.


Gasfield worker dies in Antero drill site mishap, Employee struck by falling steel object

SILT, Colo. — Joseph Buchanan, 34, of Louisiana, was fatally injured in an apparent work-related accident at an Antero Resources gas-production drilling rig Thursday evening near Silt. The accident occurred at a drilling rig near mile marker one on County Road 315, (Mamm Creek Road). According to Garfield County Sheriff's spokeswoman Tanny McGinnis, Rifle Fire Protection District (RFPD) responded to the call at 5:45 p.m. Thursday where they found Buchanan with head injuries. "It looks to be an apparent mechanical failure," said Garfield County Sheriff's Deputy Eric Ashworth. McGinnis said the RFPD crew worked on Buchanan at the scene of the accident for nearly an hour before he was pronounced dead.


Truck driver hits bridge off Interstate 84, dies

Town of Wallkill, NY — A truck driver was killed yesterday morning when the tractor-trailer he was driving veered off Interstate 84 westbound and hit a bridge. Levi L. Childress, 39, of Missouri, died on impact from multiple fractures, internal bleeding and lung and brain injuries, the autopsy revealed. No other motorists were involved in the accident, which happened at around 7:30 a.m. After he collided with the Route 17 bridge at Exit 4, two motorists stopped and one used a fire extinguisher to put out a small fire that started, state police said.It is not clear why Childress drove off the road. Early speculation pointed at either a heart attack, which was ruled out by the autopsy, or the possibility that he fell asleep at the wheel. There were no skid marks in the grassy shoulder or on the concrete, indicating he did not try to stop. The tractor-trailer stopped once it hit the concrete side of the bridge, leaving large black splotches, and flipped onto its left side.


Wounded New York Police Officer Dies

NEW YORK — An officer shot in the face during a traffic stop died Saturday, and the suspected gunmen now face murder charges, officials said. Russel Timoshenko, 23, died at a hospital of injuries suffered early Monday after he and his partner stopped a stolen SUV in Brooklyn, Police Commissioner Raymond Kelly said. Timoshenko had been paralyzed, suffered brain swelling and was unable to breathe on his own. His partner, Herman Yan, who was saved by his bullet-resistant vest, was released from a hospital Tuesday. Two suspects captured in Pennsylvania were charged in the assault; a third man suspected of driving the getaway car is being held in New York.


Police officer dies in car accident

A Montgomery police officer has been killed in a vehicle accident. Montgomery police spokesman Lt. Mark Drinkard said that the accident happened at 11:05 p.m. Friday at Old Selma Road and Hunter Loop Road. According to a press release, Montgomery police officer Robert Edward McCrory, 24, was pronounced dead on the scene when his 2005 Dodge Ram collided with a 1997 Chevy Cavalier. The driver ran a red light at the intersection when McCrory was traveling westbound on Old Selma Road and Hunter Loop Road. McCrory's vehicle left the roadway and struck a telephone pole. The Cavalier left the roadway and traveled 70 yards into a field. Micah Joel Day was the driver of the Cavalier and was transported to Baptist South Medical Center with non-life threatening injuries.


Mississippi Officer Dies in Crash

MS - It's a time of mourning at the Kemper County Sheriff's Department, where officers are remembering one of their own. 59-year-old Deputy William Walters died in a two-vehicle crash Sunday afternoon on Highway 16 East of DeKalb. "Not only was he a deputy and a worker, but he was a friend to me," said Sheriff Sam Tisdale. Tisdale says that's what makes the loss so hard. For the past seven months, Walters had worked with the Kemper County Sheriff's Department. This same department lost a dispatcher, Shandy Hudson, who was killed in an accident while on her way to work three years ago, and Sheriff Mike McKee was killed in the line of duty ten years ago.


2 firefighters die in San Pablo blaze, "Flashover" catches men searching for residents in burning house

CA - Two Contra Costa County firefighters lost their lives Saturday after they were caught in an explosion of flames as they tried unsuccessfully to rescue a couple from their burning home in unincorporated San Pablo, authorities said. A seemingly routine call for help quickly turned disastrous with four lives lost, and left the 300-member Contra Costa County Fire Protection District reeling from the first on-duty deaths in its history. Contra Costa County Fire Protection District Capt. Matt Burton, 34, of Concord, Fire Engineer Scott Desmond, 37, of Brentwood, and residents Delbert Moore, 67, and Gayle Moore, 62, died in the blaze at the couple's ranch-style, one-story home on Michele Drive in the Montalvin Manor neighborhood, authorities said. The firefighters had managed to pull one victim from the home and were trying to find the other person when they were caught in a "flashover," which happens when hot gases build up in a building and cause bursts of flame to erupt, said Fire Chief Keith Richter.


OSHA investigates Round Rock workplace death

A workplace accident at a Round Rock manufacturing company took the life of a worker this week. Heath Bice, 28, of Bartlett, was found unconscious at the TECO-Westinghouse Company on Monday afternoon. Emergency workers say Bice suffered a wound to his chest and abdomen area. The cause of death is not known at this time. OSHA, the federal Occupational Safety and Health Administration, is investigating the accident.


Man drowns in Collinsville pond

MO - An employee of a Collinsville construction firm drowned in a pond on his company's property Wednesday. Timothy L. Cochran, 53, of Highland, died when the farm tractor his was driving tipped over a small hill and rolled into a pond at 6086 Fedder Lane in Collinsville. Witnesses called 911 at about 11:30 a.m. and pulled Cochran from the water. He was pronounced dead at 12:10 p.m., according to the Madison County Coroner's Office. Drugs or alcohol do not appear to have played a role in the death, but toxicology reports will be performed, as well as an autopsy, Coroner's Office representative Roger D. Smith said. The Coroner's Office said Cochran's brother, Perry Cochran, owns the property, which is also listed as the address for the general contractor and construction company Cochran Homes. The company's Web site lists Tim Cochran as an employee. Calls to Perry Cochran and the company were not immediately returned.


Tattoo Shop Worker Shot & Killed

TN - Police are on the scene of robbery and deadly shooting at a Tattoo shop in Northeast Memphis. It happened just after 2 p.m. at Top Shelf Tattoo in the 6400 block of Summer. Investigators say a 32 year-old woman was shot at the shop. She died shortly after the shooting. Memphis police are now looking for two men and a woman. The trio was last seen driving east on Summer. The tattoo parlor is across the street from the shopping center where a Fayette County teen shot two people in May. We do have a crew on the scene. 3 medical workers, 2 patients killed as semi hits ambulance

PAULDING, Ohio — Three emergency medical workers and two of their patients were killed last night when their ambulance was struck broadside by a tractor-trailer and burst into flames in northwestern Paulding County, the Ohio Highway Patrol said. Another emergency medical worker and the driver of the semi were hurt in the crash just before 7 p.m. between Paulding and Cecil, Ohio, about 65 miles southwest of Toledo, troopers at the patrol’s Van Wert post said. The Ohio State Highway Patrol's Van Wert Post released the names this morning of the five people killed in the crash. The emergency workers from Antwerp were: Sammy R. Smith, 64; Heidi L. McDougall, 31, and Kelly J. Rager, 25.


Iron Workers Killed When Manlift Collapses - What Their Widows Need to Know...

AK - On July 19th, ironworkers Tyler Kahle and Craig Bagley were killed in Nome, when their manlift collapsed under them. Newspaper accounts report that two were working for Alaska Mechanical Inc., a contractor building the Rock Creek mine and milling facilities operated by Alaska Gold Co., a NovaGold Resources Inc. subsidiary. Here is what their widows/families need to know about Alaskan law...The widows have two (2) claims under Alaskan law: (1) a worker's compensation claim (about worthless); and (2) a claim against whomever was supported to maintain the manlift. It is presently unknown if the collapse occurred due to operator error or a mechanical problem, according to troopers. Hence, the first thing to determine is "Who was operating the manlift?" The second thing to determine is "Who employed that responsible worker?" The widows/families of these two men have claims against whomever was responsible for ensuring their safety on that manlift.


County Employee Killed In Two Vehicle Crash

MS - A Lincoln County employee was killed and a County Supervisor riding with him was injured after a two vehicle crash on a rural Marion county road. It happened on Highway 13 just inside the Marion County line near Columbia, Mississippi at 1:15 Thursday afternoon. Mississippi Highway Patrol Sergeant Perry Snyder says 46-year-old Tommy C. Kyzar was driving a county owned 2003 Chevy pick-up truck northbound when it drifted into the southbound lane. The pick-up smashed into an oncoming 18-wheeler. Kyzar was killed. His passenger, 47-year-old Jerry Wilson, was airlifted to Forrest General Hospital in Hattiesburg. Wilson injuries are not life threatening. He is district two supervisor for Lincoln County. The driver of the 18-wheeler was not injured.


Raybon killed in plant accident

AL - A man was killed Tuesday afternoon in a forklift accident at Liberty Building Products, LLC in Grove Hill. Clarke County E-911 reported that it received the call at approximately 2:54 p.m. from the facility on Industrial Park Drive off Love Road. According to Royce Kelley of the Grove Hill Police Department, the victim of the accident is William Rodger Raybon, 55, of 777 Centerline Road in Fulton. Raybon was marking off the plant floor with tape to show forklifts where to go for safety purposes, Kelley said. The driver of a forklift was moving lumber from a truck and didn't see Raybon kneeling down. Raybon was hit by the forklift. The forklift driver was not identified by authorities. Liberty Building Products manufactures wooden roof trusses and other wood products in the plant formerly occupied by Hamilton Wood Veneer. The deceased man's brother, Terry Raybon, and his nephews, Brian Raybon and Darin Raybon, are listed as principal members of the corporation. Brian Raybon said in a prepared statement Wednesday, "We are mourning the loss of a close family member" and declined further comment on the accident. District Attorney Spencer Walker said Tuesday the initial investigation revealed "no foul play. It was clearly an industrial accident," he said.


Construction worker hit and killed by equipment

SAN DIEGO, CA - A construction worker was killed at a Kearny Mesa construction site yesterday when a 600-pound excavation bucket fell on him. The county Medical Examiner's Office identified the man as Darrell Gene Morgan, 55, of Vista. Morgan was with other employees of Straight Line Engineering who were setting up equipment at the site, a cul-de-sac on Copley Drive, about 9 a.m., police Lt. Jerry McManus said. Morgan was standing in a trench when the bucket fell off the arm of the excavator and struck him in the chest, said Kate McGuire, spokeswoman for the state's Occupational Safety and Health Administration. She said the excavator was being operated by another construction worker.




Forklift fatally crushes demo worker at tannery

NORTH SIDE, IL - A demolition worker was killed Thursday afternoon in an accident at a shuttered tannery on Chicago's North Side. The accident occurred about 1:30 p.m. at the former Gutmann Leather Co. tannery, on the 1500 block of West Webster Avenue, police said. The victim, Jose Torres, 65, was part of a demolition crew that has been knocking down the tannery since it closed last year. Workers found the victim's body and called 911, Sgt. Eugene Mullins said. "It appears to be a construction accident," but police were still investigating how the man died because there were no witnesses, he said. A Cook County medical examiner's office spokesman said Torres of the 5100 block of South Avers Avenue was crushed between a forklift and a wall. He was taken to Advocate Illinois Masonic Medical Center, where he was pronounced dead at 3:40 p.m., the spokesman said.


Woman killed in fall from museum roof

BALTIMORE, MD - A woman who worked as a firefighter and paramedic in Howard County was killed early Thursday when she fell from the roof of an Inner Harbor museum, officials said. Mary Rollins, 27, of Baltimore was on the museum roof with a friend about 2:30 a.m., the museum's receptionist, according to Roland Woodward, the museum director. The two got onto the roof from a staircase on the outside of the building and were trying to climb onto the museum's adjacent neon sign that overlooks the water, Woodward said. Rollins fell some 65 feet, Officer Nicole Monroe, a Baltimore police spokeswoman, said.


Security guard killed; also justice censured

NJ - An armed private security guard was shot and killed this morning while delivering about $150,000 to a Newark check cashing store, authorities said. Officials have not released the name of the victim, who was shot shortly after 9 a.m. at Town Check Cashing on West Market and 12th Street. He was pronounced dead at nearby University Hospital at 10:02 a.m. "He attempted to draw his weapon, and he was shot in the head," said Janet Dickerson, a spokeswoman for Newark. The job usually involves two guards, but the victim's partner did not show up, said Essex County Sheriff Armando Fontoura. Police are now questioning the partner, he said. Authorities are planning a noon press conference.

Hwy 13 crash kills Lincoln County worker

MS - A Lincoln County employee was killed and a county supervisor was injured Thursday afternoon in a two-vehicle accident near the Marion-Lamar county line. State troopers say 46-year-old Tommy Kyzar of Brookhaven was killed when his Chevy pickup veered into the other lane of Highway 13 and struck an oncoming 18-wheeler. Lincoln County supervisor Jerry Wilson was reportedly asleep in the passenger side of the pickup. The extent of his injuries is unknown. The 18-wheeler belonged to Triple M Logging, and its driver, 22-year-old Joshua Carey of Richton, suffered minor injuries.


Witnesses say truck hit and killed DPW worker

NY - A "tragic accident" killed a local musician Wednesday when he was struck by a city of Syracuse dump truck at his day job at the Department of Public Works, a union official said. Donald A. Ransom, 51, a keyboardist with the Black Lites and AfterFX, was working near the DPW's asphalt plant off Midler Avenue sometime after 9 a.m. when he was struck by a city vehicle, DPW Commissioner Jeff Wright said. The truck was operated by 32-year DPW employee Charles Feigel, of Chestnut Ridge Road, Chittenango. Ransom, of Garfield Avenue, Syracuse, was pronounced dead at 10:09 a.m. at University Hospital, city police said.


Convenience store worker fatally shot

VA - Midlothian man was fatally shot last night at a convenience store in Chesterfield County. Police said the victim, 52-year-old Anwar Farooq, was discovered around 11:30 p.m. by a customer who walked into the Fast and Friendly store at 6249 Jefferson Davis Highway in the Falling Creek section of Chesterfield. Farooq, a resident of the 2800 block of Sugarberry Lane who worked at the store, was found dead on the floor. Authorities believe robbery was the motive for the shooting.


Vigil held in Venice for slain LAX worker

CA - Family and friends of a Los Angeles International Airport (LAX) worker who was shot and killed outside a Venice liquor store on Saturday, July 7th, gathered at the site of his death to say goodbye to him Saturday, July 14th. Eric Lorenzo, 20, of South Los Angeles, was murdered by two unknown assailants outside Lincoln Liquor Locker in the 2400 block of Lincoln Boulevard. The alleged gunmen then fled the scene in a car, driving east on Zanja Avenue, according to witnesses. Lorenzo was taken to UCLA Medical Center, where he died of multiple gunshot wounds to the head Monday, July 9th..


Worker killed in trench at construction site

SAN DIEGO, CA – A construction worker was killed inside a trench in a Kearny Mesa construction site Thursday morning, apparently when a 600-pound bucket fell on him. The worker, whose identity has not been released, was at the cul-de-sac on Copley Drive about 9 a.m. when the accident happened, said police Lt. Jerry McManus. The victim was with other employees of Straight Line Engineering, who were setting up equipment on the site, the lieutenant said. Medics pronounced the man dead inside the trench. A backhoe, minus its earth-scooping bucket, could be seen nearby. Workplace safety investigators from Cal-OSHA, the state's workplace safety agency, police officers and the District Attorney's office all are looking into the accident. The property is owned by Sudberry Properties, which is building office buildings, a parking structure and a bank on the location. The construction site was closed for the investigation.


One killed in Grove Hill industrial accident

AL - A person died in an industrial accident Tuesday at Liberty Building Products in Grove Hill in central Clarke County, city officials said. City police and officials with Liberty Building Products confirmed that someone had died in an accident but provided no further information Tuesday. Liberty manufactures a variety of wooden building materials, according to company profiles on the Internet.


66-year-old local man dies from Hantavirus

CO - State and local health officials have confirmed a local man has died due to complications from Hantavirus. The Tribune has learned that Fred Sharp, 66, of Silver Cliff Ranch, died last Monday, July 9, at a Pueblo hospital, after contracting the virus at a location that has yet to be determined. Sharp had been an employee at the Horn Creek Conference Center. Two deaths from Hantavirus have also been confirmed in Costilla and Park counties in recent days. According to the Colorado Department of Public Health and Environment, investigations are being conducted to determine the source of each Hantavirus exposure. Earlier this week, health officials were performing tests at Horn Creek and the Sharp home. Hantavirus is a serious respiratory disease carried by deer mice which are typically found in rural areas. The disease is not carried by the more common house mouse.


Police Seek Shooter of McDonald's Drive-thru Worker

PA - Neighbors and friends still cannot believe what happened to 40-year-old Shawnee Koch. In what may have been a random act of violence, the McDonald's employee was shot in the head during a "drive-by" at the drive-thru early Tuesday morning. She died 12 hours later. WFMZ's Joel D. Smith reports.o far police don't have anything new to tell us. An autopsy began at 3:00, and the investigation is of course, ongoing. This is the same spot where someone was murdered less than 3 years ago. Today we talked to someone working here then. I quit when that happened with another person, I quit Mellisa Ortiz was working at McDonald's in October of 2004, when Jason Stief was murdered while in line at the drive thru. Oritz now lives across the street from where Shawnee Koch lived. She says she saw her often.


Man dies in workplace accident at Round Rock manufacturing company, Company under investigation by OSHA.

TX - A TECO-Westinghouse Motor Co. employee died Monday in a workplace accident. Investigators and a company attorney would not answer questions about what killed Heath Bice, 28, of Bartlett, but Round Rock firefighters said he sustained a wound to the chest and abdomen area. Round Rock firefighters said they received a call from TECO-Westinghouse about 1:45 p.m. When they arrived, they found Bice unconscious. They performed cardiopulmonary resuscitation and used a defibrillator until paramedics arrived. TECO-Westinghouse, a manufacturer of electric motors and generators, referred questions to Austin attorney Wayne Meissner. He would not answer questions about the incident. "Heath Bice was a valued friend and valued employee, and we are all saddened by his tragic accident," Meissner said. "Our thoughts and prayers are with the family." Federal Occupational Safety and Health Administration officials are investigating Bice's death. Diane Petterson, a spokeswoman for OSHA, said Wednesday that she could not comment because of the investigation. Listen


Police make third arrest in murder of Jeffco Burger King worker

CO - Authorities said today that a restaurant employee has been arrested in connection with the robbery of a Jefferson County Burger King that left a female employee dead. Arvada Police arrested Anthony Cole, 21, on suspicion of first-degree murder and robbery. He is being held without bond at the Jefferson County Jail. Cole was working at the fast-food restaurant when two other suspects allegedly robbed the eatery and shot the woman. On Tuesday, two Arvada men suspected of being involved in the robbery and homicide were captured at a Greyhound bus station in Savannah, Ga. The pair, identified as Thomas Steven Johns, 17, and Anthony John Lowe, 21, were arrested shortly after midnight by plainclothes SWAT team members. Arvada police said Burger King employee Celia Meza was shot and killed by robbers early Sunday, after two suspects ran out with an undisclosed amount of cash.


EMS Worker Killed In Accident

NEAR COMSTOCK, MI – The crash happened Tuesday in Kalamazoo County. In the accident, emergency trainer Paul Dickens died. Dickens worked with emergency response personnel across West Michigan. "He was probably the most dedicated person to emergency medical services that I've met," said one person who knew him. While heading down I-94 Tuesday night, a tire came off an Iowa man's truck. It flew down the highway, hitting the truck driven by Dickens, who lived in Hillsdale. His truck went out of control and ran into a car driven by a Battle Creek woman. The Battle Creek woman and Iowa man were treated and released, but Dickens died at the scene. The Sheriff's Department says at 70 miles an hour, that tire became a missile. A Reading emergency sticker, an EMS license plate and a teaching manual were on the front seat. Now they are just mementos of a life that ended early. video


Syracuse DPW worker killed in accident

SYRACUSE, N.Y. -- A local musician and a long time city worker was killed after a co-worker struck him with a DPW truck about 9:30 a.m. Wednesday. Fellow DPW co-worker Edward Firnstein is shocked. "We took lunches together, breaks together, you know, we hung out at the end of the shift together," he recalls. Firnstein isn't the only one shocked by the tragedy. Police say Donald Ransom, 51, was killed after his co-worker struck him with a DPW truck. The District Attorney's office New York State and the DPW will be investigating. For Commissioner Jeff Wright, it's a tough pill to swallow.


Worker, 23, killed at drive-through

OH - North Linden resident Carla Spaulding said the Kwik n Cold Drive Thru was a lot like a neighborhood general store. "If you didn't have the right amount of money and they knew you, they'd give you credit," she said. "They had no enemies." Spaulding couldn't believe that one of those store workers was dead and another wounded after a robbery and shooting yesterday afternoon at the drive-through convenience store, 2899 Suwanee Rd. Late yesterday, Columbus police had charged Leon Mahone with aggravated murder in the shooting. Mahone, 37, of 133 E. 15th Ave., was in the Franklin County jail. He was one of four men who were stopped in a car shortly after the 1:30 p.m. shooting and questioned about it. No one else was charged late yesterday. The dead man was identified in court documents as Mohammad Khan, 23. His address wasn't available.


Taxi Cab Driver Shot In Head

SEATAC, Wash. -- According to the King County medical examiner 43-year-old Jagit Singh, the taxi driver found dead on July 10, was shot in the head. Singh’s body was discovered in a taxi engulfed in flames in Seatac in the early-morning hours of July 10. According to police, Singh had been dispatched to the Seatac address where he was dispatched to pick up a passenger before he was killed. Investigators said they have not named any suspects in the case.

Workman killed by roller at Honolulu airport identified

HI - A 51-year-old Hilo man who was run over by a pneumatic roller while working on a taxiway repaving job at the general aviation area of Honolulu International Airport was identified by the medical examiner's office as Harvey Kojiro. An autopsy determined Kojiro, an employee of Jas. W. Glover Ltd., died of multiple internal injuries in an industrial accident.
