Some Good News For A Change

Ok folks someone gave me a contact and he has been wonderful not only has he agreed to work on my messed up perl program but has also offered to host the site. How awesome is that?

The down side is that we may have some down time. I will make sure it is running while we are in DC.

Hopefully is it a smooth transition but that one is up to my host provider now.

Justice For Chad!

There are many injustices in this world and we all know life isn’t fair. For some matters, there is supposed to be a system of checks and balances. We may not agree with the final outcome, but at least we know that the matter went through the established system.

I have written about Chad Cook before this young man was only 25 years old and dammit this case is just plain and simple BS. Chad Cook's family still have not received an investigation report from any agency responsible for workplace safety. There is really no since in this load of stuff, this family cannot even began to have closer until they get some real answers.

This post will remain at the top of our blog until we get the needed signatures. As much as i hate it if we don't seem to get them on a timely basis we will go on strike. No more blog until we get them. so please take the time to send this to a friend or loved one. Help Chad's parents and show them they have our support!

OSHA the CSB and Me

This photo may seem familiar it is part of the USMWF banner and is a constant reminder of why I spend so much time trying to make a difference for other families.

My brother Shawn was directly in front of the furnace during the first blast at Hayes Lemmerz and not much further during the second which was much more intense. Believe it or not he was a tough cookie and survived only later to be taken off of life support. His internal organs were burnt beyond repair, his eye sight was gone and his limbs damaged beyond repair. There were several reports as to how much of his body was burned and it ranged from 95-98% which at that point it really doesn't matter because much of it was 3rd degree burns.

Besides taking Shawn off of life support the most reoccurring image was of the priest who spoke to us and visited the burn unit on a regular basis he stated " I am sorry sorry, I have really never seen anything quit this bad since the war so prepare yourself before you go in." the second warning I received before walking in and seeing him. But there is no preparing to see a loved one in that position.

I have been around death before Shawn and since him; I know it's smell, it is distinct and there is no other that can compare. When it hits you it drains every inch of your body's strength and leaves you with a since of lifelessness.

I really didn't intend to go into this again as I hate it. It's a haunting experience that reoccurs and never gets less painful, never fades and no one should have to endure it. No family should have to deal with this and no person should have to die in this manner just making a living.


The CSB has been a hero of sorts to families who have a loss do to combustible dusts.

The CSB has investigated many of the combustible dust explosions and can include 50 industrial sectors including chemicals, textiles, metal processing, wood and furniture, wastewater treatment, agriculture, paper products, pharmaceuticals and recycling operations for metal, paper and plastics.

In a 25 year span 119 workers have been killed and more than 700 in combustible dust explosions. The CSB has fought long and hard to gain regulations for combustible dust with no real response from OSHA.

So on Oct. 19, 2007 OSHA announced a combustible dust safety programm. The instruction is available electronically on OSHA's Web site. OSHA mostly based it's instruction on the on CSB's studies and NFPA standards and codes some of which came out of the CSB investigations.

However the CSB is skeptical of the new instruction and wonders if it is enough,

“We’re certainly pleased that OSHA is paying increased attention to this area of deadly accidents,” said Daniel Horowitz, spokesman for the safety board."

“We certainly are going to study the OSHA action closely,” Horowitz said. He noted that the safety board has made multiple recommendations to the administration for changes on OSHA safety enforcement and requirements, and the board has not had time yet to determine whether today’s OSHA action has addressed all of those safety recommendations."

I guess it's more than what we had for 819 others but time will tell. It just makes one wonder why they feel so compelled to use the CSB findings for an instruction but not new regulations. To me that is more like covering a base for both teams and that just don't happen.

Mansour Osanloo a jailed union leader in Tehran

Mansour Osanloo, the jailed leader of the bus workers' union in Tehran, faces the possible loss of his eyesight unless he receives urgent medical attention -- which the Iranian authorities are denying him.

Amnesty International has just now launched an online campaign calling on the Iranian government to allow Osanloo to receive medical attention. I urge all of you to sent off your messages today. The Iranian government must be made aware that the world is watching.

The International Transport Workers Federation (ITF), which has spearheaded the international campaign in defense of Osanloo, has now produced a short film entitled "Freedom Will Come" which tells his story. It can be viewed online on YouTube, and will shortly be available as a DVD as well.

If every reader of this message passes it on to a few friends and colleagues, we can flood the Iranian government with email messages -- but we must do this quickly, before Osanloo loses his vision.

Construction worker killed on I-70

GOLDEN, CO — Authorities say the construction worker who died on Interstate 70 was 43-year-old Kerry G. Lovelace of Denver. The State Patrol says Lovelace was picking up construction cones from the back of a truck Tuesday when he fell out and was run over. Officials say the truck was backing up at the time. Troopers say the driver apparently pulled forward and the truck hit Lovelace a second time. He was pronounced dead at the scene.


Los Banos mechanic shot, killed at business

CA - Owner of adjacent auto repair shop was seen shooting in garage before he killed himself; it's the first homicide in the city in 8 years. The owner of a Los Banos auto repair business was shot to death Saturday morning and his partner wounded while their assailant reportedly killed himself about an hour later near Gustine. Cmdr. Dan Fitchie of the Los Banos Police Department identified the shooting victim as Paulino Martinez Becerra, 46, of Los Banos. Juan Villa, 51, of Los Banos, owner of an adjacent auto repair firm, died of a self-inflicted gunshot wound in his pickup truck at Cottonwood Road in Gustine. Villa's identity was released by the Merced County Coroner's Office, Fitchie said. Martin Mendoza, 41, of Los Banos was shot in the shoulder and airlifted to Memorial Medical Center in Modesto with nonlife-threatening injuries. He was listed in stable condition Sunday.


S.F. garbage-truck driver killed in crash

FRANCISCO, CA - A San Francisco trash hauler died this morning when she crashed her truck into a utility pole in the Sunset District and was thrown from the cab. Patricia Green, 49, of San Pablo had worked nine years for Sunset Scavenger, one of San Francisco's two garbage collectors, a company spokesman said. Police said the wreck occurred at 6:30 a.m. at the corner of 28th Avenue and Noriega Street. The impact crushed the right side of the cab and Green was found on the ground nearby, said Sgt. Steve Mannina, a police spokesman. She was pronounced dead at San Francisco General Hospital. Green was alone in the truck. Police were investigating the cause of the crash. The cab had dual steering wheels, and Green had been sitting at the right-side wheel, said Sunset Scavenger spokesman Robert Reed. He said Green started her shift at 6 a.m., picking up garbage and recyclables.


Man injured in trench collapse dies

SHAWNEE, Kan. - A Kansas City, Kansas, man who was injured when a trench collapsed in Shawnee last month has died. Dennis Lewis was injured September 17th while working for Backhoe Services in Olathe at a residential development. He was installing plumbing pipes when the trench he was in gave way. The 51-year-old Lewis was buried chest deep. He had trouble breathing when he was rescued by crews working nearby. He also suffered leg injuries. He remained hospitalized until he died Wednesday evening.


Worker dies after accident at Alliance plant

ALLIANCE, Neb. - A 25-year-old worker in Alliance has died of injuries suffered in an accident at the Parker Hannifin plant. Authorities say Andrew Frey of Hay Springs died early Saturday morning at a hospital in Scottsbluff. He had been flown there Friday evening from an Alliance hospital. A company news release says the accident happened at 8:15 Friday evening during normal product testing. The plant makes industrial hoses and employs about 300 people. Frey was given first aid, then taken to the hospital. The news release did not detail what happened. A plant spokeswoman says there will no comment beyond the news release.


Wild animal farmer killed by aggressive exotic deer

BALL GROUND, Ga. - John Henry Frix ran a construction business, but it seemed his passion was raising exotic animals. At the end of a dusty dirt road that he named "The Trail of Tears," he ran his own game ranch with barbed-wired pens that held buffalo, llama, horses and other animals. But in a sad twist, it appears that Frix's passion was also his doom. The 66-year-old was found dead Sunday night inside a deer pen on his property, gored several times in the upper body by a red deer's antlers, said Sgt. Jay Baker of the Cherokee County Sheriff's Department. Frix's family told police that the red deer had been acting hostile lately, probably due to rut _ a deer's mating period.


Car leaves road, lawn worker killed

IN - An Indianapolis man died Tuesday after a car struck the lawnmower he was riding on the Far Northside. Jose De Jesus Pasillasguitter, 23, died of blunt force injuries suffered in the crash, according to the Marion County coroner's office. Oved Figuero, 44, was driving a four-door Honda that swerved off the road and struck Pasillasguitter and the lawn mower in the 8500 block of North Keystone Avenue at about 2:30 p.m. Tuesday, Sgt. Paul Thompson said. Figuero, of Indianapolis, had a learner’s permit that expired on Sept. 30, Thompson said. Figuero was not arrested, Thompson said. Police are still investigating the incident.
The driver was not arrested.


Shooting in Simi Valley tire shop leaves 2 dead, 2 wounded

SIMI VALLEY, CA – A gunman shot a woman to death and wounded two men at a tire shop Tuesday, then apparently killed himself, police said. Officers responding to reports of shooting went to the Tire Pros shop at about 7:30 a.m. and found a “chaotic” scene, with people lying in the office and garage areas, Sgt. David Livingstone said. Livingstone identified the dead woman as Susan Sutcliffe, 53. Police declined to identify the suspected gunman, who Livingstone said appeared to have died from a “self-inflicted” wound.


Public safety: Painter electrocuted at historic home

KANSAS CITY,MO - A painter working on the roof of a stately historic home near Swope Park apparently was electrocuted Thursday afternoon, Kansas City fire officials said.
When rescue workers arrived at the house on the southwest corner of 67th Street and Elmwood Avenue, they learned he was dead. They interviewed co-workers and determined that two painters had climbed a tall ladder to reach the roof and used a second smaller ladder, called a jump ladder, to reach a dormer, Kansas City Fire Battalion Chief Joe Vitale said. Vitale said electrical crews were investigating what caused the incident.


Worker killed in rail-car accident

SIOUX CITY, IA - A man performing salvage work on a rail car died Sunday when the car's wall fell and crushed him, the Sioux City Police Department reported. Jose Rodriguez,Jr. of Des Moines was pronounced dead at the scene. The accident occurred just south of 19th Street near Floyd Boulevard, according to police. The man was not working for Union Pacific Railroad, but the Gopher State Scrap and Metal firm from Mankato, Minn., a division of Alter Metal Recycling. Gopher had been contracted to work on an unused box car that had caught fire Sept. 29. Calls to Gopher State Scrap were referred to Bob Ellis of Alter Trading in St. Louis. Ellis said he couldn't comment on what happened as Rodriguez worked but said a company investigation is under way, as is an investigation by the Occupational Safety and Health Administration. "It is a real tragedy," Ellis said. "We are very saddened, obviously, about this loss." Read more of this story Wednesday in the Journal's print edition and online.


Postal worker killed by gunman buried Monday

ALEXANDRIA, LA — Hundreds of uniformed postal workers and dignitaries, including Alexandria Mayor Jacques Roy, former Mayor Ned Randolph and Alexandria Police Chief Daren Coutee, turned out Monday to mourn John Martin "Marty" Thiels, a 30-year veteran of the U.S. Postal Service. Thiels' wife, Laurie, led the mourners at Monday's funeral, along with his sons, John Carley and Daniel Thiels of Alexandria; his daughters, Stacy Anderson of Marksville and Christina Thiels of Alexandria; his mother, Janice Thiels of Chandler, Texas; and sister, Brenda Wright of Brownsboro, Texas. Marty Thiels, 50, was one of two people killed Thursday by a gunman identified by police as John C. Ashley, 63, of Alexandria. According to police, Ashley entered the Giordano & Giordano Law Office in downtown Alexandria and opened fire. Thiels had gone into the law office to deliver the mail when he was taken hostage. Following a 10-hour standoff and shootout between area officers and the gunman, Ashley was discovered dead, along with Thiels and Camille "Joey" Giordano II, 32.



Pilot found dead in plane wreckage

NEWTON, Ga. — A pilot was found dead in his crashed single-engine aircraft after a four-day search in Alabama and Georgia. The crash site was found Monday at Ichauway Plantation in rural Baker County in southwest Georgia. The pilot, Kyle Boss, 22, died in the crash. The Bellanca single-engine aircraft was reported missing Friday afternoon while on a flight from the Decatur County Industrial Airpark in Bainbridge to Weedon Field in Eufaula, Ala. The Alabama and Georgia Wing of the Civil Air Patrol had been searching for the plane amid concerns it may have crashed into Lake Eufaula. Authorities found the crash site, which was 10 miles south of Newton, after a resident told them he had seen a small plane nose-dive above a forest about two miles from his home Friday afternoon.



Trucker Shot On His First Day At Work

LITHONIA, Ga. - An employee of a DeKalb County trucking company was shot on his first day of work late Monday night, police said. The shooting happened at the Brown Trucking Company on Chapman Road in Lithonia. Police said the man was getting in his truck when someone drove up in a black vehicle and asked for directions before opening fire. The man is in critical condition. Police did not release his name. Investigators have no suspects and no motive


Ironworker falls to his death at journalism-school worksite

AZ - An ironworker fell to his death Tuesday morning while working on Arizona State University's $71 million journalism-school building in downtown Phoenix. Ian Delmar, 21, fell from the seventh floor - what is now the top of the building - 80 feet down an air-conditioning shaft in the Walter Cronkite School of Journalism and Mass Communication building around 9 a.m., Phoenix police spokeswoman Stacie Derge said. Delmar was taken to St. Joseph's Hospital and Medical Center, where he was pronounced dead. A crew from Tempe's Top Flite Construction Inc. on Tuesday was stacking steel so that workers could build an enclosure for the mechanical equipment that will eventually go on top of the building, said officials for Sundt Construction Inc., the general contractor.


Worker Killed During Controlled Burn In Webster Parish

LA - A man helping with a controlled burn was killed Thursday afternoon in Webster Parish. Webster Parish Sheriff Gary Sexton says the man worked for a private company that was hired to clear out brush along Highway 163 and Booth Cutoff Road. That's just south of Doyline. Sexton said right now he thinks the 25-year-old worker from east Texas somehow got caught up in the flames of the controlled burn. No other details are being released at this time. Asphalt worker killed in rain

FL - A 50-year-old asphalt worker got backed into by a co-worker driving a front end loader during Wednesday morning's heavy rain. He was taken to the hospital and pronounced dead. Milo H. Williams of Jacksonville had just given another co-worker some rain gear at Duval Asphalt Products on Philips Highway when the front end loader backed over him about 8:30 a.m. in the construction yard, according to the Sheriff's Office report. The driver had topped off a dump truck with asphalt and was backing toward a pile of more asphalt when it happened.


Tucumcari motel worker found dead

TUCUMCARI, N.M. - State police are investigating the death of a 60-year-old employee at the Microtel Inn in Tucumcari. State Police Sgt. Andrew Tingwall said investigators are treating the death of Nancy Archibeque of Tucumcari as a homicide because they believe she was beaten to death. Tingwall said Archibeque was found by co-workers and local police responded. State police criminal investigators were then called to the scene. Tingwall didn't have any other details about the crime or what Archibeque's job was at the motel. Investigators have identified a suspect but no arrests have been made.


Revere officer dies from gunshot wound

REVERE, Mass. --Investigators are awaiting autopsy results and ballistics tests as they try to determine who was responsible for the fatal shooting of a Revere police officer. Dan Talbot was shot once in the head early Saturday morning in a parking lot behind Revere High School. The 30-year-old Talbot was a five-year veteran of the force who was assigned to the gang unit. Spokesman Jake Wark of the Suffolk District Attorney's office said investigators hope to find out what type of bullet killed Talbot. He also said a "number of people" had been interviewed about the case but denied that any arrests had been made. WCVB-TV reported that Talbot and his fiancee had been at a restaurant, Margaritas, earlier in the evening, but it's unclear what happened next.


DeKalb, Wells machinery accidents leave 2 dead

IN - Farm machinery played a role in two separate fatalities in northeast Indiana on Friday. A Waterloo soybean farmer was killed sometime after 2 p.m. while using a combine in a field off of County Road 10 in DeKalb County. John Miller, 64, was using the combine to cut soybeans when the head of the combine struck a ditch and became stuck, DeKalb County Coroner Jeff Warner said. Miller got off the combine without turning it off and crawled underneath it to try to release the head of the combine. The head of the combine, which Warner said weighed about a ton, fell onto Miller and suffocated him. Miller was found by a friend who went looking for him after attempts to reach Miller by cell phone were unsuccessful, Warner said. A combine also played a role in the death of a Warren man in Wells County. According to the Wells County Sheriff’s Office, Clyde Snow, 72, was driving west on Indiana 218, just east of West County Road 600, behind a combine just before 5 p.m. when he pulled into the eastbound lane to pass the combine. Police said Snow then pulled back into the westbound lane behind the combine, which was being driven by Larry Couch, 62, of Poneto, and the front corner of Snow’s vehicle caught on the combine’s left rear tire and flipped over.


Man held in Buscemi's slaying; second is at large

MI - Suspect arrested in Chicago in Eastpointe shooting. One of two suspects who police say were caught on security camera shooting two pizza store employees last week -- killing one -- was in jail Sunday after being arrested in Chicago. Isaiah Mayweather was arrested about 3:30 p.m., two days after police say the 25-year-old Detroit man and 28-year-old Winfred Roger Phillips robbed Buscemi's The Original at 22044 Gratiot. A 40-year-old male employee was shot and killed as he swept the shop floor. Police said the gunmen hopped a counter and seized cash before Mayweather shot a 27-year-old man in the back of his neck. The employee was hospitalized in critical condition.


Limo driver killed in crash

CA - A 51-year-old driver of a limousine was killed in Sorrento Valley early Monday morning in a solo vehicle crash that left the limo overturned. The crash happened just before 3:15 a.m. on the Vista Sorrento Parkway off-ramp from northbound Interstate 805, the California Highway Patrol said. The crash also took out several feet of guardrail along the off-ramp. It will be closed for several hours as crews work to repair it. The man's identity has not yet been released.


Man pinned by semi-trucks dies

MN - A man died when he was pinned between two semi-trucks/trailers in the Coon Rapids Industrial Park Sunday. Patrick J. Gear, 52, East Lyme, Conn., was pronounced dead at the scene at the plant that houses the distribution center for Anheuser-Busch at 11225 Xeon St. N.W. According to Capt. Cary Parks, Coon Rapids Police, it appears that Gear was killed by the semi-truck/trailer that he had been driving. A witness told police that he and Gear were talking while standing behind a semi-truck/trailer at the loading dock. Suddenly, the truck began move. While the witness was able to escape by running under the trailer, Gear apparently tried to run toward the cab and was pinned between the semi-truck that was moving and another semi-truck next to it at the loading dock, according to the Coon Rapids Police report.


Wall tumbles at construction site, killing two

BROWNSVILLE, TX — Two construction workers with Texas Steel Erectors Inc. died Tuesday afternoon and a third one remains in stable condition at a local hospital after a wall panel collapsed at the construction site of the future T-Mobile Customer Service Center in Northeast Brownsville. Brownsville police spokesman Sgt. Jimmy Manrrique said a 9-1-1 call came into dispatch at 3:20 p.m. about the incident at the future call center site at 635 Paredes Line Road. Killed in the accident were a 52-year-old man from Austin and 49-year-old man from Toluca, Mexico, Manrrique said. Justice of the Peace Tony Torres pronounced the men dead at the scene. A third man, a 22-year-old from Austin, was rushed via ambulance to Valley Regional Medical Center, where he was listed in critical but stable condition Tuesday night, Manrrique said. Police refused to release the names of the deceased pending notification of next of kin.


Two Men Killed in Collapse

PA - Two Amish men were killed when they were buried in rubble after a barn wall collapsed. John Esh of Loganton and his 15-year-old son, Benjamin, were buried in the rubble, state police said. Crews worked for hours Tuesday to remove them from tons of stone and debris. That wall collapse happened Tuesday morning near the community of Woolrich in Clinton County. The collapse happened when a newly-built concrete block wall collapsed under the pressure of backfilled stone. Emergency officials said the men were killed when the wall toppled, burying them in stone and cinder block. Dozens of emergency responders surrounded a barn on Dugan Lane near Woolrich, where, EMA officials said the two Amish construction workers were killed around 10 a.m. A straw hat belonging to one of the workers could be seen atop a pile of stone. Emergency workers said the unstable nature of the barn kept them from moving straight into recovery mode.


CBD worker dies after 3-story fall

LA - A worker who fell from a window Thursday in the Central Business District has died, the Orleans Parish coroner's office said Saturday. Segundo Cruz-Venturo, 28, fell about 10:30 a.m. from a third-floor window while installing glass in the area of Julia and Baronne streets, chief coroner's investigator John Gagliano said. Cruz-Venturo died Friday at 1 p.m. at University Hospital of head and internal injuries. He lived in Kenner but was from Mexico, Gagliano said.


Caltrans worker dies after being hit by car

DUBLIN, Calif.— A Caltrans employee who was hit by a car as he was working on a San Francisco Bay area highway has died. Djuan Bush, 24, of San Leandro, was pronounced dead late Tuesday at a Castro Valley hospital, officials with the Alameda County Coroner's office said Wednesday. Bush was working at the Interstate 580-680 interchange when he was struck shortly after noon Tuesday. CHP Office Steve Creel said the accident occurred when the driver of a Porsche reportedly lost control, hit a Caltrans truck, bounced off and hit Bush before striking another truck. The Caltrans crew was working on guard rail maintenance. Creel said several trucks were parked along the shoulder and signs warned drivers to slow down. The driver of the Porsche was uninjured. Cause of the crash has not been determined, Creel said. Gov. Arnold Schwarzenegger said Capitol flags would be flown at half-staff in honor of Bush.


Two killed in truck crash

TX - Two men were killed Thursday afternoon in a collision between a cement truck and an 18-wheeler, officials said. The 2:30 p.m. wreck occurred on SE Loop 410, near the intersection of Southcross, when the driver of the cement truck, which was traveling north, lost control and crossed the median into the southbound lanes, said San Antonio Police Sgt. Gabe Trevino. The truck struck the 18-wheeler, driven by a Wal-Mart employee. The cement truck also hit a small white sedan, which spun to the side of the road, before crashing head-on with the Wal-Mart truck, said Trevino, the department's spokesman. Both truck drivers died at the scene. Crystal Acevedo, 19, and her 7-month-old daughter Natalie Rose, escaped from their sedan with no injuries. Shortly before 5 p.m., authorities opened one lane on the southbound access road of Loop 410. All other southbound lanes were closed at Southcross exit. Officials said buses from nearby elementary schools was diverted so that students would not sit in traffic after school.


Tree trimmer electrocuted in Marin

WOODACRE, CA - A 26-year-old San Rafael man was electrocuted by a 12,000-volt power line as he trimmed a redwood tree in Marin County, authorities said today. Carlos Rivera-Olvera was killed as he worked on the 100-foot-tall tree at 16 Silvan Way in Woodacre a little after 2 p.m. Monday, said Marin County Fire Battalion Chief Mike Giannini. He was about halfway up the tree, still tethered to the trunk, when rescue workers arrived. "His body was contacting the power line that was in proximity to the tree," Giannini said. "It's hard to really speculate exactly what happened - based on the proximity of the wire to the tree, it's certainly possible that he accidentally contacted the wire, or possibly a tool or falling branch did." Man killed in Zoo Drive wreck

MT - A 28-year-old man was killed in a wreck at the Zoo Drive and South Frontage Road intersection this afternoon. The unidentified man was driving a Big Sky Linen truck when the wreck occurred shortly before 2 p.m., Montana Highway Patrol Trooper Dell Aman said. The man was not wearing a seat belt and was killed when he was ejected during the wreck. It took emergency workers, including crews from two Billings Fire Department engines, about 45 minutes to free him from under the delivery truck. Aman said the man, who was traveling north on Zoo Drive, failed to stop and crossed South Frontage Road where the truck broadsided a Ford pickup. The linen truck rotated and flipped onto its passenger side on the east end of the median. The pickup, which was traveling west, spun and came to a stop at the west end of the median. The driver's side of its crew cab was severely damaged in the wreck.


Smoke inhalation killed 5 workers in pipeline fire, coroner concludes

CO - The Clear Creek County coroner's office has determined that five painters killed last week during a fire in a pipeline above Georgetown died from asphyxiation caused by smoke inhalation. The five men - employees of RPI Coatings Inc. of Santa Fe Springs, Calif. - died Oct. 2 when an epoxy coating they were spraying on the inside of water pipes ignited. The workers were identified as Donald Dejaynes, 43; Dupree Holt, 37; James St. Peters, 52; Gary Foster, 48; and Anthony Aguirre, 19. They were all from California. Xcel Energy owns the pipe, which is used to power a hydroelectric turbine on the north side of Guanella Pass above Georgetown. The Occupational Safety and Health Administration is investigating the cause of the fire along with the Colorado Bureau of Investigation.

Houston, TX - Two Loomis employees were shot and killed while servicing an ATM on the northeast side of Philadelphia. This morning, two Loomis employees were shot and killed while servicing an ATM on the northeast side of Philadelphia. The two were part of a three-person team operating out of the company's Pennsauken, New Jersey, branch. The third crew member was the driver of the vehicle and remained in the vehicle to radio for help, pursuant to established security procedures. He is in good condition after suffering cuts from shards of glass as a result of bullets fired by the attacker at the vehicle. The two men killed were William Widmaier and Joseph Alullo. Mr. Widmaier joined the company in 1989. Prior to joining Loomis, he served as a Philadelphia police officer for 23 years. William leaves behind his wife and adult children. Mr. Alullo has been with the company for four years. Previously, he served as a patrol sergeant for the Philadelphia Police Department for over 20 years. Joe leaves behind his wife and three daughters. "We are of course deeply saddened by the loss of our two teammates today," said Cal Murri, President of Loomis US.


Construction Worker Killed, Run Over By Dump Truck

OH - A construction worker was killed at a job site Thursday after accidentally being run over by a co-worker with a dump truck. It happened near the corner of Turkeyfoot Road and Industrial Drive in Independence. The victim is identified by police as 48-year-old Ricky Smith, of Dry Ridge, an employee with Central Rock Mineral Company based in Lexington. Air Care was called to a landing zone nearby, but 9News was told Smith died while being transported to the waiting chopper. 9News was also told that the moments before Smith was runover weren't unlike any other that you would normally see at a construction site.


Machinery crushes worker to death

AURORA, OH - Federal authorities are investigating the death of a construction worker who was crushed by a piece of falling machinery that weighed more than 1,000 pounds, officials said. Authorities were called at 1:37 p.m. Wednesday to a construction site at Indian Trail and Deerpath Roads in Aurora, where they found William Myers of Dixon, 48, lying in a trench, unconscious and not breathing, according to the DuPage County Coroner's office. A bucket from an excavator had dislodged from the machine and fallen onto Myers, crushing his chest, said Aurora Fire Chief John Lehman.


Frankfort Worker Dies After Fall

KY - An investigation is underway in Frankfort tonight after a worker falls to his death at a construction site. It happened this morning along U.S. 60 near the Interstate 64 interchange. That's where a new Kohl's department store is being built. Police say the man, 53-year-old Noble Cox of Miamisburg, Ohio, was the supervisor on scene from a subcontracting group Capitol City out of Ohio. Cox was talking to some of his workers, when he somehow fell 18 feet. Police have turned over the investigation to O.S.H.A officials, calling it a tragic accident. Officials say they expect to have autopsy results by tomorrow. video


Worker dies in fall down elevator shaft

AL - A 36-year-old construction worker died this afternoon when he fell 40 feet down an elevator shaft. The man, whose name has not yet been released, was working on a three-story house in Walton's Mountain subdivision off Airport Road, said HEMSI Chief Operations Officer Don Webster. The workers were pulling a chalk line on the roof when the Hazel Green man slipped and fell down the shaft, landing on his head. Webster said HEMSI paramedics attempted to resuscitate him, but he died on the scene.


Tree service worker dies after fall

ORANGEBURG, S.C. - A 44-year-old man trimming limbs has died after the tree he was climbing in uprooted, sending him to street below. Charles Saylor of North was climbing a tree Friday next to the one he was trimming, said Orangeburg Public Safety Department Capt. Ed Conner. Saylor, who was working for Stack Tree Service, died at the scene from blunt force trauma, Orangeburg County Coroner Samuetta Marshall said. "The tree was rotted to a point that it would not support additional weight," Conner said. "The pine tree appeared to be the remnants of the last ice storm we had. Certainly, those trees turn out to be very unsafe."


Worker dies on city job site, Man, 58, found on roof of music academy

LANCASTER, Pa. - A 58-year-old construction worker collapsed and died Monday while working on the roof of The Pennsylvania Academy of Music. Warren Askey of Phillipsburg was pronounced dead at Lancaster General Hospital after he was found unconscious on top of the building about 3 p.m., investigators said. Askey was constructing a ship's ladder on top of the building for Reynolds Iron Workers of Williamsport. Mike Callahan, executive vice president of Benchmark Construction — the company spearheading renovations of the facility — said Askey had worked for Reynolds for more than 25 years. Callahan said a Benchmark roofer found Askey motionless on a piece of plywood and called 911 at 2:55 p.m. Askey's body had no signs of trauma or injury, and investigators believe the man had a heart attack while working under a scorching sun.


Worker dies at Cronkite School site, Victim fell into 80-foot deep hole

AZ - A 21-year-old male construction worker died Tuesday morning after falling from the roof of the new building for the Walter Cronkite School of Journalism and Mass Communication at the Downtown campus. Ian Delmar was assisting a crane when he fell into a hole that was about 80-feet deep shortly before 9 a.m., said Detective Stacie Derge of Phoenix police. The man fell from the seventh floor of the building located at the intersection of First and Fillmore streets, said Charlie Boyd, a spokesman for Sundt Construction, Inc. Sundt is leading the construction on the new Cronkite building. "We know he fell through a ventilator shaft," Boyd said. "We know the ventilator shafts are normally covered and marked. [But] we don't know what happened in this case." It looked as if the man was walking backwards when he fell through the shaft, said Capt. Victor Rangel of the Phoenix Fire Department. "The shaft was covered with plywood and well marked," Rangel said. Fire officials attempted to resuscitate the man while they transported him to St. Joseph's Hospital and Medical Center, Rangel added. Delmar was working for Top Flite Construction, who had been subcontracted by Sundt, Boyd said.


Breaking news: Euclid police officer dies in line of duty

OH - A Euclid police officer with 22 years of experience died in the line of duty Wednesday evening. The officer was headed onto Interstate 90 west from North Lakeland Boulevard, near East 207th Street, when his police cruiser struck a light pole between the ramp and the highway, said Euclid Police Sgt. Andrew Veres. The officer was accelerating to catch up to a speeder when his vehicle hydroplaned, Veres said. The officer's vehicle burst into flames after striking the pole, he said.


Midstate Worker Crushed In Mining Accident

GORDONSVILLE, Tenn. - A mining accident that injured one worker happened around 9:15 p.m. Tuesday at the Mossy Creek Mining Company in Gordonsville, which is in Smith County near Carthage. According to officials, what's believed to be a machine accident crushed 62-year-old Willie Watson. Emergency officials airlifted Watson to Vanderbilt University Hospital with several broken bones, but Wednesday morning Watson was in stable condition. Last month two workers at the same mine were hospitalized after being hit by falling rock. Wednesday morning the mine was still operating.

ORANGE BEACH, AL - A construction worker fell to his death Wednesday afternoon while setting a reference line for carpenters on the 10th floor of the Phoenix West II condominiums, authorities said. Michael Long, 45, of Carthage, Miss., was pronounced dead at the scene, Baldwin County Coroner Jim Small said. Orange Beach firefighters and paramedics were called to the Gulf-front scene at about 2:30 p.m. to respond to a man who had fallen several stories and was not breathing. Working on what will eventually be Brett/Robinson's 358-unit, 32-story condominium tower, Long fell from the 10th floor, just below the building's highest completed level, police said. Authorities said that during his fall Long struck a protruding deck used to collect construction debris. "Despite aggressive resuscitation efforts, the patient was pronounced dead at the scene," Orange Beach Battalion Chief Kevin Lanford wrote in a news release. Orange Beach police are investigating the matter, but Lt. Steve Brown said that the fall appears to be an accident. Long, an employee of Georgia-based United Forming Inc., was setting a string midway between the 10th level's floor and ceiling to be used as a reference point for future work, Brown said.


Pipeline worker killed at Harriman State Park

Harriman State Park, NY — A construction worker was killed this morning when he was thrown from a machine he was operating and it rolled over him. Pat McCaffrey , 67, of Lebanon, N.J., was working on the Millennium Pipeline, an underground natural-gas pipeline that runs through the state park, said Michael Armiak, Millennium spokesman. McCaffrey was an employee of Precision Pipeline, which has been contracted by Millennium to build the pipeline. The accident happened at around 9 a.m. McCaffrey was sitting in the enclosed cab of a side-boom, picking up the new pipe and putting it in place using a belt attached to a hanging chain from the machine, when he was thrown, Armiak said. How and why he was thrown is still being investigated by State Park Police. The scene was not cleared until early afternoon and this evening, police were still interviewing people as part of the investigation.

WV - A local man dies in an early morning construction accident, at Allegheny Energy's Pleasants County power plant The individual did not work for the power company. 37-year-old Kinsey Culp of Marietta ... was an employee of Maryland-based Pullman Power Company. Culp, and another employee...were working on a construction project at a plant chimney, located between the power plant's two smokestacks...when he fell more than two hundred feet. The Pleasants County Sheriff's Department says Culp suffered serious head injuries, and was pronounced dead at the scene. Allegeny Energy Spokesman Allen Staggers says this accident happened shortly before seven o-clock this morning. The sheriff's department and the West Virginia State Police responded to the scene shortly after the fall. An official from the Occupational Safety and Health Administration arrived at mid-afternoon to begin a formal investigation of the accident. The individual was working on the construction of the chimney for an environmental emissions control project.


Wrongful-death suit targets U-Haul

CA - The company says a Bay Area customer caused his own death when he tried to help after a truck he'd rented started rolling as a worker inside tried to fix its parking brake. September 16, 2007 When the parking brake malfunctioned repeatedly, Demetrio T. Nagtalon drove the truck back to the U-Haul rental center in San Francisco to exchange it for another. After pulling into the check-in area, Nagtalon watched as a U-Haul employee slid under the dashboard with a pliers to tug on the brake cable. The worker left the engine running and the truck on an incline, neglecting to block the wheels. Suddenly, the truck began rolling downhill. Nagtalon rushed toward it, trying to help. He became pinned between the 4-ton truck and a steel pillar and suffered a crushed pelvis and massive internal bleeding. Nagtalon, 63, of Daly City, Calif., died at a hospital two hours later. Bobby M. Johnson, the U-Haul worker, who escaped injury, said Nagtalon was a hero, according to a police report on the Dec. 8 incident. "He got hurt trying to save me," Johnson told police.


Worker killed at Millennium Pipeline project in Tuxedo identified

NY - A construction accident involving a piece of pipe-laying equipment on a steep hill killed a worker on the Millennium Pipeline project in Tuxedo this morning. Pat McCaffrey, 67, of Lebanon, N.J., was an employee of Precision Pipeline, a Wisconsin-based contractor hired by the Millenium Pipeline Co. LLC of Pearl River, a spokesman for the company said. He was crushed to death when the piece of equipment rolled over him on a steep hill near the border between Rockland and Orange counties. Millennium Pipeline Co. LLC of Pearl River is building a massive underground natural gas pipeline there, stretching 182 miles across the Lower Hudson Valley into the Southern Tier. The man was not immediately identified, but authorities said he was from New Jersey.


1 officer dies after 4 are shot near Miami

CUTLER BAY, Fla. - Authorities in the Miami area are still looking for the man who killed a police officer and injured three others during a traffic stop today. The shooting triggered a manhunt in a suburban Miami neighborhood. Initially, police misidentified the suspect, based on information they were given. They now say they're looking for 25-year-old Shawn Labeet. They say the man they had initially identified as the suspect was actually hundreds of miles away at the time, and that they were "misled" by a source. Police believe they've recovered a vehicle and a gun used in the shooting. In all, four officers were shot. The mayor of Miami-Dade County confirms that 1 of them died.


FARMER KILLED IN TRACTOR ACCIDENT

CA - A 46-year-old farmer died Wednesday after being run over by his tractor while harvesting almonds on his ranch, officials said. The man, whose name was not released, had an unknown mechanical problem about 3:30 p.m. while driving the tractor near his home, on the 6100 block of Vivian Road, southwest of Modesto, sheriff's spokesman Royjindar Singh said. The tractor's engine stopped, and when he restarted it, the machine began to roll forward. The man ran in front of the tractor in an attempt to jump onto its seat, Singh said, but instead he got caught in front of it. Westport and Ceres fire departments responded. The man was flown to Doctors Medical Center in Modesto, where he was pronounced dead, Singh said.


Plane crash victims were business owner, two employees

HAVRE, Mont. - The three Havre residents killed in a fiery plane crash north of Seattle were the owner of an oil and gas exploration company and two of his employees. The crash yesterday near Washington's Skagit (ska-jit) Regional Airport killed the 60-year-old pilot, John O. Brown, and his passengers, Randall McPherson and Christopher Schafer. McPherson was 59 and Schafer 25. Brown owned J. Burns Brown Operating Co., and subsidiaries Santana Inc. and Textana Inc. The A-36 Beechcraft Bonanza was registered to Textana.


Two accused of killing guard for replacement garbage workers

CA - An Oakland man could face the death penalty after being charged with special-circumstances murder in the shooting of a man who worked for a security company guarding replacement haulers during the East Bay garbage lockout, authorities said. Jarome Collins, 19, of Oakland was charged with murder and two special circumstances accusing him of killing Byron Mitchell, 29, of Chicago on Aug. 4 during the commission of a burglary and robbery. Collins also faces enhancements of using a gun and causing great bodily injury to Mitchell, who was a contract worker for a firm that provided security for replacement workers during the 27-day lockout of Waste Management garbage haulers in July. Two other defendants, both 17-year-old girls, have been charged with murder but do not face any special circumstances.


Local PPG employee dies after industrial accident

CRESTLINE, OH - Neil T. McMichael, 61, died early Thursday from injuries sustained in an industrial accident at PPG Industries Inc., the company confirmed late Thursday night. "PPG regrets to announce the death of Mr. McMichael in an accident earlier this morning at the Ohio Glass Plant," said Betsy Bialosky, corporate public relations manager for PPG in Pittsburgh. "PPG is investigating the accident in conjunction with local officials and the federal office of Occupational Safety and Administration." The Crestline EMS confirmed it responded to a 3:22 a.m. call at PPG. No further details were available Thursday night through the Richland County Coroner's Office or the Richland County Sheriff's Department. According to his obituary, McMichael was a 1964 graduate of Colonel Crawford High School and a U.S. Army veteran who serv- ed three years in Vietnam.

UT - A 34-year-old Salt Lake City man died Thursday at a tire shop while he was placing a jack under a semitrailer to change the tires and the driver accidentally drove over him. The accident that killed James A. Boren occurred about 3:40 p.m. at 1335 W. 2100 South, said Salt Lake City police Lt. Lamar Ewell. The tire store employee was attempting to change two tires on the back of the truck and was positioning the jack underneath the vehicle when another employee told the the truck driver, who was in the truck, to pull forward, Ewell said. The truck ran over Boren. No one else was injured. The driver was not cited, Ewell said.


Worker falls to his death off of Verrazano Bridge

New York- - A worker (Manuel Pereira, 48) was killed when he plunged off the Verrazano-Narrows Bridge on the Brooklyn side Friday afternoon. Authorities say the worker fell off the bridge on the Staten Island-bound lower-level just before 1:40 p.m. The victim, who is employed by the contractor doing construction work, reportedly fell to the ground and was pronounced dead at the scene. Emergency workers converged on the the entrance of the Fort Hamilton Army Base, near where the body landed. A section of cement was also knocked over on the side of the the bridge, which appeared to be a construction area. The Staten Island-bound lower-level was closed as officials examined the damaged cement. The Verrazano-Narrows Bridge is currently undergoing a 21-month construction project requiring the around-the-clock closure of one lower-level lane in each direction.


Officials identify worker killed in power line accident

HERMON, Maine - A construction worker who was accidentally electrocuted while working on a modular home has been identified as a 24-year-old Portland man. Angel Valespi was pronounced dead at Eastern Maine Medical Center following the Wednesday morning accident at the Clearview Estates subdivision. A second worker, Robert McLeod of Harrington, was critically injured. Valespi and McLeod were working on a home that was in contact with a crane that bumped into overhead power lines, the Penobscot County Sheriff's Office said. High winds were believed to have been a factor in the accident, which was under investigation by the Occupational Safety and Health Administration


Centreville worker dies weeks after accident, Man fell off mower, was run over by it

IL - The 48-year-old Centreville city worker who fell off a mower while cutting grass on a sloped area about seven weeks ago died Tuesday at a St. Louis hospital. Chester Staten, who had been employed by the city for two years, died at St. Louis University Hospital at 5 p.m., Centreville Mayor Mark Jackson confirmed Wednesday. Jackson said Staten had undergone multiple surgeries while he was hospitalized. "It's a very sad time in Centreville," Jackson said. "We're a small community, and everybody knows everybody. The entire city and staff are saddened by Staten's death. We're having a rough time." Staten was working on Phiefer Road in Centreville cutting grass on a slope when he fell from the Bush Hog mower, Jackson said. "The rear end of the Bush Hog ran over his legs," Jackson said. Jackson said a funeral service for Staten will be held at noon Saturday at Officer Funeral Home on Missouri Avenue in East St. Louis.


Wisconsin man kills former co-worker, self

SHEBOYGAN, Wis. — A gunman in camouflage killed a former co-worker inside a Wisconsin factory early Monday, then committed suicide as officers searched the sprawling building, police said. Police identified the shooter as Shadow Yang, 40, and the victim as Christina Wollenzien, 29. Both lived in Sheboygan. Wollenzien was on the job Monday when she was shot to death around 4:30 a.m., said Lt. Michael Williams. Her attacker fired one shot, hitting her in the stomach, then, when she tried to get away, he shot her in the head, said Kirk Engholt, vice president of human resources.



Moss Point Man Dies At Pascagoula Shipyard

MS - A Moss Point man died Wednesday afternoon at Signal International. Michael Bodden had been working at the Pascagoula company for two weeks as an electrician. Though a company spokesperson could not say exactly what happened, Jackson County Coroner Vicki Brodaus told WLOX News there were no signs of foul play or injury. Bodden had been working in the Pump Room of the Dry Dock inside a narrow space and had to be cut out, which took some time. An autopsy will be done to determine the cause of death. The federal Occupational Safety and Health Administration will also investigate, because the death happened at a workplace.



Third Odessa Police Officer Dies from Injuries

TX - A third police officer is dead following a weekend shooting in Odessa. Corporal Abel Marquez passed away just after 3 p.m. Wednesday afternoon. He and his two brothers made an agreement long ago, that if one of them should be killed in the line of duty, life support would be removed, and their organs donated. Earlier on Wednesday, Sergeant Pete Marquez and Corporal Philip Marquez attended the funerals of fellow officers Cpl. Scott Gardner and Cpl. Arlie Jones. Services for each of those men were held separately in Odessa. The Lubbock Police Department provided 11 motorcycle officers to escort and monitor traffic. All three officers were shot and killed while responding to a domestic disturbance call in Odessa Saturday night. The suspect, Larry White, is in custody charged with capital murder.


Plant Fined Following Worker’s Death

GAFFNEY, S.C. -- The Nestle frozen food plant in Gaffney will have to pay a fine for safety violations found in connection with the death of an employee who was killed at the plant in July. James Redmon, 65, died while he was working on a crane, performing routine maintenance in the freezer tower. Redmon was a mechanic at the plant. Cherokee County Coroner Dennis Fowler said that Redmon was trapped and suffered head injuries. He was pronounced dead at the plant. The Occupational Safety and Health Administration cited the plant for not having specific procedures posted near the crane that Redmon was operating. OSHA also said that Redmon didn’t follow the proper lockout procedure.


I-70 worker killed while pulling cones

Morrison, CO - A worker for a state highway subcontractor was killed on Interstate 70 about 3:30 p.m. Tuesday when he apparently fell off a truck while picking up traffic cones and the truck ran over him twice. The accident occurred about a quarter-mile west of the Morrison exit in the far-left westbound lane. The workers were there because a 6 1/2-mile stretch of I-70 from West Colfax Avenue to the Lookout Mountain road is being resurfaced. Two of three westbound lanes of I-70 were closed while the sheet-covered body was removed and the incident was investigated. Trooper Gilbert Mares of the Colorado State Patrol said the accident happened when a white Chevy pickup owned by American Barricade Rental was backing up. Mares said the truck was going "very slow - obviously not highway speed." The driver was wearing a seat belt. The victim, a 43-year-old Denver man who was not identified, was sitting or standing in the back of the truck and fell as he picked up cones, Mares said. The truck driver, David A. Bostrom, 32, of Centennial, "felt some type of a bump," Mares said, and pulled forward, running over the man again.


Construction Worker Dead After Scaffolding Falls On Him, Incident Under Investigation

WESTFIELD, Ind. -- A man working at a Westfield home construction site was pronounced dead after steel scaffolding fell on him Wednesday, authorities said. Juan Rivera, 36, of Indianapolis, had just stepped onto the scaffolding from one of the home's windows when the scaffolding tipped, police said. Rivera fell about 10 feet, and then the scaffolding fell onto him, police said. Rivera was pronounced dead at the construction site, located on Downstream Drive on Westfield's east side. Police said the scaffolding, before it fell, was on a forklift. State occupational safety inspectors will investigate, authorities said


Road crew workers killed by truck

LAWRENCE, Kan. - Two women are in custody in the deaths of 2 state highway workers who were killed on U.S. 59 in Douglas County by a pickup truck that sped away. The Department of Transportation identifies the victims as 30-year-old Tyrone Korte, of Seneca, and 24-year-old Rolland Griffith, of El Dorado. Korte was an engineering technician for KDOT, and Griffith was an employee of a KDOT contractor. A Highway Patrol spokesman says two women from Washington state are being held in the Osage County Jail. They are identified as 48-year-old Ramona Morgan and 26-year-old Sabrina Morgan. The patrol says a trooper saw the pickup fleeing south this morning, then west on U.S. 56 into Osage County. Deputies apprehended the women after using "stop sticks" on the pickup.


Cabbie killed this morning in East orange

NJ - A Police in East Orange found a cab driver dead at the wheel of his vehicle this morning from a gunshot wound. The victim was identified as Maney Orius, 56, of East Orange. Relatives said Orius came to the United States from Haiti in 1990 and began driving a taxi 10 years ago. Before that, he held a factory job. The death was ruled a homicide but few other details were available, said Detective Andrew DiElmo, a spokesman for the East Orange Police Department. He did say police discovered the taxi on South Grove Street, just below Central Avenue. The Diane Dell cab had jumped the curb, struck a tree and run into a fence at the Holy Sepulchre Cemetery, said Celony Estiverne of East Orange, a brother of the victim. The two lived in the same home.


Oradell DPW Employee Killed on Job

ORADELL, N.J. - An Oradell public works employee died on the job Tuesday when a small steamroller pinned him against a parked car as he filled a pothole on a quiet side street, authorities said. Douglas Van Valen, 28, of New Milford, who was a volunteer firefighter in Oradell and in his hometown, was pronounced dead at Hackensack University Medical Center late Tuesday afternoon. Van Valen, a Navy veteran who leaves behind a fiancee, became trapped against the car on Reis Avenue when the paving machine lurched forward as another DPW worker operated it, police and town officials said. 1 killed, 1 injured in shooting at Hardee's in Newport News

VA - A single bullet fired during an early morning robbery killed one restaurant employee and wounded another Saturday, police said. The two robbers escaped and remain at large. The robbery, at the Hardee's restaurant in the 400 block of Denbigh Blvd., was reported at 5:24 a.m., said Harold Eley, a police spokesman. When officers arrived, they found three employees in the restaurant, two of them suffering from gunshot wounds. Both were taken to Riverside Regional Medical Center. The woman, identified as Diane Green, 47, of the first block of Curtis Tignor Road in Newport News, died from her injury, Eley said. The other employee, a 21-year-old man from the 11000 block of Old Court House Way, remained hospitalized Saturday with a bullet wound to his arm, but he is expected to recover. His name was not released. The restaurant manager, a 41-year-old Newport News woman, was not injured and her name also was not released.


Car wash worker killed when sucked into brushes

CA - In one of those who-would-have-believed-it accidents, an 18-year-old car wash worker was killed in Michigan when he got pulled into the machinery. Ricardo Martinez was cleaning the equipment at the end of the day with a pressure washer when, somehow, he became entangled in the machinery and received fatal injuries. While we've never really considered what would happen if a person went through a car wash, we didn't think the things were deadly. "Come summer, the work gets kind of hard," indeed.


Worker dies in Aurora forklift accident

Co - A 32-year-old worker at a Centennial masonry company was killed Friday when a forklift he was using rolled over onto him. The accident happened at about 1:45 this afternoon at United Masonry, which is in the 15300 block of East Hinsdale Circle. South Metro Fire Rescue spokesman Andy Lyon said the man was using a large forklift to raise and push a flatbed trailer up a slight incline from the a parking lot to a fenced-in yard. At one point, the man climbed out of the forklift to make sure everything was secure, and the trailer began to roll down the incline. Lyon said the trailer pushed against the forklift, causing it to tip. The man ran back to the forklift to try to move it, but the forklift fell onto him, Lyon said. The man's name was not released to give officials time to notify his family.


Drowned worker identified

IL - The Cook County medical examiner's office Thursday ruled a 16-year-old worker's death in Long Grove was a drowning. Valentin Gutierrez of Alsip drowned after falling into a swimming pool while working a job hanging drywall Wednesday on the 2300 block of Coach Road, Lake County sheriff's police said. The victim didn't know how to swim. Gutierrez was working with family members, said police, who have concluded their investigation of the accident.

Oil Field Worker Killed

This is the second accident in three weeks that happened outside Morton at a pumping station, and according to the county sheriff, this is simply a dangerous business. "Anytime people work around machinery... Some little something goes wrong... An unexpected and it happens," Cochran County Sheriff Wallace Stalcup said. Yesterday morning, a crew was removing cycle rods from the ground. Those rods go into the ground to pump the oil out and as one was being removed, it got bent, snapped and instantly killed 56-year-old Levelland resident Jesus Chavira. "He was a real good man... He was there for everyone. He was a caring man," Chavira's niece Veronica Perez Torres said. Torres added the company who owns the well, North Premium Well Services has only called the family once. "We didn't get to hear anything until today. She was supposed to be notified the same day and nobody notified her about anything," Torres said. NewsChannel 11 also called the company several times Friday, left several messages and never got a return call. But we do know that as of now this issues is in the hands of OSHA to determine an exact cause.


Storage Facility Manager Killed During Robbery

HOUSTON, TX -- A manager of a northeast Houston storage facility was killed during a robbery, KPRC Local 2 reported. Houston police said two men robbed the Value Self Storage on the feeder road of the Eastex Freeway near Lynnfield Street at about 9:30 a.m. Friday Investigators said the robbers confronted the female manager in the office, demanded money and fatally shot her."The lady was in shock. She couldn't believe it. She came to apply for a job and got caught up in a holdup and a shooting. And if it hadn't been for the fact that she told me to start the day at 10, I would have been here," employee Barbara Petite said. Petite said the manager told her to come in later than her scheduled time and that may have saved her life. "Why did you have to kill her? Why did you have to shoot her? Why did you have to do that?" Petite asked. "That's the part -- to just know it was for nothing. There was hardly any money here." The robbers escaped in the job candidate's white Dodge Caravan with Texas license plate 349 KXR. The general manager of the storage facility said he was in the process of installing surveillance cameras, but they were not yet in place to record the robbers.


State probes landfill worker's death, No witnesses as man may have been hit by a truck.

CA - The state has launched an investigation into the death of a Sacramento County worker who was killed in an apparent accident at the county landfill Monday, officials said Thursday. The California Division of Occupational Safety and Health said it will look into the circumstances surrounding the death of Douglas Aldrich, 52, of Elverta, who was pronounced dead Monday morning after being transported from the landfill on Roseville Road to a local hospital. A heavy equipment operator, Aldrich had been with the county since 1993. He was fatally injured at 5 a.m., officials said.


Farmer killed in rural Gibson City

IL - The Ford County Sheriff says it's an unfortunate but common accident that can happen this time of year. A farmer was killed Tuesday night in rural Gibson City, near the McLean County Line, as he was mowing. Sheriff Mark Doran says 61-year-old Michael Schroeder's family became worried when he didn't return at dusk, and a neighbor found Schroeder around midnight less than a mile from his home.


Postal Truck Driver Killed At Mail Processing Center

VA - A contract driver for the U.S. Postal Service, a single mother of three, was killed at the Dulles processing and distribution center when she was pulled under a tractor-trailer and dragged, authorities said yesterday. Katherine N. Sowers, 49, who lived in Leesburg, had driven her box truck into the center's docking area Tuesday when the tractor-trailer was pulling out, a Loudoun County Sheriff's Office spokesman said. "Somehow she got caught beneath the truck," spokesman Kraig Troxell said. He could not say how far Sowers was dragged. No charges have been filed, he said. Authorities were called to the center, at 44715 Prentice Dr., just after 3 p.m. Sowers was pronounced dead at the scene, Troxell said.



Men die of injuries from boom lift

GA - Two Warner Robins men hospitalized a week ago today after the portable boom lift they were in suddenly fell over have died. Travis Scott Register, 27, died Saturday at The Medical Center of Central Georgia. Robert Jay Stephens, 28, died Friday at the Macon hospital. The cause of the accident is under investigation by the Occupational Safety and Health Administration. The two men were working on the flashing near the roof line of the Flint Energies building under construction on Russell Parkway in Warner Robins when the boom lift toppled over, Warner Robins police Maj. John Wagner said. Mark Christianson, project manager for International City Builders of Warner Robins, the general contractor, said Tuesday he does not know what caused the lift to become unbalanced and tip over. He said the men were standing in the basket of the lift about 27 feet in the air and that their harnesses were attached to the lift as required. "OSHA has been on site. They have not indicated that anything was done improperly," Christianson said. "We've looked at the site ourselves. "We're not sure what the cause of the accident was. Everything seems to have been followed. ... The guys had their hard hats on, their harnesses. ... They'd been working in that area. ... We're just unsure what was the cause but OSHA was still investigating," he said. Thursday and Friday, work continued on the job site with skeleton crews. Full work crews were back on site Tuesday. Register and Stephens worked for Southern Glazing Services of Perry, which was performing subcontracting work on the site.


Farmer killed after mowing tractor rolls over

GIBSON CITY, IL — Ford County authorities have identified the farmer who was killed early Wednesday morning in a mowing accident as Michael J. Schroeder, 60, of rural Gibson City He was pronounced dead at the scene at 1:30 a.m. Wednesday at county roads 300N and 70 E west of Gibson City. Ford County Sheriff Mark Doran said Schroeder was mowing along the edge of a creek in that area when a tire of his tractor apparently slipped off the edge of the waterway. The tractor rolled on top of him. Schroeder was found when family members started looking for him when he did not return home at dusk. Coroner Doug Wallace said the accident remains under investigation.


Dayton Farm Accident Claims 15-Year-Old, Equipment Detaches From Tractor, Hits Youngster

DAYTON, VA — A farm accident claimed the life of a 15-year-old boy in Dayton Monday, according to Sgt. Felicia Glick of the Rockingham County Sheriff’s Office. Eric James Koogler of Harrisonburg was fatally injured shortly before 4 p.m. at a farm on Ottobine Road after he was hit in the torso by a piece of farm equipment that became detached from a tractor, Glick said. When a deputy arrived on the scene at about 4 p.m., Glick said, Koogler had been transported to Rockingham Memorial Hospital. Glick said he was then taken to the University of Virginia Medical Center in Charlottesville, where he died from internal injuries. Lee Roy Hensley owns the farm, according to Glick. Hensley could not be reached for comment. Jimmy Maass, safety coordinator for the Virginia Farm Bureau, said this type of accident is rare but farm workers should always be vigilant about equipment safety.


Worker dies of injuries suffered in ammonia release

WATERLOO, IA - Elizabeth Myers died Sunday of injuries suffered when ammonia was accidentally released at a Beef Products Inc. plant, state labor officials said Tuesday. Myers was one of several workers injured when a BPI employee opened a valve that wasn't locked causing the ammonia to be released on Friday, officials with the Iowa Labor Commissioner's Office said. One other worker was taken to the burn unit at University Hospitals in Iowa City. Several others were taken to a hospital but no information was available Tuesday about their conditions, officials said. The accident happened as contractors were working to hook up a new ammonia piping system. The release caused the evacuation of the plant, the neighboring Tyson Fresh Meats plant and a third business in the area. About 235 people work at the BPI plant, which produces low-fat beef products. The Iowa Division of Labor Services was investigating the ammonia release.


Carwash employee dies at Goo Goo wash

WYOMING, MI — A full-time carwash employee died after becoming entangled in the automatic brushes of a franchised Goo Goo Express Wash here, according to an Associated Press report. The Sept. 1 story said Ricardo Martinez was closing the carwash when a customer returned to have his vehicle washed again. After the vehicle entered the carwash, Martinez went inside to clean the tunnel and became entangled in its automatic brush. The article said a customer found his body in the carwash. It did not say if there were any other employees working at the express wash at the time of his death. According to a report on Fox 17, police reviewed surveillance tape to rule out foul play. The case has been ruled an “industrial accident” and turned over to the state’s OSHA office.



The TV report said Martinez graduated high school earlier this year. He had been employed at the Goo Goo wash since March and was a full-time employee trained in the wash’s safety policy.


A Marathon employee dies at Garyville plant

CA - A newly hired Marathon Oil Co. employee died in a motor vehicle accident at the Garyville refinery over the weekend, a company spokeswoman said Tuesday. Jackie Batiste, 34, of Hahnville was fatally injured after he accidentally drove a company vehicle into a retention pond at the refinery on Friday night, said Angelia Graves, a Marathon spokeswoman. Graves said it appears as though Batiste, who was on duty at the time of the accident, drowned. An autopsy is pending to confirm an official cause of death, she said. An internal emergency response crew at the Garyville refinery arrived at the scene of the accident around 11 p.m. Friday, Graves said. She said further information about how the response team was called to the plant's retention pond -- which collects rainwater -- is still under investigation.


Worker Killed at Beaumont Construction Site

CA - Authorities Tuesday released the name of a contract worker who suffered fatal injuries in Beaumont while replacing an electrical transformer that failed during the heat wave. The California Department of Industrial Relations' Division of Occupational Safety & Health has opened an investigation into the death of 50-year-old Richard Eastman, said spokeswoman Kate McGuire. Eastman, who worked for Kansas City Mo.-based PAR Electrical Contractors Inc., was fatally injured while working on a power pole in an alleyway in the 1300 block of Massachusetts Avenue about 9 Monday night. He had been sent out on an emergency call to restore power in the area serviced by Southern California Edison, said Jim Stapp, his boss at PAR's Fontana office.

Pilot found dead in plane wreckage

NEWTON, Ga. — A pilot was found dead in his crashed single-engine aircraft after a four-day search in Alabama and Georgia.

The crash site was found Monday at Ichauway Plantation in rural Baker County in southwest Georgia.

The pilot, Kyle Boss, 22, died in the crash.

The Bellanca single-engine aircraft was reported missing Friday afternoon while on a flight from the Decatur County Industrial Airpark in Bainbridge to Weedon Field in Eufaula, Ala.

The Alabama and Georgia Wing of the Civil Air Patrol had been searching for the plane amid concerns it may have crashed into Lake Eufaula.

Authorities found the crash site, which was 10 miles south of Newton, after a resident told them he had seen a small plane nose-dive above a forest about two miles from his home Friday afternoon.
