Villa's identity was released by the Merced County Coroner's Office, Fitchie said. Martin Mendoza, 41, of Los Banos was shot in the shoulder and airlifted to Memorial Medical Center in Modesto with nonlife-threatening injuries. He was listed in stable condition Sunday.
S.F. garbage-truck driver killed in crashFRANCISCO, CA - A San Francisco trash hauler died this morning when she
crashed her truck into a utility pole in the Sunset District and was thrown from the cab.
Patricia Green, 49, of San Pablo had worked nine years for Sunset Scavenger, one of San Francisco's two garbage collectors, a company spokesman said. Police said the wreck occurred at 6:30 a.m. at the corner of 28th Avenue and Noriega Street. The impact crushed the right side of the cab and Green was found on the ground nearby, said Sgt. Steve Mannina, a police spokesman. She was pronounced dead at San Francisco General Hospital. Green was alone in the truck. Police were investigating the cause of the crash. The cab had dual steering wheels, and Green had been sitting at the right-side wheel, said Sunset Scavenger spokesman Robert Reed. He said Green started her shift at 6 a.m., picking up garbage and recyclables.
Man injured in trench collapse diesSHAWNEE, Kan. - A Kansas City, Kansas, man who was injured when a
trench collapsed in Shawnee last month has died.
Dennis Lewis was injured September 17th while working for Backhoe Services in Olathe at a residential development. He was installing plumbing pipes when the trench he was in gave way. The
51-year-old Lewis was buried chest deep. He had trouble breathing when he was rescued by crews working nearby. He also suffered leg injuries. He remained hospitalized until he died Wednesday evening.

died of injuries suffered in an accident at the Parker Hannifin plant. Authorities say
Andrew Frey of Hay Springs died early Saturday morning at a hospital in Scottsbluff. He had been flown there Friday evening from an Alliance hospital. A company news release says the accident happened at 8:15 Friday evening during normal product testing. The plant makes industrial hoses and employs about 300 people. Frey was given first aid, then taken to the hospital. The news release did not detail what happened. A plant spokeswoman says there will no comment beyond the news release.

66-year-old was found dead Sunday night inside a deer pen on his property,
gored several times in the upper body by a red deer's antlers, said Sgt. Jay Baker of the Cherokee County Sheriff's Department. Frix's family told police that the red deer had been acting hostile lately, probably due to rut _ a deer's mating period.

Paul Thompson said. Figuero, of Indianapolis, had a learner’s permit that expired on Sept. 30, Thompson said. Figuero was not arrested, Thompson said. Police are still investigating the incident.

electrocuted Thursday afternoon, Kansas City fire officials said.
When rescue workers arrived at the house on the southwest corner of 67th Street and Elmwood Avenue, they learned he was dead. They interviewed co-workers and determined that two painters had climbed a tall ladder to reach the roof and used a second smaller ladder, called a jump ladder, to reach a dormer, Kansas City Fire Battalion Chief Joe Vitale said. Vitale said electrical crews were investigating what caused the incident.
Worker killed in rail-car accidentSIOUX CITY, IA - A man performing salvage work on a rail car died Sunday when the
car's wall fell and crushed him, the Sioux City Police Department reported.
Jose Rodriguez,Jr. of Des Moines was pronounced dead at the scene. The accident occurred just south of 19th Street near Floyd Boulevard, according to police. The man was not working for Union Pacific Railroad, but the Gopher State Scrap and Metal firm from Mankato, Minn., a division of Alter Metal Recycling. Gopher had been contracted to work on an unused box car that had caught fire Sept. 29. Calls to Gopher State Scrap were referred to Bob Ellis of Alter Trading in St. Louis. Ellis said he couldn't comment on what happened as Rodriguez worked but said a company investigation is under way, as is an investigation by the Occupational Safety and Health Administration. Ian Delmar, 21, fell from the seventh floor - what is now the top of the building - 80 feet down an air-conditioning shaft in the Walter Cronkite School of Journalism and Mass Communication building around 9 a.m., Phoenix police spokeswoman Stacie Derge said. Delmar was taken to St. Joseph's Hospital and Medical Center, where he was pronounced dead. A crew from Tempe's Top Flite Construction Inc. on Tuesday was stacking steel so that workers could build an enclosure for the mechanical equipment that will eventually go on top of the building, said officials for Sundt Construction Inc., the general contractor.

backed into by a co-worker driving a front end loader during Wednesday morning's heavy rain. He was taken to the hospital and pronounced dead.
Milo H. Williams of Jacksonville had just given another co-worker some rain gear at Duval Asphalt Products on Philips Highway when the front end loader backed over him about 8:30 a.m. in the construction yard, according to the Sheriff's Office report. The driver had topped off a dump truck with asphalt and was backing toward a pile of more asphalt when it happened.

60-year-old employee at the Microtel Inn in Tucumcari. State Police Sgt. Andrew Tingwall said investigators are treating the death of
Nancy Archibeque of Tucumcari as a homicide because they believe she was
beaten to death. Tingwall said Archibeque was found by co-workers and local police responded. State police criminal investigators were then called to the scene. Tingwall didn't have any other details about the crime or what Archibeque's job was at the motel. Investigators have identified a suspect but no arrests have been made.

DeKalb, Wells machinery accidents leave 2 deadIN - Farm machinery played a role in two separate fatalities in northeast Indiana on Friday. A Waterloo soybean farmer was killed sometime after 2 p.m. while using a combine in a field off of County Road 10 in DeKalb County.
John Miller, 64, was using the combine to cut soybeans when the head of the combine struck a ditch and became stuck, DeKalb County Coroner Jeff Warner said. Miller got off the combine without turning it off and crawled underneath it to try to release the head of the combine. The head of the combine, which Warner said weighed about a ton, fell onto Miller and
suffocated him. Miller was found by a friend who went looking for him after attempts to reach Miller by cell phone were unsuccessful, Warner said. A combine also played a role in the death of a Warren man in Wells County. According to the Wells County Sheriff’s Office,

The crash also took out several feet of guardrail along the off-ramp. It will be closed for several hours as crews work to repair it. The man's identity has not yet been released.

Patrick J. Gear, 52, East Lyme, Conn., was pronounced dead at the scene at the plant that houses the distribution center for Anheuser-Busch at 11225 Xeon St. N.W. According to Capt. Cary Parks, Coon Rapids Police, it appears that Gear was killed by the semi-truck/trailer that he had been driving. A witness told police that he and Gear were talking while standing behind a semi-truck/trailer at the loading dock. Suddenly, the truck began move. While the witness was able to escape by running under the trailer, Gear apparently tried to run toward the cab and was pinned between the semi-truck that was moving and another semi-truck next to it at the loading dock, according to the Coon Rapids Police report.

wall panel collapsed at the construction site of the future T-Mobile Customer Service Center in Northeast Brownsville. Brownsville police spokesman Sgt. Jimmy Manrrique said a 9-1-1 call came into dispatch at 3:20 p.m. about the incident at the future call center site at 635 Paredes Line Road. Killed in the accident were a

49-year-old man from Toluca, Mexico, Manrrique said. Justice of the Peace Tony Torres pronounced the men dead at the scene. Benjamin, were buried in the rubble, state police said. Crews worked for hours Tuesday to remove them from tons of stone and debris. That wall collapse happened Tuesday morning near the community of Woolrich in Clinton County. The collapse happened when a newly-built concrete block wall collapsed under the pressure of backfilled stone. Emergency officials said the men were killed when the wall toppled, burying them in stone and cinder block. Dozens of emergency responders surrounded a barn on Dugan Lane near Woolrich, where, EMA officials said the two Amish construction workers were killed around 10 a.m. A straw hat belonging to one of the workers could be seen atop a pile of stone. Emergency workers said the unstable nature of the barn kept them from moving straight into recovery mode.

Caltrans worker dies after being hit by carDUBLIN, Calif.— A Caltrans employee who was
hit by a car as he was working on a San Francisco Bay area highway has died.
Djuan Bush, 24, of San Leandro, was pronounced dead late Tuesday at a Castro Valley hospital, officials with the Alameda County Coroner's office said Wednesday. Bush was working at the Interstate 580-680 interchange when he was struck shortly after noon Tuesday. CHP Office Steve Creel said the accident occurred when the driver of a Porsche reportedly lost control, hit a Caltrans truck, bounced off and hit Bush before striking another truck. The Caltrans crew was working on guard rail maintenance. Creel said several trucks were parked along the shoulder and signs warned drivers to slow down. The driver of the Porsche was uninjured. Cause of the crash has not been determined, Creel said. Gov. Arnold Schwarzenegger said Capitol flags would be flown at half-staff in honor of Bush.
Two killed in truck crashTX - Two men were killed Thursday afternoon in a
collision between a cement truck and an 18-wheeler, officials said. The 2:30 p.m. wreck occurred on SE Loop 410, near the intersection of Southcross, when the driver of the cement truck, which was traveling north, lost control and crossed the median into the southbound lanes, said San Antonio Police Sgt. Gabe Trevino. The truck struck the 18-wheeler, driven by a Wal-Mart employee. The cement truck also hit a small white sedan, which spun to the side of the road, before crashing head-on with the Wal-Mart truck, said Trevino, the department's spokesman. Both truck drivers died at the scene. Crystal Acevedo, 19, and her 7-month-old daughter Natalie Rose, escaped from their sedan with no injuries. Shortly before 5 p.m., authorities opened one lane on the southbound access road of Loop 410. All other southbound lanes were closed at Southcross exit. Officials said buses from nearby elementary schools was diverted so that students would not sit in traffic after school.
Tree trimmer electrocuted in MarinWOODACRE, CA - A
26-year-old San Rafael man was
electrocuted by a 12,000-volt power line as he trimmed a redwood tree in Marin County, authorities said today.
Carlos Rivera-Olvera was killed as he worked on the 100-foot-tall tree at 16 Silvan Way in Woodacre a little after 2 p.m. Monday, said Marin County Fire Battalion Chief Mike Giannini. He was about halfway up the tree, still tethered to the trunk, when rescue workers arrived. "His body was contacting the power line that was in proximity to the tree," Giannini said. "It's hard to really speculate exactly what happened - based on the proximity of the wire to the tree, it's certainly possible that he accidentally contacted the wire, or possibly a tool or falling branch did." Giannini said the power line was about 3 feet from the tree in an area that was relatively free of branches. Rivera-Olvera worked for Efrain Landscape Construction in San Rafael, Giannini said.

28-year-old man was
killed in a wreck at the Zoo Drive and South Frontage Road intersection this afternoon. The unidentified man was driving a Big Sky Linen truck when the wreck occurred shortly before 2 p.m., Montana Highway Patrol Trooper Dell Aman said. The man was not wearing a seat belt and was killed when he was ejected during the wreck. It took emergency workers, including crews from two Billings Fire Department engines, about 45 minutes to free him from under the delivery truck. Aman said the man, who was traveling north on Zoo Drive, failed to stop and crossed South Frontage Road where the truck broadsided a Ford pickup. The linen truck rotated and flipped onto its passenger side on the east end of the median. The pickup, which was traveling west, spun and came to a stop at the west end of the median. The driver's side of its crew cab was severely damaged in the wreck.
Smoke inhalation killed 5 workers in pipeline fire, coroner concludesCO - The Clear Creek County coroner's office has determined that five painters killed last week during a
fire in a pipeline above Georgetown died from asphyxiation caused by smoke inhalation. The five men - employees of RPI Coatings Inc. of Santa Fe Springs, Calif. - died Oct. 2 when an epoxy coating they were spraying on the inside of water pipes ignited. The workers were identified as
Donald Dejaynes, 43; Dupree Holt, 37; James St. Peters, 52; Gary Foster, 48; and Anthony Aguirre, 19. They were all from California. Xcel Energy owns the pipe, which is used to power a hydroelectric turbine on the north side of Guanella Pass above Georgetown. The Occupational Safety and Health Administration is investigating the cause of the fire along with the Colorado Bureau of Investigation.

Warren Askey of Phillipsburg was pronounced dead at Lancaster General Hospital after he was found unconscious on top of the building about 3 p.m., investigators said. Askey was constructing a ship's ladder on top of the building for Reynolds Iron Workers of Williamsport. Mike Callahan, executive vice president of Benchmark Construction — the company spearheading renovations of the facility — said Askey had worked for Reynolds for more than 25 years. Callahan said a Benchmark roofer found Askey motionless on a piece of plywood and called 911 at 2:55 p.m. Askey's body had no signs of trauma or injury, and investigators believe the man had a heart attack while working under a scorching sun.

falling from the roof of the new building for the Walter Cronkite School of Journalism and Mass Communication at the Downtown campus.
Ian Delmar was assisting a crane when he fell into a hole that was about 80-feet deep shortly before 9 a.m., said Detective Stacie Derge of Phoenix police. The man fell from the seventh floor of the building located at the intersection of First and Fillmore streets, said Charlie Boyd, a spokesman for Sundt Construction, Inc. Sundt is leading the construction on the new Cronkite building. "We know he fell through a ventilator shaft," Boyd said. "We know the ventilator shafts are normally covered and marked. [But] we don't know what happened in this case." It looked as if the man was walking backwards when he fell through the shaft, said Capt. Victor Rangel of the Phoenix Fire Department. "The shaft was covered with plywood and well marked," Rangel said. Fire officials attempted to resuscitate the man while they transported him to St. Joseph's Hospital and Medical Center, Rangel added. Delmar was working for Top Flite Construction, who had been subcontracted by Sundt, Boyd said.

Orange Beach firefighters and paramedics were called to the Gulf-front scene at about 2:30 p.m. to respond to a man who had fallen several stories and was not breathing. Working on what will eventually be Brett/Robinson's 358-unit, 32-story condominium tower, Long fell from the 10th floor, just below the building's highest completed level, police said. Authorities said that during his fall Long struck a protruding deck used to collect construction debris. "Despite aggressive resuscitation efforts, the patient was pronounced dead at the scene," Orange Beach Battalion Chief Kevin Lanford wrote in a news release. Orange Beach police are investigating the matter, but Lt. Steve Brown said that the fall appears to be an accident. Long, an employee of Georgia-based United Forming Inc., was setting a string midway between the 10th level's floor and ceiling to be used as a reference point for future work, Brown said.
Pipeline worker killed at Harriman State ParkHarriman State Park, NY — A construction worker was killed this morning when he was
thrown from a machine he was operating and it rolled over him.
Pat McCaffrey , 67, of Lebanon, N.J., was working on the Millennium Pipeline, an underground natural-gas pipeline that runs through the state park, said Michael Armiak, Millennium spokesman. McCaffrey was an employee of Precision Pipeline, which has been contracted by Millennium to build the pipeline. The accident happened at around 9 a.m. McCaffrey was sitting in the enclosed cab of a side-boom, picking up the new pipe and putting it in place using a belt attached to a hanging chain from the machine, when he was thrown, Armiak said. How and why he was thrown is still being investigated by State Park Police. The scene was not cleared until early afternoon and this evening, police were still interviewing people as part of the investigation.

The tractor's engine stopped, and when he restarted it, the machine began to roll forward. The man ran in front of the tractor in an attempt to jump onto its seat, Singh said, but instead he got caught in front of it. Westport and Ceres fire departments responded. The man was flown to Doctors Medical Center in Modesto, where he was pronounced dead, Singh said.

Byron Mitchell, 29, of Chicago on Aug. 4 during the commission of a burglary and robbery. Plant Fined Following Worker’s DeathGAFFNEY, S.C. -- The Nestle frozen food plant in Gaffney will have to pay a fine for safety violations found in connection with the death of an employee who was killed at the plant in July.
James Redmon, 65, died while he was working on a crane, performing routine maintenance in the freezer tower. Redmon was a mechanic at the plant. Cherokee County Coroner Dennis Fowler said that Redmon was
trapped and suffered head injuries. He was pronounced dead at the plant. The Occupational Safety and Health Administration cited the plant for not having specific procedures posted near the crane that Redmon was operating. OSHA also said that Redmon didn’t follow the proper lockout procedure.
I-70 worker killed while pulling conesMorrison, CO - A worker for a state highway subcontractor was killed on Interstate 70 about 3:30 p.m. Tuesday when he apparently
fell off a truck while picking up traffic cones and the truck ran over him twice. The accident occurred about a quarter-mile west of the Morrison exit in the far-left westbound lane. The workers were there because a 6 1/2-mile stretch of I-70 from West Colfax Avenue to the Lookout Mountain road is being resurfaced. Two of three westbound lanes of I-70 were closed while the sheet-covered body was removed and the incident was investigated. Trooper Gilbert Mares of the Colorado State Patrol said the accident happened when a white Chevy pickup owned by American Barricade Rental was backing up. Mares said the truck was going "very slow - obviously not highway speed." The driver was wearing a seat belt. The victim, a
43-year-old Denver man who was not identified, was sitting or standing in the back of the truck and fell as he picked up cones, Mares said. The truck driver, David A. Bostrom, 32, of Centennial, "felt some type of a bump," Mares said, and pulled forward, running over the man again.

Cabbie killed this morning in East orangeNJ - A Police in East Orange found a cab driver dead at the wheel of his vehicle this morning from a
gunshot wound. The victim was identified as
Maney Orius, 56, of East Orange. Relatives said Orius came to the United States from Haiti in 1990 and began driving a taxi 10 years ago. Before that, he held a factory job. The death was ruled a homicide but few other details were available, said Detective Andrew DiElmo, a spokesman for the East Orange Police Department. He did say police discovered the taxi on South Grove Street, just below Central Avenue. The Diane Dell cab had jumped the curb, struck a tree and run into a fence at the Holy Sepulchre Cemetery, said Celony Estiverne of East Orange, a brother of the victim. The two lived in the same home.

steamroller pinned him against a parked car as he filled a pothole on a quiet side street, authorities said.
Douglas Van Valen, 28, of New Milford, who was a volunteer firefighter in Oradell and in his hometown, was pronounced dead at Hackensack University Medical Center late Tuesday afternoon. Van Valen, a Navy veteran who leaves behind a fiancee, became trapped against the car on Reis Avenue when the paving machine lurched forward as another DPW worker operated it, police and town officials said. Investigators were trying to determine if the machine malfunctioned, Oradell Police Chief Rhynie Emanuel said. "Most would say it's a routine task, but when heavy equipment is involved, it's always something to be very careful with," Borough Administrator Wolfgang Albrecht Jr. said.

bullet fired during an early morning robbery killed one restaurant employee and wounded another Saturday, police said. The two robbers escaped and remain at large. The robbery, at the Hardee's restaurant in the 400 block of Denbigh Blvd., was reported at 5:24 a.m., said Harold Eley, a police spokesman. When officers arrived, they found three employees in the restaurant, two of them suffering from gunshot wounds. Both were taken to Riverside Regional Medical Center. The woman, identified as

Worker dies in Aurora forklift accidentCo - A 32-year-old worker at a Centennial masonry company was killed Friday when a forklift he was using rolled over onto him. The accident happened at about 1:45 this afternoon at United Masonry, which is in the 15300 block of East Hinsdale Circle. South Metro Fire Rescue spokesman Andy Lyon said the man was using a large forklift to raise and push a flatbed trailer up a slight incline from the a parking lot to a fenced-in yard. At one point, the man climbed out of the forklift to make sure everything was secure, and the trailer began to roll down the incline. Lyon said the trailer pushed against the forklift, causing it to tip. The man ran back to the forklift to try to move it, but the forklift fell onto him, Lyon said. The man's name was not released to give officials time to notify his family.
Drowned worker identified
IL - The Cook County medical examiner's office Thursday ruled a 16-year-old worker's death in Long Grove was a drowning. Valentin Gutierrez of Alsip drowned after falling into a swimming pool while working a job hanging drywall Wednesday on the 2300 block of Coach Road, Lake County sheriff's police said. The victim didn't know how to swim. Gutierrez was working with family members, said police, who have concluded their investigation of the accident.
Oil Field Worker Killed
This is the second accident in three weeks that happened outside Morton at a pumping station, and according to the county sheriff, this is simply a dangerous business. "Anytime people work around machinery... Some little something goes wrong... An unexpected and it happens," Cochran County Sheriff Wallace Stalcup said. Yesterday morning, a crew was removing cycle rods from the ground. Those rods go into the ground to pump the oil out and as one was being removed, it got bent, snapped and instantly killed 56-year-old Levelland resident Jesus Chavira. "He was a real good man... He was there for everyone. He was a caring man," Chavira's niece Veronica Perez Torres said. Torres added the company who owns the well, North Premium Well Services has only called the family once. "We didn't get to hear anything until today. She was supposed to be notified the same day and nobody notified her about anything," Torres said. NewsChannel 11 also called the company several times Friday, left several messages and never got a return call. But we do know that as of now this issues is in the hands of OSHA to determine an exact cause.
Storage Facility Manager Killed During Robbery
HOUSTON, TX -- A manager of a northeast Houston storage facility was killed during a robbery, KPRC Local 2 reported. Houston police said two men robbed the Value Self Storage on the feeder road of the Eastex Freeway near Lynnfield Street at about 9:30 a.m. Friday Investigators said the robbers confronted the female manager in the office, demanded money and fatally shot her."The lady was in shock. She couldn't believe it. She came to apply for a job and got caught up in a holdup and a shooting. And if it hadn't been for the fact that she told me to start the day at 10, I would have been here," employee Barbara Petite said. Petite said the manager told her to come in later than her scheduled time and that may have saved her life. "Why did you have to kill her? Why did you have to shoot her? Why did you have to do that?" Petite asked. "That's the part -- to just know it was for nothing. There was hardly any money here." The robbers escaped in the job candidate's white Dodge Caravan with Texas license plate 349 KXR. The general manager of the storage facility said he was in the process of installing surveillance cameras, but they were not yet in place to record the robbers.
State probes landfill worker's death, No witnesses as man may have been hit by a truck.
CA - The state has launched an investigation into the death of a Sacramento County worker who was killed in an apparent accident at the county landfill Monday, officials said Thursday. The California Division of Occupational Safety and Health said it will look into the circumstances surrounding the death of Douglas Aldrich, 52, of Elverta, who was pronounced dead Monday morning after being transported from the landfill on Roseville Road to a local hospital. A heavy equipment operator, Aldrich had been with the county since 1993. He was fatally injured at 5 a.m., officials said.
Farmer killed in rural Gibson City
IL - The Ford County Sheriff says it's an unfortunate but common accident that can happen this time of year. A farmer was killed Tuesday night in rural Gibson City, near the McLean County Line, as he was mowing. Sheriff Mark Doran says 61-year-old Michael Schroeder's family became worried when he didn't return at dusk, and a neighbor found Schroeder around midnight less than a mile from his home.

VA - A contract driver for the U.S. Postal Service, a single mother of three, was killed at the Dulles processing and distribution center when she was pulled under a tractor-trailer and dragged, authorities said yesterday. Katherine N. Sowers, 49, who lived in Leesburg, had driven her box truck into the center's docking area Tuesday when the tractor-trailer was pulling out, a Loudoun County Sheriff's Office spokesman said. "Somehow she got caught beneath the truck," spokesman Kraig Troxell said. He could not say how far Sowers was dragged. No charges have been filed, he said. Authorities were called to the center, at 44715 Prentice Dr., just after 3 p.m. Sowers was pronounced dead at the scene, Troxell said.
Men die of injuries from boom lift
GA - Two Warner Robins men hospitalized a week ago today after the portable boom lift they were in suddenly fell over have died. Travis Scott Register, 27, died Saturday at The Medical Center of Central Georgia. Robert Jay Stephens, 28, died Friday at the Macon hospital. The cause of the accident is under investigation by the Occupational Safety and Health Administration. The two men were working on the flashing near the roof line of the Flint Energies building under construction on Russell Parkway in Warner Robins when the boom lift toppled over, Warner Robins police Maj. John Wagner said. Mark Christianson, project manager for International City Builders of Warner Robins, the general contractor, said Tuesday he does not know what caused the lift to become unbalanced and tip over. He said the men were standing in the basket of the lift about 27 feet in the air and that their harnesses were attached to the lift as required. "OSHA has been on site. They have not indicated that anything was done improperly," Christianson said. "We've looked at the site ourselves. "We're not sure what the cause of the accident was. Everything seems to have been followed. ... The guys had their hard hats on, their harnesses. ... They'd been working in that area. ... We're just unsure what was the cause but OSHA was still investigating," he said. Thursday and Friday, work continued on the job site with skeleton crews. Full work crews were back on site Tuesday. Register and Stephens worked for Southern Glazing Services of Perry, which was performing subcontracting work on the site.
Farmer killed after mowing tractor rolls over
GIBSON CITY, IL — Ford County authorities have identified the farmer who was killed early Wednesday morning in a mowing accident as Michael J. Schroeder, 60, of rural Gibson City He was pronounced dead at the scene at 1:30 a.m. Wednesday at county roads 300N and 70 E west of Gibson City. Ford County Sheriff Mark Doran said Schroeder was mowing along the edge of a creek in that area when a tire of his tractor apparently slipped off the edge of the waterway. The tractor rolled on top of him. Schroeder was found when family members started looking for him when he did not return home at dusk. Coroner Doug Wallace said the accident remains under investigation.
Dayton Farm Accident Claims 15-Year-Old, Equipment Detaches From Tractor, Hits Youngster
DAYTON, VA — A farm accident claimed the life of a 15-year-old boy in Dayton Monday, according to Sgt. Felicia Glick of the Rockingham County Sheriff’s Office. Eric James Koogler of Harrisonburg was fatally injured shortly before 4 p.m. at a farm on Ottobine Road after he was hit in the torso by a piece of farm equipment that became detached from a tractor, Glick said. When a deputy arrived on the scene at about 4 p.m., Glick said, Koogler had been transported to Rockingham Memorial Hospital. Glick said he was then taken to the University of Virginia Medical Center in Charlottesville, where he died from internal injuries. Lee Roy Hensley owns the farm, according to Glick. Hensley could not be reached for comment. Jimmy Maass, safety coordinator for the Virginia Farm Bureau, said this type of accident is rare but farm workers should always be vigilant about equipment safety.
Worker dies of injuries suffered in ammonia release
WATERLOO, IA - Elizabeth Myers died Sunday of injuries suffered when ammonia was accidentally released at a Beef Products Inc. plant, state labor officials said Tuesday. Myers was one of several workers injured when a BPI employee opened a valve that wasn't locked causing the ammonia to be released on Friday, officials with the Iowa Labor Commissioner's Office said. One other worker was taken to the burn unit at University Hospitals in Iowa City. Several others were taken to a hospital but no information was available Tuesday about their conditions, officials said. The accident happened as contractors were working to hook up a new ammonia piping system. The release caused the evacuation of the plant, the neighboring Tyson Fresh Meats plant and a third business in the area. About 235 people work at the BPI plant, which produces low-fat beef products. The Iowa Division of Labor Services was investigating the ammonia release.
Carwash employee dies at Goo Goo wash
WYOMING, MI — A full-time carwash employee died after becoming entangled in the automatic brushes of a franchised Goo Goo Express Wash here, according to an Associated Press report. The Sept. 1 story said Ricardo Martinez was closing the carwash when a customer returned to have his vehicle washed again. After the vehicle entered the carwash, Martinez went inside to clean the tunnel and became entangled in its automatic brush. The article said a customer found his body in the carwash. It did not say if there were any other employees working at the express wash at the time of his death. According to a report on Fox 17, police reviewed surveillance tape to rule out foul play. The case has been ruled an “industrial accident” and turned over to the state’s OSHA office.
The TV report said Martinez graduated high school earlier this year. He had been employed at the Goo Goo wash since March and was a full-time employee trained in the wash’s safety policy.
A Marathon employee dies at Garyville plant
CA - A newly hired Marathon Oil Co. employee died in a motor vehicle accident at the Garyville refinery over the weekend, a company spokeswoman said Tuesday. Jackie Batiste, 34, of Hahnville was fatally injured after he accidentally drove a company vehicle into a retention pond at the refinery on Friday night, said Angelia Graves, a Marathon spokeswoman. Graves said it appears as though Batiste, who was on duty at the time of the accident, drowned. An autopsy is pending to confirm an official cause of death, she said. An internal emergency response crew at the Garyville refinery arrived at the scene of the accident around 11 p.m. Friday, Graves said. She said further information about how the response team was called to the plant's retention pond -- which collects rainwater -- is still under investigation.
Worker Killed at Beaumont Construction Site
CA - Authorities Tuesday released the name of a contract worker who suffered fatal injuries in Beaumont while replacing an electrical transformer that failed during the heat wave. The California Department of Industrial Relations' Division of Occupational Safety & Health has opened an investigation into the death of 50-year-old Richard Eastman, said spokeswoman Kate McGuire. Eastman, who worked for Kansas City Mo.-based PAR Electrical Contractors Inc., was fatally injured while working on a power pole in an alleyway in the 1300 block of Massachusetts Avenue about 9 Monday night. He had been sent out on an emergency call to restore power in the area serviced by Southern California Edison, said Jim Stapp, his boss at PAR's Fontana office.
Pilot found dead in plane wreckage

NEWTON, Ga. — A pilot was found dead in his crashed single-engine aircraft after a four-day search in Alabama and Georgia.
The crash site was found Monday at Ichauway Plantation in rural Baker County in southwest Georgia.
The pilot, Kyle Boss, 22, died in the crash.
The Bellanca single-engine aircraft was reported missing Friday afternoon while on a flight from the Decatur County Industrial Airpark in Bainbridge to Weedon Field in Eufaula, Ala.
The Alabama and Georgia Wing of the Civil Air Patrol had been searching for the plane amid concerns it may have crashed into Lake Eufaula.
Authorities found the crash site, which was 10 miles south of Newton, after a resident told them he had seen a small plane nose-dive above a forest about two miles from his home Friday afternoon.
