Weekly Toll: Death in the American Workplace

Volney man killed in garage accident

April 19, Volney, New York – State Police in Fulton have investigated a freak accident where a flatbed truck fell on top of a mechanic killing him. At about 10 a.m. last Thursday, police responded to a report of a man pinned underneath a vehicle at 3054 County Route 4 in the Town of Volney in Oswego County. The investigation revealed that the owner of Wally's Towing, Paul Sage, 66, was performing mechanical work under his 1995 GMC flatbed tow truck. A second tow truck, owned by Sage, raised the front end of the flatbed using a linked chain, which subsequently broke, resulting in the flatbed truck falling on top of him.


Worker Dies in Accident at KCP&L Plant

April, 19, Lacygne, Missouri - A man died in an accident at KCP&L's LaCygne Generating Station early Saturday morning, according to a spokesman for the utility. According to KCP&L, the victim was a contract worker at the generating station in LaCygne, Kansas. The name and age of the victim, as well details about how he was killed, have not been released. Officials say an investigation into the accident is continuing. The LaCygne facility is co-owned by Westar and KCP&L.


Santa Ana man dies after 4-story fall at construction site

April 19, Dana Point, California – A Santa Ana man fell from the fourth floor of a Marriott hotel and died of his injuries Saturday, Orange County sheriff's officials said. Construction worker Daniel Varela Jr., 59, was declared dead at 12:05 p.m. at Mission Hospital Regional Medical Center shortly after he plunged four stories. Varela was helping with construction efforts on a new wing of the Laguna Cliffs Resort & Spa at 25135 Park Lantern in Dana Point, according to Sheriff's Department spokesman Jim Amormino.


Branford Police ID Dead Man in Truck

April 17, Farmington, Connecticut - Branford Police have identified the man found dead in the trailer part of a tractor trailer at the T.A. Travel Center Friday morning. An autopsy on the body of Dale Lynn Anderson, 47, of Redlands, California was scheduled for Saturday, police said. Investigators have labelled the death an apparent homicide. The driver called police about 10:44 a.m. Friday. Anderson's body was removed from the scene shortly after 5:00 p.m. Friday, and taken to the Office of the Chief Medical Examiner in Farmington.


Cincinnati man dies in I-75 crash

He was driving one of three semi-trailers involved in a chain reaction.

April 17, Harrison Township, Ohio — A 48-year-old Cincinnati man was killed Friday, April 17, when his semitrailer slammed into the rear of a truck that had come to a stop along Interstate 75 south to avoid blocked traffic, the Dayton post of the Ohio Highway Patrol said. Lydel Turner was pinned and crushed in the cab of his Pitt Ohio Express rig in the accident about 4:24 a.m. on the interstate between Needmore Road and Wagner Ford Road, patrol Sgt. J.C. Chesser said. The roadway was shut for about five hours after the accident, the patrol said.


2 Dead, 1 Arrested In Warehouse Shooting

April 17, Philadelphia, Pennsylvania - Two people are dead after an armed man opened fire outside of a business in Bristol Borough, Bucks County. It happened around 3:30p.m. Friday at the Simon & Schuster publishing warehouse on the 2200 block of Radcliff StreetPolice said a man, who is a former employee, walked up to the front door of the building and started shooting. He was taken into custody at the scene, wearing a t-shirt that read, "Stupidity Is Not A Crime...So You're Free To Go." Co-workers identified the suspect as Robert Diamond. They said he was a disgruntled and confrontational employee who walked off the job four months ago. ("Did you hear any gunshots?" Fox 29 Reporter Nefertiti Jaquez asked a witness. "I only heard two.") One of those gunshots killed 52-year-old Reggie Woodson, who was leaving the warehouse, but never got to his car. The husband and father of four was shot and killed by the rear-loading dock. He was a forklift operator at the company since 1988.


Forrest bus driver dies after wreck

April 17, Hattiesville, Mississippi - A Forrest County school bus driver involved in a collision with a log truck Wednesday died Thursday in Forrest General Hospital's intensive care unit. The cause of death could not be determined for Jackie Keys, 50, of Glendale, who'd been a bus driver with Forrest County School District for more than 10 years. Forrest County Coroner Butch Benedict said an autopsy will be performed. Keys had just dropped off students Wednesday morning at Earl Travillion Attendance Center and was headed back to her home when the bus she was driving rear-ended a loaded log truck on the Glendale Avenue Bridge spanning the Bouie River.


Houston building collapse kills 1 construction worker-injures 2

HouTex Inn building renovation site collapses killing 1 construction worker.

April 17, Houston, Texas – Houston Fire Department and Houston Police Department responded to a 911 building collapse call on Tuesday at the old HouTex Inn. Construction workers at the southside motel, located in the 6300 block of the Gulf Freeway, were replacing joists on the first floor when the building undergoing renovations collapsed ontop of the unsuspecting laborers. Supervisors for Camden Builders, the contractor hired by Houston city officials and New Hope Housing partnership, stated four men were working at the site when the unstable structure collapsed. When the roof and supporting structures toppled and collapsed, the building renovators were crushed by hundreds of pounds of lumber and concrete. Houston emergency search and rescue teams used dogs, cameras, and high powered tools to cut through the roof, debris and rubble to rescue the trapped workers.


2 dead, 1 critical in SoCal hospital shooting

April 16, Long Beach, California — A hospital employee shot another employee to death, critically wounded another and then killed himself Thursday, police said. Police Chief Anthony Batts said the violence erupted just before noon at Long Beach Memorial Medical Center. Batts would not identify those involved but said they were all male. He said the motive remained under investigation, but noted it came amid a flurry of recent shootings in the U.S.


4th Oakland Police Officer Dies

April 16, Oakland, California - A fourth Oakland police officer gunned down in a weekend shootout with a wanted parolee has been taken off life support. Sergeant John Hege was taken off life support this morning at Highland Hospital. The Alameda County Coroner's office confirms that his body was delivered to them under police escort this morning. Sergeant Hege was a veteran traffic officer who was shot during a traffic stop on Macarthur Boulevard and 74th avenue.


Man dies after falling at Piscataway construction site

April 15, Piscataway, New Jersey — A Burlington County man died late Wednesday afternoon after falling at a construction site on New Brunswick Avenue, police said. The construction worker, a Delran resident, fell about 10 feet and suffered head trauma minutes before 3:30 p.m., Lt. Kenneth Blair said. He was taken to Robert Wood Johnson University Hospital in New Brunswick, where he died about an hour after the fall. Police were withholding the man's identity pending notification of next of kin, and did not immediately know his age.


Worker Killed at Highway 65 Construction Site

April 15, Fair Grove, Missouri -- A Columbia man is dead after a construction accident along Highway 65 just north of Fair Grove. The Missouri Highway Patrol says 53-year-old Lance Trager was standing next to a tractor-trailer. As the semi pulled forward, Trager fell under the wheels of the trailer and was run over. He died at the scene. A patrol reconstruction team was called in to investigate. However, it appears the truck was moving to move some equipment.


Independence County Worker Struck, Killed In Roadside Accident

April 16, Little Rock, Arkansas - An Independence County worker cleaning up storm debris has died after being hit by a car. Authorities say 53-year-old Jimmy L. Jackson was cleaning up debris from last week's storms along a road when he was struck and killed Thursday morning. Police are still investigating the incident. So far, there's no word on who the driver was or whether any charges will be filed.


Truck driver killed in early morning crash

April 15, Danville, Pennsylvania - A unidentified truck driver was killed early Thursday morning when his rig ran off a exit ramp from Interstate 80 and crashed into a stream. The load on the trailer shifted and crushed the tractor. State police at Milton reported the 1:40 a.m. accident happened at Exit 199 (Mile Run). The rig apparently went down the exit ramp, failed to stop at the stop sign and continued through the intersection. It crashed through the guide rails across the intersection, then became airborne and struck the opposite bank of a small stream.


A-1 employee dies from work injury

April 14, River Falls, Wisconsin - An accident near Lakeside Foods in New Richmond at 8:30 a.m. on Tuesday, April 14, led to the death of one man. Eugene Hakes, of Cadott, was cutting a water pipe when his saw kicked back, striking his throat. The New Richmond Ambulance Service responded and transported Hakes to Westfields Hospital, where he was pronounced dead. Hakes was working for A-1 Excavating at the time of the accident. The company had been contracted by the city to install new water meters near Lakeside Foods.


Worker dies after truck runs over him on U.S. 65

April 15, Springfield, Missouri - A man doing construction work on U.S. 65 in Fair Grove died after he was run over by a tractor trailer. Columbia resident Lance Trager was 53. He was pronounced dead at the scene, according to a report from the Highway Patrol. The accident occurred around 4:10 p.m. on U.S. 65 in Fair Grove. Trager was standing next to the tractor trailer. As it pulled forward Trager fell under the wheels and was run over, the report from the Highway Patrol said.


Deadly Farm Accident

April 15, Eau Claire, Wisconsin - It happened northwest of Bloomer about 10 this morning on a farm at Highway 64 and County Road "DD". Chippewa County Coroner Kay Gerrits says 55-year-old Michael Johnson was working for a local feed mill. She says he was at the farm loading grain into a bin, when his clothes got caught in a piece of machinery. Johnson was working for Ed's Feed Service based in Bloomer. Jay Ruf, who owns Ed's Feed Service, says as Johnson knelt down to load the grain, his shirt got caught. Coroner Kay Gerrits says the farmer tried to free him by cutting his clothing and then called for help but it was too late. He was pronounced dead at the hospital in Bloomer.


Illinois man killed in accident at cement plant

April 15, Ste. Genevieve, Missouri - An Illinois man is dead after an accident at the Holcim (US), Inc. cement plant in Ste. Genevieve Tuesday. Vincent Lavite of Wood River, Illinois died after some type of accident at the plant around 3 p.m. Tuesday, according to the Ste. Genevieve county coroner. Lavite was taken to a hospital in Festus and later air-liefted to Barnes Hospital in St. Louis where he died. An autopsy is expected to be conducted Wednesday.


Schnucks security guard is shot, killed by suspected shoplifter

April 15, East St. Louis, Missouri - A grocery store security guard died Tuesday afternoon after a suspected shoplifter grabbed his gun and shot him twice, East St. Louis police said. Wayne Groom, 43, of Smithton, died in the shooting, St. Clair County Coroner Rick Stone said. Groom was shot in the back of his head and in his right leg. An East St. Louis man was in police custody. The shooting occurred at 2:20 p.m. Tuesday at the Schnucks supermarket, 2511 State St.


Texas Man Dies In Heavy Equipment Accident

April 15, Kilgore, Texas — Donald Joseph Cain, 71, of Clarksville died when a piece of heavy equipment he was operating overturned. Kilgore police said Cain was operating a Caterpillar roller Tuesday when it overturned on a slope. Cain was thrown from the cab and pinned beneath the equipment. Police say Cain was dead at the scene. An autopsy was ordered. Kilgore police said Cain was an employee of Stephenson's Dirt Contractors.


Man killed in Buckhead by tree loved family, work, church

April 14, Atlanta, Georgia - At 5:30 a.m. Monday, Faye Jones walked her husband, Ray, to his truck. She carried his lunch and coffee for him. Mrs. Jones said “I love you” and “be careful.” Mr. Jones said “bye” and kissed her. That was the last time the couple would perform their morning ritual. Four hours after leaving his Ball Ground home north of Atlanta for a plumbing job in Buckhead, Raymond Jones was crushed to death by a falling tree. Raymond Jones attended Easter Sunday services at their church, Concord Missionary Baptist, in Fairmount one day before his death. “He loved his job, he loved his work,” said Faye Jones. “I’m glad he went like that. I didn’t want him to go, of course, but he died working.” Ray Jones, 63, provided plumbing services for a well-heeled list of clients in Buckhead and north Atlanta, his wife said. At around 9:30 a.m. Monday, Jones was driving south on Nacoochie Drive toward Peachtree Battle in Buckhead, when high winds uprooted an oak tree that fell on his truck, crushing the cab and likely killing him instantly, authorities said.


Investigation into firefighters' deaths begins

April 13, Houston, Texas - It was a somber day for Houston firefighters as they honor two of their own who died battling a house fire on Easter morning. Shortly after noon Monday, the bodies of Captain James Harlow and probationary firefighter Damion Hobbs were transported from the medical examiner's office, while their colleagues stood at attention. Both men died after being trapped inside the home. Investigators from several agencies made a thorough sweep of the gutted house on Oak Vista near DeLeon in southeast Houston Monday. They were there all day with representatives from the U.S. Bureau of Alcohol, Firearms, Tobacco and Explosives, the state fire marshals' office, the National Institute of Occupational Safety and Health, as well as investigators from the Houston Fire Department's arson division. It's their task to find out every detail of the fire.


Shooter Remains At Large

April 13, Richmond, Virginia - A day after a delivery driver was shot and killed in Henrico County, other drivers across the region are taking precautions. Yong Sun, a driver for The Great Wall Restaurant in Henrico's east end, was killed Sunday night after making a delivery. Driver Tim Nelson finds the killing hard to understand. "I can't believe someone would get killed over a twenty dollar delivery," Nelson said while driving and making his usual run Monday evening.


BREAKING NEWS: Man Dies In Elevator Accident At Former Holiday Inn

April 13, Columbus, Ohio - A 52-year-old elevator technician died today when an elevator car plunged down the shaft and crushed him at the former Holiday Inn on Lane Avenue. James C. Higgins, an Oracle Elevator Company employee, was at the bottom of the shaft about 8 a.m. on a ladder. He and a co-worker were preparing the elevator so workers could remove furniture from the closed hotel, Columbus Fire Battalion Chief Dave Whiting said. He said mechanical or human error may be responsible for the accident. "Because the elevator they were working on wasn't the one that came down, they may have not shut the elevator off," Whiting said.

Gippsland Asbestos Related Diseases Support Inc. are conducting their Annual Wreath Laying Ceremony to coincide with International Workers’ Memorial Day in memory of workers who die, are injured or fall ill as a result of their work.The Ceremony will take place in Centenary Rose Garden, Commercial Road, Morwell, Victoria, Australia at 11.00am, on Tuesday 28 April. Austria

ATUF The Austrian Trade Union Federation, in cooperation with its Construction and Woodworkers Union, will organise a manifestation on April 28, 0900 in a public square in Vienna (Keplerplatz, 10th district).
The manifestation will be held next to an existing memorial of construction workers who died because of work accidents.
Apart from general information on the significance of the day, the event will focus on the particular health and safety problems of short-term and informal workers.

Benin

l’Union Nationale des Syndicats de Travailleurs du Bénin (UNSTB) Alors que les syndicats dans le monde commémorent les décès, les blessures et les maladies professionnelles lors du 28 avril, la République du Bénin à l’instar de la communauté africaine célèbrera la 13me Journée Africaine de la Prévention des Risques Professionnels à Parakou le jeudi 30 avril 2009. Le thème retenu cette année est intitulé : « Santé au travail : Mieux Prévenir les Risque, Mieux Protéger les Victimes » Quant à l’Union Nationale des Syndicats de Travailleurs du Bénin (UNSTB), elle l'a déclaré "Journée nationale de la mise en œuvre du Régime d’Assurance Maladie Universelle (RAMU)" En effet, au Bénin moins d’un million de personnes sont couvertes par des mécanismes de protection sociale, soit moins de 12,5% de la population béninoise. Or, les mutuelles de santé ont montré leur capacité à organiser les populations face à l’offre de soins, notamment dans le cadre de la recherche de la qualité des services de santé. Il s’agira pour nous de marquer notre accord et de forcer la main au gouvernement pour qu’il accélère le processus de mise en œuvre de la protection de la vie et la santé au travail à travers le RAMU.

Confederation of Benin Trades Unions The Confederation of Benin Trades Unions (UNSTB) will host events on the prevention of work hazards on 30 April. The theme will be: ‘Health at work: Better warnings about risks, mean better protection of the victims.’ The unions will highlight the need for everyone to have access to the universal health insurance scheme (RAMU). In Benin, fewer than 1 million people have this coverage, less than 12.5 per cent of the total population. The unions will put pressure on the government to quickly implement related measures to deliver better health and safety at work.

ITUC update on CTC-CLC activities


Canadian Auto Workers Canadian Auto Workers (CAW) has issued a briefing on Worlers' Memorial Day observing that this is the 25th anniversary of the National Day of Mourning. CAW local union presidents, CAW President Ken Lewenza stresses "that it's critical to reaffirm that health and safety is a fundamental right for all workers. Occupational cancer is a major concern." He continues, "we must pressure the federal government to stop the export of chrysotile asbestos and lobby all provincial governments to ban and have all forms of asbestos removed from our workplaces." Lewenza is urging all CAW members to take action, but also to take a moment on April 28 to stop and remember all workers injured or killed on the job. more


Canadian Centre for Occupational Health and Safety The Canadian Centre for Occupational Health and Safety (CCOHS) is reminding employees and employers that April 28th is National Day of Mourning in Canada. This day is set aside to honour those workers across the country whose lives have been lost, injured or disabled on the job, or suffer from occupational diseases. To help promote awareness in the workplace of this important day, CCOHS has designed a bilingual poster made available free of charge. The poster can be previewed on the CCOHS website and has more information about the National Day of Mourning

Ethiopia Ethiopia has marked Workers' Memorial Day with tripartite events since 2004. This year's theme is 'health and life at work, a basic human right' with the aim of creating a safety culture and encouraging government, employers and workers at large to incorporate OSH issues in every work activity. The planned events include a mass 3 km walk/rally on April 26th started by ministers and other officials and supported by a marching band. It is hoped there will be 1,000 participants. The march will end with a speech. Promotional materials such as posters, brochures, banners, shirt and caps will be produced and distributed. The International Transport Workers' Federation (ITF) Unions say no to violence! ITF will hold its Action Day on 28 April 2009. The main theme for this event will focus on violence in public transport. The ITF Railway Workers' Section Conference in December 2008 unanimously endorsed this campaign day and agreed to invite ITF unions who represent bus and taxi workers, where relevant and appropriate. WORKPLACE FATALITY FAMILY BILL OF RIGHTS

Wow can't get much better than this! Unless the States pick it up.

We were ask some time back if the Family Bill of Rights could be used and they are putting it to good work.


WORKPLACE FATALITY FAMILY BILL OF RIGHTS

Currently, the Occupational Health and Safety Act allows the Police, the Minister of Labour, the Company and Joint Health and Safety Committees to conduct investigations where there is a fatality or critical injury in the workplace.
The Act is the basic legal authority which states general duty requirements and principles. ( The workplace parties does not include families.)
The rights to independant investigations by the families or family representatives is not allowed.
Nor are the families allowed to receive any of the above investigative reports.
Any rights are not extended to the families in any way.
Even at a Coroners Inquest, the families are not allowed to present any evidence that may have been obtained outside of the "official" investigations.
Due to the fact that family does not have any rights under the Occupational Health and Safety Act, the only way that a family may even be allowed to view the accident scene is through the permission of the employer.

We know that is not going to happen.

The Workplace Fatalities Family Bill of Rights aims to change that.

We beleive that the familiy should have complete access to all information and we are proposing a bill to the Government of Ontario to change that and ensure that the family does have access to all investigation reports.

It is critical that the familiy has this information so they can make an informed decisions as to what the next step is. ( for instance if there is negligence involved ).

We expect to have our final draft of the Bill to be completed shortly and presented to the government for the fall sitting.

We ask for your support in this project and will provide updates as to our progress.

Tree trimmer killed

April 11, Phoenix, Arizona - A man was killed while trimming a tree. The accident occured near Earll and 26th street in Phoenix. A team of three men were trimming a palm tree when the accident happened. Rescue workers say a large palm frond fell, stuck and killed the worker. Phoenix fire rescue crews had to remove the body of the man from the tree. Two other workers in the tree and had to be helped out of the tree before the body of the thrid man could be removed from the tree.


Glass panes fall from crane in Oak Lawn, killing construction worker

April 11, Dallas, Texas - A construction worker died Saturday morning in the Oak Lawn area of Dallas when he was struck by several glass panes that fell from a crane, police said. The 24-year-old man, whose identity wasn’t released, was helping remodel a building in the 3800 block of Oak Lawn Avenue near Blackburn Street. The glass panes fell from the top of the crane at an unknown height around 8:30 a.m., police said. Paramedics took the man to Parkland Memorial Hospital, where he later died. The federal Occupational Health and Safety Administration, which investigates such incidents, came to the site after the accident.


Store clerk killed in robbery

April 11, Stockton, California - A clerk who had worked for 31 years at the busy Arco AM/PM on the corner of California Street and Alpine Avenue died early Friday following a confrontation with a robber. Ralph Dillingham, 61, of Stockton, described as a husband and father who was both a nice guy to customers but had a no-nonsense reputation when it came to working the night shift at the 24-hour minimart gas station, was pronounced dead at St. Joseph's Medical Center later that morning. Police officers found Dillingham lying unconscious behind the counter shortly after they received a call at 2:50 a.m. He was taken to the hospital a little more than 10 blocks away, where he died.


Electrical worker collapses and dies

April 11, Newnan, Georgia - A North Carolina man working at an electrical substation on Lower Fayetteville Road collapsed and died Thursday evening, but the cause of death is still under investigation. Willie Williams, 38, and a co-worker from Waukesha Electric Systems were adding a transformer to the SummerGrove substation near Highway 154 when Williams collapsed. He was pronounced dead at Piedmont-Newnan Hospital, said Deputy Coroner Bart Henson.


Injured Ashland County Green Township Road Worker Dies

April 11, Ashland County, Ohio - An injured Green Township road worker who was hurt on Tuesday morning on Township Road 2704 in Ashland County has died. Jeffrey White, 40, passed away Friday at MetroHealth Medical Center in Cleveland. A passing motorist found White unconscious, bleeding from the head and lying next to a township dump truck along the roadway and called 911. White had been clearing brush from along the roadway with a co-worker who had loaded debris into the truck and went to the township garage to retrieve straps to secure the load.


Vending machine worker killed at NW Fla. college

April 10, Defuniak Springs, Florida - Authorities are investigating the shooting death of a vending machine maintainer at Northwest Florida State College. DeFuniak Springs Police Capt. Mark Weeks says the man was found on campus Friday afternoon with two gun shot wounds to the chest. His identity hasn't been released. Police are looking for someone named Thomas Ford McCoy Jr. in connection with the shooting, but would not elaborate.

Valpo man, 47, dies in Lowell trench collapse

April 10, Lowell, Indiana - A Valparaiso man was killed when a 10-foot-deep trench he was digging with a co-worker collapsed on him Friday afternoon. Michael Faherty, 47, was pronounced dead at 1:50 p.m. at a home in the 16700 block of Mount Street in Cedar Creek Township. The Lake County Coroner's office labeled the death accidental. They will conduct an autopsy this morning. Crown Point firefighters and technical rescue team members work to secure the sides of a trench as they recover the victim after a fatal trench collapse in the 16700 block of Mount Street Friday afternoon in Lowell.


Construction accident victim loved outdoors

April 10, Washington County, Maryland — Benjamin Burger Jr., who died Thursday after falling while working on a construction project, was an outdoors person, relatives said Friday. He was particularly fond of hunting for deer and turkeys, and of fishing for trout, said Vicki Burger, his aunt. She said he would camp out so he could be the first person fishing at a stream. Burger was building a deck in the 17300 block of West Washington Street near Hagerstown when he fell onto a picket fence, according to a representative of Straightline Construction of Clear Spring, for which Burger was working.


One Dead Following Trench Collapse

April 10, Lowell, Indiana - One man is dead and another escaped injury after a 10-foot-deep trench they were digging caved behind a Lowell home, Lake County authorities said. The trench, at 16731 Mount St., in Lowell, was being dug for a new sump pump for a single-story home on a wooded lot. Two men were in the trench when it collapsed, the NWI Times reported. One of the men tried to dig the other out with a backhoe, but was unsuccessful. The trench was as much as 10 feet deep in some portions, said Mike Higgins, the Public Information Officer for the Lake County Sheriff's Department.


C-DOT Highway Worker Killed in Pueblo Identified

April 10, Pueblo, Colorado - A Colorado Department of Transportation employee who was backed over and killed by a fellow employee working in the cone-zone was identified as 31-year-old David Valdez. Valdez, who was originally from Walsenburg, leaves behind a small child. Agency spokeswoman Stacey Stegman says the CDOT crew was getting ready to repair expansion joints on a bridge when the accident happened Thursday morning.


Correctional officer killed in stabbing at Atwater prison

April 8, Merced, California - A federal correctional officer at U.S. Penitentiary Atwater is dead after being stabbed by inmates at the facility Friday, according to Merced County Sheriff Mark Pazin. Although details are still sketchy, Pazin said the officer, identified as Jose Rivera, 22, of Chowchilla, was transported to Mercy Medical Center Merced after the stabbing. The FBI is conducting the investigation into the attack and Pazin said his office will conduct the autopsy. Rivera was taken to the hospital at 3:30 p.m. and was pronounced dead at 4:15 p.m., Pazin said. Pazin said Rivera's death underscores the dangers that correctional officers face every day.


Ohio contractor worker dies from injury at plant

April 8, Cincinnati, Ohio - An employee of a contractor has died after being injured at AK Steel's Middletown Works plant in southwest Ohio. Scott Rich, president of the International Association of Machinists and Aerospace Workers Local Lodge 1943 union, says the worker leaned against a controller while working on a lift and was crushed between the lift and a pipe. The worker's identity wasn't released immediately. Alan McCoy, spokesman for AK Steel Holding Corp. ( AKS - news - people ), says the contractor worker was injured Monday and died during a surgical procedure either late Tuesday night or early Wednesday.


Factory worker crushed, killed at Alladin Plastics in Hawkins County

April 8, Phipps Bend — A longtime employee of Alladin Plastics at the Phipps Bend Industrial Park was killed Tuesday night when he was reportedly crushed by a machine that molds plastic chairs. The accident occurred Tuesday shortly before 9 p.m. According to an Alladin employee who asked to remain unnamed, supervisor Lonnie Mowell, 61, 1035 Longs Bend Road, Rogersville, was attempting to free a chair that had been jammed in the machine when the machine engaged. Mowell had been employed at Alladin Plastics, 640 Phipps Bend Road, Surgoinsville, for nearly 40 years.


Norwalk man dies in electrical fire at mine

April 8, Des Moins, Iowa - A Norwalk man died in an electrical fire at the Hallett Materials site northwest of Des Moines on Tuesday afternoon. Tadd Bainum, 35, a Hallett employee, died after a transformer spark caused a fire near the gravel mine at 5255 N.W. 26th St. The fire started around 4 p.m., said Jana Rooker, spokeswoman for the Polk County sheriff's office. The specific cause of Bainum's death is unknown and will be determined by the Polk County medical examiner. MidAmerican Energy had to shut off power to the area for rescue crews to reach Bainum. He was in an electrical area that is under Hallett's control, said Ann Thelen, spokeswoman for MidAmerican.


3 Pittsburgh Police Officers Shot Dead; Alleged Gunman Arrested

April 7, Pittsburgh, Pennsylvania - Three Pittsburgh police officers were killed and a fourth was injured in an unprecedented shooting Saturday morning in Stanton Heights, where authorities believe a man was "lying in wait" with high-powered weapons, ready to fire when the victims arrived at his home. Mayor Luke Ravenstahl identified the slain officers as Paul Sciullo, who was first to get shot, Steve Mayhle, who was backing him up, and Eric Kelly, who was on his way home but had stopped to help the others. "Our hearts and our prayers go out to the officers who paid the ultimate sacrifice," said Police Chief Nate Harper, adding that the bureau has never lost as many officers in the line of duty at one time as happened Saturday.


Suspect charged after 2 killed after Pa. robbery

April 7, Altoona, Pennsylvania - Police have identified the man charged in the shooting deaths of two people following a robbery at a west-central Pennsylvania sandwich shop. Nicholas Horner faces two counts of criminal homicide and other charges in connection with the shootings Monday afternoon in Altoona. The 28-year-old Altoona resident was arraigned Tuesday and sent to Blair County Jail without bail. Police say Horner shot two workers at a Subway shop, killing one. Authorities say the suspect then fled and shot another man about four blocks away who was retrieving his mail.


Coroner: Worker was apparently electrocuted

April 7, Princeton, Kentucky — A worker removing debris from an ice storm was killed when the lift he was standing in came into contact with a power line. The Messenger of Madisonville reports 38-year-old Raymond Morris was apparently electrocuted in the weekend accident. Providence Assistant Police Chief Brent McDowell says Morris was aloft in a scissors truck when the incident occurred about 4:50 p.m. Friday. Morris was transported to Regional Medical Center, where he was pronounced dead. Hopkins County Coroner Dennis Mayfield said it appeared 38-year-old Raymond Morris had been electrocuted. Morris, of Providence, worked for Ware Trucking.


Northside mechanic killed while working under car

April 8, Northside, Florida - A mechanic at a Northside auto-repair shop was crushed to death Tuesday when the car he was working beneath came crashing down. Lt. Reginald Lott of the Jacksonville Sheriff's Office said the employee was crushed when the jack supporting the car slipped. He was working alone at the shop, which is near the intersection of Lem Turner Road and Leonid Road when the car fell. His body was discovered by a customer, and he was pronounced dead at the scene. Authorities didn't release his name. Paramedics were alerted about 11:20 a.m., according to Times-Union news partner First Coast News.


St. Louis cab driver shot to death, cab stolen

April 7, St. Louis, Missouri - Police are talking with a woman who says she was a passenger in a Laclede cab when the driver was shot to death this morning. The driver was fatally shot in the head at the intersection of University Street and Leffingwell Avenue just before 4 a.m. Tuesday. He died at the scene. His name and age are not yet being released. The female passenger told police that she was in the backseat of the cab when a man walked up to the cab, talked to the cabbie and shot him.


Worker killed after getting caught in machinery

April 6, Jenks, Oklahoma — Jenks fire officials say a man was killed when he got caught in a piece of machinery at an area dirt company. Assistant Fire Chief Jake Rhoades says firefighters responded to a call about 4 p.m. Monday at the Gem Dirt Co. The man's co-workers had already pulled the 36-year-old from the machine and were performing CPR when the fire crew arrived. Rhoades described the machine as a soil tumbler. Medics from the Emergency Medical Services Authority pronounced him dead. Rhoades says a co-worker noticed the machine was off and when he went to investigate he found the man. The name of the victim wasn't released.

Activists to Come to Historic Hearings, Make time to Participate

As you may know, two important congressional hearings will be taking
place on April 28 to consider important health and safety issues that
concern us all. In the morning at 10 AM, the Senate HELP Committee
members Senators Patty Murray [WA] and Johnny Isakson [GA] will be
hearing testimony on the role of victims and family members in workplace
health and safety issues [Family Bill of Rights] and the penalties
levied on violators of OSHA standards. We understand there may be a
simultaneous hearing in the House Education and Labor Committee and the
reintroduction of the PAWA [Protecting American Workers Act] that will
contain substantial improvements to OSHA law including increased
criminal sanctions for egregious employers.

To make the most of these hearings scheduled on Worker Memorial Day, we
are planning to have a significant number of family members who have
lost loved one in workplace tragedies in Washington DC. You can visit
*USMWF.org*to see the new Faces Campaign, which is
putting a human face on the horrific toll of workplace disasters. In
many instances this will be the first time for many of these family
members to become active in fighting for the rights of current and
future workers as a way to remember and memorialize their lost loved
ones.

We are encouraging as many activists to come to these historic hearings
on April 28 in Washington DC. *Will you make time to participate?*

In addition to other activities, we plan to have large posters of some
of the Faces to remind us all to “fight like hell for the living”.
We will inform folks of the schedule for the day as we get closer but we
are planning to meet at 8:00 am on the steps of the Frances Perkins
building at 200 Constitution Avenue, NW (on the Constitution Avenue
side) before we march collectively to the Senate and House hearing
chambers. If you are a musician and would be willing to lead us in songs
of solidarity, your guitars, banjos or other instruments would be most
welcome.

For those not able to be in DC, we hope you will send a message to you
representatives about these important issues. Please feel free to
respond if you need additional information. Please forward to other
interested activists.
