Weekly Toll: Death in the American Workplace

Gas station clerk killed in armed robbery

July 26, Seattle, Washington - First on My Ballard: A man wearing a bandana over his face shot and killed a clerk at the 76 Station and “Pit Stop” on 15th Ave. this morning, police say. The employee suffered gunshot wounds to the stomach and thigh, and he died a short time later — the first murder in Ballard since March of last year. The suspect is at large.


City worker shot at housing unit

July 26, Philadelphia, Pennsylvania - A Philadelphia Housing Authority carpenter was shot once in the back of his head yesterday as he prepared to work at a low-rise public housing unit in North Philadelphia. Rodney Barnes, 46, of Elkins Park, had just removed tools, paperwork, and sneakers from his red pickup truck parked at 41 W. Norris St. when a bullet came from close range, said Richard A. Zappile, chief of the authority's Police Department.


Worker dies days after falling off roof in Armonk

July 25, Armonk, New York - A contractor who fell off scaffolding July 17 while working on the roof of a Fordham University building has died of his injuries, officials said. Rosendo Bernao, 43, of 22 Madison Ave., Port Chester, died Thursday of skull fractures and brain injuries stemming from a 20- to 30-foot fall off scaffolding, the medical examiner and university officials said.


Worker killed in pipe rack collapse ID'd

July 26, Bakersfield, California - The man killed Saturday in an industrial accident at Dole Enterprises off Allen Lane has been identified as 49-year-old Jimmy Shannon. Kern County fire crews were called to the business on a report that the worker was trapped under several steel pipes. Shannon was pronounced dead at the scene.


54-year-old Riverton City employee found dead in manhole

July 22, Riverton, Utah - A 54 year old city employee was found dead in a 20 foot deep manhole near 11800 South and 1850 West in the city of Riverton. The Riverton City employee is Danny Crump. Police say Crump was reported missing after he did not respond to radio calls, and was later found by County Sheriff deputies. The incided does not look suspicious, however investigators are not sure how he died. Stay tuned to ABC 4 News as we bring you more on this story.


Fiery crash kills truck driver

July 24, Tuscon, Arizona - A Tucson couple is talking about the giant fireball from today's I-10 crash that killed one. It happened right before their very eyes. Earlier today there was a single-vehicle wreck, involving a tractor-trailer. It shut down the freeway and backed-up traffic for miles and hours. The driver, Shari Linder, died. Her husband Ricky Linder survived. News 4 learned a load of cleaning and automotive products created a Haz-Mat situation.


Autopsy: 3 NC Slim Jim plant workers were crushed

July 26, Raleigh, North Carolina — Autopsies show three workers killed in the collapse of a Slim Jim factory in North Carolina were crushed to death. The News & Observer of Raleigh reported Friday that autopsy results were released for workers Barbara McLean, Lewis Watson and Rachel Pulley. They died from blunt force trauma in the June 9 explosion at the ConAgra plant in Garner, where nearly 40 were injured.


Retired Farmer Dies After Tractor Rolls

July 23, Callaway, Minnesota - A retired farmer is dead after the tractor he was driving rolled one time, trapping him underneath. According to the Becker County Sheriff's Office, a 90-year-old man was operating his tractor in rural Callaway, Minn. when he attempted to drive onto the shoulder area located next to a culvert. Authorities said it appeared the front wheel of the tractor collapsed the culvert, causing the tractor to roll one time, trapping the man underneath the tractor at the bottom of the ravine.


Caltrans worker hit, killed on Highway 99 in Lodi

July 23, LODI, California — A California Department of Transportation worker hit and killed this afternoon on southbound Highway 99 has been identified as Donald Lichliter of Modesto. The 53-year-old Caltrans worker was doing abatement alongside the road when he was fatally struck by a van, according to the California Highway Patrol. “Donald Lichliter was a devoted and hard-working public servant, who worked for Caltrans for over 27 years,” said Gov. Arnold Schwarzenegger. “Maria and I join all Californians in expressing our appreciation for Donald’s service and we send our thoughts and prayers to his family and friends during this difficult time.”


Man killed in industrial accident remembered as active, friendly

July 24, West Valley City, Utah — Frank Guzman always told people "you're as old as you feel." "He was 63, but he acted like he was 18," said his son-in-law, Rex Randall. "He was just full of energy. Ready to take on the world at 63." Now Guzman's family is looking back on the good times, not Friday morning's unexpected phone call informing them that their father, husband, uncle and grandfather had been killed in a tragic industrial accident at Brody Chemical. Police say that, around 9 a.m., Guzman was trying to transfer a thick cleaning solution from a 55-gallon drum into smaller 1-quart containers when the lid of the large container blew off and struck him in the head and chest.


Orchard worker dies in ATV crash

July 23, Portland, Oregon - An orchard worker died last night when his ATV crashed and rolled on top of him in The Dalles. Lt. Gregg Hastings, a spokesman for the Oregon State Police said Ricardo Romero Camacho, 49, of The Dalles, was killed while working at the Geiger Orchards, 2845 E. 12th Street. Hastings said Romero was found underneath the ATV he was riding through the orchard at around 8 p.m.


Female Worker Crushed by Machine

July 22, Los Angeles, California - Authorities identified a worker crushed to death by a packaging machine at a food processing plant in Industry. Ana Maria Vital, 40, of La Puente was fatally injured about 8 a.m. Tuesday at Golden State Foods in the 600 block of South Sixth Avenue, said Assistant Chief Ed Winter of the coroner's office.


SC utility worker electrocuted by power line

July 22, Macedonia, South Carolina - A South Carolina electric utility worker has died after he was electrocuted while working on a power line. Multiple media outlets reported that 25-year-old Jeremy Thomas of Moncks Coroner was electrocuted late Tuesday morning while working on a high-voltage power line near Macedonia. Berkeley County Chief Deputy Coroner Bill Salisbury says the Santee Cooper lineman was working on the line on U.S. Highway 17A about five miles north of Moncks Corner when he was shocked.


UPDATED: Allens Inc. Worker Buried Under Ton of Beans, Sheriff's Office Says

July 22, Little Rock, Arkansas - A worker - possibly employed by Staffmark - died Wednesday morning at the Allens Inc. plant near Siloam Springs, and the Benton County Sheriff's Office identified him as John D. Haney, 44. Sheriff's investigators were told that a coworker found Haney "buried under a two-thousand pound bag of red beans," according to a statement issued by the Criminal Investigations Division.


Worker found dead outside middle school

July 21, Palm Springs, California - An autopsy was pending today in the apparent electrocution of a 50-year-old man at John Glenn Middle School in Indio. James Jeffredo, apparently a groundskeeper, was fatally injured at the school at the school at 45250 Dune Palms Road about 12:45 p.m. Monday, according to a coroner investigator. The man came in contact with an electrical circuit, he said.


Man Found Dead at Bottom of Manhole in Riverton

July 21, Riverton, Utah - A 50-year-old Riverton city water worker was found dead at the bottom of manhole Tuesday night at 11800 South 1850 West. The Salt Lake County Sheriff's Office call it a tragic accident. The dead man was last seen alive at 3 p.m. When he did not return to the city office, other city workers started searching for him. One worker spotted an open manhole cover, looked into it and saw the victim at the bottom of a 20 foot drop manhole.


Worker found dead at Calif. food processing plant; authorities believe death was accidental

July 21, Industry, California — A 40-year-old woman has been found dead in a machine at a Southern California food processing plant that is a major supplier for McDonald's restaurants. Los Angeles County sheriff's detectives say the woman's body was found early Tuesday at Golden State Foods in the City of Industry, an east Los Angeles suburb. Investigators believe her death was accidental.


Worker Dies After Being Pinned Under Forklift


July 20, San Benito, Texas - A construction worker injured in a forklift accident in San Benito has died. NEWSCHANNEL 5 confirmed Ruben Villegas died at the hospital last week. We're told his injuries were too severe. Officials say he was trying to put bricks on a scaffold, when the forklift tipped over. Villegas was trapped underneath.


NJ police officer dies from shootout wounds

July, 21, Jersey City, New Jersey — A city police officer who was critically wounded in a shootout with two armed robbery suspects last week died Tuesday, one day before his 38th birthday and the same day police engaged in another gun battle nearby. Detective Marc A. DiNardo, a 10-year veteran who had been on life support, was pronounced dead around 9:35 a.m. after doctors harvested his organs for transplant. The 37-year-old was hit by a shotgun blast to the face July 16 as his SWAT team stormed the apartment where the suspects had taken cover. DiNardo had no signs of life when he arrived at Jersey City Medical Center that day and had to be revived several times before being stabilized in critical condition.


Coroner: Pa. farmer killed in equipment accident

July 20, Dayton, Pennsylvania - A coroner says an 88-year-old farmer has been killed on his western Pennsylvania tract in an accident involving machinery. Armstrong County Coroner Robert Bower on Monday announced that Beryle (BAYRL) Lee Calhoun was found dead by relatives Saturday afternoon. The farmer was last seen alive on his property in Wayne Township Friday night. That's about 50 miles northeast of Pittsburgh.


Update: Worker killed by accident at TVA Kingston plant

July 20, Knoxville, Tennessee - A driver delivering 20-inch diameter pipes to the TVA Kingston plant has died, after part of the load fell on him. Larry LaCroix was delivering the pipes on a flat bed. Upon arrival, he released the straps to unload the pipes. LaCroix was taken to Roane County Medical Center and later declared dead.

New Mexico Sergeant Killed in Stakeout

July 17, Santa Fe, New Mexico - "I'm going to catch that guy." That was the last thing Sgt. Joe Harris told a secretary at the Sandoval County Sheriff's Office before setting out to bust a burglar known as the "Cookie Bandit," who had eluded police in the Jemez Mountains for close to a decade. The man was wanted for a string of burglaries of cabins and trailers in the Jemez Springs area in which he would allegedly steal food, water, clothes, boots and other items to help him survive in the mountains. Harris was fatally wounded, becoming the first Sandoval County sheriff's officer killed in the line of duty. Harris was shot in the groin, and the bullet struck a large artery, Sheriff John Paul Trujillo said.


WATCH: Construction worker dies of heat-related issues

July 16, Austin and Travis County E.M.S. Paramedics tell KVUE that a man who died from heat-related issues on Wednesday was a construction worker in his forties. The construction worker is believed to have worked several days in a row in the heat. Paramedics responded to a South Austin business where a co-worker of the victim called 911 after the construction worker passed out in his car in a parking lot. "He was demonstrating signs of altered level of consciousness which is always one of the late stages in a severe heat injury," said Mike Benavides with Austin/Travis County.


Massport worker killed at Black Falcon Terminal

July 17, Boston, Massachusetts - A Massport worker was killed this afternoon at the Black Falcon Cruise Terminal on the waterfront in South Boston after being hit or pinned under a Jersey barrier. State police say the 50-year-old East Boston man was fatally injured at 3:03 p.m. today at the massive terminal at the end of Summer Street. He was pronounced dead at the scene.


Truck driver from Cedartown killed in tractor-trailer wreck

July 18, West Point, Georgia — A Cedartown man died in a tractor-trailer wreck that scorched the ground near Interstate 85 about 11 a.m. Thursday. Georgia State Patrol Sgt. first class D.M. Blevins said the driver, John Lloyd Parris, 37, lost control of the vehicle on southbound I-85 near the two bridges above Ga. 18. The truck traveled between the two overpass bridges, crashed into a concrete support below them, then burst into flames. Smoke billowed from the truck’s cab, the office furniture in the holding compartment burning as well.


Longtime Monarch employee found dead inside work vehicle

July 17, Salida, Colorado - Mark Weeks, 47 of Salida and employee at Monarch Mountain, died Thursday while working at the ski area west of Salida. Emergency responders were called to the mountain about 4 p.m. when employees found Weeks slumped over the steering wheel in a company vehicle, Greg Ralph, Monarch marketing director, told The Mountain Mail. Weeks was a building maintenance and water quality employee who was recently promoted, Ralph said. He was found between the base lodge and ski patrol building after he failed to answer radio calls, Ralph said.


Baldwin Shop Owner Shot, Killed

July 16, Macon, Georgia - A Baldwin County shop owner was shot and killed by a robber Wednesday night, according to Sheriff Bill Massee. He said the victim was 51-year-old Danny Patel, owner of Lina Food Store at 2693 Irwinton Road. Massee said Patel died at the scene.


Miramar hospital employee shot dead at work

July 16, Miami, Florida - A Miramar hospital employee was shot dead at work before dawn Thursday morning. The 55-year-old man died inside his Toyota after he was shot in the parking lot of Memorial Hospital Miramar, 1901 SW 172nd Ave., police said. Police identified the victim Thursday as Antonio Noisette, a Pembroke Pines resident and father of four who works at the hospital as a cook.


Pastor dies at his church office in Thousand Oaks

July 14, Camarillo, California - Parishioners at St. Maximilian Kolbe Catholic Church of Thousand Oaks are in shock after the sudden death of their pastor, the Rev. Patrick O’Dwyer. He was 61. O’Dwyer collapsed in his office at the church about 7 p.m. Monday, church officials said. An autopsy Tuesday indicated the cause of death was heart disease, according to the Ventura County Medical Examiner-Coroner’s Office.


Worker dies in crane accident at Ohio paper plant

July 15, West Carrollton, Ohio — Authorities say a contract worker died after a boom from a crane fell on him at Appleton Papers in western Ohio. The accident happened Wednesday at the West Carrollton plant, which is temporarily shut down this week so that equipment can be serviced. The crane is used for moving large rolls of paper. The identity of the victim was not immediately released. West Carrollton is about 8 miles south of Dayton.


Field worker dies in farming accident

July 15, Patterson, California - A man died following what police described as an industrial accident in an orchard near the corner of Zacharias and Rogers roads Tuesday. The man had been struck by a piece of farming equipment, Stanislaus County Sheriff spokeswoman Gina Leguria said. Representatives with Patterson Police Services and the Patterson Fire Department had few details about the accident. The Stanislaus County Coroner’s Office said they had tentatively identified the man Wednesday, but would not release his name because his family had not yet been notified.


Construction worker killed by Amtrak train

July 15, Bay Point, California - A man who was killed Tuesday when his pickup truck was hit by an Amtrak train in Bay Point has been identified as 45-year-old Antioch resident Juan Tejada, according to the Contra Costa County coroner's office. Tejada, an employee of the construction company ARB in Pittsburg, had been laying pipe in the area, according to Sgt. Bryan Reynolds at the coroner's office.


Two Blue Earth County Employees Killed In Train Accident

July 14, Mankato, Minnesota - Two occupants of a dump truck struck by a train near Lake Crystal yesterday have died.The State Patrol says forty two year old Jack Baker of Le Center was driving the truck.Forty three year old Todd Ziegler of Mankato was a passenger.Both men died in the collision.And both were Blue Earth County employees.


Sheriff: Explosives at home of deputy's killer

July 14, Houston, Texas - More than a hundred incendiary devices — including homemade pipe bombs, explosive boxes with booby traps and altered tear gas devices — were found in the Baytown area home of the former police officer authorities said fatally shot a Chambers County deputy and then killed himself...Shane Thomas Detwiler, who had worked for the sheriff's office for two months, was killed while investigating a shooting at Ortez's home.


Worker Crushed by Scissor Lift at Oklahoma Linen Service Company

July 14, Tulsa, Oklahoma — An employee of Superior Linen Service was fatally crushed
June 12 when the hydraulic scissor lift he was working under fell on him, according to Officer Leland Ashley of the Tulsa Police Department. Douglas Ray Ramey, 33, was working on a lift used to load trucks at the plant when the accident occurred. Ramey disconnected an improperly working hydraulic hose, Ashley says in quoting from the police report, and the lift fell. Ramey was trapped underneath the equipment and was pronounced dead at the scene.


McIntyre man dies in Gordon train accident

July 14, Macon, Georgia - An employee for Rescar Industries died Friday evening after sustaining injuries while working on a train in Gordon. Damien Whipple, 24, of McIntyre, died when he fell off one train car and under another while the train was being switched, said Wilkinson County Coroner Billy Matthews. Matthews and Gordon police said it was an accident. Officials from OSHA are investigating the scene, something that is standard procedure when a death is involved, said G.T. Breezley, the area director of OSHA’s Atlanta office.


Man Killed in Shredder Accident at Recycling Plant

July 13, Los Angeles, California - A man was killed Monday morning in a freak accident at the recycling plant in Lincoln Heights. The adult man was working on a cardboard shredder at the center on North Mission Road when he got stuck inside the machine, according to Los Angeles City Fire spokesman Devin Gales. It's unclear if he fell in or how he otherwise became entangled inside the shredder, Gales said. It happened at the recycling facility shortly after 8:00 a.m.


Man killed in Ludlow when tractor-trailer overturned identified

July 13, San Bernadino, California - A Missouri man who died Friday when the tractor-trailer he was riding in overturned has been identified as 62-year-old John Steven Hovenga. Hovenga, from St. Joseph, Mo., was asleep in the sleeper cab of a 2007 Yellow Freight tractor-trailer that was heading west on the 40 Freeway in Ludlow when the driver lost control about 4:50 a.m. The driver drifted onto the dirt median and over-corrected as he tried to edge the tractor-trailer back onto the roadway, causing it to overturn, California Highway Patrol officers said.

Combustible Dust: An Insidious Hazard

After much debate from one of the Companies that had a Combustible Dust Explosion in 2003 the CSB has produced a new video "Combustible Dust: An Insidious Hazard"

It really is amazing and a little emotional if you have a family member that was involved. The CSB did a terrific job of putting Combustible Dust in perspective. This should be sent to every industry that could potentially have a dust hazard. This video shows in detail the three combustible dust explosion in 2003 that spurred an extensive investigation and push for reform in the House and Senate for OSHA oversight and regulations.

Stated in the CSB's in a news release "No company wants to see its facility blown up and destroyed and its employees killed,” CSB Chairman John Bresland says in the video. “But they just don't understand what the hazard is, they don't realize that they
have a hazard here, until that one day when the explosion occurs, and it's a
terrible tragedy for them. And they look back and say, ‘If we'd only known.’”

The video is wonderful for awareness but unfortunately this will not be enough. We do need an OSHA standard so that there is a clear set of guidelines for the companies who really want to insure safety and for the ones who get by thinking their near misses are a part of the job...well there would be no room for that type of attitude.

Over 2000 Signatures

Remember the Sheharbano "Sheri" Sangji petition? Well now they have collected over 2000 signatures and is being sent to the CA senators, Governor, local representatives, Cal/OSHA head (and the head of the investigation), the DA and anyone else that may be able to impact this issue. If you live in the area please give them a call. Calls are monument, I know many have ask does it really make a difference, Believe me it does!

Sheri's family would like to thank you for the support you have given so far. You have all been amazing in answering the call.


Just in case you need a refresher:

Only three months into her job in the lab, Sheri was extracting t-butyl lithium into a syringe and when when the syringe broke and a of flash fire engulfed her sweeter. Sheri spent 18 days in a burn unit with third-degree burns over 43% of her body. I don't really know the details however I have been there and know how excruciating it can be to have to see your loved one in this condition. Such a beautiful young women and a loss to many. He has toured Europe and the US, and has produced many CD’s delivering his message of solidarity through his music. Mike wrote this song and performed it live at Workers Memorial Day In Pittsburgh. This song is about my brother, Gary Puleio, who was killed on August 15, 2001 due to unsafe working conditions. Gary was killed on the job at Meadville redi-Mix concrete plant He had been employed there only 3 months as a cement truck driver and fell 25 feet to his death, from a cement tower, while shoveling gravel off the hopper to clean it. The company claimed Gary just wandered up there on his own, without wearing any safety equipment, at the end of his driving shift rather than being assigned this dangerous task because he was the “new man”.

Sheboygan gas station worker killed identified

July 13, Sheboygan, Wisconsin - A gas station employee who died after a van backed into the side of the store has been identified. Sheboygan police say a 32-year-old Sheboygan man and his wife had just filled up, and his wife went into the store to pay 11 p.m. Saturday. As he was backing the van up to pick up his wife, the van crashed into the store's window, coming to a rest inside the store. The employee is identified as 18-year-old Emily E. Hughes of Sheboygan.


Lincoln stunt flier dies in crash

July 12, Omaha, Nebraska - A champion aerobatic pilot from Lincoln was killed in a plane crash during a practice session Friday for an air show near Tarkio, Mo. An Edge 540 aircraft piloted by Chandy Clanton crashed into a bean field about 2:30 p.m. Friday near U.S. Highway 136 and State Route O near Tarkio, according to the Missouri Highway Patrol. She was pronounced dead at the scene. Clanton, 36, was the daughter of Harry Barr, an employee of Duncan Aviation in Lincoln. Clanton often appeared at events in Clarinda, Iowa, her father's hometown.


Heart attack precedes crash in taxicab driver death

July 12, Pittsburgh, Pennsylvania - A taxicab driver who crashed his Yellow Cab into a bridge abutment in the city died of natural causes, according to the Allegheny County Medical Examiner's Office. Michael Brown, 53, of Mt. Lebanon, died of natural causes, according to a supervisor. Mr. Brown lost control of his cab at 12:16 a.m. and it crashed into the on-ramp to the West End Bridge off of Reedsdale Street.


Cottage Hills man killed in workplace accident at Olin Brass

July 12, Belleville, Illinois - A Cottage Hills man was killed in an industrial accident at an East Alton plant Saturday morning. Michael B. Niswander, 41, died in the industrial accident, which occurred about 10:30 a.m. at Olin Brass, reported the Madison County Coroner's Office. The coroner's office reported that Niswander was struck in the chest by industrial machinery. Niswander was transported to Alton Memorial Hospital, where he was pronounced dead about 50 minutes later.


1 dead, 1 hospitalized after trench collapse

July 10, Coushatta, Louisiana — An independent contractor is dead and another man is hospitalized after both were trapped underground while working on a sewer line in Coushatta. Police Chief Charlie Adams says the accident happened just after 9 a.m. Friday in an open field along U.S. Highway 71. The men, who are employed with TGS Contractors Inc. in Minden, were working for the town to replace a collapsed sewer line.


Worker Killed in Tree-Cutting Accident

July 10, Elm Grove, Wisconsin - A 31-year-old worker died when he fell about 40 feet as he and other workers were cutting down a tree in a Milwaukee suburb. The Elm Grove police department says the accident about 9:50 a.m. Friday killed Dale Simek of Trevor, who was hired to remove a large tree at a private residence.


Worker crushed at recycling center

July 10, Fort Wayne, Indiana - Police say a worker at a Fort Wayne metal recycling facility died after being run over by a front-end loader. The man was injured Thursday at the OmniSource plant east of the city's downtown and died a short time later at a hospital. Police say the worker was standing near the 30-ton loader when the driver didn't see him while turning it. Authorities did not immediately release the worker's name.


MCC employee dies after being injured by machinery

July 9, Kingman, Arizona - The Mohave County Medical Examiner's Officer is conducting an autopsy today on the body of an employee who died on the Kingman Campus of Mohave Community College. Sean McMahon, public information officer for the college, confirmed that a maintenance employee was fatally injured in an accident around 1:30 p.m. Tuesday. His name is not being released at this time per the family's request.


MeadWestvaco employee dies

July 9, Silsbee, Texas - An employee from Kirbyville with the MeadWestvaco plant in Evadale who was injured last Tuesday afternoon at the mill died from his injuries Friday afternoon. Otis Deon Collier, 41, died Friday at Memorial Hermann Hospital in Houston. He had been employed with the mill for 16 years. According to information from the company’s public relations manager, Penny Sullivan, a flash fire occurred at about 2:45 p.m. on Tuesday, June 30.






Man dies after he's burned while inspecting tanker

Water truck operator dies in logging operation accident

July 8, Concow, California - A John Wheeler Logging water truck driver died Wednesday morning after rolling his vehicle over while working on a logging operation about 20 miles northeast of Concow. At approximately 11:15 a.m., a fellow logger passed the overturned truck on the remote U-Line on Sierra Pacific Industries property, an area only accessible by a single-lane gravel and dirt road. Police: RI worker died after hitting wrong button

July 8, North Kingston, Rhode Island — A worker who died at the Electric Boat Corp. plant in North Kingstown this month was crushed when he pushed the wrong button and closed a hangar door instead of opening it. The police report of the July 1 incident says 31-year-old Ritchie Morse was moving a section of submarine with a double-cabbed transport device that was partially inside and partially outside the hangar door. The Providence Journal reports that Morse realized he had forgotten his hardhat in the front cab of the vehicle. When he went to retrieve it, he was unable to squeeze through the small gap between the door and the transport.


Worker dies after industrial accident in West Bottoms

July 8, Kansas City, Missouri - A 56-year-old man died this morning after an industrial accident in Kansas City’s West Bottoms...The victim was trying to remove an electric motor from the scaffold and he activated the motor to place some tension on a cable. The action apparently caused the bracket on the roof to dislodge and fall. It struck the victim on the top of the head, police said. He was taken to a hospital where he was pronounced dead.


Worker dies after fall into Hershey’s-bound chocolate

July 8, Philadelphia, Pennsylvania - A temp worker at a Camden chocolate processing plant died this morning after he fell into an eight-foot vat that was mixing and melting chocolate to be used in Hershey's candy. Vincent Smith II, 29, of Camden, was standing atop a platform and tossing blocks of solid, raw chocolate into the tank, Jason Laughlin, spokesman for the Camden County Prosecutor's Office, said. The tank was heated at 120 degrees Fahrenheit, and paddles inside stirred the chocolate as it was thrown in.


We Energies worker dies from steam burns

July 8, Milwaukee, Wisconsin - A We Energies employee who suffered extensive burns from steam escaping from a pipe last month has died from his injuries. The Milwaukee County Medical Examiner's Office released a report Wednesday on the death of 58-year-old William Kaiserling, a steam services mechanic who was a 30-year employee of the utility. The report said he was working to release water during heavy rains June 19 in downtown Milwaukee when the accident happened, leaving him with extensive burns and inhalation of heat from the steam.


Worker dies after being pulled from muddy east side sinkhole

July 7, Detroit, Illinois - A construction worker has died after being buried up to his shoulders in a sinkhole on Detroit’s east side this afternoon. Detroit Police Sgt. Eren Stephens Bell said the 36-year-old Detroit man, who was not identified, was working on a residential road reconstruction project when the road collapsed and debris, mud and a slab of concrete fell on him. The man was pronounced dead at a nearby hospital.


Business owner shot to death

July 7, Danville, Illinois — A local business owner was shot and killed outside his car washing and polishing shop Monday morning. Bennie Moten, 45, was pronounced dead at Provena United Samaritans Medical Center following the shooting. A worker at Moten’s business, JB Professional Detailing Shop, received minor injuries from a gunshot in the morning incident. City detectives and county investigators responded to an original call of shots fired at the scene. Police tape blocked off the street and entrance in front of Moten’s business at 729 Cleveland St.


Truck driver killed in fiery crash on I-75 in Hernando County

July 7, Ridge Manor West, Florida — Heading south on Interstate 75 in the predawn darkness Monday, Aubrey Pinion felt like he had the highway to himself....According to the Florida Highway Patrol, a semitrailer truck rear-ended Pinion's 1995 Dodge van just south of State Road 50, sending the van into the guardrail and causing the semi to overturn and catch fire. The southbound lanes were closed for about four hours. The truck driver, who has not been identified, was killed in the crash.


Park employee dies mowing lawn

July 7, Houston, Texas - A Harris County employee who worked outside was found dead on the job Monday afternoon. David Reynolds, 48, was a Precinct 1 Park Attendant who was assigned to mow the grass at Dow Park around 3pm. Shortly after that, he was found unconscious. With the mid-day heat bearing down on him, Reynolds started his evening shift at the park. Around 3pm, his white truck was unloaded and a co-worker had just filled him in on what to do.


Norwalk man dies from injuries in barge accident

July 7, Fairfeild, Connecticut - The crew job foreman injured on a barge replacing pilings in Pine Creek last month has died. Paul Thomas Rysz Jr., 33, a lifelong resident of Norwalk, was apparently struck in the head with a steel pin used to secure heavy steel rods at the mouth of the creek on June 26. At the time of the accident, Rysz' crew was using a crane to pull an assembly of rods, cables and pins that are pushed into the river bottom to hold the barge in place. Rysz was an employee of Norwalk Marine Contractors, which specializes in projects that involve marine pile-driving. It has been in business since 1985, according to the company's Web site.


Cab Driver Dies After Hitting Tree

July 7, Lake City, Florida - A cab driver has died after his taxi hit a tree this morning. Lake City police say it happened just after 9 this morning on Lake Desoto Circle. Police found the driver, Thomas Edkin, unconscious and not breathing. An officer started CPR and called for rescue, who took Edkin to the hospital. Edkin was pronounced dead less than two hours later. Investigators believe that possibly an unknown medical issue caused the him to crash the yellow cab taxi into the tree.

July 6, Lenoir City, Tennessee — Authorities say one of two workers buried in concrete at a Tennessee construction site has died and the other has been taken to a hospital in critical condition. Police Lt. Mike Tinnel tells The Knoxville News Sentinel that a form for a 30-foot wall collapsed during a concrete pour Monday afternoon at the Lenoir City Utilities Board sewage treatment plant. Deputy Fire Chief Tony Brock says rescue crews used rappelling equipment and baskets to pull the workers from the concrete.

July 6, Ocracoke, North Carolina - Two more workers who had been preparing for an Independence Day celebration died of injuries suffered when a truckload of fireworks exploded on a dock, authorities said. Disney Employee Killed In Monorail Crash

July 5, Orlando, Florida -- A 21-year-old Walt Disney World employee was killed when two monorails carrying several guests collided at Magic Kingdom, emergency officials said. Austin Wuennenberg, the operator of one of the monorails, was pronounced dead at the scene after the monorails collided at about 2 a.m. Sunday near the Ticket and Transportation Center. Disney said it was the first fatal crash in the monorail's 38-year history in the park.


Truckload of fireworks explodes in N.C., killing 2

July 5, Ocracoke, North Carolina - A truckload of fireworks exploded Saturday morning on a dock at a remote North Carolina island, killing two workers and critically injuring three others preparing for an Independence Day celebration, authorities said. The explosion shook homes and businesses and was heard across a village on the southern end of Ocracoke Island, which is the only inhabited part of the 15-mile spit of land, accessible only by boat.


N.C. officer dies in crash with Horry County patrolman

July 3, Myrtle Beach, South Carolina - A North Carolina correctional officer was killed Thursday in a two-vehicle crash involving an Horry County police officer on S.C. 544 and Singleton Ridge Road. Around 2:15 p.m., Ricky Ruggerio, an Horry County patrol officer, was traveling west on S.C. 544 in a department cruiser and collided with a Dodge traveling east on S.C. 544, as the Dodge attempted a left turn onto Singleton Road, according to Sonny Collins of the S.C. Highway Patrol. Timmy Fulmore, a 42-year-old from Orrum, N.C., was killed. He worked for the Department of Corrections in Tabor City, N.C., according to the Horry County Coroner's office.


Cab Driver Shot to Death in East Palo Alto

July 3, East Palo Alto, California - Police in East Palo Alto are searching for a gunman, after a 31-year-old Yellow Cab driver was killed overnight. The initial investigation suggests robbery was not the motive in the shooting. The San Jose man still had cash on him when police arrived on the scene, on the 2700 block of Hunter Street. "The first call was fireworks, through our shots fired system," explained East Palo Alto Police Department Captain Carl Estelle. "While officers were en route to the location, it was determined that it was multiple gunshots."


Officers believe robbery was motive

July 3, Dallas, Texas - taxi driver was found stabbed and then burned Thursday night in rural Denton County. The Freedom taxi driver’s name has not yet been released. Justin firefighters found his body near State Highway 114 and John Day Road, about five miles west of Texas Motor Speedway, as they were extinguishing a grass fire at a petroleum well lease site about 9 p.m. Denton Sheriff’s Office officials said the man was stabbed in the chest and his throat cut.


Man killed in collision with train

July 2, Springfeild, Missouri - A 30-year-old Ozark man died Wednesday afternoon when a coal train slammed into the van he was driving east of Springfield. Michael Biggs was pronounced dead at the scene of the fiery crash on River Walk Lane, just west of the intersection of Farm Roads 193 and 199, south of Sunshine and southwest of Turners. An employee of Long Refrigeration in Ozark, Biggs was hit by a Burlington Northern-Santa Fe engine pulling empty coal cars as he crossed railroad tracks that intersect River Walk, according to Sgt. Dan Bracker of the Missouri Highway Patrol.


Worker dies from electrical shock suffered while repairing machine at Fort Wayne bakery plant

July 2, Fort Wayne, Indiana — Authorities say a worker died after he suffered an electrical shock while repairing a machine at a Fort Wayne bakery plant. The Allen County coroner's office ruled that the death of 50-year-old Richard Warren of Morris, Ill., was accidental following an autopsy Wednesday. Warren died at a Fort Wayne hospital soon after he was shocked Tuesday while working at the Aunt Millie's Bakeries plant.


Worker killed at RI submarine plant

July 2, North Kingston, Rhode Island -- A North Kingstown man has been died while working at the Electric Boat Corporation manufacturing plant. North Kingstown police said Wednesday that the 31-year-old worker, who was not identified, had been using a transport device to move a section of steel outside. When it started raining, the steel had to be moved back inside. The machinery was not moving as the worker got out. But he became trapped between it and a large door that was being closed. Officials say the employee, who had worked at Electric Boat for 10 years, was treated on the scene this morning and died at Kent County Hospital.


Willows orchard worker killed in tractor accident

June 30, Artois, California — A 53-year-old man was found dead Monday night after a tractor he was operating near County Road 35 apparently ran over him, pinning him beneath a rear wheel. The victim, Reginaldo Correa Talamantes, of Willows, was discovered by his wife, who went to look for him about 9:30 p.m. when he failed to return from work, and didn't answer his cell phone. Colusa police officer Juan Perez, a relative of Talamantes, was assisting in a search for the man and said he was directed to a shop area in a field owned by Trinity Orchards of Chico. Perez told Glenn County Sheriff's officials he found the tractor wedged against a tree, still in drive, with its rear wheels spinning. Talamentes was pronounced dead at the scene.


3 cleaning workers killed in putrid NYC well

June 30, New York, New York — Three workers hired to clean out a putrid well at a waste transfer station on Monday apparently were overcome by toxic fumes, fell down a narrow shaft and died, the fire department said. Initially one worker went into the shaft, and the other two went in after him when he didn't return, said John Sudnik, fire department deputy assistant chief of the Queens borough command. They all fell through the shaft and into the well.

Teen summer worker dies falling nearly 100 feet from cliff in Ohio state park

June 1, Calcutta, Ohio — Authorities say an Ohio teenager fell nearly 100 feet to his death inside a state park where he was doing tree trimming work. The State Highway Patrol says 17-year-old Dustin Huffman suffered head trauma in Tuesday's fall down a cliff and was pronounced dead at the scene in Beaver Creek State Park, about 80 miles southeast of Cleveland. Troopers say Huffman apparently lost his footing while exploring in the park during a break from his summer job cutting branches away from utility lines.


WVa worker dies after being pinned by equipment

June 1, Pleasant Valley, West Virginia - A Fairmont man has died after being pinned between an aerial lift's platform and a crane. Police say 48-year-old George Green was standing on the platform as he backed the lift out of a United Rentals garage Tuesday morning. The lift backed into a crane parked behind it, pinning Green between the platform and the crane. Marion County sheriff's Detective Sgt. Chip Phillips says Green died at the scene.


1 Dies, 2 Injured After Fall From Construction Site

June 1, Dublin, Ohio —A Columbus construction worker fell to his death Wednesday at a Dublin construction site after a scaffolding collapsed, injuring two co-workers as well...The three workers fell different distances from the scaffolding: one fell to the ground, a second worker fell two stories and a third worker fell one story, according to Dublin Public Information Officer Mike Racey. Wilbert Johnson, 52, of Columbus died after he fell an unknown distance.


Electric Boat worker killed on job

June 1, North Kingston, Rhode Island - An Electric Boat worker injured on the job Wednesday morning has died from his injuries, according to a company news release. In a bulletin sent to employees, the company said the victim was a rigger at the plant. The accident happened at 7:25 a.m. at the company's Quonset manufacturing facility in North Kingstown. The 31-year-old worker was then taken to Kent Hospital in Warwick, where he was pronounced dead, the release said. Company officials said the victim worked for Electric Boat for 10 years.


Pedmont Trucker Killed In Grinding Accident

June 1, Piedmont, Alabama - log truck driver died Tuesday when he ran into the rear of a dump truck on U.S. 278 near Hokes Bluff. James Franklin Beecham, 49, was killed in the wreck but the driver of the dump truck, 38-year-old Paul Taylor Pledger, of Ball Play, was only slightly injured. The impact caused the dump truck to turn over, dumping its load of chert. The logs shifted, crunching the cab of the Beecham’s truck and pinning him inside the wreckage. It’s the third wreck in less than two years involving a large truck overturning and spilling its load.


Farmers market worker electrocuted

June 30, Philadelphia, Pennsylvania - Maintenance supervisor Earl W. Yerkes, 85, was killed while working on a rooftop air conditioning unit. A faithful employee who worked for the Quakertown Farmers Market for nearly 50 years died there Saturday afternoon when he was shocked by at least 450 volts of electricity, officials said. Earl W. Yerkes, 85, was pronounced dead at St. Luke's Hospital Quakertown, but indications are he might have died immediately, said Richard Woldow, owner of the farmers market on Station Avenue in Richland.

June 26, Albertville, Alabama - Progress Rail’s scrap yard, on Industrial Boulevard in Albertville, was the scene of a fatal accident shortly after 10 p.m. Wednesday.

Recycling employee killed in Irwindale work accident

June 25, West Covina, California - A recycling service employee was killed Thursday when a metal cable he was replacing on a dump truck snapped and struck him in the head.

Man fatally shot in head in Castro Valley hydroponics store


June 26, Castro Valley, California - The Alameda County Sheriff's Office is searching for at least two suspects after a man in a Castro Valley hydroponics business was fatally shot in the head Friday afternoon. The man was taken to Eden Medical Center in critical condition, and died there Friday evening....Miles did not identify the man — nor whether he was the business owner, an employee or a customer — pending notification of relatives. He said some witnesses had provided descriptions of possible suspects, but no description was released to the media.


2 workers killed in fatal Palm Beach County crash

June 25, Jupiter, Florida - Authorities say two road construction workers have died in accident in Palm Beach County. Police say a third worker was taken to the hospital with non-life threatening injuries suffered in the crash early Friday morning in Jupiter. The worker's condition was not known. The workers were repaving the roads as part of an ongoing project. The names of the workers were not released. Authorities have not released any additional details of the crash.


Man dies in workplace accident in Durant

June 25, Durant, Oklahoma - An employee died today at a Durant business when he was hit by a large piece of granite. The man and a coworker at Allied Stone, 2201 W. Arkansas St., were moving slabs of granite on a forklift at 8:45 a.m. when one of the slabs split down the middle, falling on the man, Durant police Lt. Carrie Wyrick said. The man suffered a head injury and died at the scene, Wyrick said. The identity of the man has not been released. Durant police are still investigating the accident.


Houston Officer Killed

June 25, Houston, Texas - Two men are facing murder charges in connection with the death of veteran Houston police officer Henry Canales. The Harris County District Attorney has charged the suspected illegal aliens 41-year-old Andres Nava-Maldonado and 36-year-old Xiomara Mendez-Rosales with fatally shooting the undercover officer during a robbery. The shooter, 37-year-old Roberto Pedroza Carrillo was shot and killed at the scene.


I-35 construction worker killed in Austin area

June 24, Austin, Texas — The driver of a dark-colored SUV is sought after a construction worker along Interstate 35 in the Austin area was struck and killed. Police Sgt. Kenneth Conner says the accident happened before dawn Wednesday along southbound I-35. Conner says the SUV drove into the construction area, striking and killing the worker, then sped away.


Construction worker's death blamed on heat

June 25, North Little Rock, - A 44-year-old North Little Rock man has become the first person in Pulaski County to die because of the heat. Frank Goy died Wednesday afternoon at a North Little Rock hospital after becoming ill while doing construction work at a home Wednesday morning. Goy's body was sent to the state Crime Laboratory for an autopsy but Pulaski County Coroner Garland Camper says the death fits the profile of being heat related.


Man Doing Yard Work Struck, Killed By Car

June 24, Allentown, Pennsylvania - One person was killed after being hit by a car in Monroe County. It happened on Route 196 in Tobyhanna in Coolbaugh Township yesterday afternoon. Police say Lisamarie Ostuni, 43, of Tobyhanna, was traveling south on Route 196 when her vehicle left the road. They say the vehicle was on the berm of the road when it struck Robert Apuzzi, who was doing yard work. Apuzzi, 49, was killed.


Man found dead after falling at Huntsman facility in Port Neches

June 24, Port Neches, Texas - A cause of death has not been determined for a man who was found dead after falling this morning at the Huntsman facility in Port Neches. Around 7:15 a.m., workers at the facility on the corner of FM 366 and Hogaboom Road found the man on the ground near a rail car, according to Brent Weaver of the Jefferson County Sheriff's Office. Port Neches fire and EMS as well as the Sheriff's Office responded to the call.


Consol Mine Worker Crushed To Death In Pennsylvania

June 24, Steubenville, Ohio - An investigation is under way into a fatal mining accident at a Consol mine in Greene County, Pa. A 54-year-old worker was putting bolts into the roof of the mine when a rock fell, crushing him, according to Pennsylvania Department of Environmental Protection spokesman Tom Rathbun. Rathbun said the mine will be idled while the DEP and other officials investigate the accident.

June 23, Titusville, Pennsylvania - A coroner says a New York man has died of head injuriesJ from falling from a roof he was helping demolish at a former hardware store in northwestern Pennsylvania. The Erie County Coroner says 69-year-old Millard Young, of Akron, N.Y. died Sunday at Hamot Medical Center in Erie from complications of injuries he received in the fall. Young was working for Rockford Construction Co. of Grand Rapids, Mich. when he fell while doing roof demolition work in Titusville on June 12. The building is being renovated, possibly for use as a video rental store.


Construction worker from The Colony run over, killed by truck he was repairing

June 23, Dallas, Texas - A construction worker in The Colony was killed earlier today when he was run over and dragged by a truck he was repairing. The incident happened about 9:30 a.m. at a city pump station in the 4100 block of Main Street. The construction site has been closed while the death is being investigated. It appears the victim was working on the truck when another worker, not realizing anyone was under the vehicle, got in and drove off, said Lt. Darren Brockway, of The Colony Police Department.


Ex-jockey killed in S.J. track accident

June 22, Stockton, California - A 47-year-old man died after he was hurt while working Sunday on the San Joaquin County Fair's horse-racing track, fair and law enforcement officials confirmed Monday. David Fowler-Wright, a Humboldt County resident, was a former jockey who was employed this year as a rake man, responsible for raking the track smooth of tire tracks that can frighten horses....While the gate was moving, Fowler-Wright either slipped or tried to jump off it and fell. He was run over by the gate's back tires.


Family of woman killed in lab fire raps regulators

June 22, Los Angeles, California —The family of a 23-year-old research assistant killed in a lab explosion at UCLA says the state's investigation was shoddy and the university's penalties too light. The family of Sheharbano Sangji issued the criticisms and asked for harsher penalties in documents filed with the California Division of Occupational Safety and Health. The family's requests were denied last week because only an employer may appeal a fine.


Maryland Construction Worker Struck and Killed by Dump Truck

June 22, Maryland Police say a construction worker is dead after being hit by a dump truck. The worker was hit around 10:15 p.m. Thursday at a construction site in Capitol Heights. June 22, Grandview, Indiana — Marilyn Gentry said her only son, Frankie Jordan, finally had found the right woman in his life. "They got married on June 13 and went on their honeymoon in Gatlinburg (Tenn.)," the 75-year-old Spencer County woman said. "The 13th has always been my lucky day. I talked with him when he got back, and he was in great spirits." Jordan was about nine hours into a 12-hour shift when he was found dead early Monday morning after an accident at the ThyssenKrupp Waupaca foundry in Tell City, Ind., where he was a maintenance man. Perry County Deputy Coroner John Gebhard said Jordan was pinned between two pieces of equipment when rescue workers arrived at 3:41 a.m.


Mass. Man Dies After Being Shot At Gas Station

June 22, New Bedford, Massachusetts - A 32-year-old man shot while visiting his friend at a New Bedford gas station has died. Bristol district attorney spokesman Gregg Miliote says John Martin of New Bedford died Sunday. He was shot in the head a day earlier during an attempted robbery at the Petro Mart. Martin was visiting his friend, who is a store clerk, when another man walked into the store, showed a handgun and said he would rob the business.


Four killed in Metro Red Line collision

June 22, Gaithersburg, Maryland - At least four people were killed, including a female Metro train operator, and at least 70 people were injured Monday evening when two Red Line trains collided between the Takoma and Fort Totten stations about 5 p.m., according to Metro and Takoma Park police officials. The collision occurred when a train heading toward the Shady Grove station slammed into another train from behind during rush hour. The names of the four people killed have not been released, but one reportedly was a Metro employee. There are conflicting reports on the number and severity of the injuries. But according to Metro's Web site, some injuries were serious. Metro did not know the cause of the collision.


Foundry worker found dead

June 22, Tell City, Indiana - A Grandview man dies while working his shift at the Waupaca Foundry in Tell City. The Perry County Deputy Coroner, John Gebhard, says 42-year-old Frankie Jordan was found dead around 3:30 this morning. Gebhard says the cause of death has not been determined, but there is trauma to Jordan's chest. An autopsy will be performed, but it is not yet scheduled. A family friend tells 14 News Jordan just got married last week.


Man Killed at U-M

June 22, Ann Arbor, Michigan - Authorities say a 48-year-old worker was killed when a load of roofing materials fell from a crane and hit him at a University of Michigan hospital construction site.Gary Winisky Jr. of Garden City was struck Monday at the site of the new C.S. Mott Children's Hospital in Ann Arbor. He died in the emergency room at the university's medical center. The Ann Arbor News says it's the third construction death on campus in the past 16 months.


Truck driver in fiery crash is identified

June 21, Long Beach, California - Authorities on Sunday identified a truck driver killed in a fiery crash on a freeway interchange. Stanley Jones, 51, was killed June 13 when the tanker truck he was driving plunged off the road while making the transition from the eastbound Artesia (91) Freeway to the northbound Long Beach (710) Freeway, said Los Angeles County coroner's spokesman Jim Blacklock. The truck spilled about 8,000 gallons of gasoline, igniting an intense blaze that melted guardrails and left only the truck's axles and small portions of the cab.
