Owner of Wrecker Business Found Dead

September 18, GREENVILLE, NC - 59 year old Walter Clay Malloy Sr, was found dead outside his business, Clay's Wrecker Service. They say the business was not broken into or robbed. Police are not saying how Malloy died, but they are treating this an a death investigation.


Police Officer Found Dead

September 19, ORWIGSBURG, PA - 47 year old William Bradley, a police officer, was found dead Sunday in the borough police station, state police said. Bradley was found with an apparently self-inflicted gunshot wound, troopers said. Investigators did not specify where on his body he was shot.


Ohio Man Dies in Farm Accident

September 18, PARIS, OH - 50 year old Steven Shockling, working atop a silo at his township farm fell about 50 feet to his death Saturday morning. Shockling was working on equipment at the top of the empty silo at his family farm when he fell inside about 10:45 a.m., said Rick Walters, Stark County Coroner’s Office investigator.


Man Killed Making Repairs to Car

September 18, GREENSBORO, NC - 19 year old Talon James Brandon Gravely, an employee for Whip Appeal Auto, LLC, was killed Saturday when part of the vehicle he was working underneath collapsed on him. Detective M.D. Matthews said other people were working at the shop at the time, however no one was with Gravely, who was underneath a car outside the shop.


Man Working Vineyard Dies in Truck Rollover

September 17, WINDSOR, CA - 73 year old Leo John Becnel died when his truck overturned at his vineyard in unincorporated Sonoma County west of Windsor on Friday evening, according to the California Highway Patrol. Becnel was backing a dump truck down a steep slope made up of loose dirt, and he was not able to stop the truck, which turned perpendicular to the slope and rolled over. He was not wearing a seat belt, and the truck did not have a windshield. He was ejected from the car and suffered fatal injuries, according to the CHP.


Backhoe Operator Crushed by Falling Tree

September 17, BARRETT, MN - Mitch Brethorst, owner of Brethorst Excavating, was killed when a large cottonwood tree he was trying to clear fell on the full-swing backhoe he was operating, crushing him. Brethorst had dug on two sides of the tree when the tree toppled over, landing on top of the cab of the backhoe. The tree crushed the cab of the backhoe, trapping him inside.


Truck Driver Killed in WV Accident

September 17, WHEELING, WV - 58 year old Ronald Kiser of Fayetteville died after he crashed his truck into another. Kiser drove into the back of another tractor trailer around 2:30 this morning on I-70 in that area. The Ohio County Sheriff says the driver of the first tractor-trailer was traveling at a slow rate of speed with his hazard lights on when Kiser hit him. Kiser's cab became pinned under the tractor of the truck he hit and burst into flames. Kiser died at the scene.


Forklift Overturns, Killing Operator

September 17, PHOENIX, AZ - 29 year old Lewis Acuña, who lived just outside of Arizona City, was killed Friday in an accident involving a forklift. Acuña, who worked for Evergreen Turf, was delivering sod to a home had just dropped off one load and was returning back to end of the very steep driveway for a second load when he lost control of the forklift. The forklift picked up quite a bit of speed before it flipped on its side, ejecting Acuña and pinning him underneath. He was pronounced dead at the scene.


Man Killed Working on Farm Tractor

September 16, BARBER, OK - 56 year old Richard Cinnamon has been killed when the tractor he was working on rolled over him. Investigators say the tractor was running while Cinnamon worked on it and it apparently slipped into gear and ran over him.


Road Construction Worker Struck by Truck

September 17, FOREST VIEW, IL - 33 year old Aurelio Rosales, an employee of Midwest Fencing, was killed Friday afternoon by a semitrailer-tractor truck coming off of the Stevenson Expressway in southwest suburban Forest View, authorities said. Rosales was making repairs to a barrier fence when he was struck, said IDOT spokesman Guy Tridgell.


Construction Worker Dies in Fall

September 17, OVERLAND PARK, KS - A construction worker died and two other men were seriously hurt Friday when a scaffold collapsed during construction of a middle school in south Overland Park. The men were believed to be working on the scaffold in a three-story interior stairwell of the school when the scaffold rig, which was erected from the ground, as opposed to being suspended, collapsed. An Overland Park Fire Department spokesman estimated the fall at 30 to 35 feet. UPDATE: Authorities have identified the worker who died as 52 year old James D. Tyree of Eudora, KS.


Young Worker Dies in Grain Elevator Incident

September 17, COLUMBUS, OH - A 21 year old year old man was killed this morning in a Marion County grain elevator. He and another employee were cleaning a grain bin and died when he fell and was trapped in the auger mechanism. The auger, located at the bottom of the bin, was working as the two men were cleaning, shoveling grain into the auger instead of shoveling it out of the bin. Assistant Chief Dan Williams said it is suspected the victim's foot went through the grating and got caught in the auger, which pulled him down.


California Firefighter Dies After Training Exercise

September 16, IRVINE, CA - 56 year old James M. Owen died today after a routine training exercise. Battalion Chief Kris Concepcion of the Orange County Fire Authority said Owen collapsed in full cardiac arrest after an urban search-and-rescue drill in Irvine. Concepcion did not know if Owen had just finished a classroom session or an active part of the drill.


Accident Claims Farmer's Life

September 16, WESTMORELAND, KS - 44 year old Frederic Biesenthal was found dead just before 10 p.m. Thursday, the victim of an apparent farm accident, Pottawatomie County Sheriff Greg Riat said. Biesenthal may have become entangled with a moving part of the tractor when he was moving hay.


Laundry Store Employee Gunned Down

September 16, TRENTON, NJ - 33 year old Ambreesh D. Patel was fatally shot during the daylight robbery at a West Ward laundry after he told the gunman he did not have any money, police said. An armed man who allegedly walked into the store with a T-shirt covering his face, shot Patel once before fleeing. The suspect remained at large.


Worker Killed at Plant, Crushed by Machinery

September 15, NORCROSS, GA - an 48 year old man was was crushed on Wednesday by a stacker at the RockTenn plant. RockTenn produces paperboard and packaging products. His name or other details have not been released.


Officer Accidentally Shot During Training Exercise

September 15, ST.JOSEPH, MO - 35 year old Officer Dan De Kraai was fatally shot Wednesday afternoon while on a break from a training session, the Police Department said. was hit in the back after another officer’s handgun discharged about 2 p.m. at a closed school that was the site of the training session, according to a written statement from police. The shooting appeared to have occurred during some sort of demonstration, he said. De Kraai and the other officer were longtime friends.


Trucker Dies in Fiery Crash

September 15, TRENTON, NJ - 44 year old Michael Hyland of Smyrna, DE died in a fiery crash on the New Jersey Turnpike Wednesday. "For reasons that are under investigation he lost control of the vehicle, veered to the right and hit the concrete barrier, veered to the left and hit the median, then veered to the right again and struck the barrier," Trooper Brian Polite said. The series of impacts caused the tractor to burst into flames, and rescue crews were unable to rescue him before he perished.


Park Volunteer Dies in Tractor Accident

September 15, ELLENSBURG, WA - 80 year old Robert Arce of Presidio, TX was killed after he was pinned under a tractor near Ellensburg, Wednesday afternoon. Arce was attempting to pull a stuck forklift off the yard using a tractor, when it flipped over on top of him. KNDO spoke to Arce's wife who said she and her husband drove up each summer in their motorhome to volunteer at the park. This was their third year to help out at the event. Arce's wife would give tours to park visitors, while he took care of the grounds that would include lawn care.


Forklift Accident Claims Life of Business Owner

September 15, CHANDLER, OK - 64 year old Bill C. Matthews, owner of Lincoln County Farm Center, died Wednesday in an incident involving a forklift. Details of what happened are not clear but it is being reported that he was not driving the forklift.


Student Driver Hits and Kills UPS Employee

September 15, DES MOINES, IA - 64 year old Daniel Raber, a United Parcel Service employee, died after he was hit by the trailer of a semitrailer truck at the Des Moines airport early Wednesday. Raber was opening a gate so the truck could make a delivery when the trailer struck him. He was pinned by the trailer against a guard shack, but not crushed, police said. Raber then fell, and the rear wheels of the trailer ran over his legs. The driver of the truck that hit Raber was a trainee, and the instructor was a passenger in the truck.


Man Dies in Mower Accident at State Park

September 14, GERARD, MO - 62 year old Donald D. Lamb of Hepler was killed after the mower struck him while he tried to free it from mud. Lamb apparently got off the mower to try and free it when the machine moved forward, pinning him.


Asphalt Paving Company Employee Killed in Accident

September 14, LIVERMORE, CA - 36 year old Ramiro Vargas died Tuesday after he was struck by runaway paving machine in unincorporated Livermore, the Alameda County Sheriff's Office said. Sheriff's Lt. Kelly Miles reported that while making repairs to a private driveway the paving machine apparently malfunctioned and rolled down a hill. It crashed through a barbed wire fence and stuck a worker standing next to an asphalt truck. The worker did not hear the paver coming because of the generator noise and didn't see it because his back was turned, Miles said.


Accident Involving Heavy Equipment Kills Worker

September 14, MINOT, ND - 24 year old Joel Douglas Larsen of Gillette, WY was killed late Tuesday afternoon near Minot. Larsen was working on a skid-steer loader when the front end came down on top of him, killing him. Ward County Sheriff’s Capt. Todd Keller says some safety pins apparently were not in place. Keller said Larsen was working for Clark’s Heavy Equipment and Field Service Repair, an oil field service company based in Belle Fourche, S.D.


Man Dies in Mower Accident

September 14, STUART, FL - a 26 year old man lost his life Tuesday in a landscaping accident at an apartment complex in Plantation. The man was driving a riding industrial mower adjacent to a man-made lake when somehow the mower ended up in the lake. It is not clear if he experienced a cardiac episode before or due to the accident as he was in full arrest when rescuers brought him to the surface after being pinned under the mower.


Driver Dies When Refuse Container Truck Hits Bridge

September 14, CLEVELAND, OH - 39 year old Marvin Matos, who may have been unaware that the vehicle’s boom was in the upright position, died Tuesday after a violent collision with the Hull Street Amtrak bridge. The truck is believed to hit the bridge at considerable speed, police said. A police officer working a nearby traffic detail was immediately on the scene to provide aid, police said.


Gas Station Worker Dies After Falling Off Ladder

September 14, ATLANTA, GA - an unidentified worker fell to his death Tuesday night. Investigators said the man was changing letters on a sign at Cheshire Bridge and La Vista roads when he fell off a ladder. The man suffered major head trauma and died at the scene.


Driver of Semi Truck Dies in Crash

September 14, CLEVELAND, OH - a car caused an accident sending a Tank Integrity Services semi-truck that was traveling westbound over the embankment and onto Lee Road below causing the death of the semi driver. The victim's identity has not been released.


Virginia State Trooper Dies in Training Incident

September 14, RICHMOND, VA - 41 year old Virginia State Trooper Mark D. Barrett died Tuesday during an annual in-service training in the southeastern portion of Lake Anna on the Louisa County side. Trooper Barrett and his “dive buddy,” a fellow trooper, were conducting a training dive. As they were ascending, the dive buddy noticed Trooper Barrett was having difficulty. The dive buddy aided Trooper Barrett to the surface and immediately signaled for help from the other divers. He died after being transported to a hospital in Henrico County.


RV Mechanic Killed in Garage Accident

September 14, ARROYO GRANDE, CA - 65 year old Ronald Arthur Kelsey died of blunt force trauma after being run over by a motorhome Tuesday afternoon in Arroyo Grande, and a Guadalupe resident was arrested on suspicion of felony driving under the influence and gross vehicular manslaughter while intoxicated. Kelsey was working underneath the motorhome when for some unknown reason the suspect put it into motion and ran over the victim.


Highway Worker Struck and Killed

September 14, FRAMINGHAM, MA - 57 year old Gregory Vilidnitsky, an inspector for the state’s Department of Transportation, was concentrating on his work Tuesday night. He was checking the pavement temperature as part of quality control when red pickup carrying two allegedly drunk individuals ran into Vilidnitsky and then smashed into an oil truck, prosecutors and police said. Vilidnitsky, who would have turned 58 on the 16th, was pronounced dead at the scene. The occupants of the pickup are in police custody.


Water Truck Driver Thrown Under Truck Dies

September 13, LEXINGTON, SC - 47 year old Dewayne Harrison of New Zion was driving a water truck used to keep dust down. He parked the truck but the parked truck began moving as Harrison was exiting the vehicle, and he lost steering control. The truck hit Harrison, who was thrown under the vehicle as the truck hit a ditch.


Driver Dies When Cement Truck Overturns

September 13, DETROIT, MI - 60 year old Arlander Eustice Rogers Jr. died Monday, hours after the rear tire of the cement truck he was driving blew, forcing it to overturn on westbound I-94 near Outer Drive. The truck flipped on its side and went off the roadway into the median, striking the guardrail, according to the Michigan State Police. The truck was hauling two trailers, and one broke loose as the truck flipped, hitting the overhead road sign. The sign then fell on the trailer.


City Employee Electrocuted

September 13, FAIRVIEW, OK - 25 year old Matthew Meister a lineman for the municipal electric services, died while installing an electric meter at the Mennonite Brethren Church. The cause of the accident remains under investigation but authorities think Meister slipped while working and fell onto a set of wires, Fairview City Manager Dale Sides said.


Farm Tractor Accident Kills Man

September 13, WATONWAN COUNTY, MN - 83 year old John James Nasman was killed Monday on his farm in southern Minnesota after his tractor apparently rolled over onto him, pinning him underneath. The accident appeared to have occurred earlier in the day, according to the sheriff's office.


Lowe's Employee Dies After Fall From Ladder

September 13, SEARCY, AR - 76 year old Jerrell Cockrell has died after apparently falling from a 10 foot ladder inside the store. He was found unconscious by a store customer.


Man Crushed in Tree Trimming Accident

September 13, SIMSBURY, CT - 24 year old Luke Stetson of Windsor Locks was killed Monday by a falling tree. Police in Simsbury say Stetson was about 30 feet up a tree trimming branches and wearing a safety harness when the white oak fell over and landed on top of him. The owner of Wayne's Tree Removal says the tree Stetson was working on looked healthy and strong, but he learned later it had dry rot inside. He called it a freak accident.


Steel Beam Falls, Kills One at Science Park

September 13, NEW HAVEN, CT - One worker was killed and three others injured, when an I-beam fell at a construction site in the Science Park area Monday morning. Based on preliminary accounts two workers were inside a mechanical lift when the “significant” horizontal beam fell from about 30 feet on the west side of the structure, struck the lift bucket and crashed to the ground. He said it was unclear if the other two injured workers were somewhere elevated in the structure and fell to the ground with the beam falling on top of them or they were on the ground when the collapse happened. UPDATE: Authorities have identified the victim as 44 year old Robert Adrian of Coxsackie.


Man Dies in Construction Truck Rollover

September 13, LEXINGTON, SC - An unidentified man is dead after an accident at a construction site in Lexington County. Lexington police said a crew at a construction site was getting ready to leave for the day when, according to officers, one of them was behind the wheel of a water truck. For some reason, the truck began rolling, and the driver exited the vehicle. Police said the truck hit a ditch, which shifted the water in the truck. Somehow, the man became pinned beneath the vehicle.


Preston County Farmer Killed in Tractor Accident

September 12, KINGWOOD, WV - 59 year old Terry Delaney was killed Sunday after his tractor rolled over on top of him. He was found pinned under his tractor by his wife and neighbor. An investigation by the Preston County Sheriff's Department concluded that Delaney was working on fencing, on an embankment, when the tractor overturned.


Farmer Dies After Being Trapped in Equipment

September 12, ARCADIA, MI - 60 year old Rick Williams died Sunday when he became trapped in farm machinery. Williams was cleaning equipment used to bind hay when he apparently became trapped shortly after 4 p.m. Sunday, Manistee County sheriff's officials said. A neighbor turned off the baler and freed Williams, who died at the scene, sheriff's officials said. Williams wanted to make his farm on St. Pierre Road accessible for children with disabilities to fish and ride horses. Family members said that goal will continue as a tribute to Williams, who died in a weekend farm accident.
