The Weekly Toll: Death in the American Work Place

CHP Officer Struck and Killed During Traffic Stop

June 27, BARSTOW, CA - 28 year old CHP Officer Justin McGrory was struck and killed Sunday when a car veered onto the shoulder of the freeway during a traffic stop in San Bernardino County, authorities said. The incident happened about 3:40 a.m. when two officers stopped a vehicle for speeding on the northbound 15 Freeway in Barstow, CHP Officer Mario Lopez said. A 2009 Pontiac G6 traveling north clipped the right rear of the patrol car and struck Officer Justin McGrory. He was airlifted to a hospital where he died of his injuries.


Firefighter Dies From Heart Attack During Storm Clean Up

June 27, MACON, GA - 42 year old Jay C. Brown died from an acute heart attack Sunday night while trying to clean up from the severe weather that rolled through the area. The fire department was called out to cut down and remove trees that fell during the storms on Friendship UMC Road, that's in the Gresston area. Dodge County Coroner Joe Smith says Brown was chief of the Gresston Volunteer Community Fire Department. It's part of the Dodge County Fire Association.


Pilot That Pulls Salt Lake Bees Banner Killed in Crash

June 27, WEST JORDAN, UT - 34 year old Quinn Mark Falk of Cottonwood Heights died when his plane crashed after taking off about 5 p.m., West Jordan Police Department Capt. Dan Gallagher said. Witnesses who saw the crash told police they saw the plane take off and do a quick loop around the airport as attendants laid out the Bees banner advertising the team. The plane flew low and hooked the banner. It began to climb, but the banner dragged along the ground as witnesses heard the engine begin to lose power, Gallagher said. Falk released the banner, but the plane could not continue its ascent and nose dived into the ground. Witnesses began running toward the plane when it burst into flames. When fire crews arrived, the plane was completely engulfed, Gallagher said.


MS Fire Captain Suffers Heart Attack on Duty

June 26, CLINTON, MS - 54 year old Captain John Thurman died from a heart attack after exercising while on duty, investigators found. Thurman was jogging alone on a paved trail behind his fire station as part of his fire department's physical fitness program when he collapsed, a NIOSH report released Thursday said. A civilian called 911 after finding him lying on the trail, unresponsive and not breathing. Firefighters initiated began CPR, but he died en route to a local hospital.


Man Dies Setting Up Motocross Camera

June 26, MORRISON, CO - 57 year old Stuart Keene of Lakewood died from a fall while setting up a camera for a nationally televised Motocross event. Keene of Lakewood fell about 20 feet Friday afternoon. He was setting up a video camera at Thunder Valley Motocross in Morrison.


Lowell Man Dies When Trapped Under Tractor

June 26, LOWELL, MI - 78 year old Ivan Blough died when he became trapped under his tractor which had rolled on a hill. "He was on a bank and he was trying to come up the hill and the tractor rolled over and rolled on top of him," Kent County Sheriff's Sgt. Eric Rakow told 24 Hour News 8. Blough had volunteered for the Boy Scouts since at least 1958, said Troop 102 Committee Member Dirk Ritzema. His two sons were Eagle Scouts. He had mowed the lawn every summer for the last 30 years.


Subway Worker Crushed to Death in NY

June 26, BROOKLYN, NY - Richard Lang of New Jersey was killed in a gruesome accident in Brooklyn Friday morning. He was crushed to death while unloading heavy material from a truck, police said. Lang, a truck driver, was unloading a haul of subway panels at the Avenue M station in Midwood shortly after 8 a.m. when the load suddenly shifted and fell on top of him, police and NYC Transit officials said. Lang was an employee of Preferred Transit Inc., a trucking company with a base in Whitehouse, N.J.


Farmer Dies When Tractor Rolls Into Creek

June 25, ROSALIA, WA - 73 year old Robert Clausen died when his tractor rolled into a creek east of Rosalia near the Spokane and Whitman County line Thursday. Deputies say Robert Clausen was spraying crops with the tractor in a field near the 1500 block of Waterman Road Thursday evening. Investigators say the tractor rolled when it got too close to the bank of the creek. Firefighters say Clausen was already deceased when they arrived at the accident scene just after 9:00 p.m.


Employee of CA Marijuana Dispensery Killed

June 25, HOLLYWOOD, CA - an un-named worker at a CA Marijuana Dispensary was killed Thursday night during an apparent robbery, Los Angeles police reported, according to MyFoxAtlanta.com. "It is a homicide," said LAPD Officer Karen Rayner. "It appears to be a robbery." UPDATE: The worker killed during a robbery attempt has been identified as 27 year old Matthew Benjamin Butcher.


U.S. Helicopter Pilot Dies From Crash in Malaysia

June 25, KUALA LUMPUR, MALAYSIA - 51 year old William Charles Scott of Meridian, MS, helicopter pilot from the United States, has died in a crash. The helicopter was used to transport timber in a remote Malaysian rain forest on Borneo island. The helicopter went down Friday in a logging zone in Malaysia's eastern Sarawak state. The helicopter was operated by the Malaysian subsidiary of Portland, Oregon-based Erickson Air-Crane Inc., which harvests timber for Malaysian loggers in Borneo.


Lightning Strike Kills TX Coastal Crabber

June 24, DALLAS, TX - 30 year old Charles Franklin Weaver, Jr. of Liberty is dead after lightning struck the boat from which he and another man were working on the Intracoastal Waterway. A Chambers County Sheriff's Office statement says Weaver Jr. was dead at the scene of the lightning strike, while 47-year-old Jerry Duff, also of Liberty, was airlifted to a Houston hospital. His injuries were not thought to be life-threatening, according to the statement. The incident happened about 9:30 a.m. Thursday as Weaver and Duff were crabbing in the canal near the Texas 124 bridge. A passing tugboat contacted authorities.


VT Man Dies in Farm Tractor Accident

June 24, E. MONTPELIER, VT - 56 year old Bruce Butler of E. Montpelier was brush hogging a field on his property on Blazier Road over the weekend when the tractor became stuck on a rock. Butler tried to jack up the tractor to free it, but it rolled on top of him. "The report came in on Monday, stating that he had not shown up for work, and it was out of character for Mr. Butler, so they had called the Vermont State Police to do a welfare check," explained Sgt. Tara Thomas of the Vt. State Police. "We reported to his residence Monday afternoon. It is believed Mr. Butler was deceased for a day, day and a half from when we conducted the welfare check." The medical examiner ruled Butler suffocated after the tractor fell on him.


Saddle Bronc Rider Dies at Jackson Hole Rodeo

June 24, CHEYENNE, WY - 20 year old Nicolas Gillett of Cody died from a skull fracture when his spur became tangled in his stirrup after he came out of the gate during the Jackson Hole Rodeo. Jackson police Lt. Bob Gilliam said Gillett's left spur got caught on his saddle after he came out of the gate. "The horse drug him for a while... and, according to one of the witnesses, may have stepped on him," Gilliam said. "The victim came free of the horse, and the horse rolled over the right side and crushed him; that's my understanding of what happened."


PA Firefighter Dies From Heart Attack

June 24, WESTOVER, PA - 41 year old Roy Everett Westover of Westover died of a heart attack after returning a hose to a fire truck. Westover complained of a headache, tightness in his chest and shortness of breath just before he became semiconscious, a NIOSH report released Wednesday said. The ambulance crew on scene found him to be sweating heavily and carried him to the ambulance for transport to a local hospital, where he died.


Worker Killed Installing A/C Unit

June 24, CARROLLTON, GA - 31 year old Matthew Hughes of Tallapoosa was killed and another injured Thursday morning while placing an air conditioning unit in the attic of a home on Canterbury Drive in Carrollton. Hughes received an electrical shock while performing wiring work on the unit at about 9:30 a.m. Hughes reportedly dropped the torch he had and set some insulation on fire. Thompson said Alan Best of Carrollton suffered burns on his bare hands while trying to help Hughes, whose clothing caught on fire. Best put the fire out and gave Hughes CPR. Both were taken to Tanner Medical Center where Hughes was pronounced dead. The exact cause of death is yet to be determined.


Fire District Chief Killed in Accident

June 24, TRI-CITIES, WA - North Franklin Fire District 4 Chief Chet Bauermeister of Basin City was fatally injured when an ATV flipped backward on a steep slope. The Franklin County sheriff's office says another firefighter riding with Bauermeister was taken to a hospital with minor injuries. The wildfire in the Wahluke Slope area near the Saddle Mountain National Wildlife Refuge was controlled Wednesday night.


Kentucky Coal Miner Dies In Underground Accident

June 24, FRANKFORT, KY - 29 year old Bobby L. Smith of Vicco was killed at James River Coal's Leeco 68 mine at Jeff in Perry County near Hazard. Details of the accident were not immediately released. Smith, who operated a machine used to dislodge coal inside the mine, was pronounced dead at a Hazard hospital. State inspectors were notified at about 1:30 p.m. EDT. This is the second fatality in as many weeks and fifth in the state this year.


Two Dead After TX Grain Elevator Collapse

June 24, RUSSELL, TX - 21 year old Max Greve of Hays and 19 year old Sean Banks of Russell were killed Thursday when part of a grain elevator collapsed on them. Two tubes -- silos of the elevator -- filled with grain, went down in what officials are calling a structural collapse. Greve and Hays, who were unloading a semitrailer, were buried in the grain. Their bodies were recovered shortly before 5 p.m. The collapse of the AgCo elevator happened just after 2 o'clock.


Two Concrete Workers Killed in Boom Collapse

June 24, LEWISTON, TX - Two workers for a Fort Worth-area company were killed and two others were injured early Thursday when the boom from their concrete pumping truck collapsed on them in Lewisville. 34 year old Gerardo Horta of Dallas died in the accident, the name of the other worker who died was unavailable Thursday afternoon. "It appears the boom from a concrete pumping truck had some type of failure, which caused it to collapse," Leggett said. The boom collapsed about 5 a.m. at 940 Lakeside Circle, where a park-and-ride lot for a new commuter-bus and rail-line project is being built. It is on the south side of the city. UPDATE: Officials have identified the 2nd person killed as 41 year old Rafael Ruiz-Garcia of Denton, TX.


Botanist Killed By Grizzly in Yellowstone Park

June 24, YELLOWSTONE, WY - 70 year old Erwin Evert of Oak Park, IL was killed by a grizzly after entering an area on the Shoshone National Forest that was officially posted as "closed" while bears were being trapped and tranquilized. Evert was killed on June 17th by a grizzly that had been tranquilized and released just hours before his death. The incident occurred about 10 miles from the East Entrance of Yellowstone National Park. Evert was a prominent botanist. His primary residence was in Oak Park, IL, where he was a research associate with the Morton Arboretum. Evert spent decades at a summer cabin on Kitty Creek near Wapiti, WY. He recently published a 750-page magnum opus on "Vascular Flora of the Greater Yellowstone Area."


Young Worker Dies in Farm Equipment Accident

June 24, GREENWICH TWP, PA - 18 year old John Richard Smith of Lenhartsville was pronounced dead by the Berks County Coroner’s Office after a reported farming equipment accident. Smith died Wednesday night when he was sucked into a hay baler at a farm on Dunkels Church Road in Greenwich Township, according to the Berks County coroner's office.


Man Dies in Farm Accident

June 24, DEKALB CO., AL - 25 year old Christopher George Bertram of Fyffe died Wednesday in an accident while working on a seed farm near Collinsville. Bertram worked at Murphree Seed Farm and was in the bucket of a vehicle, cutting limbs. The bucket was extended about 30 feet in the air and the vehicle was on a slight hill when it turned over, with Bertram still in the bucket, Wilson said.


Charter Captain Working Oil Spill Kills Self

June 23, FORT MORGAN, AL - 55 year old William Allen "Rookie" Kruse of Orange Beach, AL was found dead on the flying bridge of his boat, The Rookie, at the dock at Fort Morgan Marina just before 7 a.m., Baldwin County Deputy Coroner Rod Steade said. Kruse, a charter boat captain, shot and killed himself this morning just before his vessel was scheduled to set out to take part in oil cleanup and protection efforts, investigators said. "He had just let his deckhands off the boat and sent them to get something," Steade said. Baldwin County Coroner Stan Vinson said witnesses told investigators that Kruse had been upset about the loss of business caused by the closing of fishing grounds and public perceptions of the Deepwater Horizon oil spill.


Man Dies From Injuries Suffered in June 5 Accident

June 23, SAN JOSE, CA - 51 year old Jose Herrara has died after being severely injured in an electrical accident in Orinda this month, Santa Clara County officials said. Herrera suffered second and third-degree burns over most of his body when the cherry picker he and another worker were using came into contact with high-voltage electrical lines the morning of June 5. He was an employee of the Santa Clara-based Delta Window Cleaning.


Owner of Junkyard Found Dead

June 23, LINCOLNTON, NC - 67 year old Dan Mullinax, owner of a local junkyard, was found dead inside the building. A fire started at a junkyard on Highway 150 just after 5:30 p.m. A driver called for help after spotting smoke and Mullinax was found inside. Investigators haven’t said how Mullinax died or what caused the fire. They also have not discussed a potential motive in Mullinax’s death.


Maintenance Worker Killed in Explosion

June 23, BRONX, NY - 38 year old Joseph McLendon died Tuesday after the barbecue grill he was cleaning exploded outside a Bronx senior center in a huge blast that blew out nearby windows. Fire officials said McLendon used the wrong kind of cleaning agents on the grill, setting off a giant chemical explosion. "It was a large barbecue grill, but he must have been using other ingredients to clean the grill - solvents that interacted with the propane," said Deputy Fire Chief Kevin Scanlon.


CHP Officer Dies After Being Struck by Car June 9

June 23, LOS ANGELES, CA - 48 year old CHP Officer Philip Ortiz died Tuesday, about two weeks after being struck by a vehicle as he issued a ticket on the shoulder of the 405 Freeway. On June 9, he was issuing a ticket on the shoulder of the northbound 405 (San Diego Freeway), south of National Boulevard in West LA. Officers said several witnesses reported that an Infiniti driver was passing vehicles on the shoulder before striking the officer. Emergency crews worked to free Ortiz, who was pinned in the wreckage. He was transported to Ronald Reagan UCLA Medical Center in critical condition. CHP officials said he died Tuesday afternoon.


Man Killed When Equipment Falls on Him

June 23, GARDEN CITY, KS - 21 year old Jeffery Schiffer died Tuesday afternoon after a front-end loader fell on him at Plymell Dairy, 10550 S. Road 14, in Finney County. Schiffer, an employee of Garden City Tire Center was changing tires on the front-end loader, which was raised on jacks, when the loader slipped off and fell on him, according to Finney County Sheriff Kevin Bascue.


Florida Keys Officer Killed in Single Car Crash

June 23, KEY LARGO, FL - 36 year old Deputy Melissa Powers was killed in a single car accident in Key Largo Tuesday night. She is survived by two children, and her husband who is a Detective with the Homestead Police department.


Man Dies Following Construction Accident

June 22, ST.THOMAS, V.I. - 29 year old Ashorne Daniel has died after being struck by a car June 17th. Daniel was working as a flag man at a construction site on Brookman Road in the area of Estate Charlotte Amalie when he was struck by a tan Camry driven by a 53 year old man.


Man Killed in Logging Truck Accident

June 22, SEDRO-WOOLLEY, WA - 35 year old Bradley B. Pederson of Burlington was killed in a logging accident Tuesday near Sedro-Woolley. A 25-year-old Sedro-Woolley man was driving a large truck with a 40-foot trailer down a logging road off of Cokedale Road just after 8 a.m. when the accident occurred. Pederson was standing on the running board outside of the truck, said State Patrol Trooper Keith Leary. Pederson was riding on the driver’s side of the truck to learn how to navigate the corner of the road. When the truck went around a corner to the right, it rolled onto its left side, Leary said, crushing the man.


Worker Found Dead On Roof

June 22, NEW HAVEN, CT - an unidentified worker was found dead on the roof of a Carriage House at Edgerton Park in the city’s East Rock neighborhood Monday. The man had been working on a flat-roofed section of the Carriage House, which is located near the stables in the western section of the 22-acre park. Police did not say whether he was a city employee or a private contractor.


Walmart Employee Dies After Being Injured

June 22, FORT WORTH, TX - Bruce Florence has died after trying to stop a shoplifting incident. About 2 p.m. June 14, Florence, working in the lawn and garden department, spotted a man carrying a 26-inch Vizio television and heading toward the exit. Florence stepped in front of the man, but he barreled right over him, dropping the TV in the process, Westworth Village police Detective John Moore said. Florence was hospitalized with a head injury until he died on Father's Day.


Gold Miner Killed in Drilling Accident

June 20, FAIRBANKS, AK - 20 year old Jacob Ryan Branham of Nevada was killed early Monday morning after his clothing was caught in a piece of equipment, according to the Alaska State Troopers. Branham was working at Livengood gold mine near Fairbanks. At about 5:40 a.m. Monday morning he suffered a possible broken neck after his clothes were caught in a large drill used for drilling into the ground, according to trooper spokeswoman Megan Peters. A spokeswoman for the Mine Safety and Health Administration, Amy Louviere, said an inspector who visited the mine Tuesday verified that the accident took place during exploratory drilling at an abandoned mine.


Firefighter Dies of Apparent Heart Attack

June 22, OSWEGO, NY - 46 year old Scott W. Davis died Sunday after completing several emergency responses. Davis, 46, had completed a tour of duty on Saturday when he became ill, according to the USFA. He died the following day due to an apparent heart attack.


Contractor Killed At North Carolina Army Post

June 22, FORT BRAGG, NC - a contractor has been killed and two other men have been injured in a live-fire training accident at Fort Bragg in North Carolina. Post spokesman Tom McCollum says the accident happened Tuesday afternoon on a weapons range during an exercise involving small arms. He didn't have any other details and says the accident is under investigation. Two of those struck were civilian contractors for Echota Technologies, a defense contractor based in Alcoa, Tenn. UPDATE: The contractor killed has been identified as 57 year old Edward Jenkins.


Three Die in Plane Crash

June 22, LOCK HAVEN, PA - 29 year old Dan Snider of Pentress, W VA and 46 year old Rodney Whiteman of Morgantown, W VA, employees of the US Forest Service died in a plane crash. Also dead is the pilot, Patrick Jessup of New York. Snider and Whiteman worked for the Forest Health Program of the Northeastern Area and were conducting an aerial pest detection survey, the Forest Service said. The Federal Aviation Administration said the plane went down Monday as it approached William Piper Memorial Airport. The Cessna 210 struck a house, a utility pole and several vehicles and ended up in the street, but no injuries were reported on the ground.


Deputy Killed in Car Accident

June 22, TWIGGS, GA - 27 year old Deputy Richard L. Daniels was killed in an accident early Tuesday morning. Daniels had just made an overnight security check at Reece Funeral Home when he headed east down U.S. 80 to do to the same at some churches. On a dark, lonely stretch of road that time of morning, he apparently never saw what hit him. A huge oak toppled over at mile marker 6. The impact on the 55 mph highway killed him instantly and tore the roof off his patrol car. It kept going another 271 feet after clipping the tree trunk that was suspended off the ground.


Man Dies After Farm Tractor Rolls On Him

June 22, CAMBRIDGE TWP, MI - 77 year old Charles Austin Brighton died Sunday after a tractor he was operating on his property overturned and rolled on top of him. According to a report released today by the Lenawee County Sheriff’s Department, Brighton was operating a 1974 Ford tractor and baling hay at 6:30 p.m. on his property in the 9000 block of U.S. 12 when the weight shifted on the baler, causing the tractor to overturn. Brighton was thrown from the tractor, which then rolled over onto him. He was pronounced dead at the scene by Lenawee County medical examiners.


Man Dies in Tractor Trailer Wreck and Fire

June 22, WHEATLAND, NY - 49 year old Freddie R. Hatfield of Decatur, AL died after his semi truck, carrying construction materials for Crete Carriers, veered off the roadway and crashed. State Police Capt. Michael Nigrelli said it appeared the tractor-trailer was weaving on the Thruway this morning, just before it veered off the right shoulder into a wooded area about 50 feet from the highway, near Winslow Road. The rig then burst into flames from a ruptured fuel line.


Fort Drum Civilian Worker Dead

June 22, FORT DRUM, NY - Timothy Clermont, who worked for Drum's 'Department of Logistics,' died, according to a statement from Drum's press office. Clement was discovered between a fence and the lawn tractor he was operating. There were deep tire marks in the ground and a hole in the chain link fence. The incident, which occurred on Camp Hale Road just before 8 a.m., is under investigation by the U.S. Army Criminal Investigation Division.


AZ Mine Employee Dies in Wreck

June 22, FLORENCE, AZ - 52 year old Thomas E. Benavidez was killed and another seriously injured Sunday when a 240-ton hauling truck ran over their pickup at the Asarco LLC Ray mine east of Florence, according to preliminary accident reports. It appears that Benavidez and another mechanic, William Hyde, pulled in front of the large hauling truck, and the driver of the hauling truck could not see their pickup, Evans said. "These haul trucks are literally two to three stories tall," Evans said. "They have blind spots. The pickup was broadsided and the haul truck rolled over the pickup with the two employees inside."


Worker Dies From Possible Refrigerant Leak

June 21, MONTGOMERY, AL - 22 year old Nicholas Burdette of Millbrook was found dead at the Montgomery Eastdale Mall. Officers had blocked off every entrance of the mall, while the Montgomery Fire Department's HAZMAT team tried to find the source of an R-22 refrigerant leak near the mall's ice skating rink. Lt. Mark Drinkard, a spokesman for the Montgomery Police Department, says the fire department received a call Sunday morning about the leak, and police received a call reporting a man was dead. Drinkard says Burdette apparently was performing janitorial services when he died.


TX Police Officer Killed in Shoot Out

June 21, LANCASTER, TX - 37 year old Officer Craig Shaw died after being shot in the line of duty. Shaw responded about 6 p.m. Sunday to a report of a domestic disturbance that was upgraded to shots fired, police said. "At some point the shooter was confronted, there was an exchange of gunfire, at some point Officer Shaw was shot," Lancaster Police Chief Keith Humphrey said during a Monday morning news conference. "There was an additional exchange of gunfire, at some point the shooter was killed." Humphrey said it was the first time an officer in his department had been fatally shot in the line of duty. He identified the slain lawman as Officer Craig Shaw, 37, who had been with the department for five years. Shaw was married with a 13-year-old daughter and a 7-year-old son.


Police Employee Dies In The Line Of Duty

July 21, MANASSAS, VA - 60 year old Donald E. Reid, a civilian employee supply manager of the Prince William County police department died in the line of duty Friday. Reid died of an apparent heart attack while responding to a gas leak in the Manassas area Friday, police officials said Monday. Friday afternoon, police officers responded to the intersection of Sudley Road and Sudley Manor Drive to direct traffic around a large gas leak, which closed roads in the area for hours. At about 7 p.m., Reid was delivering water and other supplies to those officers, when he pulled his car over to the side of the road, in “apparent medical distress,” police said. A Virginia State trooper saw the stopped car, checked on Reid and called an ambulance. Reid was taken to Prince William Hospital, where he died later Friday night.


Farmer Killed In Tractor Accident

June 21, FAY, OK - 84 year old Cleo Leon Shifflet died of massive injuries as he was undergoing surgery at OU Medical Center in Oklahoma City on Sunday. Troopers say the accident occurred about 8 a.m. as Shifflett was standing on the ground in front of the tractor's rear wheels, trying to start the vehicle. The patrol says the tractor was in gear and ran over Shifflett when it started.


New Sewickley Trucker Killed as Rig Flips


July 21, NEW SEWICKLEY, PA - 72 year old John S. Fuse of New Sewickley Township was killed Saturday when the tractor-trailer he was driving near Meadville flipped onto its side and crushed the truck cab. The crash occurred around 10:30 a.m. as Fuse, driving north on Interstate 79 with a fully loaded trailer, failed to negotiate a turn on an exit ramp in Vernon Township, Crawford County. The truck flipped onto its side, crushing the cab.


CA Man Dies in Fiery Truck Crash in Nevada When Runaway Truck Ramp Fails

June 21, SAN DIEGO, CA - 41 year old Fredrick Matthews of San Diego died after the 18-wheeler he was driving in Nevada was going more than 100 miles per hour when it went airborne, slammed into a home and burst into flames. Witnesses said the 41-year-old was hauling a load of lumber while driving at least 100 miles per hour. He went into a runaway truck ramp, which should have slowed him down, but his sister said it did not. The gravel and steep incline of the runaway truck ramp was designed for this very problem. Nevada’s highway patrol said it is investigating why the ramp failed to stop the big rig and will also check the truck’s maintenance records.


Truck Kills TX Road Worker

June 21, PORT ARTHUR, TX - Santos Granja was killed when a tractor trailer truck slammed into a road construction convoy. The crash happened just before 10:30 p.m. on Texas 73 westbound about 8 miles west of Port Arthur. According to the Texas Department of Public Safety, the road crew was painting lane stripes and laying down reflective markers when the truck slammed into all three vehicles of the convoy. Granja (GRAHN'-hah), who was on the trailer of one vehicle, was killed. Four others were taken to Christus St. Elizabeth Hospital in Beaumont. June 20, PORTSMOUTH TWP, MI - 52 year old Randy Vandriessche was killed when his tractor rolled over onto him in Bay County. State Police said a tractor that Vandriessche was operating rolled into a drainage ditch, pinning him underneath. He died at the scene.


Oregon Man Dies in Mine Collapse

June 20, ONTARIO, OR - 42 year old Jose Aguiar, Jr. of Ontario died Saturday morning when the walls of a mining cave collapsed on him as he was digging inside. Oregon State Police Senior Trooper Tracy Howard said that at about 8:30 a.m., Aguiar was digging inside a small mining cave with three other people near an Old Lime Plant along the Burnt River in Baker County. Investigators determined that as Aguiar was digging about 20 feet inside from the entrance, the rock and dirt wall caved in on top of him. Two other men, whose names were not available, spent five minutes digging him out before they could remove him from inside the cave.


Train Conductor Shot, Killed In Robbery

June 20, NEW ORLEANS, LA - a 52 year old CSX Train Conductor was shot and killed in a robbery early Sunday, police said. New Orleans Police Department officials said the train was traveling west when it came to stop due to traffic delays. That's when police said the gunman got on board, shot at the conductor and ran. The shooting occurred in the city's Lakeview neighborhood, just west of City Park. The 52-year-old conductor was shot several times and died at the scene. His name wasn't released. UPDATE: Conductor killed has been identified as 52 year old Fred Gibbs of Biloxi.


Fisherman Dies After Being Thrown From Boat

June 20, ORLAND, IL - a 41 year old fisherman died after another boat rear-ended his boat Sunday shortly after midnight, throwing him into the Cal-Sag Channel in the south suburbs, authorities said. The accident occurred when the second boat swerved to avoid a log and hit the fishing boat, which was not moving, Coast Guard Petty Officer Nathaniel Parks said. The man thrown into the water could not swim and was not wearing a life jacket, Parks said. State officials said at least one boat was conducting maintenance tests at the time of the collision. The Illinois Department of Natural Resources identified the victim as a 41-year-old man from Orland Park but would not release his name, pending identification of family members.


Truck Driver Dies in Hopkins Co. Accident

June 20, HOPKINS CO.,KY - 54 year old Byron E. Nuss Jr. of Warren, IN died in an accident today. Kentucky State Police said another driver pulled off the road to read a map. Authorities said Nuss' tractor trailer swerved behind and hit the pickup causing both vehicles to fall down an embankment. Police said Nuss's tractor trailer overturned, and it took a rescue crew about an hour to remove him from the vehicle.


Former Football Star Dies in Mine Accident

June 19, SILVERTON, ID - 29 year old Timothy A. Bush of Pinehurst died Friday of internal injuries from a rock fall in a morning accident at Galena Mine near Silverton, Idaho. Bush was a contract employee at the underground silver mine. He was working with his cousin when he was hit by a falling slab of rock about 8 a.m., family members said. Bush had to be carried up a ladder before he could be taken to Shoshone Medical Center, where he died. His dad, Ken Bush, who works as a trainer at the Galena Mine, was by his side.


Migrant Worker Dies

June 18, VALDOSA, GA - an un-named migrant worker was found Thursday night near the edge of a field off Etheridge Road in Brooks County. Sheriff Mike Dewey said the man was picking squash earlier in the day. The man has not been identified. Authorities say the cause of death could be heat-related.

June 18, LEXINGTON, KY - 37 year old Brian Keith Meyers of Salem ,IN died Friday morning in a crash with another tractor-trailer on Interstate 64 in Scott County, said Coroner John Goble. Meyers was wearing a seat belt when he crashed into a tractor-trailer parked in the emergency lane about 6 a.m. near Exit 69. Meyers died of blunt force trauma, Goble said. Meyers worked for Ace Doran in Indiana and was hauling dry concrete.


Man Dies in Armored Van Wreck
June 18, EDNA, TX - 24 year old Ray Wausen of Yoakum, died Friday morning when a Triple D Security Armored Service van wrecked on U.S. Highway 59 near Edna. The van was northbound about a half mile south of Edna when the right rear tire blew out, Bryant said. The van crossed the median, flipped several times and ended up in the southbound lane.


Taxi Driver Killed In Car Wreck

June 18, ST.JOHN'S CO., FL - 55 year old Keith Seltenreich was killed when the taxi he was driving crashed into another vehicle. The Checker Cab taxi was headed south, while an SUV was traveling north in the inside lane. For unknown reasons, the taxi driver crossed over the grassy median, and hit the passenger side of the SUV. After the collision, the taxi spun around and came to rest in the grassy median. The SUV came ended up on the east shoulder of the road, near a private driveway for Irriscape Trucking. Seltenreich was taken by air ambulance to Shands Jacksonville Medical Center, where he was pronounced dead. All three occupants in the van were transported to Baptist South Hospital for minor injuries. There were no passengers in the taxi.


Man Killed by Lawn Tractor

June 18, SPOKANE, WA - A 44-year-old cemetery employee died early Friday when the lawn tractor he was operating dropped off an eight-to-ten foot embankment and landed on top of him. Co-workers found the worker trapped beneath the machine about 7:20 a.m. Friday and said he appeared to be breathing at that time. However, fire department employees pronounced him dead of apparent head injuries shortly after they arrived at the scene. The worker had been mowing grass at Greenwood Memorial Cemetery, 211 N. Government Way, in an upper terrace area designated "hand mow only," Deputy Mike Beckman reported. The deputy said the machine the victim was operating has hand levers for steering, rather than a steering wheel. He said it appeared the machine bounced after hitting a depression. A wheel may have lost traction sending the mower over the embankment. UPDATE: 44 year old Robert A. Villa has been identified as the worker killed in this accident.


Alternative Energy Company Employee Killed In Explosion

June 18, SIMI VALLEY, CA - 28 year old Tyson Larson was killed at his family business, Realm Industries, when a pressurized container used to heat water exploded accidentally during an apparent attempt to make alternative fuel. Fire department investigators said the blast was accidental but its exact cause remained undetermined but according to initial reports, one of the employees, probably Larson, was processing a fuel mixture of some type when the explosion occurred, said Dean Fryer, a spokesman for Cal OSHA.


Railroad Worker Drowns After Bridge Collapses Over River

June 18, NORFOLK, NE - 38 year old Jeffrey Scholl died when a bridge over the Elkhorn River collapsed. The bridge collapsed early Tuesday afternoon while Scholl and two other railroad employees were inspecting it for damage. The two other employees - who have not been identified - were not injured after they also fell into the flood waters of the river. His body was recovered Thursday night.


Man Killed In Train Derailment Cleanup

June 18, JACKSON TOWNSHIP, IN - 43 year old Michael Bowling of Dyer, IN was directing crane operations at the site of a train derailment. He was killed Thursday when a crane toppled onto him, according to the Porter County Sheriff's Department. Bowling was overseeing the clearing of rail cars and containers when one crane became unstable and fell on him just before noon, Sheriff's Department spokesman Chris Eckert said. Bowling was not operating the crane. Cranemasters, the crane's owners, were contracted by CSX to clear the derailment scene and worked through the night after Wednesday's incident, Eckert said. Two large cranes and earth-moving equipment were clearing damaged track sections and lifting derailed cars at the time of the accident, he said.

June 17, SAN LUIS OBISPO, CA - a 67 year old man of Arroyo Grande, CA, who reportedly owned a company doing roadwork on Highway 135 was killed Thursday when he was run over by a construction truck, officials said. The CHP said that 46-year-old Paul Michaelson of Lompoc was parked in a 1997 Peterbilt dump truck with an attached trailer on a closed southbound lane of Highway 135. Michaelson backed the rig up five to 10 feet to clear a truck parked in front of him and started to pull forward when he noticed a man lying on the ground where his truck had been. Construction Worker Killed While Working Along US Route 30

June 17, PARIS TWP, OH - 26 year old Kevin Meyer of Grove City was electrocuted while working along U.S. Route 30 on Thursday when a boom truck struck a live power line, according to an Ohio Highway Patrol investigation. “A boom truck struck a live power line while (Meyer) was holding onto a brace attached to it, at which time he was electrocuted,” Shirkey said. Smith Ambulance took Meyer to Aultman. The Norwalk, Ohio-based Lake Erie Construction Co. was removing guardrail beams from the roadway when the incident occurred, Shirkey said.


Worker Dies in Trench Collapse

June 17, FAIRFIELD, IL - 53 year old Ricky Clendenin of Mount Carmel was killed Thursday afternoon when a long section of a trench he was working in collapsed. Witnesses at the scene said that workers had dug a four- to six-inch trench for an electrical line, then moved over less than two feet and used a backhoe to dig a much wider trench. Pottorff said about a 30-foot section of the soil between the two trenches collapsed, crushing Clendenin under more than seven feet of earth. "He was brought to the surface at 3:22 p.m.," Pottorff said. "We started CPR immediately and transported him to Fairfield Memorial Hospital, where he subsequently died."


Tractor Trailer Driver Killed in Wreck

June 17, KNOXVILLE, TN - 69 year old Donald Wayne Sluder of Leicester, NC was killed when his rig left the road and hit a bridge early Thursday morning in Jefferson County. According to a release, Sluder was traveling west on I-40 when he traveled into the right shoulder and struck the guardrail race. He then continued and struck a bridge pillar. After striking the pillar the trailer attached to the back of the truck separated. The truck continued to travel westbound where it caught on fire. It then stopped blocking both lanes trapping the driver inside of the vehicle.


Machine Shop Owner Killed in Accident

June 17, HIGHLAND HEIGHTS, OH - 69 year old John Bennington, owner of Sneller Machine and Tool in Highland Heights was killed Wednesday afternoon when a forklift, carrying a 7,000-pound tree stump grinder, tipped and fell on him. “The forklift tipped over and he was pinned,” Dowdell said. “I believe it was only two or three feet off the ground.” Bennington was a Chesterland resident. According to the Sneller Web site, the business is in its third generation of family ownership.


Greensboro Trucker Dies in Wreck

June 17, BLAIRS, VA - James Willie McKinzie of Greensboro, NC was killed when the tractor trailer he was driving ran off the road and flipped around 4 a.m. on Thursday, Roanoke NBC affiliate WSLS reported. Police said McKinzie was not wearing a seat belt. The crash caused the tank on his truck to leak liquid oxygen onto the road, which was closed until around 3 p.m. Thursday.


Man Dies After Bee Attack

June 17, ENCINITAS, CA - 54 year old Marco Tulio Lazaro died Wednesday after he was stung more than 500 times by swarming bees as he used a backhoe to clear brush from property in a rural area of eastern Encinitas, officials said. He was working with his 20-year-old nephew on property off Manchester Avenue near Pacific Ranch Drive when he was attacked by the bees. The man ran about 200 yards to an outhouse and went inside to seek shelter from the attack. That’s where firefighters found him, in full cardiac arrest, Henry said. Neighbors described the victim as a longtime gardener, landscaper and maintenance man who lived on the property, called Wiegand Ranch.


Miner Killed by Falling Beam at KY Coal Mine

June 17, HOLMES MILL, KY - 42 year old Jim Carmack of Barbourville was hit by a falling steel beam has been killed on the job, the fourth mining-related fatality of the year in Kentucky. Dick Brown, a spokesman for the state office, says coal dislodged from the mine wall and bumped a support holding up the beam. The Kentucky Office of Mine Safety and Enforcement says the miner died Wednesday at Lone Mountain Processing’s Clover Fork Mine No. 1 at Holmes Mill in the southeastern part of the state.


Worker Killed Hailed As Hero

June 16, NAUGATUCK, CT - 59 year old Anthony Mariano of Middlebury cleared his co-workers and was about to remove the excavating machine he was operating from beneath the Route 63 Salem Bridge when a 100-foot section fell to the ground Tuesday morning, seriously injuring him. Mariano was brought by ambulance to Waterbury Hospital, according to the state police. He later died and was hailed as a hero. The bridge spans the Naugatuck River and the Metro-North Railroad. The construction crew, from the Brunalli Construction Co. in Southington, was working on a 100-foot section of the northbound side, 40 feet of which was over land, Kevin Nursick, a spokesman for the state Department of Transportation said.

Shawano Co. Co-Op Worker Killed in Grain Bin Accident

June 16, ZACHOW, WI - 50 year old Gregory Siolka of DePere, WI died after falling into a grain elevator in Shawano County Wednesday. It happened around 10 a.m. in Zachow, which is east of Bonduel. Siolka was working inside to scrape some corn down that was hung up, then it collapsed on him, pulling him into the grain. Fire and rescue crews spent several hours trying to save Siolka. Farmers compare falling into a grain elevator full of corn to walking on quicksand. Stuart Sorenson is owner of the co-op and tearfully remembers Siolka as not just an employee...but a long-time friend.


Driver Pulling Logging Equipment Dies in Crash

June 16, LADYSMITH, WI - 86 year old Anthony Vacho of Ladysmith died when his vehicle crashed west of Ladysmith. Rusk County authorities said Vacho had medical conditions – but it’s not known if they had anything to do with the accident. Officers said Vacho was driving north when his vehicle went off the road and struck a culvert.


NJ Trooper Kills Self in Cruiser

June 16, WALL TOWNSHIP, NJ - 35 year old Trooper Scott Graham was found dead in his patrol car of a self-inflicted gunshot wound. Graham was found by another officer who was passing by shortly after 7:30 a.m. Wednesday. Graham was in a marked patrol car on an exit ramp off the Garden State Parkway in Wall Township and was pronounced dead at the scene. He is survived by a wife and three young children.


Mechanic Dies After Being Trapped Under Truck

June 16, GULFPORT, MS - 63 year old Charles Long died Wednesday morning, trapped under a company truck whose brakes he was working on. Harrison County Coroner Gary Hargrove said Long died of chest injuries when the truck rolled on top of him and stopped. The coroner said preliminary investigation showed Long was working on the brake system. “When he released the air-brake cylinder, the truck rolled forward on top of him and stopped on top of him,” Hargrove said.


Farmer Killed in Tractor Mishap

June 16, WHITE HALL, IL - 79 year old Richard C "Dick" Reynolds of White Hall was killed when a runaway tractor ran over and fatally injured him as he and a farmhand were attempting to move a water tank, authorities said Wednesday. Authorities said Reynolds and the farmhand, Chris Norris of White Hall, had been using a truck to try to move a water tank from a hillside on farmland; however, the truck could not get traction on the wet grass, and the men tried to use the farm tractor instead. Reynolds was standing behind the tractor as the farmhand put it in gear, authorities said. The farmhand told investigators that the tractor's brakes and transmission failed, and the tractor began to roll backward down the hill, running over Reynolds.

June 16, LOWER POTTSGROVE TWP, PA - An employee at Lower Pottsgrove Elementary School was killed Wednesday when a lawn mower he was working on fell on him, officials said. The staff member, a member of the grounds crew, was not identified by Lower Pottsgrove police or Pottsgrove School District officials who said "out of respect for the family" they would not release the man's name. An official with the Montgomery County Coroner's office did not have information on the death Wednesday afternoon.


Warehouse Worker Dies After Being Crushed

June 16, O'FALLON, MO - 23 year old Kelly S. McDaniel of O'Fallon died early Wednesday after being crushed by a sofa. McDaniel was operating a stockpicker inside the Rothman Furniture warehouse about 6 p.m. at 2101 East Terra Lane when a sofa-love seat fell on top of him and pinned him. McDaniel had gone to retrieve furniture for a customer but didn’t return, police said. His manager went looking for him and found him pinned between the furniture and the stockpicker, industrial equipment that raises and lowers heavy objects. The manager lowered McDaniel, removed the sofa and immediately began CPR until paramedics arrived, authorities said.

June 16, MOUNTAIN HOME, AR - 47 year old Robert Tuell died yesterday after falling from a construction site at a residence in Norfork. Baxter County Sheriff John Montgomery says Tuell was pronounced dead at the scene by the County Coroner.Initial investigation into the incident indicates Tuell was helping the unidentified homeowner with construction when he apparently fell 15 feet with his head striking the ground.

June 16, EAST MEADOW, NY - 45 year old Stacie Williams was shot to death outside a hospital early Wednesday morning. A spokeswoman for the hospital said it appeared to be a domestic dispute. Williams, a 45-year-old nurse's aide who worked the night shift in the maternity ward of Nassau University Medical Center, was found shot multiple times at about 1 a.m. while on a break outside the Long Island hospital's visitor's entrance.


EMA Employee Found Dead

June 15, MARSHALL CO.,TN - 67 year old Delwin Lee was found dead outside of his EMA truck alongside Hatchet Hollow Road on Monday. A spokesperson with the Marshall County Emergency Management Agency said Lee was unloading water from his truck on the private road when he apparently collapsed. An autopsy will be performed to determine if Lee died from the heat or some kind of medical condition. Additional details concerning the incident were not known.


Driver Dies When Truck Hits Pole

June 15, GARDENA, CA - 57 year old Richard Granath died while driving in Torrance on Tuesday, but it was not known if his death was caused by a medical incident or his crash into a power pole. Granath owned and operated Sonic Plating in Gardena. Witnesses told police that Granath was southbound on Western Avenue around 10:30 a.m. when his truck began to drift slowly, according to police Lt. Jeremiah Hart. Suddenly, the truck accelerated and hit a utility pole just south of 190th Street, Hart said. Officers found Granath, the vehicle's sole occupant, behind the wheel with no pulse, Hart said.


Lightning strikes, kills St. Louis-area postal worker

June 16, ST LOUIS, MO - 34 year old Christina Jones, a mail carrier, was killed when lightning struck a tree while she was seeking shelter from a storm in the St. Louis suburb of Ferguson. Neighbors say the force of the strike knocked Jones' shoes and hat off and tattered her clothes. They held an umbrella over the fallen postal worker and prayed until emergency workers arrived. Co-workers and friends say Jones had just returned from the Bahamas with her fiance, and that she was the mother of a 6-year-old son.


Roofer Dies After Fall Through Roof

June 16, MONTGOMERY, NY - A 26-year-old man from Beacon died Tuesday morning after falling from the roof at the shop where Paul Teutul Jr. is building his new bike business. Police declined to release the man's name pending notification of his family. The man worked for OCS Industries of Middletown, which had been hired to repair the roof by the building owner, Olori Brothers, Lt. Alex Landolina of the Montgomery Police Department said. Just before 9 a.m., the police received a call that a man had fallen. The roofing repair company had cut a hole in the roof and the man stepped backward into the space and fell, Landolina said. UPDATE: Police have released the name of the man killed as 26 year old Gary Shatlaw of Beacon.


Cab Driver Drowns After Helping Passenger To Safety

June 15, LAWTON, OK - 50 year old Miguel Lopez's cab got stuck in rushing water at 12th and Euclid. He had a passenger and called for another cab driver to pick them up. He helped the passenger to safety, but in the process, police said he unknowingly stepped into a canal where the water quickly pulled him under. Lopez worked for AAA Cab Company for several years and he left about two and a half years ago to work for Church's Chicken right next to his house. He just returned to AAA two weeks ago. His coworkers at the cab company said he's a hero, risking his own life to save a passenger and another driver.


EMA Employee Found Dead in Marshall Co.

Contract Employee Dies at Sandstone Mine

June 15, LESUEUR, MN - 46 year old Thomas Edwards, an employee of MOR PPM, Inc, died in an accident June 12 at a Minnesota sandstone mine. According to the LeSueur County Sheriff’s Office, Edwards,of Erwin, TN was seriously injured when he fell from a ladder approximately 47 feet to the ground at Unimin Minnesota’s Ottawa Plant and Pit in LeSueur. He was transported to LeSueur Hospital and later pronounced dead.


Postal Worker's Death Ruled Homicide

June 15, HAYWARD, CA - 61 year old Nancy Rogers was found dead at a dump in San Leandro about 8 a.m. on Monday after co-workers reported her missing. On Monday, Rogers came to work at 1 a.m. Several hours later, the small group of employees who worked with her that day noticed that she was missing and called their supervisor, Rickher said. The supervisor then called police. The early morning employees "are a very close-knit group" and it was "out of the ordinary" for Rogers to disappear, Rickher said. Orrey said officers who responded to the post office determined that its Dumpster had recently been emptied. Officers next went to Waste Management's transfer center at 2615 Davis St. in San Leandro and determined where the trash from the post office had been unloaded. The piles were searched, and amid the trash officers found a body that matched Rogers' description, Orrey said.

MLGW spokesman Glen Thomas said crews were doing maintenance work on the wooden pole when part of it broke off, striking the men. June 15, RAYMOND, NE - 30 year old Neil Cary, of Lincoln died Tuesday after a 10-foot fall, a Lancaster County Sheriff's sergeant said. Around 3 p.m., co-workers found Cary on the concrete floor of the Benes Heating and Air Conditioning warehouse at 5580 W. Mill Road. Investigators believe Cary fell from a loft used to store supplies, resulting in a fatal head injury, Brookhouser said.

Idaho Man Dies in Oil Drilling Accident

June 15, VERNAL, UT - 32 year old Alan J. Eichelberger of Hansen, ID died in an accident on an oil rig in Uintah County on Sunday. Eichelberger was struck in the head by drilling equipment shortly before 1 a.m. while working on the oil rig, said John Laursen, Uintah County undersheriff. "He was wearing a hard hat, but that didn't do him any good," Laursen said.

June 15, MINNEAPOLIS, MN - 47 year old Jesse Paul Stygar of Rosemound, a mechanic for Delta Air Lines, died at Minneapolis-St. Paul International Airport Tuesday morning after being injured while working on a plane, a spokesperson with the Metropolitan Airports Commission said. MAC spokesperson Pat Hogan said Stygar was working in the gear/wheel well area when he was injured. Stygar died by the time emergency crews arrived at the scene.


Census Crew Leader Dies After Being Shot

June 15, BALTIMORE, MD - 22 year old Spencer Williams was working for the Census Bureau has died after being shot while dropping off a co-worker, the first killing of a Census worker on the job this year. Williams died Friday after being shot June 7, according to Baltimore police. He was found inside his car, resting in the median near an intersection, and had multiple gunshot wounds, police said. Census Bureau spokesman Steven J. Jost said Williams was returning home after driving a co-worker home at the end of the day and was considered to still be on the job.


Second Lincoln Explosion Victim Dies

June 14, STANFORD - The Stanford man who was critically burned in a massive gunpowder explosion at an ammunition business earlier this year has died. James Greenwood, 57, died Tuesday at University of Kentucky Medical Center in Lexington, said John Sallee of Spurlin Funeral Home in Stanford, which is handling Greenwood’s arrangements. Greenwood never returned home from the hospital following the Jan. 12 explosion at the home of Shannon Corman, who operated a gun and ammunition business behind his residence off of Herndon Avenue. Greenwood worked for Corman. Corman and Greenwood were apparently dividing black powder from a large stockpile into smaller amounts when the explosion occurred, destroying an outbuilding behind the home and setting off hundreds of rounds of live ammunition.


Caterer, Nurse Dies in Wreck

June 14, HEMPFIELD TOWNSHIP, PA - 28 year old Bridget McGrath of Hannastown, PA died in a car crash in Hempfield Township, Westmoreland County. She was a registered nurse and a part-time caterer for DeNunzio's Italian Restaurant. Her family said that she was likely on her way home from a catering event early Monday morning, according to the county coroner. State police think she fell asleep at the wheel."Unfortunately, after she struck the telephone pole, the wires were still hot, still active wires, and we believe it charged the vehicle, which would mean there was electricity still flowing -- a possibility that the vehicle still had electricity emitting through the vehicle," Trooper Stephen Limani said. "We believe that, at some point in time, maybe she tried to exit the vehicle. The door was ajar." The county coroner's office said McGrath may have burned to death or been electrocuted while trying to get out of her car.


NE Trucker Killed When Struck by Equipment

June 14, MILBANK, SD - 58 year old Vincent Nelson of Jackson, NE was killed when a piece of equipment fell off a forklift and struck him in the head. Sheriff Mike McKernan says Nelson died Friday after a loader frame being moved off his semitrailer slid off the forklift and hit him.


Nemacolin Employee Killed In Route 381 Crash

June 13, WHARTON TOWNSHIP,PA - A worker at Nemacolin Woodlands was killed in a van crash Sunday. The van, carrying seven passengers, ran off Route 381, just north of Route 40 in Wharton Township at 7:30 a.m. It struck a utility pole and a tree. "They were mainly employees from Nemacolin Woodlands finishing up their shift and returning home. We did have one fatality -- the back passenger of the van," said Farmington VFD Chief Brian Vansickle. The female victim has not yet been identified. Authorities said she was not wearing a seatbelt.


Texas trucker killed in crash on Louisiana highway

June 13, CONVERSE, LA - 26 year old Jadarion Montez Taylor of Longview, TX died after his diesel fuel truck flipped several times, ejecting him. Taylor was hauling 5,000 gallons of fuel in a 2010 International truck. The Times of Shreveport reports that Taylor apparently entered a slight left hand curve in the highway, ran off of the road to the right and began traveling in the ditch. The truck turned sideways and started rolling, ultimately coming to rest on the driver's side in a pasture.


Veteran Firefighter Dies After Lightning Strike Visiting Firefighter Memorial

June 12, DENVER, CO - 53 year old William John Carr of Pompana Beach, FL was taken off of life support on Tuesday night at St. Anthony Central Hospital after suffering an injury which doctors say he did not have a chance of surviving. "He suffered catastrophic brain damage from the lightning and from a lack of oxygen," his brother Bob Carr said. Carr, a veteran firefighter in Broward County, Florida for more than 30 years,was riding across the country with three fellow firefighters. They were on their way to visit the Fallen Firefighters Memorial in Colorado Springs. Witnesses say they saw the men near mile marker 138, when the lightning struck. The lightning hit Carr directly on the right side of his head, shattering his helmet and knocking him to the ground.


New Milford Zoner Dies After Collapse at Meeting

June 12, NEW MILFORD, CT - 60 year old Charles H. Bradford, who was a member of the New Milford Zoning Commission, died following his collapse Monday evening during a regular meeting of the local Republican Town Committee. Mr. Bradford elected to the zoning board last November. In the candidate’s information he provided during the last municipal election season, Mr. Bradford, who was 60 at the time, noted that he and his wife, Pat, then married for 37 years, had eight children and 12 grandchildren.


Local Fire Chief Killed in Farm Accident

June 10, CROPSEY, IL - 63 year old Richard Brucker died in a farm accident Wednesday. Authorities found Brucker's body unresponsive before he was pronounced dead on scene. Brucker began serving as the Cropsey Fire Chief in 1998. Preliminary autopsy results indicate he died from multiple injuries consistent with a fall. Coroner Beth Kimmerling says Brucker might have fallen from a grain bin. In addition to his duties as fire chief, he worked as a farmer and was retired from State Farm.


Conductor Killed When Hit by Train

June 10, RICHMOND, VA - 54 year old Conductor Marshall W. Conner, a member of UTU Local 610, died after being struck by a train near here, June 10, reports the Richmond Times-Dispatch. Conner, a 26-year veteran railroader was hit by a northbound freight train traveling a parallel track while he was conducting an air brake test.


Manure Gas Killed 2 Workers

June 9, WEST POINT, NE - 63 year old Alan Borhart and 36 year old Robert Holiday appear to have died of suffocation due to gas at a pig manure pump house. Cuming County authorities are awaiting laboratory test results to see what type of gas suffocated the two workers. The bodies of both men were discovered Friday night in the pump house on their workplace, the Maple Creek Pig Co., south of West Point along U.S. Highway 275. Co-workers say Borhart and Holiday were pumping pig manure from one containment lagoon to another.


Semi-Truck Driver Dies in Accident

June 4, KENNEWICK, WA - 55 year old Ronald W. Collins from Lorimor, IA was killed June 2 when his trailer overturned and his truck crashed into a guardrail near the Vernita Bridge the Washington State Patrol said. Collins was driving a 2000 Freightliner tractor east on the highway when he took a curve too fast causing his trailer to overturn troopers said. The truck then rolled onto its top and stopped on the guardrail. Collins, who was wearing his seat belt, died at the scene.

Officer Dies After Chasing Suspect

June 12, WHITE PLAINS, NY - 43 year old Michael Perry collapsed and died Saturday after a foot chase with a larceny suspect in downtown White Plains. Perry, a 7 1/2-year veteran of the White Plains police, began suffering chest pains and was rushed to White Plains Hospital Center after arresting Ricky Pryor, whom he chased from Macy's in the Galleria mall to Court and Main streets at 4:41 p.m. Perry collapsed en route to the ambulance, police said. He was pronounced dead at the hospital at 5:50 p.m.


Former ECC President Killed in Farm Accident

June 12, ROCKY MOUNT, NC - 67 year old Hartwell Fuller died after an accident involving a tractor at his Dortches farm on the 5900 block of Brake Road. Fuller was a longtime educator and former Edgecombe Community College president. Since the late 1960s, Fuller had remained a key figure in the community college. Relatives have said Fuller was attempting to change out lawn mower blades so he could mow the farm’s grass. It later was reported that his tractor was not secure and that it flipped over and fell on top him around 9:30 a.m.

Two Men Killed On The Job in Valley

June 12, VALLEY, AL - 53 year old Clemmie Dawsen and 43 year old Lorenzo Hodge of Opelika died while doing their jobs this morning. Dawson and Hodge work for a cleaning company out of Opelika, Top Notch Cleaning. They were at their last cleaning of the night at the O.D Alsobrook Center of East Alabama Mental Health. It is an outpatient counseling center. Valley Police said the two men were buffing the floors with machines that use propane gas. A worker found the men when she came into work this morning and immediately called 911. Both the worker and the police smelled gas when they walked in.


Idaho Truck Driver Dies of Heart Attack

June 11, CANNONVILLE, OR - A 59 year old Idaho man died following an apparent heart attack about 9:20 a.m. today while driving a commercial truck on Interstate 5 in the Canyonville area, Oregon State Police reported. The tractor-trailer rig, operated for Motorwest Inc. of Caldwell, Idaho, was found stopped and undamaged in the center median, said Lt. Gregg Hastings, a state police spokesman. Emergency responders gave CPR and took the man to Mercy Medical Center in Roseburg, where he died an hour later. His name was withheld pending notification of family. UPDATE: 59 year old Frank R. Forsythe of Marsing, ID died of a heart attack.

June 11, SKIATOOK, OK - 60 year old Lawrence E. Barham of Bartlesville was killed when the semi-trailer truck he was driving wrecked just west of the Skiatook city limits in Osage County at about 3:25 p.m. Thursday. Lawrence E. Barham, 60, was traveling southbound on Lenapah Street when his semi truck left the roadway for 106 feet, striking a cement culvert and overturning on its top.


Top Alcohol Funny Car Driver Was killed in Crash

June 11, OLD BRIDGE, NJ - 58 year old Neal Parker was on his way to a career best run. His 2005 Monte Carlo accelerated down the quarter mile track, crossing the finish line at Raceway Park in Old Bridge in less than 6 seconds at nearly 250 mph. But then things went terribly wrong. The race car’s parachute, used to slow the vehicle, failed to deploy, sending the vehicle and driver hurtling off the end of the track, shooting past a safety runoff area, through a gravel trap, breaching several safety nets and finally crashing into water-filled barrels, said Graham Light, National Hot Rod Association’s vice president of race operations.


CHP Officer Killed in Redlands Crash

June 11, REDLANDS, CA - Tom Coleman of Redlands, a motorcycle officer with the California Highway Patrol was killed early Friday in a crash at a Redlands intersection. The crash, which involved the officer's cycle and a big rig, occurred during a pursuit. Authorities said the pursuit began after the motorist committed a traffic violation and failed to yield to the officer. The suspect's vehicle made it through the intersection, but the officer collided with the big rig.


Off-Duty MD Trooper Working Security Shot and Killed

June 11, FORESTVILLE, MD - 24 year old Wesley Brown escorted a disorderly customer from an Applebee's on Thursday evening and when he left the restaurant around 12:40 a.m. Friday, State Police Superintendent Col. Terrence Sheridan said that customer shot him. Brown, who was wearing a jacket that clearly identified him as a police officer on the back, was talking on the phone in the restaurant's parking lot when he was shot, Sheridan said. Covered in blood, Brown made it back inside the restaurant and passed out, police said. Brown was pronounced dead at a local hospital. Many troopers do security work as part-time jobs, but it must be approved by the state police command, he said.


2 CareFlite pilots killed in helicopter crash

June 11, GRAPEVINE, TX - 44 year old Guy del Guidice and 23 year old Stephen Durler of Dallas were killed in a post-maintenance helicopter crash June 2. Their helicopter went down about 10 minutes after taking off on a test flight from a hangar in Grand Prairie. Pieces of the chopper were spread across a field near Midlothian. The National Transportation Safety Board has not released a cause for the crash.


Inmate Worker Dies in Accident

June 10, GULFPORT, MS - 33 year old Mireyel Scott of Leflore County died Thursday of a mowing accident. Scott was working for a city beautification crew and workers had to bring in a machine to lift the mower off the inmate. Authorities say the inmate was on a mower when it overturned, pinning the inmate underneath in a ditch with water in it. The incident occurred along the Clower-Thornton Nature Area.


Worker Dies in Tree Trimming Accident

June 10, CORPUS CRISTI, TX - a man in his 50's was killed today while working in the 100 block of Pearse Street. Just after noon, neighbors noticed him laying motionless on the ground and called 911. Police believe the victim fell off a ladder from a height of at least 10 feet. The victim is said to be a Hispanic man in his 50's.


Driver Killed When Cement Truck Rolls

June 10, CHICAGO, IL - 59 year old Jose Ruiz of the 5100 block of South Winchester Avenue died when the cement truck he was driving rolled over as it exited the Bishop Ford Expressway on the Far South Side this afternoon, authorities said. The Ozinga Brothers, Inc., mixer, which was carrying a full load of concrete, was exiting the southbound Bishop Ford (Interstate Highway 94) to westbound 111th Street when it collided with a compact van and rolled over about 12:44 p.m., Illinois State Police said. It appeared Ruiz was driving too fast for conditions, police said.


Councilman Dies After Collapse in Office

June 10, LOUISVILLE, KY - 67 year old George Unseld has died following a collapse in his office earlier this afternoon. The 67-year-old city lawmaker was found by Councilwoman Madonna Flood, D-24, and Democratic caucus spokesman Tony Hyatt, who heard the councilman fall and calling for help. Unseld was alone in his office when he collapsed. Behind the councilman’s desk and by the office door were a pools of blood. After calling EMS, Councilwoman Vicki Welch, D-13, who is a registered nurse, administered CPR until the ambulance arrived. Once EMS arrived Unseld was unresponsive, says Hyatt.


Accident Kills One at AG Equipment Plant

June 10, NEWTON, IL - 48 year old James Humphres of Hildago is dead after an apparent accident at a plant that makes agricultural equipment. Jasper County Coroner Jason Meyer says Humphres died Monday at GSI Group plant in Newton. Hidalgo is 75 miles south of Champaign and Newton is about 10 miles farther south. Few details are available, but Meyer says an autopsy found that Humphres suffered multiple cranial fractures due to blunt force trauma.


Two Die in Separate But Related Semi Truck Accidents

June 10, ESSEX, CA - 35 year old Sergio German Preciado of Hinkley struck the rear of another tractor trailer rig which was parked in the #1 lane of eastbound Interstate 40 east of mile marker 102 in Rural Essex due to another accident which was blocking the freeway ahead of them. Emergency personnel responded and Preciado was pronounced dead at the scene.
Then 61 year old David Mark Flores of Victorville struck the rear of another tractor trailer rig which was parked in the #1 lane of eastbound Interstate 40 east of mile marker 101 in Rural Essex due to now yet another accident which was blocking the freeway ahead of them. Emergency personnel responded and Flores was pronounced dead at the scene. The California Highway Patrol is investigating the incident.


Alabama Coal Miner Killed in Surface Explosion
June 10, PARRISH, AL - 38 year old Phillip W. Gustofson became the 37th coal miner to die on the job this year in the United States. Gustafson died in an explosion at Taft Coal Sales & Associates Inc.’s Choctaw Mine, a surface operation near Parrish, in Walker County, Ala., northwest of Birmingham. Gustafson was a service truck operator with about seven years experience. According to a report: The operator was in the process of fueling the drill rig when an ignition/explosion erupted into a fire, engulfing him in flames. He was airlifted to a local hospital where he was pronounced dead on arrival.


Farmer Killed As Antique Tractor Overturns

June 10, ROCHESTER, IL - 73 year old Henry Spengler died Wednesday, one day shy of his 74th birthday, from injuries he suffered when an antique tractor he was driving overturned on him. Authorities estimated Spengler had been pinned under the tractor between 60 and 90 minutes, said Spengler’s sister, Delores Hobbs of Edinburg. Spengler was trying to pull a piece of old culvert on an incline when the tractor overturned, Hobbs said.


Waste Collection Worker Dies After Being Hit By Car

June 10, RAISIN TWP., MI - 28 year old Scott Terrance Cousino of Toledo was working a route Thursday morning for Countryside Disposal of Ottawa Lake when he stopped to pick up some garbage at a residence on Sutton Road between Raisin Center and Rogers highway, Raisin Township Police Chief Scott Lambka said. “He had just stepped out of his truck to cross the westbound lane to get to the garbage,” Lambka said. Cousino was headed east on Sutton, and the stop was a few hundred feet west of the crest of a hill. He was hit by a westbound Mercury Mariner SUV driven by Laura Kathleen Hubbard, 40, of Britton shortly after it cleared the top of the hill. Cousino’s truck was being followed by another Countryside Disposal truck driven by his older brother, Mike. Both Cousinos live in Toledo, Lambka said. Mike Cousino, 31, had apparently finished his route and was following his brother as he finished his route.


Former Calhoun officer dies in Afghanistan

June 9, CHATTANOOGA, TN - 44 year old Wayne Willard, a former Calhoun, GA police officer died in a Taliban attack in southern Afghanistan on Monday while working for DynCorp, a private security company. Willard spent his free time in the war zone finding coats for Afghan children, his former boss said. Mr. Willard is survived by his, wife, Liz Willard, and two teenage children, police said.


TECO Worker After Contact With Electric Line

June 9, TAMPA, FL - 39 year old John Hudson was shocked Monday while working on a power line in the Tampa Palms area. Hudson, 39, was in a bucket truck working on an overhead line about 10:20 a.m. Monday when he came into contact with a live 7,620-volt power line, Tampa police said.


Providence City Worker Dies

June 9, PROVIDENCE, RI - 45 year old Anthony Bento died when the vehicle he was driving overturned. Parks Superintendent Robert McMahon says Bento was transporting dirt into a greenhouse at the Botanical Center at Roger Williams Park when the vehicle overturned at about 10 a.m. Tuesday. The John Deere hauler is described as similar to a golf cart. Bento was taken by ambulance to Rhode Island Hospital where he died a short time later.


Two Workers Killed in Second Natural Gas Explosion

June 9, HOUSTON, TX - 32 year old Steven Douglas Odell and 44 year old Johnnie Mike Renner were killed when a natural gas pipeline exploded. The blast near the small town of Darrouzett in Lipscomb County, a few miles from the Oklahoma border, was the second deadly blast in two days in Texas involving workers who accidentally hit natural gas lines. A crew working for a dirt-contracting company was removing clay from a pit Tuesday afternoon when the bulldozer hit the gas pipeline and cause it to explode.


Exeter Man Electrocuted

June 8, EXETER, NE - 45 year old Jay Luzum died early Tuesday after apparently being shocked by the power cord of a grain auger, according to Fillmore County Sheriff William Burgess. Investigators believe Luzum was using a grain auger to transfer grain from a bin into a truck when the auger malfunctioned. Luzum tried to unplug the auger's power cord and was shocked, Burgess said in a release. The incident happened at about 6:30 a.m. on a farm near Milligan, about six miles south of Exeter in eastern Fillmore County.


Lawrence County man dies from injuries

June 8, ASHLAND, KY - 29 year old Chaunchey Neal of State Route 243 near Deering was injured Tuesday while cleaning an ice machine at Tri City Ice in Ashland, Ky., where he was employed. The 29-year-old died Saturday at Cabell Huntington Hospital. Neal’s mother, Brenda Neal, said her son was cleaning the machine when a bolt came loose, causing the machine to fall on him. The machine knocked him into a puddle of water. “His face was in that water and he didn’t have any oxygen going to his brain,” Neal said.


One Killed in I-91 Crash

June 8, Littleton, MA - 25 year old Ryan Murphy was killed when a tractor-trailer struck a disabled big rig parked on the side of Interstate 91 in North Haven on Tuesday morning, according to state police. Murphy, a mechanic was working on the disabled 18-wheeler when the crash happened. was with the service truck called to aid the tractor-trailer driver, police said.

June 8, JEFFERSON CITY, TN - an unidentified tractor trailer driver was killed in a fiery crash Tuesday morning at the I-40/I-81 split in Jefferson County. The driver's name hasn't been released. The tractor-trailer was hauling watermelons from Alabama on I-81 when it ran off the road, caught fire and became lodged in the I-40 overpass.


Truck Driver Dies in Crash

June 8, HOPE TOWNSHIP, NJ - 65 year old Barbara Feather, of Valencia, PA died Tuesday in a one vehicle roll over on Interstate 80. Feather was driving a 2006 Freightliner on I-80 West when she tried to exit the highway at the Hope/Blairstown exit, police reported today. The rig overturned on the exit ramp, slid left and ended up on its side with the cab atop a guardrail, police said. Feather had to be cut out of the rig after the 3:13 p.m. crash. She was pronounced dead an hour later at the scene, police said.

June 8, STROMSBERG, NE - Phillip Sundberg was killed in a weekend farming accident. Sheriff Dwaine Ladwig says Sundberg was farming Sunday near Stromsburg and was trying to unhook a planter from his tractor when the tractor slipped into gear. Sundberg became caught in the planter and died at a hospital.


Collision Kills Tractor Driver

June 8, CARLISLE, NY - 56 year old David Huse of Warnerville died after his farm tractor was hit by a car on a rural upstate New York road. Troopers say Mr. Huse of Warnerville was driving the tractor with an attached mower on a road in Schoharie (skoh-HAYR'-ee) County Monday evening. They say a car driven by 20-year-old Courtney Chichester of Carlisle crossed into the oncoming lane and hit the front of Huse's tractor, flipping it backward onto him. Huse was pronounced dead at the scene.


Wantage farmer is found dead in tractor accident


June 8, WANTAGE, NJ - 45 year old Raymond Hamming died after he was pinned under a tractor, which sat on a steeply graded portion of a Turner Road field at about 6:20 p.m. Monday. Authorities believe Hamming had been working in the field on the tractor when the machine’s tire dipped into a hole and the tractor overturned.

June 8, KNOBEL, AR - 34 year old Jason Scott Rotton of Lake Village is dead after his crop duster crashed in a rice feild off Clay County Road 250. According to Clay County Sheriff Gerald McClung, Rotton was spraying a rice field and as he was banking, he went over to a field and crashed.


Gunman's rage over estranged wife leaves 5 dead at Hialeah restaurant

June 8, HIALEAH, FL - The estranged husband of a waitress showed up at Yoyito Cafe-Restaurant in Hialeah just as his estranged wife was ending her waitress shift, her second day on the job. Armed with a .45-caliber pistol that he owned, he pulled the trigger nine times, killing Molina, three co-workers and himself. He also wounded three others in a shooting spree that was described by witnesses as total mayhem. 24 year old Liazan Molina, wife of the gunman was killed along with her co-workers 32 year old Maysel Figuero and 56 year old Zaida Castillo.

June 8, MEMPHIS, TN - 29 year old Raphael Wilson was killed outside a used car lot on Covington Pike, working as a security guard, when his Mercury Mystique was slammed into from behind. Police say both the driver and his passenger were drunk when their van slammed into and killed Wilson. Wilson’s family says he worked weekend nights as a security guard, so he could spend his days taking classes toward his college degree.


Livery driver shot to death in NY

June 8, BRONX, NY - 48 year old Santos Cesar, a livery driver who worked seven days a week to support his family became the first hack killed this year - after two men he picked up shot him after an argument. Santos fought for his life after the passengers plugged him in the stomach Sunday afternoon. Santos - a married father of three children, two stepkids and two grandchildren - died at St. Barnabas Hospital just before 9:30 p.m.


California worker dies in wood chipper accident

June 8, WATSONVILLE, CA - 60 year old Henry Lira, a Santa Cruz County maintenance worker who died when he was pulled into a wood chipper had been an employee of the county for more than 40 years. According to sheriff's reports, Lira was part of a four-person crew that was clearing brush on a rural road outside Watsonville on Monday morning when the accident happened. Lira was pronounced dead at the scene.


BIA officer dies while responding to call on Utah reservation

June 7, ROOSEVELT, UT - 33 year old Joshua Yazzie, A police officer for the U.S. Bureau of Indian Affairs was killed in a car accident while responding to an incident near Bottle Hollow Reservoir Monday evening. Yazzie was responding to assist ambulance personnel with an incident involving an intoxicated person who was becoming violent. Yazzie was just a couple miles from the incident near U.S. 40. UHP troopers believe Yazzie was going just a little too fast when he entered a curve and lost control, overcorrected and rolled the Ford Crown Victoria patrol car. Yazzie was ejected from the car and killed instantly. UHP Sgt. Ted Tingey said Yazzie was not wearing a seat belt. He leaves behind a wife and two children.


McAllen man electrocuted

June 7, MCALLEN, TX - an unidentified 46 year old worker died Saturday after being electrocuted while working on a water heater. Investigators told Action 4 News the man was working on a water heater at an apartment complex on the 500 Block of East Camelia Avenue when it happened. Police said fellow workers called in the report around 11:21 a.m. Saturday.


Barnegat man dies while working underwater at N.Y. nuclear plant


June 7, BUCHANAN, N.Y - 47 year old Lloyd Beare of Barnegat an employee of Dryden Diving Co. of Woolwich, Gloucester County, N.J. was working as a diver at the Indian Point nuclear power plant died Monday after losing consciousness underwater while preparing to repair a wall at the facility, a spokesman said. He was about 14 feet deep in the Hudson River near the wall to the Indian Point discharge canal when colleagues realized he was not responding to routine communications checks at about 9:25 a.m., said Indian Point spokesman Jerry Nappi. Beare had entered the water about 10 minutes earlier and was tethered to a dock. A co-worker pulled Beare out and an emergency medical technician who was at the dive site began CPR. Beare was then taken by ambulance to Hudson Valley Hospital Center in Cortlandt, where he was pronounced dead at 10:20 a.m.


Man Killed in CA Gold Mine Explosion

June 7, KERN, CA - 62 year old James Steiner Bennett of Kern died after a recent explosion at a gold mine in a remote area of California's Siskiyou County. On May 28, Bennett and another worker were checking on explosives they thought had already detonated at the Know Nothing Mine when the explosives went off, according to the Siskiyou County Sheriff's Office.


Oklahoma Truck Driver Killed in Texas Accident

June 7, OKLAHOMA CITY, OK - 27 year old Ryan Lively of Sharon was killed when his semi rolled over as he was trying to merge onto a highway near Canadian, Texas. The agency says Lively's semi was traveling at a speed greater than he could control when he tried to merge from U.S. 60 onto U.S. 83. Authorities say the vehicle overturned, ejecting Lively.


Man Killed in Tractor Accident

June 7, EASTON, KS - 62 year old James Crum of Leveanworth was killed Saturday in a tractor accident at a rural Easton farm, according to Sheriff’s Office officials. Crum had left his residence in Leavenworth to mow grass at a family farm. After his wife had not heard from him for a period of hours, she began calling his cell phone. He didn’t answer, according to the report. She went to the farm to check on him and found him dead.


One Worker Killed in Gas Explosion

June 6, DALLAS, TX - A work crew digging a ditch accidentally struck a natural gas line on Monday, causing a giant explosion in a rural area near Cleburne, about 50 miles southwest of Dallas. Brian Fine, the Hood County emergency management coordinator, told The Associated Press that one worker’s body was found Monday night some distance from the blast site. The victim has been identified as 45-year-old Oklahoma resident James Robert Neece. Thirteen other workers were accounted for, and Capt. Jerry East with the county sheriff’s office said that six people were taken to a hospital and that four of them had been released. He did not know the status of the others.

Police Trooper hit and killed

June 6, HOWELL, NJ - 29 year old State Trooper Mark K. Castellano was struck and killed by a car Sunday morning while searching for two occupants of an abandoned vehicle on Interstate 195. The driver of the abandoned vehicle, which was found in the same location but was not the vehicle which struck Castellano, was found hiding in the bushes of this heavily wooded area a short time later. Diana Hoffman, 30, of Blackwood, Camden County, was arrested and charges are pending. Authorities are questioning the validity of Hoffman's claim that an unidentified man was a passenger in her vehicle. A search of the area for that suspect, described as a bald black man, wearing a white T-shirt and blue jeans, was suspended Sunday night, State Police said. State Police revealed few details as to why Castellano was involved in the search for Hoffman, why she abandoned the car and apparently was fleeing, and how the chase began. Castellano leaves behind a wife and two children.


Tractor Trailor Wreck Kills One

June 5, ROCKWOOD, TN - 41 year old Tony McMahan of Knoxville, TN Friday night when his tractor trailer collided with another tractor trailer on Interstate 40 in Roane County. was driving west on Interstate 40 when his tractor trailer struck the rear of a tractor trailer driven by Jennifer Murray, 50, of St. Petersburg, Fla., according to a Tennessee Highway Patrol report. McMahan’s truck caught on fire and he was killed. Murray was injured in the accident.


Trucker killed in Highway 44 wreck identified

June 5, REDDING, CA - 44 year old David Delany died Saturday morning in a solo wreck on Highway 44. Delaney was driving a combination tractor-trailer west on the highway at an undetermined speed when the crash occurred on a curve about two miles east of Viola at roughly 8:45 a.m. Saturday. He lost control of the tractor-trailer, which drifted into the eastbound lane. The trailer flipped onto its left side and the big rig went down a steep embankment, the CHP said.


Worker killed in trench collapse

June 5, KNOX COUNTY, NE/SD border - 48 year old Galen Jueden, a Lewis and Clark Rural Water District employee died Friday after a trench collapsed around him in northern Knox County, near the Nebraska/South Dakota border. Jueden and another worker were replacing water lines in the trench, about 5 or 6 feet deep, when it collapsed around Jueden at about 11 a.m. Friday, Sheriff's Capt. Ernest Ibach said. "He was only really buried about up to his waist," Ibach said. Telephone workers in the area pulled him from the trench and performed CPR, but neither they nor rescue workers could revive him. Jueden was pronounced dead at the scene. "We're thinking it was internal injuries," Ibach said.

Jackson VA Employee Dies After Falling From Truck In Texas

June 4, JACKSON, MS - 40 year old Amy Joette Wheat was killed when she fell out of a truck in Texas, police said. WFAA in Dallas reported that two men and a woman were in a Ford F-250 truck owned by the federal government on Wednesday at about 11:30 p.m. A witness told police that a woman, who was later identified as Wheat,fell out of the truck and hit her head on a pole. The two men put the woman back in the truck and said they were taking her to a hospital, police said. Wheat was taken by ambulance to a local hospital, where she was declared dead, police said. Neighbors said Wheat was a single mother with a young daughter. Wheat worked as a nurse recruiter at the VA Medical Center, officials said.


Maintenance worker killed

June 3, LAS VEGAS, NV - 47 year old Adrian Arego-Gonzalez was shot and killed while on the job last week at an east valley apartment complex Las Vegas police said Abrego-Gonzalez was one of two maintenance men fixing a window around noon at the Village at Karen Apartments on Sahara Avenue, between Lamb and Nellis Boulevards. The workers encountered a group of five men who were in the middle of what appeared to be a burglary. Police said the maintenance workers had a "verbal altercation" with the five men. One of the five confronted the complex employees with a pistol and shot the men.


Farm Worker Killed in Accident

June 3, CORNING, CA - 38 year old Manuel Jose Rameriz fell to his death early Tuesday morning in an accident at the Sobarea Ranch northwest of Corning, the Red Bluff Daily News reported. Ramirez fell into the hay baler sometime between 4:30 a.m. and 6:30 a.m., at the farm located on Gyle Road. Ramirez's body was discovered by the ranch's owner around 7:15 a.m. t was not clear why Ramirez climbed into the bailer, but authorities think it may have been to clear an obstruction or blockage.


OSHA probing electrocution in Milford

June 3, MILFORD, CT - 33 year old Victor Larranaga-Marquez of Bridgeport was killed Tuesday evening when an aluminum ladder he and his uncle were using came in contact with an electrical wire, police said. According to police, Larranaga-Marquez and his uncle, Gerrardo Marquez-Hernandez, 45, also of Bridgeport, were working at 65 Point Beach Drive and were maneuvering the ladder at the front of the home when it came in contact with the power line, which was not insulated. Representatives from the Occupational Safety and Health Administration and police are investigating the incident.


Death at Madison County Landfill

June 3, LINCOLN, NY - 30 year old Vincent R. Unczur of Frankfort was in the process of emptying the garbage from the 10-wheel truck he was operating when the upright, hydraulic-controlled box fell on top of him. A nearby commercial hauler rushed to the man’s aid while a county employee called 911. The county employee then used a backhoe to raise the box off his body. The accident happened on the working face of the landfill where solid waste is dumped. County Administrator Paul Miller said Wednesday’s death was the first that has happened at the county’s Buyea Road landfill.


Postal Worker Dies After Apparent Dog Attack

June 3, OCEANSIDE, CA - 33 year old Hao Yun "Eddie" Lin has died after a Rottweiler lunged at him along his route in Oceanside, causing him to fall and suffer a fatal head injury, a postal official said Friday. According to postal inspector Slivia Torres, Lin encountered the dog while delivering mail May 25 in the 500 block of Stanley Street. Some details of the incident are unclear, but someone called Oceanside police about 4:13 p.m. May 25 to report the attack. Lin was found lying in the street with a severe head injury and was transported to Tri-City Medical Center.


Two Killed in Tractor Trailer Crash


June 2, New BRAUNFELS, TX - 62 year old Richard Hardacre, of Ferndale, MI and 53 year old Richard Gallagher of Southfield, MI died after their 18-wheeler drove off of northbound Interstate 35, crashed and became fully engulfed in flames. The men were employed by Troy-based Point Dedicated Services, which also owned the truck, police said. According to police, the truck was loaded with automotive parts being taken from Laredo, Texas to Kansas City, Mo. Wells said it’s unclear who was driving or what caused the truck to veer from the road, causing the one-vehicle crash. He said there was a substantial about of fuel spilled on the roadway, as well as where the truck came to rest.


Two Dead After Tractor Trailer Fire

June 1, VIRGINIA - Two people are dead after the tractor-trailer they were riding in caught fire during a crash this afternoon. About 2:45 p.m., the tractor-trailer’s left, front axle malfunctioned, causing it to swerve off the road, strike several trees and catch on fire, said Sgt. Michelle Cotten, a Virginia State Police spokeswoman. The vehicle was engulfed in fire and the two passengers suffered severe burns, so it will take awhile to identify them and notify their families, she said.


Convenience Store Clerk Shot in Robbery Attempt

May 30, ARLINGTON, TX - 54 year old Prem Sharma of Arlington died during the robbery of a convenience store in the 4500 block of South Bowen Road. Customers found the clerk in the store after the robbery and called police and paramedics. Sharma died at the scene.


Firefighter Dies at Fire Station

May 30, BELLEVILLE, WIS - 63 year old Donald A. Schneider Jr of Belleville died Saturday, May 29, 2010, at one of his treasured spots, the Belleville Fire Station, of an apparent heart attack. He was a 38-year member of the Belleville Fire Department serving more than 20 years as fire inspector.


Restaurant employee murdered

May 28, OLIVE BRANCH, MS - 39 year old Alejandro Gonzales of Memphis and assistant manager of the Mi Pueblo restaurant at 8230 Camp Creek has died after being shot in a robbery attempt at the restaurant. Olive Branch Asst. Police Chief Mark Kimbell said Gonzalez was married, and Kimbell thought Gonzalez had either two or three children.
