Cab Driver Dies in Crash

September 26, BAY SHORE, NY - 55 year old Leon Williams, a driver for Tommy's Taxi, died Sunday morning in a crash. Williams was on duty, but had no passengers, when he lost control of the cab and ran into a utility pole. An investigation is ongoing.


Man Dies After Altercation With Another

September 26, KENSINGTON, PA - 30 year old Ray Santiago, a driver for Saini's Towing, was killed after another tow-truck driver allegedly struck him with his tow-truck after an argument in Kensington. Police tell NBCPhiladelphia another tow truck driver who works for the Straight Up Towing ran over Santiago several times after an argument. The man faces several charges including homicide by vehicle.


Motor Carrier Inspector Killed in Crash

September 26, WAYNE CO, IN - 64 year old Robert E. Pitcher of Pershing was killed Sunday in a head-on crash with a car hauler that closed the eastbound lanes of Interstate 70 in Wayne County, police said. Pitcher was on duty and hauling a piece of equipment utilized for testing brakes of commercial motor vehicles, when he, for an unknown reason, hit a guardrail, crossed the median and struck a tractor-trailer hauling eight cars.


Kansas Man Dies When Semi Hit by Train

September 25, MANHATTAN, KS - 51 year old Paul Cross died when his semi was hit by a train Saturday. Cross had stopped his semi at the railroad tracks just north of K-61 and North East 30 Avenue Saturday afternoon. He then pulled forward in front of the oncoming train, according to the Kansas Highway Patrol accident report. The train was unable to stop and hit the vehicle on the driver side door.


Man Killed Working on Heavy Equipment

September 25, MCFARLAND, CA - 49 year old Jose Luis Esquivel of Orland was killed Saturday in an industrial accident. He was crushed while doing repairs on the hydraulic system of a pistachio harvester.


Jockey Killed During Race

September 25, HOBBS, NM - 44 year old Mark Anthony Villa suffered fatal injuries when the horse he was riding fell. According to his wife Krystal, Villa was trying to crawl under the rail to safety when another horse raced by - it's hoof hit Villa in the back of the head and kicked off his helmet. Krystal told ABC-7 Villa began riding when he was 16-years-old and won more than 1700 races.


Landscaper Mowing Grass Killed by Train

September 25, ROSENBERG, TX - 19 year old Walter Barcenas was mowing grass near some railroad tracks when he was struck and killed Saturday morning. According to police, Barcenas was mowing grass along the railroad line when a BNSF train struck him. He was working with a landscape crew. An investigation continues into the accident.


Tree Trimmer Killed on the Job

September 25, TAMPA, FL - 44 year old Miguel Varas was killed while trimming trees. He became entangled in the rope that was wrapped in the tree branches. The victim fought for his life, but unfortunately, the force of the rope pulling against him caused his fatal injuries. He was not pulled into the chipper.


Firefighter Dies in Line of Duty

September 24, NEW CHURCH, VA - 54 year old William "Hal" Harold Clark died while battling a large brush fire in the town of New Church on the Eastern Shore. No details as to how Clark died or why is available at this time.


Man Killed in Explosion Testing Equipment

September 24, LAFAYETTE, LA - 27 year old Justin Wood of Gretna was killed after an explosion during an oil field equipment test on Friday. State Police say a piece of equipment exploded during a pressure test at a company called Supreme Services. Workers were using nitrogen to conduct the test. Wood was killed when a piece of pipe blew off during the explosion and struck him.


Ohio Firefighter Dies Battling Blaze

September 24, LONDONDERRY, OH - 26 year old Ryan Seitz of Vinton County was killed Friday battling a brush fire in Ross County. He was killed when an end cap blew off a pressurized metal tank and struck him. The tank was being used to fight fires that broke out between Ross-Vinton County lines. Seitz worked for the McArthur Fire Department in Vinton County.


Man Dies When Farm Tractor Rolls Backwards

September 24, BUSHNEL, NE - Marty Webb of Dix died when a tractor he was working on rolled over him. Police say Webb was doing some welding on the machine when it rolled back over him. The tractor was not running at the time.


Tow Truck Driver Struck and Killed

September 24, SEATTLE, WA - 51 year old William Anthony "Tony" Padilla from Burien, working for GT Towning in Seattle, was struck and killed on an entrance ramp while working on a vehicle from an earlier collision. Padilla was kneeling down at the left front side of a disabled vehicle when a south bound car lost control in the HOV lane. The car then crossed five southbound lanes and struck Padilla. He died at the scene.


Construction Worker Killed on Job

September 24, FAIRFIELD, OH - 38 year old Terry D. Garrison of West Union was killed when a dump truck backed over him Friday night. He was standing in the construction zone on Ohio 4, when the dump truck, which was loaded with fresh asphalt, backed over him around 10 p.m., according to Fairfield Township police.


Woman Killed in Semi Crash

September 23, LAKE COUNTY, FL - 65 year old Winifred Brune of Leesburg was killed Thursday when a tractor-trailer truck she was driving crashed into a fence in Lake County, according to Florida Highway Patrol Sgt. Kim Montes. Troopers say the woman was driving west on County Road 46 in the right lane when she began drifting onto the shoulder. The truck slammed into a fence near the roadside. Montes said troopers are trying to determine if a medical condition may have caused the woman to drive off the road.


Logger Killed Working on Machine

September 23, DUNLAP, TN - 39 year old William Eric Posey of Hixon died Thursday when he became entangled in a machine he was working on. For reasons unknown he fell into the machine. No further information is given.


Tractor Trailer Driver Dies in Fiery Crash

September 23, GOVERNMENT CAMP, OR - 54 year old Dusty Lee Mings of Vancouver, WA, a driver for Western Boxed Meat Distributors, died when his rig hit a big cow on an Oregon highway. The truck then hit some trees and burst into flames.


Worker Killed When Crushed in Equipment Accident

September 23, PENDERGRASS, GA - 50 year old Bradley Keith Marlowe was killed when he was pinned between a trailer and bulldozer Thursday. Marlowe was working on a bulldozer when a trailer that was parked on an incline rolled into Marlowe, trapping him. A witness used a pickup truck to move the trailer, but Marlowe died at the scene.


Worker Dies in Trench Collapse

September 23, DOTHAN, CT - 47 year old Ray Anthony Blackmon of Dothan was killed Thursday when the trench he was working in collapsed around him. Blackmon and another worker were laying a sewer line down behind a junior college when the trench they dug collapsed around them. The second worker is in intensive care.


Florida Worker Dies After Being Electrocuted

September 23, PALM BEACH, FL - an unidentified man, working in a condominium in Palm Beach has died after being electrocuted. The victim had been doing work in the bathroom and was found to have burn marks on his hands. He was in full cardiac arrest en route to the hospital.


Idaho Man Dies in Accident Involving Forklift

September 23, HAZELTON, ID - 36 year old Michael Louder died after he was pinned between a forklift and a potato cellar door while working Thursday morning. Jerome County sheriff’s deputies and Jerome County paramedics responded shortly after 9 a.m. in attempts to free Louder and revive him. Despite their efforts and assistance from air ambulance paramedics, Louder died at the scene.


Worker Killed When Forklift Overturns

September 22, PHOENIX, AZ - an unidentified employee of a Phoenix Recycling facility is dead after a forklift rolled onto him. Authorities believe the worker was operating the forklift in the backyard area when it collided with a front loader. The larger piece of equipment knocked the forklift over and in the process, the operator fell out of the lift and the forklift rolled onto him and killed him.


Tractor Trailer Crash Kills One

September 22, MEMPHIS, TN - an unidentified 57 year old tractor trailer driver from North Carolina was killed when the truck he was riding in crashed on Interstate 40 near Midtown Memphis, police said. The victim was on his break in the sleeper section of the cab. A preliminary investigation found that the driver lost control of the eastbound truck, causing it to flip over and slide along the pavement edge of the exit ramp for traffic heading north on the interstate loop. The truck hit a guardrail, and the cab struck a support pier for a flyover overpass, police said. Both the victim and driver were found outside the cab of the truck. UPDATE: Authorities have identified the victim as Milton Arthur Daugherty from Englewood, FL.


Farmer Dies in Accident While Clearing Trees

September 22, NATALIE, VA - 73 year old Gene Raymond "Baby" Crews of Lennig Road died at the scene from injuries sustained when he was struck in the head by a tree limb. “A witness who saw what happened said Mr. Crews was bush hogging, and he was knocking down some tree limbs with the loader,” Sheriff Noblin said. “One of the limbs came back and struck him in the head, knocking him off the tractor.”


Construction Worker Dies After Support Beam Gives Way

September 22, BENTON HARBOR, IN - a 47 year old construction worker was killed when a support beam in the building he was working in gave way and knocked him off his ladder. According to authorities, a support beam fell and hit the worker and took out his 12-foot ladder. The victim fell from the ladder onto acetylene tanks. When he fell on the tanks, Connell said it caused internal injuries to the worker.


Business Owner Found Dead

September 21, CHINO, CA - Lenny Woods was found dead at the Chino Hills Ford car dealership that he owned for many years before having to close it after extreme financial difficulties. Friends and employees of Woods said he was found dead inside the dealership after apparently committing suicide. At the time, Woods was distraught over the loss of his life's work.


Man Dies When Truck Overturns

September 21, CARLSBAD, NM - 29 year old Marcus Perez was killed Tuesday evening when the brine water-filled truck he was driving rolled, throwing him out of the cab. Investigating officers concluded that Perez was approaching a right hand bend in the roadway. He was unable to negotiate the turn at the speed he was driving, and as a result, the 2004 Mack semi-truck and trailer entered the southbound lane of traffic and then over-corrected to the right, causing the semi-truck to go into a skid. The weight of the brine water in the tanker trailer caused the truck to roll over, subsequently throwing Perez from the cab.


Second Ohio Realtor Victim of Homicide

September 21, KENT, OH - 51 year old Andrew VonStein, a realtor for Cutler Realty, was found dead under "suspicious circumstances" after he was reported missing. Authorities located his car via OnStar at a vacant house and upon checking the residence deputies found his body inside the house. His death is being investigated as a homicide but have not released the manner of his death.


Sheriff's Deputy Dies in Crash

September 21, TAMPA, FL - 48 year old Mark A. Longway of Hillsboro, a Hillsboro Sheriff's Deputy, died from injuries sustained in a crash involving a tractor trailer in downtown Tampa. Authorities say Longway and the truck collided early Tuesday morning in an intersection near an interstate overpass. One of the cars had a red light, but authorities did not immediately say who was at fault. The deputy's cruiser was under the semi and he died at the scene.


AT&T Employee Dies on the Job

September 21, COLUMBUS, OH - 52 year old Bryan R. Green of Pataskala was found crushed to death Tuesday morning. According to Columbus police, the worker’s van crushed him when he was working on an AT&T distribution box. The victim’s van was not in parking gear, police said. His van trapped him against the cable-company box. A homeless person found the victim’s body. He went for a help and called 911.


Deputy Fire Chief Dies After Training Exercise

September 20, HINSDALE, IL - Mark Johnson, Deputy Fire Chief for the Hinsdale Fire Department, had just returned from an emergency services call and was taking part in physical training when he collapsed. On duty members tried to revive him, but he was pronounced dead shortly after being transported to the Hinsdale Hospital emergency room.


Realtor in Ohio Found Dead

September 20, KENT, OH - 67 year old Vivian Martin, owner of Essence Realty, who was showing a vacant house for sale, was found dead on the kitchen floor in the burning home Monday in Youngstown, said police chief Jimmy Hughes. An autopsy showed she was strangled before the home burned down, and the fire was ruled an arson. Martin's death may be related to a robbery of another realtor a week before and again to the death of another agent Tuesday. Two male suspects have been arrested.


State Trooper Dies in Crash

September 20, BOWIE, TX - 56 year old Cpl. David Ralph Slaton, a trooper for the Texas Dept.of Public Safety, was killed Monday night after hitting a cow and then colliding head-on with a tractor-trailer in Montague County. The impact (of hitting the cow) pushed his vehicle into the northbound lanes, where he hit a southbound semi, said Tela Mange, a spokeswoman for the Texas Department of Public Safety. Slaton died at the scene.


High School Teacher Dies Unexpectedly

September 20, IRINE, CA - 46 year old Kent Bentley, a Woodbridge High School English teacher collapsed and died in his classroom during lunch break on Monday. A group of students was walking by his classroom and spotted Bentley on the ground. A staff member tried to provide CPR but was unable to revive Bentley, according to Irvine Unified spokesman Ian Hanigan.


Worker Dies in Forklift Accident

September 20, ARDMORE, OK - 36 year old Jason Chastain died Monday after a forklift accident at DOT Foods in Ardmore. Chastain hit something with his forklift and became pinned. A full investigation has been launched.


Postal Worker Dies When Van Hit by Train

September 20, MOBILE, AL - 64 year old Nathan Williams III was killed when the truck he was driving was struck by a freight train in southern Washington County. Williams was not wearing a seat belt and was ejected from the truck when the southbound Norfolk Southern railway freight train hit it.


Man on Way to Make Deposit, Killed in Robbery

September 20, WOONSOCKET, RI - 49 year old David Main was shot and killed outside a Rhode Island bank on Monday. Main was shot in the head by a man in a black mask who took off by foot. Main was a gas station manager and was at the bank to make a company deposit. Police and the FBI are searching for two suspects.


Man Electrocuted Dismantling Light Poles

September 20, PORT ORANGE, FL - 59 year old Wade Leonard Madole of Boynton Beach, was killed when he grabbed a live wire, according to Port Orange's fire marshal. Madole and another man were dismantling the bases of light poles no longer needed at a food service warehouse expansion project. The electricians were pulling out wires without problems, but just after 5 p.m. Madole was trying to get out of a small hole they had dug, grabbed onto a bare wire and was electrocuted. He then went into cardiac arrest and died at a nearby hospital.


Semi Driver Killed in Fiery Crash

September 20, HACKENSACK, NJ - 45 year old Gregory Griffaton of Springfield, PA was killed early Monday morning after the vehicle careened into a guardrail on Route 17, struck a tree and burst into flames, officials said. The truck overturned on an embankment before catching fire, and a police official described the cab as “unrecognizable.” The official added that the condition of the truck was making it difficult for investigators to determine for whom Griffaton was driving.


Highway Maintenance Worker Dies From Fall

September 20, BERKLEY SPRINGS, WV - 48 year old Robert Fleece Jr. was killed after being thrown from the back of a DOH dump truck near Berkeley Springs Monday morning. State Troopers say Fleece and two others workers were on the back of the truck making sure gravel was spread evenly on a road that was being tarred. Witnesses say the truck was slowly backing up when its bed suddenly raised to maximum height and threw Fleece across the safety chain and onto the pavement where he hit his head. He was pronounced dead at a nearby hospital a short time later.


Taxi Driver Killed During Carjacking

September 19, LYNWOOD, CA - 56 year old Erasmo Amaral who died Sunday after being stabbed and dragged from his taxi by an apparent robber. Detectives say the suspect stabbed Amaral in the upper torso, then drove the taxi westbound on Imperial Highway while the victim clung to the outside of the driver's door. Amaral had been pinned between several vehicles as he was dragged for "quite a distance," detectives said.
The suspect turned northbound on Jackson Avenue and sideswiped the driver's side of the taxi into three parked vehicles. He then got out of the cab and fled on foot.


Explosion Kills Truck Driver Offloading Propane

September 19, ALTON, MO - 39 year old James Dewayne Smith, a driver for Huellinghoff Brothers Inc. of Union, Mo., died as a result from an explosion September 13. He was offloading propane from his tanker truck to a storage tank at Updike's Gas Service and an explosion occurred for a reason that officially remains a mystery.


Farm Tractor Flips, Killing Driver

September 19, WORCESTER, NY - 40 year old Adolph Schrull Jr died when a tractor flipped over on top of him. Schrull was attempting with a friend to remove an old steel hay rake from the brush in a field by hooking a chain to it and pulling it free. The action at some point caused the front wheels to come off the ground, said State Police, and the tractor flipped backward on to itself while Schrull was still in the driver seat of the older-model Ford 8N tractor.


Missing Campground Worker Found Dead

September 8, ATLANTA, GA - Todd Tumlin went missing on August 28th. On September 8th his body was found in the woods off Pulliam Mill Rd. near the site of River’s Edge, a gay campground where Tumlin worked. Local law enforcement investigators have ruled the death a murder and are investigating the crime as a “homicide by gunshot.”

Owner of Wrecker Business Found Dead

September 18, GREENVILLE, NC - 59 year old Walter Clay Malloy Sr, was found dead outside his business, Clay's Wrecker Service. They say the business was not broken into or robbed. Police are not saying how Malloy died, but they are treating this an a death investigation.


Police Officer Found Dead

September 19, ORWIGSBURG, PA - 47 year old William Bradley, a police officer, was found dead Sunday in the borough police station, state police said. Bradley was found with an apparently self-inflicted gunshot wound, troopers said. Investigators did not specify where on his body he was shot.


Ohio Man Dies in Farm Accident

September 18, PARIS, OH - 50 year old Steven Shockling, working atop a silo at his township farm fell about 50 feet to his death Saturday morning. Shockling was working on equipment at the top of the empty silo at his family farm when he fell inside about 10:45 a.m., said Rick Walters, Stark County Coroner’s Office investigator.


Man Killed Making Repairs to Car

September 18, GREENSBORO, NC - 19 year old Talon James Brandon Gravely, an employee for Whip Appeal Auto, LLC, was killed Saturday when part of the vehicle he was working underneath collapsed on him. Detective M.D. Matthews said other people were working at the shop at the time, however no one was with Gravely, who was underneath a car outside the shop.


Man Working Vineyard Dies in Truck Rollover

September 17, WINDSOR, CA - 73 year old Leo John Becnel died when his truck overturned at his vineyard in unincorporated Sonoma County west of Windsor on Friday evening, according to the California Highway Patrol. Becnel was backing a dump truck down a steep slope made up of loose dirt, and he was not able to stop the truck, which turned perpendicular to the slope and rolled over. He was not wearing a seat belt, and the truck did not have a windshield. He was ejected from the car and suffered fatal injuries, according to the CHP.


Backhoe Operator Crushed by Falling Tree

September 17, BARRETT, MN - Mitch Brethorst, owner of Brethorst Excavating, was killed when a large cottonwood tree he was trying to clear fell on the full-swing backhoe he was operating, crushing him. Brethorst had dug on two sides of the tree when the tree toppled over, landing on top of the cab of the backhoe. The tree crushed the cab of the backhoe, trapping him inside.


Truck Driver Killed in WV Accident

September 17, WHEELING, WV - 58 year old Ronald Kiser of Fayetteville died after he crashed his truck into another. Kiser drove into the back of another tractor trailer around 2:30 this morning on I-70 in that area. The Ohio County Sheriff says the driver of the first tractor-trailer was traveling at a slow rate of speed with his hazard lights on when Kiser hit him. Kiser's cab became pinned under the tractor of the truck he hit and burst into flames. Kiser died at the scene.


Forklift Overturns, Killing Operator

September 17, PHOENIX, AZ - 29 year old Lewis Acuña, who lived just outside of Arizona City, was killed Friday in an accident involving a forklift. Acuña, who worked for Evergreen Turf, was delivering sod to a home had just dropped off one load and was returning back to end of the very steep driveway for a second load when he lost control of the forklift. The forklift picked up quite a bit of speed before it flipped on its side, ejecting Acuña and pinning him underneath. He was pronounced dead at the scene.


Man Killed Working on Farm Tractor

September 16, BARBER, OK - 56 year old Richard Cinnamon has been killed when the tractor he was working on rolled over him. Investigators say the tractor was running while Cinnamon worked on it and it apparently slipped into gear and ran over him.


Road Construction Worker Struck by Truck

September 17, FOREST VIEW, IL - 33 year old Aurelio Rosales, an employee of Midwest Fencing, was killed Friday afternoon by a semitrailer-tractor truck coming off of the Stevenson Expressway in southwest suburban Forest View, authorities said. Rosales was making repairs to a barrier fence when he was struck, said IDOT spokesman Guy Tridgell.


Construction Worker Dies in Fall

September 17, OVERLAND PARK, KS - A construction worker died and two other men were seriously hurt Friday when a scaffold collapsed during construction of a middle school in south Overland Park. The men were believed to be working on the scaffold in a three-story interior stairwell of the school when the scaffold rig, which was erected from the ground, as opposed to being suspended, collapsed. An Overland Park Fire Department spokesman estimated the fall at 30 to 35 feet. UPDATE: Authorities have identified the worker who died as 52 year old James D. Tyree of Eudora, KS.


Young Worker Dies in Grain Elevator Incident

September 17, COLUMBUS, OH - A 21 year old year old man was killed this morning in a Marion County grain elevator. He and another employee were cleaning a grain bin and died when he fell and was trapped in the auger mechanism. The auger, located at the bottom of the bin, was working as the two men were cleaning, shoveling grain into the auger instead of shoveling it out of the bin. Assistant Chief Dan Williams said it is suspected the victim's foot went through the grating and got caught in the auger, which pulled him down.


California Firefighter Dies After Training Exercise

September 16, IRVINE, CA - 56 year old James M. Owen died today after a routine training exercise. Battalion Chief Kris Concepcion of the Orange County Fire Authority said Owen collapsed in full cardiac arrest after an urban search-and-rescue drill in Irvine. Concepcion did not know if Owen had just finished a classroom session or an active part of the drill.


Accident Claims Farmer's Life

September 16, WESTMORELAND, KS - 44 year old Frederic Biesenthal was found dead just before 10 p.m. Thursday, the victim of an apparent farm accident, Pottawatomie County Sheriff Greg Riat said. Biesenthal may have become entangled with a moving part of the tractor when he was moving hay.


Laundry Store Employee Gunned Down

September 16, TRENTON, NJ - 33 year old Ambreesh D. Patel was fatally shot during the daylight robbery at a West Ward laundry after he told the gunman he did not have any money, police said. An armed man who allegedly walked into the store with a T-shirt covering his face, shot Patel once before fleeing. The suspect remained at large.


Worker Killed at Plant, Crushed by Machinery

September 15, NORCROSS, GA - an 48 year old man was was crushed on Wednesday by a stacker at the RockTenn plant. RockTenn produces paperboard and packaging products. His name or other details have not been released.


Officer Accidentally Shot During Training Exercise

September 15, ST.JOSEPH, MO - 35 year old Officer Dan De Kraai was fatally shot Wednesday afternoon while on a break from a training session, the Police Department said. was hit in the back after another officer’s handgun discharged about 2 p.m. at a closed school that was the site of the training session, according to a written statement from police. The shooting appeared to have occurred during some sort of demonstration, he said. De Kraai and the other officer were longtime friends.


Trucker Dies in Fiery Crash

September 15, TRENTON, NJ - 44 year old Michael Hyland of Smyrna, DE died in a fiery crash on the New Jersey Turnpike Wednesday. "For reasons that are under investigation he lost control of the vehicle, veered to the right and hit the concrete barrier, veered to the left and hit the median, then veered to the right again and struck the barrier," Trooper Brian Polite said. The series of impacts caused the tractor to burst into flames, and rescue crews were unable to rescue him before he perished.


Park Volunteer Dies in Tractor Accident

September 15, ELLENSBURG, WA - 80 year old Robert Arce of Presidio, TX was killed after he was pinned under a tractor near Ellensburg, Wednesday afternoon. Arce was attempting to pull a stuck forklift off the yard using a tractor, when it flipped over on top of him. KNDO spoke to Arce's wife who said she and her husband drove up each summer in their motorhome to volunteer at the park. This was their third year to help out at the event. Arce's wife would give tours to park visitors, while he took care of the grounds that would include lawn care.


Forklift Accident Claims Life of Business Owner

September 15, CHANDLER, OK - 64 year old Bill C. Matthews, owner of Lincoln County Farm Center, died Wednesday in an incident involving a forklift. Details of what happened are not clear but it is being reported that he was not driving the forklift.


Student Driver Hits and Kills UPS Employee

September 15, DES MOINES, IA - 64 year old Daniel Raber, a United Parcel Service employee, died after he was hit by the trailer of a semitrailer truck at the Des Moines airport early Wednesday. Raber was opening a gate so the truck could make a delivery when the trailer struck him. He was pinned by the trailer against a guard shack, but not crushed, police said. Raber then fell, and the rear wheels of the trailer ran over his legs. The driver of the truck that hit Raber was a trainee, and the instructor was a passenger in the truck.


Man Dies in Mower Accident at State Park

September 14, GERARD, MO - 62 year old Donald D. Lamb of Hepler was killed after the mower struck him while he tried to free it from mud. Lamb apparently got off the mower to try and free it when the machine moved forward, pinning him.


Asphalt Paving Company Employee Killed in Accident

September 14, LIVERMORE, CA - 36 year old Ramiro Vargas died Tuesday after he was struck by runaway paving machine in unincorporated Livermore, the Alameda County Sheriff's Office said. Sheriff's Lt. Kelly Miles reported that while making repairs to a private driveway the paving machine apparently malfunctioned and rolled down a hill. It crashed through a barbed wire fence and stuck a worker standing next to an asphalt truck. The worker did not hear the paver coming because of the generator noise and didn't see it because his back was turned, Miles said.


Accident Involving Heavy Equipment Kills Worker

September 14, MINOT, ND - 24 year old Joel Douglas Larsen of Gillette, WY was killed late Tuesday afternoon near Minot. Larsen was working on a skid-steer loader when the front end came down on top of him, killing him. Ward County Sheriff’s Capt. Todd Keller says some safety pins apparently were not in place. Keller said Larsen was working for Clark’s Heavy Equipment and Field Service Repair, an oil field service company based in Belle Fourche, S.D.


Man Dies in Mower Accident

September 14, STUART, FL - a 26 year old man lost his life Tuesday in a landscaping accident at an apartment complex in Plantation. The man was driving a riding industrial mower adjacent to a man-made lake when somehow the mower ended up in the lake. It is not clear if he experienced a cardiac episode before or due to the accident as he was in full arrest when rescuers brought him to the surface after being pinned under the mower.


Driver Dies When Refuse Container Truck Hits Bridge

September 14, CLEVELAND, OH - 39 year old Marvin Matos, who may have been unaware that the vehicle’s boom was in the upright position, died Tuesday after a violent collision with the Hull Street Amtrak bridge. The truck is believed to hit the bridge at considerable speed, police said. A police officer working a nearby traffic detail was immediately on the scene to provide aid, police said.


Gas Station Worker Dies After Falling Off Ladder

September 14, ATLANTA, GA - an unidentified worker fell to his death Tuesday night. Investigators said the man was changing letters on a sign at Cheshire Bridge and La Vista roads when he fell off a ladder. The man suffered major head trauma and died at the scene.


Driver of Semi Truck Dies in Crash

September 14, CLEVELAND, OH - a car caused an accident sending a Tank Integrity Services semi-truck that was traveling westbound over the embankment and onto Lee Road below causing the death of the semi driver. The victim's identity has not been released.


Virginia State Trooper Dies in Training Incident

September 14, RICHMOND, VA - 41 year old Virginia State Trooper Mark D. Barrett died Tuesday during an annual in-service training in the southeastern portion of Lake Anna on the Louisa County side. Trooper Barrett and his “dive buddy,” a fellow trooper, were conducting a training dive. As they were ascending, the dive buddy noticed Trooper Barrett was having difficulty. The dive buddy aided Trooper Barrett to the surface and immediately signaled for help from the other divers. He died after being transported to a hospital in Henrico County.


RV Mechanic Killed in Garage Accident

September 14, ARROYO GRANDE, CA - 65 year old Ronald Arthur Kelsey died of blunt force trauma after being run over by a motorhome Tuesday afternoon in Arroyo Grande, and a Guadalupe resident was arrested on suspicion of felony driving under the influence and gross vehicular manslaughter while intoxicated. Kelsey was working underneath the motorhome when for some unknown reason the suspect put it into motion and ran over the victim.


Highway Worker Struck and Killed

September 14, FRAMINGHAM, MA - 57 year old Gregory Vilidnitsky, an inspector for the state’s Department of Transportation, was concentrating on his work Tuesday night. He was checking the pavement temperature as part of quality control when red pickup carrying two allegedly drunk individuals ran into Vilidnitsky and then smashed into an oil truck, prosecutors and police said. Vilidnitsky, who would have turned 58 on the 16th, was pronounced dead at the scene. The occupants of the pickup are in police custody.


Water Truck Driver Thrown Under Truck Dies

September 13, LEXINGTON, SC - 47 year old Dewayne Harrison of New Zion was driving a water truck used to keep dust down. He parked the truck but the parked truck began moving as Harrison was exiting the vehicle, and he lost steering control. The truck hit Harrison, who was thrown under the vehicle as the truck hit a ditch.


Driver Dies When Cement Truck Overturns

September 13, DETROIT, MI - 60 year old Arlander Eustice Rogers Jr. died Monday, hours after the rear tire of the cement truck he was driving blew, forcing it to overturn on westbound I-94 near Outer Drive. The truck flipped on its side and went off the roadway into the median, striking the guardrail, according to the Michigan State Police. The truck was hauling two trailers, and one broke loose as the truck flipped, hitting the overhead road sign. The sign then fell on the trailer.


City Employee Electrocuted

September 13, FAIRVIEW, OK - 25 year old Matthew Meister a lineman for the municipal electric services, died while installing an electric meter at the Mennonite Brethren Church. The cause of the accident remains under investigation but authorities think Meister slipped while working and fell onto a set of wires, Fairview City Manager Dale Sides said.

September 13, WATONWAN COUNTY, MN - 83 year old John James Nasman was killed Monday on his farm in southern Minnesota after his tractor apparently rolled over onto him, pinning him underneath. The accident appeared to have occurred earlier in the day, according to the sheriff's office.

September 13, SEARCY, AR - 76 year old Jerrell Cockrell has died after apparently falling from a 10 foot ladder inside the store. He was found unconscious by a store customer.


Man Crushed in Tree Trimming Accident

September 13, SIMSBURY, CT - 24 year old Luke Stetson of Windsor Locks was killed Monday by a falling tree. Police in Simsbury say Stetson was about 30 feet up a tree trimming branches and wearing a safety harness when the white oak fell over and landed on top of him. The owner of Wayne's Tree Removal says the tree Stetson was working on looked healthy and strong, but he learned later it had dry rot inside. He called it a freak accident.


Steel Beam Falls, Kills One at Science Park

September 13, NEW HAVEN, CT - One worker was killed and three others injured, when an I-beam fell at a construction site in the Science Park area Monday morning. Based on preliminary accounts two workers were inside a mechanical lift when the “significant” horizontal beam fell from about 30 feet on the west side of the structure, struck the lift bucket and crashed to the ground. He said it was unclear if the other two injured workers were somewhere elevated in the structure and fell to the ground with the beam falling on top of them or they were on the ground when the collapse happened. UPDATE: Authorities have identified the victim as 44 year old Robert Adrian of Coxsackie.


Man Dies in Construction Truck Rollover

September 13, LEXINGTON, SC - An unidentified man is dead after an accident at a construction site in Lexington County. Lexington police said a crew at a construction site was getting ready to leave for the day when, according to officers, one of them was behind the wheel of a water truck. For some reason, the truck began rolling, and the driver exited the vehicle. Police said the truck hit a ditch, which shifted the water in the truck. Somehow, the man became pinned beneath the vehicle.


Preston County Farmer Killed in Tractor Accident

September 12, KINGWOOD, WV - 59 year old Terry Delaney was killed Sunday after his tractor rolled over on top of him. He was found pinned under his tractor by his wife and neighbor. An investigation by the Preston County Sheriff's Department concluded that Delaney was working on fencing, on an embankment, when the tractor overturned.


Farmer Dies After Being Trapped in Equipment

September 12, ARCADIA, MI - 60 year old Rick Williams died Sunday when he became trapped in farm machinery. Williams was cleaning equipment used to bind hay when he apparently became trapped shortly after 4 p.m. Sunday, Manistee County sheriff's officials said. A neighbor turned off the baler and freed Williams, who died at the scene, sheriff's officials said. Williams wanted to make his farm on St. Pierre Road accessible for children with disabilities to fish and ride horses. Family members said that goal will continue as a tribute to Williams, who died in a weekend farm accident.

September 11, WRIGHTSTOWN, PA - 67 year old Fran Gorski was killed Saturday afternoon while repairing one of the trucks from his trash collection business on Cedar Lane in the Penns Park area of Wrightstown. He and an employee had been repairing the vehicle when Gorski was pinned beneath it. Mr. Gorski was also a volunteer for 51 years and deputy chief of Lingohocken Fire Co.


Rayville Officer Shot, Killed During Burglary Call

September 11, RAYVILLE, LA - 57 year old Sgt Thomas M. Alexander was shot and killed while responding to a burglary-in-progress call at a home on U.S. Highway 80, east of Rayville,Saturday night. Richland Parish Sheriff Charles McDonald tells The News-Star that Alexander was shot when he confronted the suspects inside the home. McDonald said Alexander was pronounced dead at the scene.


Man Dies in Farm Tractor Rollover

September 10, ALBANY, MN - 28 year old Christopher Louis Sabrowsky died Friday afternoon in an accident involving a farm tractor that rolled over near Mapleview Rd. south of Albany. Sabrowsky had been attempting to pull two other tractors free from a muddy field when the tractor he was on tipped over, pinning him. Rescue personell were able to remove him from beneath the tractor, but were unable to revive him. He was declared dead at the scene.


Tragic Accident Kills Longtime Employee

September 9, CAPE GIRARDEAU, MO - 61 year old Glen Sinclair, a 33 year employee with Teen Challenge was killed in a terrible accident involving a mower. Sinclair was using a tractor and a brush hog out behind a home on the Teen Challenge property where he somehow fell off the tractor, and was found by a person in the home. He was partially under the mower. He died at the scene.


Firefighter Dies of Work Related Injury

September 9, AUGUSTA, GA - 56 year old Terry Nielsen attended a training event on June 22, 2010 where he suffered a minor injury to his left arm. Later, he began experiencing severe pain in that arm, nausea and a fever. He had contracted Necrotizing Fasciitis, commonly known as the “flesh-eating” disease, and died September 9 at Doctor’s Hospital in Augusta. Doctors were able to halt the spread of the virus, but not before its effects caused the loss of skin and muscle tissue to a large portion of his upper body. Nielson was a seven-year veteran of the fire service and former State of Georgia Rookie Firefighter of the Year.


Ranch Accident Kills Farmer

September 9, MONTGOMERY COUNTY, TX - 77 year old Frank Joseph Mock was killed Thursday in an accident involving his farm tractor. According to Precinct One Justice of the Peace Lanny Moriarty Mr. Mock had been shredding the pasture. For an unknown reason the Massey Ferguson tractor died. Using his pickup he attempted to jump the tractor. The key had been left on and when the battery cable came in contact with the battery the tractor and shredder ran over him. He went into cardiac arrest and was unable to be revived.


Bridge Worker Crushed in Equipment Accident

September 9, BROOKLYN, NY - 50 year old Jerry Burgess of New Haven, CT died Thursday when he was crushed on the Williamsburg Bridge. Burgess was on a mechanical lift under the Brooklyn side of the span preparing the bridge for a paint job when the platform got stuck officials said. Burgess' partner stepped off the lift and onto the steel supports of the bridge as the two men tried to reactivate the mechanical platform, officials said. It was unclear what happened next - but officials said the platform suddenly moved, pinning Burgess between the metal lift and the steel supports of the bridge.


Two Employees of Kraft Foods Killed by Co-Worker

September 9, PHILADELPHIA, PA - 47 year old Tanya Renee Wilson and 36 year old Latonya Sharon Brown were shot to death in the break room of the Kraft Foods plant Thursday. A worker who had been suspended and escorted off the property just prior to the shooting returned minutes later with a gun and shot Wilson and Brown and another person who is in critical condition at a local hospital. The suspect turned herself in and has been charged with the crime.


Man Dies From Injuries From Combustible Sawdust

September 9, HUTCHINSON, KS - 46 year old Kevin Lewis of Burrton died and a co-worker was injured in a fire Wednesday at Woodwork Manufacturing in Hutchinson. Hutchinson fire official Wes Stewart says Lewis was standing on a platform about 12 feet above ground near a sawdust collector when the sawdust suddenly burst into flame. Stewart says Lewis either fell for jumped from the platform. Highly combustible sawdust burst into flame when the collector was opened, allowing oxygen in.


Electrician Killed at Yakima Sawmill

September 9, WHITE SWAN, WA - 62 year old James Espinoza was working on a planer when its heavy canopy door collapsed on him Thursday morning, said Yakima County Coroner Jack Hawkins. "He was caught between the (door) and the frame of the planer," Hawkins said. It appears that a hydraulic cylinder holding the door up may have failed he said.


Tractor Trailer Driver Dies When Rig Overturns

September 9, CABOOL, MI - 70 year old Eugene E. Hale of Cabool died when he was ejected from his tractor-trailer, according to the Missouri State Highway Patrol. Hale's vehicle was traveling northbound on Highway 17 at Eunice when it ran off the left side of the roadway and overturned, ejecting Hale. He was not wearing a seat belt.


Texas Worker Slips and Falls From Water Tower

September 9, KOKOMO, IN - 30 year old Rodolfo Torrez Vasquez of Dallas, TX died in Kokomo Thursday night as a result of slipping and then falling off the water tower. He didn’t fall to the ground, but reports say he was dangling 50 feet off the ground when deputies arrived. Howard County Coroner Jay Price said, “The safety line caught the victim… However he became entangled in some cables. It appears he may have suffered a broken neck during the fall.


Flagger Killed by Truck in Construction Zone

September 8, ROBINSON, IL - 34 year old Wayne Petersen of Orland Park, a flagman at a Crawford County construction zone, was struck and killed by a grain truck as he was running to avoid it. The flagman was headed for a ditch on the western side of the road as the grain truck bore down on the construction zone, but was hit by the truck in the ditch after its driver swerved to avoid stopped traffic at the last minute, police said.

September 8, TOMAH, WI - 45 year old Richard J. Conner was killed Wednesday in a farm accident. When Monroe County Sheriff's deputies arrived they found that Conner had been caught in a piece of farm equipment.


Farmer Struck by Car Trying to Save Calf

September 8, SPENCER, OH - 54 year old Keith Wetmore was struck and killed by a car on a dark road while trying to rescue a stray calf. He had stopped his tractor just after 11 p.m. Wednesday, got off and was trying to rescue the calf when he was struck by the car driven by an 18-year-old. There were no tractor or street lights.

September 8, EL MONTE, CA - 48 year old Hector Urias was found shot to death inside his El Monte auto shop on Wednesday. Detectives said there were signs of struggle inside the business. Urias specialized in drag racing Volkswagon Beetles and was active in the Pro Racing Association.


Farmer Dies in Tractor Accident

September 8, ROCKLAND, WI - 92 year old Kenneth Beron died when he was accidentally ran over by a tractor. Beron and his 82 year old brother were attempting to disconnect a mower from the back of the tractor around 2:15pm. The brother's foot slipped, releasing the clutch, and the tractor reversed over the victim.


Man Electrocuted Trimming Trees Has Died

September 8, PILOT MOUNTAIN, NC - 28 year old Alan D. Daley, an employee of Crump Tree Service, was shocked while trimming trees last month and has died of his injuries. Daley was trimming trees in a bucket truck on State Street on Aug. 28 when he came into contact with a primary electrical line. He went into cardiac arrest after being shocked, but was revived at the scene. He died this week at Wake Forest University Baptist Hospital.


Ambulance Driver Dies in Crash

September 8, AVONDALE, WV - 35 year old James Bailey died when the ambulance he was driving hit a guardrail and rolled down an embankment. The vehicle rolled more than 20 feet down the embankment before coming to a rest with Bailey pinned underneath it. Witnesses to the accident say Bailey was still alive, but died later on scene.


Man Dies After Being Pinned in Truck

September 8, WISTER, OK - 52 year old Alonzo Evans died after being pinned in a dump truck for more than an hour Wednesday afternoon. According the report, Evans lost control of his northbound 1995 International dump truck on a curve, and the truck went off the left side of the roadway. According to the report, the road was wet at the time. The truck flipped once, and Evans was pinned in the wreckage for about an hour and 20 minutes before being freed by Wilburton firefighters, according to the report.


Worker Dies When Lawnmower Rolls Into Canal

September 7, LAUDERHILL, FL - Police are investigating the death of a maintenance worker after he crashed his lawnmower into a canal behind an apartment complex. Police said the victim was pinned under the mower under water. Divers were called out to the scene to rescue the victim. Once they freed him, rescue crews transported him to the hospital in critical condition where he was later pronounced dead.

September 7, TAYLOR, MI - 54 year old Lt. Dan Kromer of Newport, a 20-year veteran of the Taylor Auxiliary Police Department, died Tuesday night after being struck on the shoulder of westbound I-94 near Monroe Street while assisting a stalled motorist, Taylor police said. Police are searching for the vehicle that hit Kromer.


Worker Dies in Lineman Accident

September 7, AUSTIN, TX - 48 year old Barry Hoke of Fentress was killed and a co-worker injured in an incident Tuesday while power was being restored to lines in Caldwell County, according to Bluebonnet spokesman Will Holford. According to Holford, the two men were in a bucket truck and somehow made contact with electricity. The company does not know what caused the incident.


Man Dies as Result of Fall at Work

September 7, KENNETT, MI - two unidentified workers have died from a work related accident. According to Pemiscot County officials, deputies there were called to the Oakley Incorporated site at the Pemiscot County Port Authority regarding two male employees who had reportedly fallen from a cat walk inside the plant. Lieutenant Ryan Holder said that once authorities arrived on the scene, it was determined that one of the victims, a Kennett man who's name has yet to be released, died as a result of the accident. The second victim died of his injuries after being flown to a hospital.

Worker Dies in Rainbow Dam Accident

September 7, GREAT FALLS, MT - 44 year old Taun Kultgen who grew up in Whitlash, MT was killed Tuesday in an accident at the Rainbow Dam construction project just outside of Great Falls. He died when he was crushed between two trucks that were driving by each other. According to Taun's younger brother, he just got engaged the Saturday before he died and planned to move to Havre, where his fiancee lives.


Firefighter, Village Worker Killed in Tragic Accident in Tarrytown

September 6, TARRYTOWN, NY - 48 year old Anthony Riggiero, a public works employee and 51 year old John Kelly, a volunteer firefighter died after apparently becoming overcome by fumes in a manhole. Public works was investigating a call about backed up sewage early Monday afternoon, said Blau. Officers were called to the scene and found the two men unconscious. The DPW worker had gone into a manhole to find the cause of the backup. Blau said the DPW worker apparently passed out and fell. Firefighter John Kelly went in after him and also apparently passed out. "The fire volunteers are not trained in close quarters rescue," Blau added, explaining why Kelly wasn't wearing protection. "I think he just wanted to save his friend," he sighed.


Three DynCorp Employees Killed in Afghanistan

September 6, Afghanistan - 51 year old William Allen of Munhall, PA, an electrician and 48 year old Alan Herzel of Milton, FL, a plumber foreman, and a third unidentified person were killed in an attack on a forward operating base in Howz-e-Madad, in Kandahar province, Afghanistan September 6. All were subcontractors working on the LOGCAP IV program. DynCorp International will defer to the third victim's employer to release any personal details.


Farmer Dies When Tractor Overturns

September 6, BATH, NY - 48 year old Mark E. Quinby died Monday after the tractor he was operating struck a round hay bale and rolled the tractor, pinning the man, according to the Steuben County Sheriff's Office. Quinby appeared to have sustained a head trauma but the cause of death will await autopsy results.


Murdered Security Guard Identified

September 5, CUMMING, GA - As reported in last week's Weekly Toll, a security guard for the Ingles Grocery Store, was killed during a robbery of the store. Authorities have identified 37 year old David Casto as the guard who was murdered. One of the suspects in the robbery has been identified as a store employee who was there when the robbery occurred.


Teen Dies in Tractor Accident

September 4, MINDEN, IA - 18 year old John Martin Dea died in an accident involving a farm tractor. Dea was driving a model 1466 International tractor and was having trouble getting the machine started. When he tried to roll start it by going down a hill, the tractor began to bounce, went out of control and rolled over on a terrace. Dea was thrown from the tractor during the incident.

September 3, SCHROEDER, MN - 47 year old Kyle Damberg, a longtime Minnesota Power worker, was found unresponsive Friday at a maintenance building under construction in Schroeder. The company says Damberg was checking on the structure being built near the Taconite Harbor Energy Center. His death is under investigation.


Border Patrol Agent Dies in Wreck

September 2, CASA GRANDE, AZ - 32 year old Michael V. Gallagher "died far too young". Gallagher died last week in a wreck while patrolling on the Tohono O'odham Nation. The driver of the vehicle that struck Gallagher has been charged with negligent homicide, criminal damage and driving under the influence, Tohono O'odham officials have said.

Robbers Kill Store Security Guard

September 5, CUMMING, GA - An Ingles Grocery Store security guard was shot and killed during a robbery of the store. The guard was one of three employees in the Canton Road grocery store at the time of the robbery. The other two, a man and woman, were not hurt. They were tied up with duct tape and left in the cooler, too.


Staunton Man Dies in Combine Accident

September 5, STAUNTON, IL - 50 year old Herbert F. Braundmeier died after being hit by a combine Sunday on a farm near Alhambra. Braundmeier was cutting corn in a field with a farmer, who was unloading corn from a combine into a grain truck. The farmer moved the combine into reverse to shift the load in the truck. The farmer did not know and could not see that Braundmeier had walked behind the combine to make an adjustment on the combine. When the combine backed up, a rear tire on the combine hit Braundmeier and he suffered a fatal head injury.


Tractor Trailer Driver Dies in Crash

September 4, SALINA, KS - 58 year old Henry Richardson of Little Rock, AR died when his semi left the road for unknown reasons according to a Kansas Highway Patrol report. The semi struck a guard rail before entering the ditch, the report said. Richardson was not wearing a seat belt, according to the report.


Man Dies After Farm Tractor Tips Over

September 4, VIGO, IN - 45 year old Shawn Biddle died when his farm tractor overturned and caught fire Thursday along Dallas Drive south of the city in Riley Township. Biddle was found trapped under the John Deere tractor in a ditch after the vehicle slid down a hill and overturned. The tractor caught fire a short time after overturning.


Train Accident Kills One at McDuffie Coal Terminal

September 4, MOBILE, AL - An investigation is under way after a Terminal Railroad employee was struck and killed by a train today at the McDuffie Coal Terminal. At the time of the accident, the man, in his 20's, was a ground person on a train crew. He had been in contact with a Terminal Railroad engineer in a locomotive that was moving train cars. When the engineer lost contact with the man, the train was stopped, port authority CEO Jimmy Lyons said. UPDATE: Authorities have identified the man killed as 27 year old Joshua Alan Roper.


UPS Cargo Plane Crashes, Kills Pilot and First Officer

September 3, DUBAI, UAE - 48 year old Pilot Doug Lampe of Louisville, KY and 38 year old First Officer Matthew Bell of Sanford, FL lost their lives in the crash of Flight 6 on Sept. 3, in Dubai, United Arab Emirates. The two were pilots for United Parcel Service. The Civil Aviation Authority of United Arab Emirates (UAE) issued a preliminary report on Saturday, indicating that the UPS cargo plane informed the tower that it was having radio problems, and the crew had detected smoke in the cockpit shortly after takeoff. The pilot and first officer were attempting to maintain altitude and return to Dubai's main airport when the plane went down.


Bread Deliveryman is Shot Dead

September 3, ELMHURST QUEENS, NY - 39 year old Jorge Martinez, who was driving his bread delivery route through Brooklyn on Friday night, was killed after bullets apparently fired by someone on a building’s rooftop pierced the ceiling of his work van and struck him in the head, the police said. It was not immediately clear if Mr. Martinez was the intended target or if the bullets were stray shots meant for someone, or something, else.


Electrical Worker Found Dead at Job

September 3, TWIN CITIES, MN - Robert J. Germundson of Coon Rapids didn't come home from work Friday night. His family reported him missing. Police found his body Saturday morning in the ceiling area of concourse E at the Lindbergh Terminal of the Twin Cities International Airport. He was doing electrical work in that area of the airport. September 3, CATHLAMET, OR - 51 year old Jerrold Samuel Florek was killed on the job Friday while logging across the river in Clatsop County. Florek was working in the Matoaka logging area Friday when he was struck by a falling log, according to the Clatsop County Sheriff's office.


Road Construction Worker Hit and Killed

September 3, YUMA, AZ - 29 year old Anthony Mosher of Queen's Creek was killed Thursday at a construction site near Dateland after being run over by a road-grading machine. Upon their arrival, deputies discovered a worker had sustained fatal injuries after having been struck by a Caterpillar road grader. The victim was conducting survey-related work in preparation for the construction of a solar power plant.


Cafeteria Cashier Dies in Robbery

September 3, KNOXVILLE, TN - 60 year old Daniel Charles Giles died after he was shot during a robbery at the S&S Cafeteria. Giles was shot once during a 7:48 p.m. Thursday robbery at the popular cafeteria at 4808 Kingston Pike. One of the customers told police that he saw the cashier and the robber in a "tug of war" over the cash register immediately before the shot was fired. Employees rushed to Giles' aid before paramedics arrived, but he was unconscious and unresponsive. Giles moved from Ohio just 3 months ago to start a new life.


Truck Crushes Longtime Business Owner

September 2, NEW PORT RICHIE, FL - 68 year old John David Parker, owner of J.D.Parker & Sons, who spent a lifetime in the tough garbage collection business, had been planning his retirement. This year, he figured. But while working just before midnight Thursday at the company compound he was underneath a 2001 International garbage truck, performing maintenance. For some reason, police said later, it rolled forward and crushed Parker. He died on the floor of the garage.


CT State Trooper Hit and Killed During Traffic Stop

September 2, ENFIELD, CT - State Trooper Kenneth Hall was killed Thursday afternoon when a pickup truck crashed into his parked cruiser on the right shoulder of Interstate 91, state police said. Trooper Hall had stopped a commercial truck. He was sitting in his cruiser to write a ticket when a pickup truck ran off the right side of the road and hit the cruiser.


Tractor Trailer Driver Dies in Crash

September 2, DENHAM SPRINGS, LA - A man died Thursday night after the tractor trailer he was driving on Interstate 12 near the 4-H Club Road overpass ran off the road and into a concrete culvert, a Denham Springs Police spokesman said. Witnesses told authorities the tractor trailer left the road and went into the culvert for no apparent reason.


Drugstore Employee Killed During Store Robbery

September 2, NORTH HIGHLANDS, CA - 27 year old Tania Guskaya was shot during a robbery at a Rexall Pharmacy in North Highlands Thursday afternoon,authorities say. The victims were behind the pharmacy counter when two individuals came in, one of them armed, and demanded drugs, said Sgt. Tim Curran with the Sacramento County Sheriff's Department. Guskaya's cousin George Shalnev said she was a devoted singer and piano player in her local church.


Man Crushed When Farm Equipment Falls on Him

September 2, BALLSTON SPA, NY - 71 year old David A. Lipscomb Sr. was killed when jacks holding up a trailer he was working under, which was missing a wheel, collapsed, causing the trailer to fall on his chest. The tow-behind trailer had three drums of asphalt on it and various smaller pieces of equipment. Lipscomb was transported to Saratoga Hospital by Community Emergency Corp. where he was pronounced dead.


Semi Driver Killed When Disabled Truck is Hit by Another

September 2, PALMER TWP, PA - 52 year old Mario Chacon was killed in a fiery collision between two tractor-trailers and a car Thursday morning. From the accident scene, it appeared as if one truck, possibly Chacon's, may have been stopped on the shoulder of the road when a second truck drove into the back of his trailer dislodging the rear axle of the trailer and sending the cab down an embankment. A set of axles was on the shoulder of the road just west of the two trailers and car. The car burned up in flames while under the rear of one of the truck trailers.


Factory Worker Trapped in Machinery Dies

September 1, ROCHELLE, IL - 25 year old Mackenzie Downing became trapped and was killed inside a piece of machinery at Transworld Plastic Films Inc. plant in Rochelle. Downing was freed from the machinery by emergency crews and some employees and was rushed to Rochelle Community Hospital where he died.


Maintenance Worker Killed in Accident With School Bus

September 1, VANCOUVER, WA - 51 year old Almaz Kucukovic, a maintenance worker, was killed by a school bus when it rolled over him at a bus barn in Battle Ground. Kucukovic was working on the bus when it started to roll. He was trying to climb aboard when he fell under the wheels. The Columbian reports he died immediately from head and internal injuries.


Local Electrician Killed on the Job

September 1, NASHVILLE, TN - 36 year old David Tittle was killed on the job Wednesday when he was shocked to death while working on a 110-volt electrical junction box at a Mooreland Drive home, according to a sheriff’s office report. Tittle was found unconscious and unresponsive. Another worker at the home told investigators he went up into the attic and attempted to turn Tittle over, but was also shocked by the electrical current upon touching him.


Deputy Trainee Dies After Completing a Run

September 1, DES MOINES, IA - 46 year old Cass Nemmers died Wednesday after a training run at the Iowa Law Enforcement Academy in Johnston, a spokeswoman said. Nemmers had just completed a 1½ mile run as part of a pre-academy physical to become a deputy, Rooker said. He was "having trouble standing, and having trouble breathing" before he collapsed. Nemmers worked the security desks at the Polk County Courthouse's entrances. He transferred to the job after a stint working in the old downtown jail, and was training to become a deputy.


Boyd Law Construction Worker Dies

September 1, IOWA CITY, IA - 49 year old Tom Fosdick of Cedar Rapids died after being injured Monday morning while working for Swanson Glass, a sub-contractor renovating the exterior of the Boyd building, according to a statement released by UI. Kevin Swanson, a representative for Swanson Glass, acknowledged early Wednesday that Fosdick was involved in a "serious" accident, but declined to elaborate. UPDATE: It is now being reported that Mr. Fosdick died from injuries sustained in a fall.


Robbery the Motive for Deliveryman's Death

September 1, HYDE PARK, MA - Robbery was the likely motive in the murder of a Domino’s Pizza delivery man who was stabbed to death on the job early this morning, said Boston Police Commissioner Ed Davis. The victim, a man in his 50s, was lured to a vacant home at 742 Hyde Park Avenue to deliver a pizza when he was killed some time after midnight, Davis said. He was pronounced dead at the scene. UPDATE: Authorities have identified the victim as 58 year old Richel Nova and three suspects in his murder have been arrested.


Alabama Farmer Dies After Tractor Overturns

September 1, DEKALB CO., AL - 81 year old Bobby Standridge, of the New Harmony community near Geraldine, died after becoming pinned underneath his tractor. Family members found Standridge's tractor overturned in a pond in the pasture behind the home. The family contacted emergency personnel and a next-door neighbor, who found Standridge's body in the water. Sheriff's deputies say Standridge was working near a pond on a tractor with a front-end loader when the tractor rolled over on top of Mr. Standridge. He became trapped when the tractor continued rolling into the pond.


Employee Found Dead at American Apparel

September 1, LOS ANGELES, CA - 44 year old Danarichie Lyndon Sindo, a data analyst, was found dead after he reported feeling sick. Details are still scarce, but LAPD spokesman Lt. Andrew Neiman confirmed to Gawker that Sindo reported feeling sick and vomiting before leaving to "get some air" on the roof. He "then went back inside and eventually went either into the bathroom or locker room and later was discovered deceased." According to a source, the body was discovered around 5 p.m. in one of the building's fifth-floor restrooms.


Worker Electrocuted While Inspecting Circuit

September 1, ORD, NE - Dean "Vegas" Lodewyck died after being electrocuted on the job Tuesday morning in Burwell. The Garfield County Attorney said Lodewyck died from injuries after coming into contact with a live electrical wire at the Loup Valley Alfalfa Plant. Lodewyck worked for Ricks Electric in Ord. Witnesses said he was troubleshooting a suspected faulty circuit at the time when he was electrocuted.


Worker Found Dead at Rice Storage Facility

September 1, STUTTGART, AR - 41 year old Monique Harper was found buried in a rice storage facility on Tuesday. The Stuttgart Fire Department received the call about 5:30 a.m. According to a police report Harper's co-workers became concerned about her whereabouts when they were unable to contact her by radio. That's when employees began searching for Harper.


Railroad Employee Killed on Northstar Rail Line

September 1, FRIDLEY, MN - 54 year old Andrew Kim Weaver was killed after being struck by a Northstar Commuter Rail train headed into the Twin Cities Wednesday morning. According to the Anoka County Sheriff's Office, Weaver was doing some testing on the rails at a double train track crossing at Egret Boulevard at the time. He was a BNSF Railroad employee. The sheriff's office said Weaver was near a couple of parked rail cars. He was talking to someone on a cell phone and had a stack of papers in his hands when he walked into the path of the oncoming Northstar train.


Landlord Killed in Possible Murder Suicide

August 31, TYLER, TX - Responding to a call of shots fired at a multi-apartment residence police discovered the body of 70 year old Socorro Nunez in one unit. They also found her tenant in another unit, wounded in what police are investigating as a murder and then an attempted suicide. Police are investigating what led to the shootings and whether financial problems were involved.


Miner Killed in Vehicle Accident

August 31, KNOX COUNTY, IN - 25 year old Brian W. Mason of Linton was killed Tuesday in a truck accident. Indiana State Police say Mason was operating an articulating dump truck at the Triad Mine in Freelandville when for unknown reasons, the truck drove off the roadway into a deep ravine. After being extricated from the vehicle, Mason was airlifted to Methodist Hospital in Indianapolis where he later died.


Truck Driver Hits Bison, Dies in Crash

August 31, ARAPAHOE COUNTY, CO - 74 year old Maynard Bontrager of South Hutchingson, KS died Tuesday morning after slamming into a bison on U.S. Highway 36, about 10 miles east of Byers. The 18-wheeler went off the road and flipped, crushing the cab. He was hauling salt, but it's not clear where the big rig was coming from or where it was headed.


Flight Crew for Medical Helicopter Dies in Crash

August 31, LITTLE ROCK, AR - Kenneth Robertson, Pilot, Kenneth Meyer, Jr., Flight Nurse and Gayla Gregory, Flight Paramedic, have all died when their medical helicopter crashed yesterday in the hills of Central Arkansas. The Air Evac Lifeteam helicopter went down near the Scotland community in Van Buren County at around 4 a.m., according to the company and the Federal Aviation Administration. They were trying to reach a person injured in a traffic accident.


Three Killed When Helicopter Crashes

August 31, BOISE, ID - 47 year old Larry Barrett and 34 year old Danielle Schiff, both Idaho Fish and Game Biologists, along with pilot, 43 year old Perry J. Krinitt of Belgrade, MT, were killed when suspected mechanical problems caused the helicopter they were in to crash in the small town of Kamiah, ID. The Department of Fish and Game had chartered the aircraft from Clarkston, Wash.-based Leading Edge Aviation to count salmon spawning nests on the Selway River.


Chain Reaction With Tractor Trailer Causes Farmer's Death

August 31, MABEN, MS - 41 year old Sammie Jones of Maben was killed in a highway accident. A patrol spokesperson says a tractor was traveling in the right lane and was being followed by a pickup. According to the MHP, an 18-wheeler hit the back of the pickup, knocking it into the tractor, killing the driver of the tractor. Maben was the driver of the tractor.


Farmer Killed in Tractor Mishap

August 31, SCOTT CO, MO - 77 year old Bobby Wolf of Chaffee died Tuesday in a farm tractor accident. Scott County Coroner Scott Amick says Wolf was on an incline when his tractor hit a stump. Amick says that caused the tractor to pop-up and roll over. Wolf was pinned underneath the tractor and died.


Man Killed In Crane Accident

August 31, OKLAHOMA, OK - Michael Parsons was killed Tuesday on the Chesapeake Energy Campus in an accident involving a crane. Police said it appeared the accident happened as the crane was being shut down for the night when something fell off of a crane at the construction site.


Tractor Trailer Driver Killed in Crash

August 31, CAROLINE CO, VA - A driver of a tractor trailer was killed in a fiery crash on I-95 South in Caroline County. The tractor trailer was traveling southbound on I-95 when its front left tire blew, causing it to run off the right side of the roadway. The tractor trailer struck approximately 650 feet of guardrail and struck several trees before overturning onto its right side and catching on fire.


Man Dies When Tractor Rolls Down Embankment

August 31, MARIETTA, WI - 66 year old Jeffrey C. Glasbrenner of Boscobel died Monday afternoon in a tractor accident on Meadowbrook Road in Marietta Township. Glasbrenner was operating a tractor on private property and was on a logging road. According to the Crawford County Sheriff’s Department, the tractor slipped off the edge, went down a steep embankment, rolled and pinned Glasbrenner underneath. He was pronounced dead at the scene by Crawford County Coroner Joe Morovits.


Man Crushed When Forklift Failed

August 31, PATERSON, NJ - an unidentified worker was killed when he was crushed under a car. The employee died and another was seriously injured when a forklift collapsed as the men were working on an auto at Beji Dismantling. Police said "the forklift gave way".


Pilot/Mechanic Killed in Plane Crash

August 30, BOZEMAN, MT - 34 year old Jarrod Dearden, of Ogden, UT, died when his plane went down in Gallatin County, MT. Dearden was a pilot and mechanic who borrowed his employers plane to fly to Bozeman to work on another pilot's plane. An investigation continues into the accident.


Man Killed Cutting Lawn at County Park

August 30 - NEW CASTLE, DE - 68 year old James Hagerty of the Sherwood Park community in Milltown was killed this afternoon when the mower he was riding on toppled down an embankment into a creek bed, pinning him underneath it, officials said. According to police, Hagerty was preparing to cut the grass on a hill near a pond when for an unknown reason his lawn tractor slid back down the hill. The tractor then went over an embankment and overturned, trapping Hagerty under the equipment, Weglarz said.


Truck Driver Dies in Collision With Train

August 30, REMSEN, IA - 37 year old Richard Martin died when his dump truck collided with a train. Investigators say Martin was driving a dump truck, which was owned by Walker Excavating of Le Mars. Authorities say Martin failed to yield to an oncoming train. The locomotive hit the dump truck on the driver's side. Martin was then ejected.


Truck Rollover Leaves One Dead

August 30, MATTAWAMKEAG, ME - 50 year old Donald Pepin was killed when the chip truck he was driving rolled over and hit a guardrail just after 11:00 a.m. State Police say witnesses reported seeing Pepin pass two-vehicles before he returned to the travel lane and lost control of the truck. Authorities are not sure what caused the accident.


Man Killed in Industrial Accident

August 30, INTERNATIONAL FALLS, MN - 51 year old Nikolas Charles Kochaniuk died Monday of what appears to be an industrial accident at Boise Paper in International Falls. “They were doing some special maintenance work on a brick wall in a building where a kiln is located,” Boise Paper spokesman Bob Anderson said this morning.


Farm Worker Dies When Pinned Beneath Mower

August 30, CLEVELAND, OH - 53 year old Genovevo "Johnny" Garcia died Monday after being pinned underwater by a riding lawnmower for 26 minutes. William “Bill” Russell, who owns the farm on Phifer Road, said Garcia had been mowing the grass along the edge of a pond on the property when the mower tipped over and rolled into the water on top of Garcia. Russell said Garcia had worked for him a few hours every morning for the past three years and that he lived with one of his friends down the road. “He was a real hard worker. He was honest,” Russell said. “When we’d go out of town, he’d watch over.”


Semi Driver Dies in Crash

August 30, MADISONVILLE, TX - Andres Hinojosa died Monday when the tractor trailer he was driving overturned. Hinojosa was eastbound in a 2007 Peterbilt tractor-trailer rig, hauling a load of gravel, when he veered to the right, over-corrected and then went into a broadside skid, causing gravel in the rig to spill on the roadway, according to DPS reports. The tractor-trailer rig then slid off the roadway, stopping on its side. Investigators speculated he swerved to avoid a cow in the roadway.


Worker Dies From 6 Story Fall

August 30, MCKEESPORT, PA - 29 year old Robert Barncord died after falling from the roof of an apartment building. Barncord was working on the roof of the six-story apartment building on Royal Oak Drive about 2 p.m. when the incident occurred, the spokesman said. Investigators are trying to determine whether a worker jumped or fell to his death.


County Employee Killed Cutting Trees

August 30, WOOD COUNTY, TX - 31 year old Matthew Don Crowell was killed while cutting a tree from the county road right of way. Crowell, who worked for Wood County Road and Bridge Precinct #2, became trapped between two very large limbs. The preliminary cause of death has been ruled blunt force trauma pending the results of the Autopsy.


Man Killed Working to Clear Farm Field

August 30, CLAREMONT, NH - 54 year old David LaValley, of Claremont, was using a chain hooked to a Jeep to pry loose an antique horse-drawn sickle from some trees that had grown up around it in what once was a farm pasture died Monday after the old equipment suddenly broke free and struck him in the head. LaValley died at Dartmouth Hitchcock Medical Center in Lebanon, N.H., shortly after he was airlifted there, Vermont State Police Detective Sgt. John Hagen reported Tuesday.


Police Officers Gunned Down in Alaska

August 28, HOONAH, AK - 32 year old Anthony "Tony" Wallace was on a ride along with his mother, her first trip to visit him in Alaska, when he was shot by a lone gunman. Wallace's partner, 39 year old Matthew Tokuoka was off duty at the time but was talking with Wallace when, while trying to protect Wallace after he was shot, was also shot by the same gunman. Tokuoka was in his vehicle, with his family, when the shootings occurred. The suspect, who had been arrested by the officers the previous year, was finally arrested. It is worth noting that Wallace was legally deaf, yet still a good officer.


Trucker Killed in Highway Crash

August 27, SAN LEANDRO, CA - 65 year old Manuel Suarez Palma was killed in a solo crash on Highway 101 near San Benito County on Friday. The driver was northbound in a 2005 International Tractor at an unknown speed when he made an unsafe movement to the right, with the vehicle to turning off the east edge of the road and coming in contact with the guard rail, according to a statement from the CHP.


Worker Crushed at Recycling Facility

August 27, SALISBURY, NC - 40 year old Maurice Jay Alexander died Friday at Presbyterian Hospital, the day after an 800-pound bale of cardboard fell off a forklift and crushed him. Police said Alexander was moving the cardboard bale on a forklift and stepped off to adjust something. As he stood in front of the forklift, the bale fell off the forks and onto him, pinning him against the forklift and crushing him. Medical responders found Alexander unresponsive. They revived him with CPR, but he died the next day in the intensive care unit at Presbyterian.


Driver Dies in Truck Rollover

August 27, RAEVILLE, NE - 46 year old Gerald Lesko of Brohead, WI was killed in a roll-over accident last Friday, Aug. 27, on 110th Street east of Raeville, NE. Lesko had delivered equipment to the Laredo Ridge Wind Farm and was driving a 1988 Peterbilt tractor-trailer westbound on 110th Street when he apparently lost control. The vehicle flipped into the south ditch. Lesko was apparently not wearing a seatbelt at the time of the accident, the Sheriff’s Department reported.


Man Dies in Oil Battery Explosion

August 24, WILSON, KS - 48 year old Randy "Rudy" Wayne Ehrlich of Russell died Fwhen the oil tank battery he was working on exploded east of Wilson. Ellsworth County Sheriff Tracy L. Ploutz says the crew was cleaning the tank battery, which had been struck by lightning and caught fire earlier Friday. Ploutz says the men were using cutting torches, but the specific cause of the explosion may never be known. The Hays Daily News reports that two other employees of Western Sales were uninjured but Ehrlich's body was found near the rubble of the exploded tank.


Tower Accident Causes Death

August 19, FARMERSVILLE, TX - 30 year old Oscar Maza-Garcia of New Mexico has died after falling 160 feet from a Caddo Basin Special Utility District water tower. The accident occurred about 7:30 a.m. Aug. 13, according to Farmersville Fire Chief Kim Morris. Garcia was painting the tower and was attached via a rope, according to Farmersville Police Department reports. He fell from the tower, hitting the concrete below. Though he did not work for the Caddo Basin Special Utility District, he was part of a group that had been subcontracted out to paint the tower.
