Weekly Toll: Death in the American Workplace

Construction Worker Dies From Fall

November 24, FORT LEE, VA - 40 year old Clinton Duke Clark, an employee for Richmond based Manning Construction died from injuries he received when he fell from scaffolding at a construction site at Fort Lee. Clark was working as a laborer on a five-story barracks building on post when he fell approximately 45 feet. Preliminary reports indicate that he was wearing a safety harness at the time of the accident.


Farmer Crushed Under Tractor

November 24, CURRITUCK COUNTY, NC - 65 year old Jimmy Lyons of Pasquotank County, a mechanic who worked for Morgan Famrs, was working on a tractor around 9:30 a.m. when investigators said the tractor accidentally backed over him.


Worker Killed in Industrial Accident

November 23, GHENT, KY - 48 year old Robin Kelley of Madison, IN was killed and four others were injured on Tuesday when a large tire on a piece of equipment exploded at an industrial plant in Northern Kentucky. The five workers were attempting to change a tire on a large piece of equipment when it exploded.


Owner Found Dead in Bakery

November 23, BAKERSFIELD, CA - 49 year old Juanita Alvarado Francisco, owner of Panaderia Bakery, was found dead from trauma in a back storage room of the store Tuesday. The person who called deputies said the business appeared to be ransacked and the woman owner could not be found. Her death has been ruled a homicide.


Sheriff Deputy Dies in Shootout With Car Theft Suspect

November 23, WELD COUNTY, CO - Weld County Sheriff Deputy Sam Brownlee was killed in a shootout with a man who was suspected of stealing a car. The suspect lead police on a long car chase which ended in a neighborhood in Evans where the officer and the suspect were shot. Brownlee was transported to a hospital where he later died.


Worker Dies When Dump Truck Overturns

November 23, OVERTON, PA - 72 year old Ernest Compton of New Milford was killed Tuesday when his dump truck loaded with stone overturned. Troopers said he lost control of the dump truck he was driving along Overton Road west of New Albany after it went off the road and began to sink on the side of the road. The truck went down an embankment and hit some trees and a fence before it overturned.


Mine Worker Killed in Crash

November 23, FRANKFORT, KY - 32 year old Rhett Mosley of Perry County was killed early Tuesday morning after he lost control of a coal truck on site at the Rex Coal Strip Mine #1. According to a preliminary report Mosley either attempted to jump out or was thrown from the vehicle, and the truck struck an embankment and overturned onto him.


Ski Patrol Director Dies in Avalanche

November 22, DENVER, CO - Scott Kay, Colorado's Wolf Creek Ski area's ski-patrol director was killed Monday in an inbounds avalanche. Resort co-owner and spokeswoman Roseanne Pitcher declined comment beyond a statement from the resort that said the patrol director, "while working to protect others, was caught in an avalanche which, to our great sadness, he did not survive." Kay joins an elite group of ski-patrol heroes who sacrificed their lives while safeguarding the slopes.


Waste Management Worker Killed on the Job

November 22, MOUNTAIN BROOK, AL - 47 year old Edward Miller Chaffin was killed when he was hit by a brush pick-up truck. Chaffin and another co-worker were using a hose to suck up leaves into the truck, and at some point when they moved the truck, they ended up running over one of the employees." An investigation is underway to determine how the incident happened.


Maintenance Worker Shot and Killed

November 22, MIAMI BEACH, FL - 60 year old Alberto Gomez was shot and killed by a tenant who was being served an eviction notice at a condo complex. Gomez was accompanying Miami-Dade police in serving the notice and an officer was wounded in the exchange of gunfire. When the tenant did not answer the officer at the door, Gomez proceeded to work on the door lock when the tenant suddenly opened the door and started shooting.


Professional Wrestler Working Security Shot and Killed at Nightclub

November 21, ATLANTA, GA - 33 year old Chris Long was shot and killed by unruly patrons that he and two other bouncers had tossed out of the DeKalb County's Shooter Alley. Police didn't release details about the incident, but one of Long's friends said he thinks the men returned later and targeted Long in the shooting.


Sanitation Worker Killed in Accident

November 20, BUCKHANNON, WV - 40 year old Joseph Robinson, who was struck by a garbage truck on November 18, has died. He was an employee for the City of Buckhannon. The driver of the garbage truck was attempting to back up, and although he saw Robinson in his rear view mirror, Robinson was hit and crushed under the rear tires. Robinson was transported from the scene to St. Joseph's Hospital and then onto Ruby Memorial Hospital where he later died.


Fireworks Company Employee Dies in Accident

November 20, UTICA, NY - 56 year old John Hatter of Canastota died after being injured while setting off fireworks at a city Christmas festival. Hatter was injured at the city's Christmas on Main Street festival Saturday evening. He was taken to St. Elizabeth Medical Center around 5:45 p.m. and later airlifted to Albany Medical Center, where he died.


Taxi Driver Found Dead Inside Cab

November 20, POWDER SPRINGS, GA - 23 year old Ricardo Peter Francois was found shot to death at an apartment complex Saturday night. A couple who resides at the complex saw the taxi with the engine running about 6 hours before reporting it to the police after they returned home and the taxi had not moved. An investigation is on-going.


Phoenix Cab Driver Shot to Death

November 19, PHOENIX, AZ - 37 year old Brian Mann, a driver for Discount Cab, was shot to death by a female passenger. According to witnesses, the cab left Highland Park apartments around 27th and Glendale avenues about 6 a.m. Friday, then pulled over in the middle of the street and fell out the driver's side door, police said. The woman, who was later arrested, got out of the back of the cab and calmly walked back to the apartment complex.


Highway Department Worker Killed in Accident

November 19, RUSK CO, WI - 45 year old Michael Bigalk of Catawba was killed Friday while working at the Rusk County Highway Shop. Bigalk was apparently servicing a dump truck and salt spreader and became entangled in the auger on the salt spreader, according to the Rusk County Sheriff's Department. He was pronounced dead at the scene.


Fire Captain Dies in Line of Duty

November 19, GRAYSON, KY - 86 year old Captain Worne Hall, the captain and creator of the Hitchins Volunteer Fire Department died in the the line of duty on Friday from a heart attack. He died while helping to establish a landing zone for an EMS helicopter. Capt. Hall served the department for 34 years.


Paving Company Trucker Dies in Crash

November 19, WILKESBORO, NC - 62 year old Rex Stone, a truck driver for Carl Rose & Sons, died Friday night when his tractor trailer ran off US 421 in Wilkesboro. Police say witnesses said the truck ran off 421 traveling west, traveled off the road about 150 feet down an embankment 30 to 40 feet high. The driver was dead when emergency units arrived, but it was not clear whether he died of a medical condition or from injuries sustained in the wreck.


State Park Workers Dies in Crash, Fire

November 16, DAVIS, WV - 68 year old Robert DeMent was found dead in a burning vehicle at Blackwater Falls State Park. Troopers say DeMent was finishing his shift as a security guard when a visitor reported a vehicle on fire near a park bridge Monday. Lt. J.A. Wise in Parsons says it appears DeMent tried to move the van from the ditch.


Corrections Officer Dies During Training Session

November 16, WINDHAM, ME - 58 year old John Paskewicz, a guard at the Maine Correctional Center who died of a heart attack Tuesday during a training exercise with his son and other officers. Paskewicz had just finished a course and was joking with his son and other officers when he collapsed. A couple of officers gave him CPR before paramedics arrived. John P. Paskewicz, who worked with his father for the past nine years, assisted the officers.

Lunch Truck Owner Hit by Car

November 18, NEWARK, NJ - Jose Goncalves, owner of Joe's Cafe, a lunch truck, was standing with his wife outside of his lunch truck near Park and 7th St. when a speeding BMW slammed into him Thursday morning. He was taken to University Hospital where doctors pronounced him dead just before 9 a.m., said Katherine Carter, spokeswoman for the Essex County Prosecutor's Office.


Missouri Man Hit and Killed on CA Job Site

November 17, MADERA, CA - an unidentified Missouri man, who was part of an excavating crew, was killed Wednesday in Madera when he was accidentally run over by a tractor, police said. The worker was standing near a large front-loading tractor about 12:45 p.m. when the operator put the tractor in reverse and ran over the man. He died of his injuries a short time later, police said.


Roofer Setting up Equipment Falls From Roof

November 17, SOUTH HADLEY, MA - 42 year old Brian LaBonte, a roofer setting up a chute to remove waste shingles, was killed when he lost his balance and fell from the roof of a commercial building at Village Commons on College Street, an official said. LaBonte was working for a company called J.D. Rivet Roofing of Springfield. OSHA and other officials are investigating.


Semi Driver Killed in Accident With Wrong Way Driver

November 17, CLINTON COUNTY, IL - 29 year old Jessica M. Williams of Old Town, FL was killed early Wednesday in a wrong-way crash on Interstate 64 at the Clinton County/Washington County line, Illinois State Police said. Another driver, going the wrong way on the interstate, crashed head on with the semi sending it over an embankment where it caught on fire. Williams husband, who was riding in the truck, was taken to an area hospital with serious burns.


Flight Instructor and Student Dies in Plane Crash

November 17, PAYSON, UT - 34 year old Jamie Bennee, a certified flight instructor for the Utah Valley University aviation training program, died when the plane he and a student pilot were in appeared to stall just before it plunged towards the ground. They said the engine restarted about 75 yards above the ground and the pilot appeared to be trying to pull the plane out of the plunge, but it was too late. The cause of the crash is under investigation.


Truck Driver Looses Control and is Killed in Crash

November 16, LA GRANGE, KY - 46 year old Mark Miller died at University Hospital Tuesday morning after he was ejected from his work truck during an accident in Pleasureville, Jefferson County deputy coroner Larry Carroll said. He said some tires on Miller’s truck dropped off the road, and Miller over-corrected trying to get his truck back on the road. Kentucky State Police in Campbellsburg are investigating.


Construction Worker Crushed Under Equipment

November 16, KEARNEYSVILLE, WV - 42 year old Doug J. Lippincott of Pittsburg, PA was killed in a drilling rig accident Tuesday at the IRS Enterprise Computing Center in Kearneysville when an industrial-sized drill came loose and toppled onto him, officials said. Officials don't yet know why the drilling rig broke free from the main support beams of the truck and toppled over. Lippincott was working in the area at the time and sustained fatal injuries when he became pinned between the drilling rig and the ground.


Hospital Employee Crushed Under Materials

November 16, LIBERTYVILLE, IL - 26 year old Raymond Zalewa of Prospect Heights, an employee in the environmental services department of the Advocate Condell Medical Center, was killed when several sheets of heavier than normal lead-lined drywall fell on top of him in a hospital storage area, authorities said Wednesday. The lead-lined drywall sheets were used during renovation of an X-ray room. The incident is being investigated by OSHA.


Houston Tow Driver Killed

November 16, HOUSTON, TX - 38 year old George Soriano was killed Tuesday afternoon while towing a FedEx truck back to its distribution center, according to a news report. Soriano was killed at the building while towing the FedEx truck, although the details of what exactly happened are still being investigated. Officials say it appears to be a tragic accident, the report states. Soriano had just changed employment to another towing company because he was concerned about towing cars on busy highways, according to the report.


Road Worker Hit and Killed

November 16, BUFFALO, NY - 27 year old David Baldy of Hamburg was struck and killed by a truck along that stretch of highway just after 5 AM. Baldy was standing on the right side of the roadway when the accident happened. The work crew was using two arrow board to close the right lane, when a tanker hauling propane struck the paint truck.


Store Clerk Killed by Young Robber

November 15, ALGONA, MN - 47 year old Vicky Bowman-Hall, an employee of Crossroads Gas Station and Convenience Store and mother of 11, was murdered during a robbery by a 17 year old boy. After Bowman-Hall surrendered cash to the robber, he shot her, authorities allege. She later died at Kossuth Regional Health Center in Algona. She leaves behind a husband and children who range in age from 4 to 27. The young gunman is in custody.


Another Store Clerk Killed by Same Young Robber

November 15, HUMBOLT, MN - 61 year old Sheila Myers, an employee of the Kum and Go Convenience Store was murdered during a robbery by the same youth who killed another store clerk only hours earlier in Algona, MN. Myers also handed over money to the robber who shot and killed her. Myers died at the store where customers often stopped by to chat with the upbeat clerk.


Fish and Game Manager Found Dead at Lake

November 15, CASSCOE, AR - 63 year old Don McSwain, manager of the Arkansas Fish and Game Commission, was found dead in Cook's Lake near Casscoe in Arkansas County, according to Arkansas County Sheriff Allen Cheek. Cheek said McSwain was found in the water near his boat with his truck parked and running nearby. A cause of death has not been determined at this time. Cheek said McSwain went to the area to load a boat.


Icy Road Causes Semi Crash

November 15, SHAKOPEE, MN - 32 year old Dana Ryther of Elk River died after he lost control on an icy highway ramp Monday near the Shakopee-Savage border, according to the Minnesota State Patrol. After hitting the side of the ramp, the tractor and trailer separated, leaving the trailer to block one lane of traffic while the cab rolled into the ditch, Savage Assistant Fire Chief Bruce Von Bank said. Ryther was crushed in the tractor and pronounced dead at the scene.


TX DPS Officer Killed in Wreck

November 15, AUSTIN, TX - Officer Jonathan McDonald was killed in a car wreck near Post late Monday night. McDonald was responding to a wreck in Garza County around 10:30 p.m. when he crashed his vehicle into a culvert about 2.5 miles south of Post. He is survived by his wife and a 6-month-old daughter.


Injured Corrections Officer Has Died

November 15, MEADVILLE, PA - 49 year old Gary M. Chapin of Conneaut Lake, a corrections officer at the Crawford County jail, who allegedly was assaulted at the jail last month, has died. Chapin was assaulted by an inmate on October 13. An autopsy is scheduled this afternoon in Erie to determine the cause and manner of death, Crawford County Coroner McHenry said. The inmate has been charged in Chapin's assault.


Tree Trimmer Dies in Chipper Accident

November 15, WALNUT CREEK, CA - 33 year old Antonio Barajas died Monday when he was loading tree debris into a wood chipper and a piece of rope also entered the machine, entangling the man and dragging him into the mouth of the chipper, Concord police Lt. Andrew Gartner said. "He wasn't actually sent through the chipper, he was in the opening of it," Gartner said. The Contra Costa Times reported that the accident happened while Barajas was working in a team of about eight workers.


Caltrans Worker Hit and Killed Along Highway

November 14, CHICO, CA - 57 year old Gary Wayne Smith, a veteran Caltrans employee who was due to retire next month, was hit and killed along Highway 99. Smith was rerouting traffic away from a fatal accident site that occurred earlier Sunday morning when he was hit and killed by a driver who did not heed the detour. The driver of the car who hit Smith was later arrested and booked into jail.


Truck Driver Dies After Losing Control of Rig

November 14, LUCILE, ID - 61 year old Harvey Atagi of Boise died when he lost control of the semi he was driving early Sunday morning on U.S. 95 near Lucile. Atagi was driving southbound at mile post 207 around 6:30 a.m. when he swerved for unknown reasons and over-corrected. He crossed the highway and crashed into the rocks on the northbound side of the road.


Construction Worker Dies From Fall

November 13, WEST CALDWELL, NJ - 43 year old Richard Peter DeMary of Rockaway was working on the roof of an office building Saturday in West Caldwell when he fell 35-feet through a skylight, the township’s police chief said. Responding officers found DeMary in a boiler room and helped others perform CPR until the West Essex First Aid Squad arrived. OSHA is investigating the incident.


Young Firefighter Killed in South Carolina

November 13, COLUMBIA, SC - 23 year old Chance Zobel was killed in the line of duty Saturday afternoon after being hit by a car on Interstate 20. Zobel and another firefighter were putting out a grass fire in the median of the interstate when they were hit by a Chevy Impala that had been rear-ended by a van. Both firefighters were rushed to the hospital where Zobel died. He was set to be married to his fiancee in April.


Construction Accident Kills Worker

November 13, SALEM, MA - 39 year old Juan Alvarado of Lynn died from crushing injuries Saturday when a 60 pound bag of gravel broke free, fell three stories and hit him on the head. The bag was being hoisted up when it somehow broke free and fell. Alvarado was part of a crew of construction workers repairing the building's roof. The incident remains under the investigation of Salem PD and OSHA.


Tractor Trailer Driver Dies in Crash

November 13, MIDDLE PAXTON, PA - 37 year old Jose Lopez-Vazquez of Wilmington, DE died when the tractor-trailer he was driving went off the highway near Mountain Road and rolled about 100 yards hitting trees and telephone poles, troopers said. Lopez-Vazquez was a driver for Chester County-based Manfredi Mushroom Inc.


Store Clerk Shot & Killed for $11.

November 12, SPENCER, NC - 20 year old Dee Rios managed to hit the store's panic button and tell police the suspect was a black male before she was taken to the hospital where she later died of a gunshot wound to the neck. The man reportedly only got away with about $11. Rios was shot and killed early Friday morning at the convenience store Latin Mix (formerly Real's Varieties) on North Salisbury Avenue.


Worker Dies After Being Knocked Into River

November 12, MEMPHIS, TN - 36 year old Rene Mendez died Friday after falling near the Thomas Street bridge over the Wolf River. Memphis police spokesman Vince Higgins said a crane malfunctioned and knocked Mendez into water behind a coffer dam built as part of construction work being done at the river bridge.


Worker Dies After Falling From Height

November 12, PITTSFIELD, IL - 41 year old Paul E. Ruble, of rural Nebo, died after a fall from a cat walk early Friday at Pike Feeds in Pittsfield. Authorities said Ruble fell while loading a tandem truck with feed. The accident remained under investigation by the Pike County Sheriff’s Department.


Worker Dies in Golf Maintenance Accident

November 12, BELFAIR, SC - 24 year old Joseph Belesky of Bluffton died Friday morning when he drove a utility vehicle under a parked semitrailer used to haul pine straw, authorities said. "He was operating a John Deere Gator and ran up underneath a parked, 40-foot box trailer," Beaufort County Coroner Ed Allen said. "The vehicle was pinned under the trailer.


School Bus Driver Dies at the Wheel

November 12, AUSTINTOWN, OH - 62 year old John "Jack" Cabot of Canfield, died at the wheel Friday morning as he was taking 51 high school students to the Mahoning County Career & Technical Center. The bus steered off the road, running over a sign and into a tree outside of 5938 Kirk Road, before it stopped. The nine students aboard "an unbelievable job organizing themselves and getting off the bus" said the school superintendent. They were taken to area hospitals with minor injuries.


Game Officer Shot Trying to Stop Poacher

November 11, GETTYSBURG, PA - 31 year old David L. Grove, a Pennsylvania Wildlife Conservation officer, was killed Thursday in a shootout with a poacher. Grove was patrolling late Thursday when he heard shots and noticed a pickup truck on the side of a two-lane road that had a spotlight focused on a field just south of Gettysburg. Grove ordered the driver from the truck and tried to handcuff him, authorities said but the driver, a convicted felon, pulled a gun and opened fire. The officer shot back but was killed. The suspect has been arrested and charged with murder.


Man Hit by Airport Tram Identified

October, HOUSTON, TX - in our blog for Oct 26 we reported an unidentified individual that was hit and killed by an airport tram in Houston. 57 year old Travis Turner was the person who was killed in this accident. We regret not seeing an alert with an update at the time this happened.

California Police Officer Dies in Shoot Out

November 7, RIVERSIDE, CA - Officer Ryan Bonaminio, a four-year veteran of the department and an Iraq war veteran, was shot by a trucker who the officer had tried to pull over after a hit and run incident. The trucker eventually pulled over in front of nearby Fairmount Park where he got out of the vehicle and fled on foot with Bonaminio running after him. The two engaged in a gunfight and Bonaminio was hit at least once, police said. When other officers arrived at the scene, they found Bonaminio on the ground with a gunshot injury.


School Van Driver Dies in Crash

November 5, MONROEVILLE, PA - 77 year old Russell Bangham died in a crash that also injured two children he was transporting. He had just picked up two sisters when his van struck a barrier on the side of the road, then a light pole near 14th Street, rolling the vehicle twice, Pittsburgh police said. Bangham loved his job and everyone liked him. Bangham's age did not affect his ability to drive, and he was current in meeting state and federal regulations said his boss.


Flagman Killed in Work Zone

November 5, YORK, PA - 85 year old George Waltermyer, a flagman, died as a result of being struck by a vehicle in a work zone. Waltemyer was seriously injured and suffered cardiac arrest. He was pronounced dead after being transported to York Hospital.


Fast Food Restaurant Worker Shot During Robbery

November 4, MOBILE, AL - 21 year old Russell Lee Allen was shot and killed during a robbery at the Domino's Pizza. The suspect walked into the store, demanded money and shot Allen. He was taken to USA Medical Center where he later died. Two other fast food robberies occurred on the same night.


Service Worker Aiding Trucker Hit and Killed

November 4, HEBRON, IN - 63 year old Philip Amsden was killed by a drunk driver in a chain reaction crash while helping a trucker who had a flat tire. Amsden parked his truck behind the semi and he was between both vehicles when another driver struck the truck's left rear side, pushing it into the rear of the semi crushing and killing Amsden.


Worker Crushed by Industrial Machine

November 4, ATLANTA, GA - 56 year old John Henry Brown of Marietta died after being trapped under heavy equipment at the Forsyth County Water Treatment Plant off Antioch Road. “He was not operating any machinery,” a sheriff's spokesman said. “He was standing beside some machinery that was operational and it overturned on him.” It is not clear how Brown became trapped under the machinery, which is similar to a forklift.


Truck Driver Dies in Collision at CA Pier

November 4, LONG BEACH, CA - 55 year old Bert Tufele of Carson, a dockworker at Pier A, Port of Long Beach, was killed after his truck collided with another inside a marine terminal. Investigators say Tufele failed to yield to another truck and the force of the collision "caused the the victim to be ejected from his vehicle and under the rear wheels" of his own rig, where he suffered fatal injuries, said Long Beach police Detective Sirilo Garcia.


CHP Cadet Dies After Falling Ill During Training

November 3, SACRAMENTO, CA - 24 year old Randy Atchison of Ceres died Wednesday after falling ill during physical training last week. Atchison was participating in the first physical fitness challenge of the 27-week training but after completing all of the challenges, complained of feeling ill afterward. He was taken by ambulance to Sutter General Hospital and then was transferred Saturday to Sutter Memorial Hospital, where he died at 1:29 a.m. Wednesday.


Man Killed at Recycling Facility

November 3, RALEIGH, NC - 39 year old Simon Martinez, a contract worker for Sonoco Recycling, was killed in an accident involving a forklift. No one witnessed the accident although Martinez had been operating a forklift but "he was not on the forklift when the accident occurred," a company spokesman said. This is the second death at the Sonoco facility in a little more than two months.


Highway Construction Worker Killed

November 3, ELLISVILLE, MS - 46 year old Medgar Evers, a construction crew member working on the site of a highway improvement project, was killed when he was run over by a forklift. It happened Wednesday morning along Interstate20 where crews are rebuilding the bridge over Highway 51. A spokeswoman for the Mississippi Department of Transportation would only say that the accident is under investigation.


Construction Worker Killed in Wall Collapse

November 3, WEST TRAVERSE, MI - 50 year old Mark Woods of Pellston is dead and another hurt after a wall of a basement area of a cottage they were working on collapsed. The cottage was being renovated by Collie Construction when the wall collapsed, trapping the two men inside. The accident remains under investigation by MIOSHA.


Cleaning Worker Found Dead

November 3, KALAMAZOO, MI - an unidentified 57 year old cleaning man was found dead this morning in a restroom at the Kalamazoo Chamber of Commerce. Police said it appears the man was in the process of cleaning when he fell and struck his head. He was pronounced dead at the scene Wednesday.


Farmer Killed After Being Hit by Train

November 2, SMITHS GROVE, KY - 49 year old Jimmy Wilson was killed when the tractor he was driving was struck by a train. According to Warren County Deputy Coroner Harold Sanson, Wilson was pulling a piece of machinery behind his tractor, was talking on his cell phone and might have had the radio on inside his cab as the tractor crossed the railroad tracks at about 10 a.m. Tuesday. “He probably just didn’t hear the train.”


Young Store Clerk Shot and Killed During Robbery Attempt

November 2, OLAR, SC - 19 year old Alicia Dyches was shot and killed at the Tiger Express Convenience Store where she worked. A 20-year-old Bamberg County man has been charged in a fatal robbery attempt that Sheriff Ed Darnell said was caused by pure "meanness." Two other employees who were also shot were taken to the hospital in stable condition.


Machine Operators Killed in Industrial Accident

November 2, CAROL STREAM, IL - 67 year old Otto C. Nowack has died from injuries he suffered when the excavating machine he was operating rolled over on him. Nowack, a hydraulic excavator, was working on a site in Milan. He died of head and chest injuries, the coroner's office said.


Farmer Killed in Tractor Accident

November 2, POLKTON, MI - 75 year old Roger Goeree was killed in an incident involving his farm tractor. Goeree was planning to pump liquid from the spreader when he went to start the tractor around 2 p.m., apparently not realizing it was in gear. The tractor lurched into motion, throwing the man from the side of the tractor and into its path, police said.


Two Northern Regional Jail Employees Killed In Crash

November 1, WEST VIRGINIA - 31 year old Valarie Fortesque of Wheeling, an office assistant for NCC, and 24 year old John Mayfield of New Martinsville, a corrections officer, were on their way to the West Virginia Corrections Academy in Glenville for three weeks when there were killed in an accident. Investigators said Mayfield pulled their vehicle into the path of an oncoming rock truck around 9:52 a.m. A gas line truck was also involved in the crash. Both were pronounced dead at the scene. Fortesque was a single mother of a 4-year-old daughter and Mayfield leaves behind a wife and 18-month-old baby.


Young Tow Truck Driver Found Shot Dead Inside Truck

November 1, LINTHICUM, MD - 23 year old Andrew Joyce, a tow truck driver for 2 months, was found dead of a gunshot wound inside his truck in the 500 block of Mosher St. about 2:30 a.m., police said. Police said they located his tow truck in the Upton neighborhood with its bed down, about to load a vehicle. He had been shot once in the chest and was pronounced dead at the scene, said Detective Kevin Brown. Andrew had been at the site of a tow request, confirmed by the vehicle's global positioning system. But nothing was stolen, and the motive was unclear.


Truck Driver Dies in Crash

November 1, BELLMONT, IL - 47 year old David Locke died Monday afternoon when the Mack Truck he was driving ran off the road, overturned and came to rest in an embankment. Authorities say Locke swerved left to avoid a striking a vehicle in front of him. Locke died as a result of his injuries.


Two Die in Double Vehicle Crash

November 1, WINSOR, IL - 47 year old James W. Painter of Teutopolis, a tractor trailer driver, and 70 year old Harold E. Keck of Mascoutah, the passenger in a Garda Armored Courier truck, were killed when their respective vehicles collided on Illinois 16 about two miles west of Windsor. The Illinois State Police said in a telephone news release that Painter was trapped inside the cab of his truck and died as a result of the fire that engulfed his vehicle upon impact. The ISP said Keck had been wearing his seatbelt, but was ejected from the truck nonetheless.


Firefighter Dies of Heart Attack While on Duty

November 1, TAYLORSVILLE, IN - 39 year old Lt Richard Drake, volunteer firefighter for the German Town Fire Dept, died in the line of duty Monday afternoon of a heart attack. He also worked full time as a corrections corporal with the Bartholomew County Sheriff's Office. Drake is survived by his wife and four children.


High School Teacher Kills Himself at School

November 1, LAS VEGAS, NV - 58 year old Michael Vernon Roberts, a science teacher at Western High School, fatally shot himself Monday afternoon, Clark County School District police said. Police were called to the school after school staff heard one gunshot about 2:30 p.m. School was not in session Monday. Sources said Roberts had met with an administrator before he shot himself in the school's outdoor quad. One source said he shot himself in the head. With the exception of law enforcement, it is against district policy for staff and students to bring firearms on campus.


Construction Owner Killed in Roof Collapse

November 1, EPHRATA, PA - 47 year old Kevin J. Sensenig, VP for R.L.Sensenig Construction, was killed Monday when the roof of the Hill School in Pottstown he was working on collapsed. Sensenig was on the roof with other employees showing them what he wanted done during routine maintenance of the building when the roof collapsed and he fell more than 50 feet to the concrete floor below. He was only one not wearing a safety harness. Another worker was pinned in the rubble but is expected to be fine. The other two workers were unharmed.


Worker Killed While on Construction Site

November 1, VALLEY CENTER, CA - 65 year old Dirk Vamoorot died after he was pinned by a large concrete barrier on a job site in Colton. He was working Monday when a K-rail fell off another barrier and pinned him, according to the San Bernardino County sheriff-coroner. He died after being taken to a hospital.

Worker Dies After Fall From Roof

November 1, BRADENTON, FL - 41 year old Guillermo Cruz-Osorio died after falling from a roof he was working on and landing on his head, police said. He was found by co-workers about 9:30 in a pool next to the house without a pulse. Police say it appears accidental.


Tractor Overturns and Kills Woman

October 31, WEST UNION, SC - 52 year old Eva Cooper Simmons was operating a farm tractor on Sunday when it flipped backwards and pinned her underneath. She was attempting to drive up an embankment on a mid-sized tractor, but it flipped instead. Simmons a suffered a laceration to the heart due to multiple rib fractures from blunt force chest trauma.


Would Be Robber Kills Attendant in NY

October 31, BRONX, NY - 35 year old Lamine Diop was shot and killed when he refused to give up the cash during a robbery attempt on Sunday. The suspect grabbed a candy bar and then announced the robbery but he fled empty handed. Cops weren’t sure if he was trying to rob the gas station, since its cash register was closed throughout the incident. Diop leaves a wife and two children in Senegal.


Truck Driver Dies After Teen Ignore Stop Sign

October 31, MIDLAND, TX - 39 year old Dominic Caccavale of Scranton, PA was killed Saturday when a 2002 Ford Explorer ran a stop sign and collided with the semi, authorities said. A teen of Midland, ran a stop sign traveling north and collided with the semi, according to a spokesperson with the Texas Department of Public Safety.


DC Officer Killed in Crash

October 30, WASHINGTON, DC - 32 year old Officer Paul Dittamo, who died after a car crash Saturday morning "loved" the job, which he had done for almost a year and a half, his sister said Saturday. Dittamo was responding to a call when the car he was driving crashed into a utility pole about 1:30 a.m. He was pronounced dead at a hospital a half-hour later, police said. The accident is under investigation.


Veteran Fire Lt. Dies on Duty

October 30, DAYTON, OH - 52 year old Fire Lt. Kevin Quinn died Saturday, while on duty at the Montgomery County Regional Dispatch Center in Miamisburg. The lieutenant, on a 24-hour work shift, died early Saturday after lying down to take a break, on a cot provided for officers required to work long days. The cause of Quinn’s death is unknown at this time.


Restaurant Cook Shot and Killed During Robbery

October 30, BOND HILL, OH - 45 year old Antonio M. Smith, who lived with his grandmother so he could care for her, was shot to death by robbers Saturday night had trouble opening or was unable to open the cash register, a homicide detective said Monday. Two black men wearing masks and armed with handguns entered the restaurant and shot Smith after he failed to open the cash register, police said.


Gas Tanker Overturns, Kills Driver

October 30, AUSTIN, TX - 38 year old Hector Rolondo Garcia of Taylor was killed in a tragic trucking accident in Austin. Garcia was driving his 6,100 gallon gasoline tanker that failed to negotiate a curve on a freeway flyover and flipped on its side. When the truck flipped, the tank ruptured and a fire started, blazing for about two hours. Emergency responders could get to the cab of the truck because of the flames. Garcia leaves behind a wife and three children, among other relatives.


Electric Lineman Killed in Helicopter Crash

October 30, LEAD MINE, WV - 52 year old Gary L. Bland of Georgia, a lineman for PAR Electrical Contractors, died in a helicopter crash. The crash occurred Saturday morning after a crew had finished installing wire for Allegheny's Trans-Allegheny Interstate Line, Doug Colafella said. Three others aboard the helicopter survived.

October 29, ROANOKE, VA - 26 year old Joe "Buck" Ruggiero of Pen Argyl, PA, known by his colleagues as an outstanding craftsman and genuine friend,died Friday evening following an accident at Explore Park in Roanoke County on the set of a Christian film. was operating a utility vehicle, which roughly resembles a sturdier version of a golf cart, police said. Ruggiero was taken to Carilion Roanoke Memorial Hospital, but authorities offered few additional details.


Taxi Driver Hit and Killed While Changing Tire

October 29, NEW ORLEANS, LA - 19 year old Hariz Alagic had just received his taxi permit in May and was working for American Taxi, was struck and killed while trying to change the tire on his cab. Alagic was parked in the grassy area well off the shoulder of the interstate. New Orleans Police say a 35-year-old man driving a Ford F-150 pickup truck lost control and struck the man.


Man Dies in Fall From Grain Elevator

October 29, SALINA, KS - a 27 year old employee of a Salina company was killed Friday morning when he fell from a grain elevator owned by the Perryton Equity Exchange. He was an employee for Central States Enterprises. It was a tragic accident, and I just don't have information yet on what happened," the company president said. "Our hearts definitely go out to the family and all of the employees who were there." UPDATE: Authorities have identified the victim as Preston M. Tolbert.


Driver Killed When Run Over by His Tow Truck

October 29, BUFORD, GA - 67 year old Julian Baugh of Lula Roger Everett died after being hit by his truck Friday. Police said Baugh was killed when his truck rolled forward after he had exited the vehicle. Police said Baugh fell while trying to hit the brake pedal with his hand. He died at the scene.


Helpful Officer Run Down by Drunk Driver

October 29, SAN ANTONIO, TX - 25 year old Sergio Antillon, a rookie officer, was critically injured October 14 in a wreck moments after stopping to help a stranded motorist. Antillon died Friday at University Hospital after being taken off life support. A suspected drunk driver veered into Antillon, another officer and a stranded motorist which sent the motorist over the guard rail but pinning Antillon against it. The other officer narrowly missed being hit. The female drunk driver was charged Friday with one count of intoxication manslaughter and one count of intoxication assault.


Driver in Hurry Causes Crash That Killed Deputy

October 29, ALBUQUERQUE, NM - 48 year old Deputy Dean Miera was killed in a head-on crash with a semi-truck Friday morning. Police said a German tourist was in a hurry to return his rental car when he darted out of the Valero gas station and tried to cross four lanes of traffic. He cut off the driver of the semi-truck, who swerved, crossed the median and slammed head-on into Deputy Miera's unmarked car. There is no word on how fast Miera or the semi were going. Sources said Miera's partner was behind him in another car. He rushed to Miera's side and found him unconscious and his legs pinned in the wreckage, but there was nothing he could do. Miera died moments later.

October 28, CLEVELAND, OH - 53 year old Daniel Kondas was shot and killed as he walked his mail route on Corridon Ave. in Maple Heights Thursday morning. "It was just a random victim, apparently there was a struggle of some sort and the gun went off," said Maple Heights Lt. Tim Love. A 19 year old man has been arrested and charged with aggravated murder.

October 28, FRANKLIN, TN - a 25 year old roofing sub-contractor was working on a home on Orchid Trail in the Sullivan Farms subdivision around 8 a.m. Thursday when he fell to his death. The 25-year-old man has not yet been identified. He worked for A&B Restoration out of Murfreesboro.


Semi Driver Killed in Collision With Train

October 26, WILLISTON, ND - 56 year old David Eastlund was killed in a fiery collision of his semitrailer hauling oil and a cargo train in northwest North Dakota. Authorities have said it appeared Eastlund was trying to cross the tracks and didn't make it. No one on the train was hurt.


New Store Owner Murdered During Robbery

October 20, HOUSTON, TX - 58 year old Sahib Ali Khan, who only 2 months ago purchased a convenience store and was having it remodeled, was stabbed to death during an apparent robbery. A homeless man that frequented the area discovered Khan’s body; he alerted the owners of a Chevron station across the street who called 911.
