Rail Yard Worked Killed When Hit by Train

April 29, WAYCROSS, GA - 45 year old Roberto Martinez was killed when his pickup truck was struck by an unmanned rail cargo car at the company’s Rice Yard in Waycross early Friday. As Martinez attempted to cross the tracks, his pick up was struck by the first of a three-car unit of box cars. He was trapped in the truck that burst into flames after the rail car flipped it and crushed it. He was an employee for CSX Transportation.



Bridge Worker Crushed to Death Under Equipment

April 29, TUNICA, LA - 38 year old Christopher Montgomery of Greenville, MS died when a construction machine he was operating overturned and crushed him. The police report that Montgomery was operating a telescopic "high reach" machine on a bridge construction project on La. Highway 66 near the gates of Louisiana State Penitentiary Thursday afternoon when the machine overturned on a slight incline. He was employed by Austin Bridge Co.



Attorney Suffers Medical Emergency at Work

April 27, ELYRIA, OH - 52 year old Robert Flanagan, a Lorain County Assistant Prosecutor, suffered a medical emergency while at work and died. Prosecutor Dennis Will said Flanagan suffered a medical emergency in his office on the third flood of the Lorain County Justice Center. Several staff members assisted Flanagan until LifeCare emergency crews arrived on scene and Flanagan died at EMH Elyria Medical Center.


County Worker Dies of Head Injuries

April 27, YAZOO CITY, AL - 48 year old Charles Ricky Shivers died Wednesday of head injuries he suffered when a tree branch hit him in the head. Shivers, employed by Yazoo County, was with a crew who were clearing storm debris. Workers were attempting to clear a large tree off the road when the tree shifted and a limb rolled back onto Shivers, striking him in the head.


Maintenance Worker Electrocuted

April 27, HARTWELL, OH - 49 year old Rob Cole of Okeana, a maintenance electrician for Emery Oleochemicals, was found unconscious in a manufacturing area of the plant on Wednesday. He was apparently electrocuted. He died at University Hospital and the exact cause of death has not been released.


Farm Worker Killed in Equipment Accident

April 27, LEBANON CO, PA - 45 year old Jason L. Martin of Newmanstown died when the farm equipment he was driving overturned and crushed him. Martin was backing up a manure loader to a spreader when the left rear tire went over the edge of an elevated concrete slab where the manure was stored, troopers said. They said the vehicle fell several feet to the ground and rolled onto its left side and on top of Martin.


Truck Driver Killed in Semi Roll Over

April 26, EL RENO, OK - 44 year old Bobby Lee Wilkerson of Oklahoma City was killed Tuesday after his tractor-trailer rig overturned near El Reno, Oklahoma Highway Patrol troopers said. Wilkerson's rig went off the side of the highway and as he over corrected, the vehicle over turned on the passenger side. Wilkerson was thrown from the vehicle and pinned underneath it.



Private Contractor Killed Overseas

April 26, KABUL, AFGANISTAN - 55 year old James McLaughlin of Santa Rosa, CA, a civilian contractor who worked for L-3 MPRI of VA died and eight members of the U.S. Air Force died Wednesday when the Afghan pilot opened fire during a meeting in which he targeted trainers, authorities said. NATO forces shot and killed the gunman.




Worker Dies After Being Trapped Under Concrete Slab


April 26, GALVESTON, TX - 65 year old Tauelangi Angilau of Salt Lake City, UT and an employee for Grant Mackay Demolition died after being trapped for 20 minutes beneath a 1,000 lb concrete slab that collapsed at the Flagship Hotel during demolition. Angilau suffered a crushing injury. Firefighters used hydraulic tools and pry bars to free Angilau, who was taken by ambulance to the University of Texas Medical Branch, where he was pronounced dead at 4:27 p.m.


Farmer Dies of Injuries Sustained in Incident

April 26, DAUGHERTY, IA - 82 year old Edward Noss, injured in a farming accident, has died. Noss was helping his son load corn on a farm near Dougherty in northern Iowa on April 16 when a truck hit the grain auger causing the auger to fall on Noss.


Utility Worker Electrocuted

April 25, CLARENCE, LA - 35 year old Jason Warner Burr of DeSoto Parish died Monday after he stepped on a live electrical wire that had just been knocked down by high winds. He was a lineman for Cleco Power. Investigators said Burr and another utility worker were checking on a downed line on Smith Thomas Road. Burr was walking and looking up toward where the line fell from when he stepped on the power line, deputies said.


Death at Ethanol Plant

April 25, STANLEY, WI - 31 year old Paul Gulcynski of Thorp died after collapsing at work at ACE Ethanol in Stanley. He had “no trauma to his body,” and it is said his death is medically-related. The exact cause of death, however, remains undetermined at this time. Gulcynski collapsed while helping to clean out a corn bin, said Neal Kemmet, president and general manager of ACE. The company informed the Occupational Safety and Health Administration of the incident and it will be investigating. ACE will fully cooperate with OSHA’s investigation, Kemmet said.


Man Dies in Explosion of Pressurized Tank

April 25, RAINESVILLE, AL - 76 year old James White died when a pressurized tank of ammonia exploded while working in his shop. White worked on air conditioners from his home shop and just gotten some new equipment his wife said.


Farmer Dies After Becoming Trapped In Equipment

April, 25, NEILLSVILLE, WI - 69 year old Richard J. Shaw died after getting caught in a skid steer that he was trying to repair. Investigators say Shaw was apparently trying to repair mechanical problems. He became pinned between the loader arms and the engine hood at the rear of a skid steer.



Worker Dies in Mine Accident

April 25, NEW SPRINGFIELD, OH - 31 year old Jason Gudat of Washingtonville was killed early Monday in an accident at a mine in Mahoning County. Gudat, an employee of the East Fairfield Coal Co., died in the accident at the Subtropolis Limestone Mine on East Garfield Road in Springfield Township. Gudat died of multiple blunt force traumas to the chest and abdomen after being hit in the chest with a rock Monday. He was wearing a hard hat.



Miner Found Dead After Rescue Attempt

April 24, MULLAN, ID - 53 year old Larry Marek, who was trapped nine days ago in a tunnel collapse at a northern Idaho Lucky Friday silver mine was found dead Sunday when rescuers reached him. Crews for Hecla Mining Co. labored around the clock to dig through the collapsed debris and build a second intersecting tunnel to reach Marek. Marek and his brother Mike had just finished watering down blasted-out rock and ore in the mine when the ceiling collapsed in a 6,150-foot deep tunnel April 15. Mike Marek escaped unharmed. The reason for the collapse has not been established.


Argument Between Co-Worker Turns Deadly

April 23, WEST MIAMI, FL - 26 year old Erick Ibarra was shot and killed by a co-worker who he was involved in an argument with. The argument escalated with the co-worker, who was hired about a week ago, and the suspect pulled out a gun and shot Ibarra. The gunman was arrested.



Texas Officer Gunned Down in Line of Duty

April 23, VENUS, TX - 31 year old Officer Clifton Taylor died during a furious shoot-out with a domestic violence suspect. Officers were responding to a home for a domestic disturbance call. When they arrived, they were told the man was hiding in a backyard shed. When they approached the shed, the suspect opened the door and opened fire hitting Taylor several times. The suspect was shot and killed by return fire from the other officers.


Unarmed Security Guard Shot to Death

April 23, ST. LOUIS, MO - 22 year old Joshua L. Miller was shot to death Saturday while on patrol at the Sinai Village redevelopment project. He was an employee for KLM Security. Miller was found shot in the head. Initially police responded to a report of shots fired but didn't find any evidence, however a few hours later they responded to a report of someone finding Miller shot inside a car. Miller was working two jobs to provide a home for his girlfriend and two young children.


Police Officer Shot During Traffic Stop

April 22, EUGENE, OR - 43 year old Officer Chris Kilcullen was shot and killed Friday afternoon during a routine traffic stop on Interstate 105. After the suspect's car attempted to elude Kilcullen,spawning a brief pursuit he was shot at the intersection of I-105 and 52nd Street in Springfield. Springfield Police resumed the pursuit near Main Street, with multiple law enforcement agencies responding. The chase ended with the female suspect being taken into custody after a standoff and she has been charged with aggravated murder.


Road Construction Worker Killed on Job Site

April 22, HOUSTON, TX - 47 year old Manuel Seja was crushed to death by a cement truck along the South Freeway on Friday afternoon. Seja was in front of the truck under the freeway at North McGregor Way about 4 p.m., when it "jumped out of gear" and struck him, Houston Police Department spokeswoman Jodi Silva said. Two other workers suffered severe but non-life threatening injuries. The workers were part of a crew doing shoring work along Brays Bayou, according to published reports.



Former NASCAR Driver Killed in Industrial Accident

April 21, DORA, AL - 66 year old Darrell Brown, owner of Brown's Trucking Co. and former NASCAR driver was killed Thursday in an industrial accident on his business property. Brown was using the front-end loader to move debris into a gulley when the entire vehicle fell into the gulley. He was pronounced dead at the scene. Cause of death was not specified.



Two Die of Accidental Electrocution

April 21, IBERIA, LA - 33 year old Jesse Clingan and 41 year old Neal Lavergne died while working on a home improvement project for a family in Iberia Thursday. They were insulation installers and working in the attic space of a home. The homeowner became concerned when the installers did not come out of the attic after all day and found them. "He had stapled through one of the power wires in the radio flex and when he touched his buddy it grounded off and the electricity ran through both of them and got them both" says the homeowner.



Longtime Disneyland Employee Dies of Natural Causes

April 20, ANAHEIM, CA - 58 year old Philip Doyle of Corona collapsed inside a backstage bathroom after making a delivery from a kitchen near one of the amusement park attractions. Co-workers became concerned when Doyle did not return to his post. They found him in the bathroom, his breathing very shallow. He was transported to a nearby hospital where he died. Doyle had been under a doctor's care for a previous medical condition.


Worker Killed at Air Station

April 18, ONSLOW, NC - 52 year old Carl Anthony "Tony" Gwynn of Jacksonville died at the New River Air Station when he was hit and killed by a dump truck. He died at Onslow Memorial Hospital. No details on the cause of the incident are available.
