Weekly Toll: Death in the American Workplace

Wrecker Employee Shot and Killed

February 24, ATLANTA, GA - 44 year old Travis Fenty was killed when a man opened fire on him and another worker who were hired to remove cars from a property. The shooter insisted the property belonged to him and didn't want the vehicles moved even though the property had been lost to him in a tax foreclosure sale two years ago. Several attempts had been made to remove the cars to no avail. This time the suspect went inside and began firing from an upstairs window. Fenty worked for Quick Drop Impounding. The suspect has been arrested and charged with murder.


Worker Found Dead At Gravel Facility

February 24, SALT LAKE CITY, UT - 56 year old Allan K. Sherman, an equipment operator, was found dead at the Bolinder Resources facility on the Great Salt Lake’s Stansbury Island on Thursday. Sherman was not conscious, breathing or responsive when paramedics arrived and found him face down on a conveyor belt, a Sheriff spokesman said. He reportedly fell onto a conveyor belt and became trapped. His body then traveled about 25 feet on the belt, which carries dirt and rocks to a crusher. That's when his body became stuck under a giant magnet.


Tow Truck Driver Dragged to His Death

February 23, COLORADO SPRINGS, CO - 35 year old Allen Rose, who worked for J&J Towing, was killed when he was tangled in the chain of his truck and dragged for more than a mile to his death. Rose was hooking up an SUV parked illegally when a woman jumped into the vehicle and took off. Rose's legs became entangled in a tow cable as the driver drove off. Rose was a married father of two.


Logger Killed by Falling Tree

February 23, MAGNOLIA, AR - 51 year old Donnie Moss of Camden was killed in a logging accident on Wednesday. A crew from East Camden-based Rogers Lumber Co. was working off U.S. 371 when a falling tree unexpectedly bounced off another tree on its way down and struck Moss. Federal OSHA is investigating what appears to be an accident.


Truck Driver Dies in Crash

February 23, LOUISVILLE, KY - 46 year old Ronnie Grubbs of New Paris, OH died as a result of injuries suffered in a truck crash. Grubbs worked for a Richmond, Ind., delivery service and was driving a new garbage truck to Waste Management Co. in LA. For an unknown reason Grubbs went off the shoulder of the road and struck three trailers in a sales lot.


Truck Driver Found Dead

February 23, HINKLEY, CA - 61 year old Michael Earl Morris of Escalon suffered an unknown fatal medical ailment causing him to crash his semi truck off of westbound Highway 58 eight miles west of Hinkley Wednesday morning. A witness discovered the crash and called 911. Responders found Morris slumped over the wheel of his semi. The California Highway Patrol is investigating.


Man Dies While Driving Business Van

February 23, EUFALA, AL - 56 year old Ricky Lamar Kent, part owner of Marshall Memorial Funeral Home, died Wednesday after he collapsed while driving the funeral home van. Kent and another employee were transporting someone who had died when he slumped over the wheel while driving. The other employee took the wheel but couldn't control the van which then veered into a gas station and into another vehicle. The employee and the other driver were seriously injured and flown to a local hospital.



Cab of Jackknifed Tractor Trailer Falls Off Bridge

February 23, INDEPENDENCE, OH - 52 year old Larry Cunningham of Cleveland died when his semi crashed on the Valley View Bridge. Another vehicle caused Cunningham to try and stop his rig which caused it to jackknife. The trailer went to one side of the bridge overpass and the cab went over the other side. Cunningham had been a truck driver for 32 years and was driving for Gray Container, a Cleveland company owned by his cousin Anthony Gray.



Contractor Working on Home Shot and Killed

February 22, NEW ORLEANS, LA - 53 year old Boris Scott was shot and killed while he worked on a vacant house Tuesday. Scott, a self employed general contractor, was working with another man when a gunman approached Scott and shot him several times. Police have not released details on a possible motive or suspect.


Restaurateur Killed in Robbery

February 22, COLUMBIA, SC - 66 year old Steve Kim was shot once in the back with a stolen pistol during a robbery Tuesday night at his Dino’s on Decker restaurant. A 23 year old man went on a two hour crime spree which resulted in four armed robberies which then resulted in Kim being killed. Kim stayed late to prepare a meal for a late hungry customer when the suspect walked in demanding money and shot Kim.





Church Bus Crash Claims Life of Driver

February 21, LAKE ARROWHEAD, CA - 61 year old Won Seok Chae of Los Angeles, died of injuries he suffered in a bus crash Monday. The bus carrying teenagers and their chaperon's home from a weekend religious retreat collided with another vehicle on a treacherous mountain road and careened down a steep, snow-covered embankment that killed Chae and left at least 10 seriously injured. John Cho, a deacon at the Light of Love Mission Church, described Chae as a professional tour bus driver who volunteered to drive the church's bus on weekends.



Worker Dies in Drilling Rig Accident

February 21, MIDLAND, TX - 44 year old Ronnie May of Sequin died when he and another man were working on a rig owned by Tulsa, OK based Helmerich & Payne Inc. The two men were working on some drilling pipe on a rack when the pipe slipped. A spokesman said one man’s leg was caught by it, and May was caught between the pipe and some drill collars, crushing May.


Radio Tower Worker Falls to Death

February 20, OKEECHOBEE, FL - 43 year old Michael Anthony Scott of Hazelhurst, GA plunged to his death from a radio tower in Okeechobee on Sunday. Scott and another worker, working for Davis Specialties of Waycross, GA, were on a radio tower installing antenna wires for South Florida Water Management when Scott fell approximately 120 ft. to his death. His safety harness appeared to be intact. The case is being investigated by OSHA.



Third St. Petersburg Officer Killed

February, 20, ST. PETERSBURG, FL - 46 year old Officer David Crawford was shot and killed when he responded to a call Monday. He is the third Florida officer killed in less than a month. A 16 year old male suspect has been arrested in the death of Officer Crawford.




New York Officer Shot to Death by Suspect

February 18, POUGHKEEPSIE, NY - 44 year old Officer John Falcone of Marlboro, was killed by a gunman who had just shot his wife and was holding his 3 year old daughter in his arms. Falcone was able to rescue the child from her father before he was shot. After firing on Officer Falcone, he turned the gun on himself. Falcone had responded to a domestic situation at a train station.



Firefighter Dies Two Days After Ceiling Collapse

February 18, LOS ANGELES, CA - 61 year old Glenn Allen, a veteran firefighter nearing retirement died at a hospital Friday, two days after he was pulled without a heartbeat from the rubble of a burning Hollywood Hills home where the ceiling had collapsed, authorities said. Unknown to firefighters, the blaze had burned through sprinkler pipes in the attic and filled the space with water, collapsing a drop ceiling held in place by 2-by-4-inch wood struts and burying Allen, Capt. Haro said.


Falling Boulder Crushes Worker Clearing Trail

February 17, GALES CREEK, OR - 59 year old Randy Hodges of Medford was killed when a boulder broke loose on a hillside and rolled into him while he was clearing a trail. Hodges, a contract employee for the Oregon Department of Forestry, was hired to open up a trail near a Gales Creek campground. When Hodges family hadn't heard from him in several days, a state worker searched and found him dead on the trail.



Police Officer Dies in Cruiser Crash

February 17, DELRAY BEACH, FL - 39 year old Adam Rosenthal of Parkland, a Sgt. of Delray Beach Police Department, died in a car crash Thursday. Rosenthal crashed his cruiser into a tree with such force that the trunk tore through the passenger side and half the car, investigators said. The Palm Beach County Sheriff's Office is investigating what made the decorated officer's car leave the street and crash into a tree on the median, including any mechanical malfunctions to the cruiser.


Worker Falls Three Stories from Roof

February 17, NEW BEDFORD, MA - Luis Gilberto Tenezaca Palaguache died Thursday when he fell while stripping off the roof of a house on Hathaway St. Palaguache was speaking with a co-worker on the ground when he suddenly fell off the roof and landed on the asphalt driveway below, said New Bedford Police Capt. David Lizotte. Palaguache was not wearing a safety harness. There were no indications of drug or alcohol use, police said. It is believed Palaguache worked for Chelsea Enterprises Construction for general contractor A Plus General Contracting of New Bedford.



Police Captain Dies of Heart Attack During Training


February 16, DAYTONA BEACH, FL - 52 year old Captain John I. "Jay" McDonough suffered a fatal heart attack while participating in a department training exercise. He was jogging in the area of International Speedway Boulevard and South Peninsula Drive when he collapsed. Captain McDonough served as a member of the Volusia County Beach Patrol for 13 years. He is survived by his wife, son, father, sister, and brother.


Tractor Trailer Driver Dies in Fiery Crash

February 15, FORESTVILLE, MD - 57 year old Osborne H. Bailey of Charleston, SC died when his trailer struck a bridge embankment of the D'Arcy Road overpass in Forestville at about 11 p.m. Tuesday, according to Maryland State Police. His vehicle also collided with several cars, injuring one man. Investigators are investigating the cause of the crash.


Oil Rig Worker Dies Two Weeks After Being Injured

February 15, CHEYENNE, WY - 50 year old Mitchell Lemley of Riverton, who was injured on February 2 when he fell 60 feet while working at a drilling rig, has died. Lemley worked for Casper-based SST Energy, which was drilling the well for another company.

Delivery Driver Killed in Robbery Attempt

February 17, ST. LOUIS, MO - 40 year old Truman L. Smith, a delivery driver for E & K Truck Service of St. Clair was shot and killed in a robbery attempt Thursday morning. He was about to make a delivery when he exited his truck to check the address. He was then accosted by the robbery suspect who shot him and ran off. Smith was pronounced dead at Kenneth Hall Regional Hospital in East St. Louis.


Firefighter Dies Months After Being Struck by Burning Tree

February 17, FRANKFORT, KY - 58 year old Donald S. Lam, an employee for the Kentucky Division of Forestry, who was injured while fighting a fire on September 7, 2010 has died of his injuries. Lam was clearing a fire break when a burning tree truck rolled off a bluff and struck him from behind. The blow left Lam unconscious with serious injuries, including second-degree burns. He had been hospitalized in critical condition since the accident.


Semi Driver Dies in Truck Crash

February 17, LEXINGTON, KY - 56 year old David A. McHenry of Massillon, OH died when his tractor trailer, belonging to LoriMar Materials Management of Massillon, left the roadway, went over a ravine and overturned. First responders reported that McHenry was able to communicate while at the scene for a short period of time and reported that he had fell asleep at the wheel. Moments later, McHenry's condition become critical and he later died from injuries sustained in the collision.


Construction Worker Struck by Car

February 17, CLIFTON, NJ - 44 year old Albert Marques of Staten Island, NY, an employee for JM Construction Co., was killed early Thursday morning when he was hit by a car on the Garden State Parkway, police said. Marques was crossing the roadway near a construction site at the Colfax Street overpass about 1:40 a.m. when he was hit in the center lane by a car, according to a State Police press release. The left of the parkway was closed for construction.


Farm Supply Worker Killed When Pinned by Car

February 17, MOREHEAD, KY - 48 year old Billy G. Bowling, an employee for Eldridge & Son Farm Supply, was killed when the driver of a car backed into him and another man, pinning them between the car and a dump truck. Witnesses said the two men were preparing to load goods into the trunk of an automobile inside a storage building when the auto was backed into the building, pinning both of them to the front of the truck. The other employee hit is listed in critical condition.


Semi Driver Found Dead After Crash

February 17, KICKAPOO, IL - 75 year old Donald E. Steidinger of Fairbury was found dead, partially submerged in a ditch after a crash Thursday morning. His truck had crashed into the ditch after apparently exiting a parking area behind a cafe, and the cab of the vehicle blocked visibility of the area where his body was found which may explain why he wasn't found for up to six hours after the crash. It wasn’t immediately clear whether Steidinger suffered from some type of medical condition that led to the crash, or from drowning in the ditch.


Deputy US Marshall Shot Serving Warrant

February 16, CHARLESTON, WV - 24 year old Deputy Derek Hotsinpillar was killed and two others were wounded Wednesday in West Virginia when a drug suspect opened fire on them with a shotgun and then was shot dead, authorities said. "Immediately upon entry into Smith's residence, three deputy marshals were fired upon by a shotgun blast and struck," US Marshall spokesman Oney said. "Deputies returned fire on Smith and he was shot dead on the scene."


Woman Strangled When Scarf Caught in Industrial Dryer

February 16, QUEENS, NY - 32 year old Limin Min Huang, an employee for An Huan Laundry Service, was crushed yesterday when her scarf became caught in the machine, police sources said. Huany was loading towels into an industrial steamer at a Queens dry cleaner and apparently forgot she had on a scarf that was caught on the machine which pulled her in.



Offshore Worker Dies After Fall Off Platform

February 16, LAKE CHARLES, LA - Joseph Bruno of Breaux Bridge died after he was pulled from the Gulf waters after falling from a Apache Corporation platform. Bruno was on a swing rope, moving from a vessel to a platform when he fell into the Gulf waters. Apache reported he was wearing a flotation device and other workers were able to pull him from the water alive. But then he became unresponsive and soon died on the platform. Bruno was a fire watcher for H.I.S. Fire and Safety Company out of Delcambre. Apache Corporation had hired H.I.S. to do maintenance work on its platform this year.


Woman Killed in Farm Equipment Accident

February 16, HERMITAGE, VA - 53 year old Debra S. Clive died in a farm equipment accident on the family farm in Swoope. Trooper Hyden said Cline was in a barn installing a plow onto the end of a tractor operated by her father. Something caused the tractor to lurch and its tire ran over Clive. She died en route to the hospital.


Worker Killed in Accident at Steel Mill

February 16, CANTON, OH - 57 year old Arthur Smith of Lorain died from massive chest injuries sustained while working at Canton’s Republic Engineered Products Wednesday night, according to Stark County Coroner’s Office investigator Richard Walters. Smith, a contractor with Superior Environmental Solutions, was trapped when a hydraulic truck door came down on top of him.


Store Owner Shot to Death in Robbery Attempt, Saved Wife

February 15, MIAMI GARDENS, FL - 35 year old Salim Ullah, co-owner of Fast Cash and Urban Wear, was fatally shot during a robbery attempt. Two suspects entered the store, asked for money and then shot Ullah twice. They then turned their guns on Ullah's wife but before they could fire he threw himself in front of her taking two more bullets, but saving her life. Ullah had one final message for his wife, saying “I will see you in heaven” before the rescue helicopter took off.


Aspiring Young Schoolteacher Collapses in Classroom

February 15, FLINT, MI - 28 year old Angel Bauswell, died Tuesday morning after collapsing in the classroom at Neithercut Elementary School in front of 14 students. Family members said they were blindsided by her death, because she was young and had no history of medical complications. With her heart set on becoming a teacher she spent her days working with 4-year-olds as a paraprofessional in a prekindergarten class and her nights studying early childhood education at Mott Community College. An autopsy will be performed to determine actual cause of death.


Farmer Dies Under Farm Tractor

February 15, SLOCOMB, AL - 68 year old Louie Williford died in a tractor accident while he was spreading fertalizer on a cattle pasture at his farm located on Grant Road off of Alabama Highway 103. The local coroner said he found the tractor up against a tree on the property, and part of Williford’s body underneath the tractor.


Truck Driver Dead Before Crashing Into House

February 15, LANCASTER, PA - 47 year old John Brasher of Bridgeport died of natural causes prior to crashing into a house along Route 30 and not from injuries he sustained in the Tuesday crash. Brasher was hauling 24 tons of garbage when police said his truck hit at stone walkway, several signs along the road before crossing three lanes of traffic and crashing into a parked car and porch of the house.


Officer Directing Traffic Hit and Killed

February 15, UNIONTOWN, OH - 46 year old Daniel P. Stiles, Uniontown Police Captain, died Tuesday morning after being struck by an SUV as he was directing traffic for Lake Center Christian School just before 8 a.m. Stiles was wearing a fluorescent jacket while he directed traffic but the possibility of sun glare made have contributed to the driver not seeing Stiles.


Industrial Accident Kills Worker

February 15, MOORESTOWN, NJ - 52 year old Robert Durocher of Merchantville, NJ, an employee for Mayberry Rigging was killed Tuesday morning when a truck trailer he was repairing fell on top of him, police said. Durocher was working under the equipment-hauling trailer at 10:55 a.m. when the undercarriage fell and crushed him. He suffered fatal head and chest injuries and was pronounced dead at the scene, police said.


U.S. ICE Agent Killed in Ambush in Mexico

February 15, MEXICO - Jaime Zapata, a U.S. Immigration & Customs Enforcement (ICE) Agent, was killed and another wounded while driving through northern Mexico Tuesday, in a rare attack on American officials in this country which is fighting powerful drug cartels. Zapata was on assignment from the office in Laredo, Texas, where he served on the Human Smuggling and Trafficking Unit as well as the Border Enforcement Security Task Force.


Worker Crushed Between Pieces of Equipment

February 15, NORFOLK, NE - 24 year old De'Andra Moore of Battle Creek was killed Tuesday at Norfolk Iron and Metal warehouse. Sheriff's investigator Jon Downey said a forklift accidentally hooked a large steel tray and pushed it into Moore, crushing him momentarily between the tray and a steel rack.


CHP Officer Killed in Crash

February 15, MENDOCINO, CA - 24 year old Thomas Adams, a CA Highway Patrol Officer, was killed while on patrol when another vehicle crossed the center line and hit his cruiser head-on. It is unclear at this time when the other vehicle veered into Adams' lane.


Worker Dies in Trench Collapse

February 15, CUMMING, GA - 20 year old Aaron Banks of Covington became trapped in a trench in Forsyth County Tuesday but died by the time rescuers were able to reach him. Forsyth County Fire Department Capt. Jason Shivers said the 20-year-old man was buried under dirt in the trench on Trammel Road near Buford Highway.


Worker Dies in Industrial Accident

February 14, WILLIAMSPORT, PA - 21 year old Darryl E. Richards was killed when he became trapped in an auger at Eastern Wood Products,township police Chief William C. Solomon said. At the start of his shift Richards was assigned to the auger at the east side of the building," Solomon said. "He was grinding up scrap wood and feeding it to the incinerator." "Something got stuck in the auger and when he tried to free it up, his clothing got caught in there," Solomon said. He was working alone at the time.


Forest Worker Clearing Brush Found Dead

February 14, CLEARWATER, WA - Sam Gaydeski of Beaver, WA, a member of a pioneer West End family was killed while on the job clearing brush for the Department of Natural Resources. His body was found late Monday. He apparently was killed by the blade of his brush cutter, said Jefferson County Sheriff's Chief Criminal Deputy Joe Nole. Gaydeski was clearing weeds near the side of a DNR road, he said.


Maintenance Worker Pinned by Truck Dies

February 14, FAIRVIEW, NJ - Jeffrey Daus, a longtime mechanic for the Department of Public Works as well as a volunteer fireman, was killed after the garbage truck he had been driving pinned him against the wall of a public works building on Fairview Avenue on Monday morning, authorities said. The Fairview police, the Bergen County Prosecutor’s Office Fatal Accident Unit and the Bergen County Police Commercial Vehicle Inspection Unit were looking into how the accident occurred, authorities said.


Driver For Blood Center Dies of Medical Event

February 14, ROCKFORD, IL - 61 year old Thomas Washington died on his way to a blood drive event. An employee for a local blood center, Washington, was driving a truck to an event when he suffered a "cardiac event" according to the local coroner's office. Police report Washington's truck veered off the road and into a small tree.


Tribal Officer Killed by Stray Trailer

February 13, SAN CARLOS APACHE, AZ - Tribal Officer 26 year old Aaron Peru was killed Sunday was driving to a mandatory work meeting when a trailer detached from another vehicle and collided with his. The officer had only been with the department for 30 days at the time of the accident. The Arizona Department of Public Safety is investigating before further details are released.


Volunteer Fireman Collapses and Dies After Call

February 13, SALTILLO, MS - 26 year old Joshua Wilkes collapsed and died shortly after leaving the scene of a fire. Wilkes and another firefighter stopped to fill their truck at a gas station and Wilkes collapsed inside the convenience store where he went to get a drink. An autopsy will be done to determine the exact cause of death.


Worker Falls Through Ceiling

February 12, GALESBURG, IL - 63 year old Walter Cassiday died of injuries when he fell from the ceiling area of Midstate Manufacturing Corp. According to employees working in the paint booth at the time, Cassiday had been working on lighting in the ceiling above the paint booth. A co-worker, who had been working with Cassiday, said Cassiday had stepped off the catwalk onto the mesh ceiling and had fallen through, hitting his face on a metal box in the paint room on his way down. He died at a local hospital.


County Worker Found Dead in Cubicle, a Day After She Died

February 12, DOWNEY, CA - 51 year old Rebecca Wells, an L.A. County employee, apparently died while working in her cubicle on Friday, but no one noticed for quite some time. Wells was found by a security guard on Saturday afternoon. She was slumped over on her desk in the L.A. County Department of Internal Services. The exact time or cause of death is unclear but the last time a co-worker saw her alive was Friday morning around 9:00 a.m., according to Downey police detectives.


Worker Killed When Struck by Equipment

February 12, ERIE, PA - 39 year old Paul Benes died Saturday outside a coke plant in northwestern Pennsylvania. The coroner said Benes died of blunt force trauma when he apparently walked in front of the front-end loader and the operator didn't see him.



Plant Worker Killed by Falling Beam

February 12, LOUISVILLE, KY - 54 year old Ronald "Ronnie" Cassidy, a long time employee of Ford's Kentucky Truck Plant, died Saturday of multiple blunt force trauma's when he was struck in the head and chest by an "I-beam" that fell inside the plant. Family members say Cassidy had worked as a millwright at the Kentucky Truck Plant for 16 years as a millwright at the plant and worked in maintenance. OSHA is investigation the circumstances surrounding the incident.


Worker Run Over by Giant Grader

February 12, WAUCHULA, FL - 40 year old Jose Antonio Soto was killed at a phosphate mine when a giant 15 ton grader hit and backed over him as crews were just beginning their shifts. Soto was contracted by McDonald's Construction working at CF Industries mine. A large crowd of witnesses saw the incident and alerted the driver that he'd just hit someone.


Farmer Dies Under Flipped Tractor

February 12, COLBERT HEIGHTS, AL - 73 year old James Ray Winsted died when his tractor flipped, pinning him underneath. Deputies say it appears Winsted was pulling a trailer of rocks behind the tractor when the tractor slid as it went down a small hill and flipped over.


Worker Clearing Snow Falls Through Roof Skylight

February 11, STERLING, NY - 41 year old Carmen Z. Garcia fell to his death from the roof a Sterling onion processing plant Friday. Garcia was clearing snow off the roof of the warehouse at Bland Farms in Sterling when he fell backward through a skylight, and died when he crashed to a concrete floor about 30 feet below, police said. An investigation into his death is ongoing.


No Lock Out Tag Out on Machine Causes Worker's Death

February 11, TERRA BELLA, CA - 30 year old Fernando Santiesteban was the victim of a fatal accident at the Setton Pistachio plant in Terra Bella Friday. Santiesteban was performing maintenance on a machine that separates nuts from hulls and his legs were severed by the machinery. Krisann Chasarik of Cal-OSHA said it appears the machine was not "locked out," so it could not be started, and another person turned it on while Santiesteban was working on it.


Tree Trimmer Dies in Work Site Accident

February 11, CENTREVILLE, IL - 38 year old Damon Carpenter died of a head injury when when a tree branch fell onto him. The incident occurred when a tree trimmer in a bucket several feet in the air cut a large branch, about 6 inches in diameter, that fell and struck Carpenter in the head, Deputy Coroner Haskenhoff said. Carpenter worked for Darnell Belt Tree Service.


Man Dies in Farming Accident

February 11, CALDWELL, ID - 40 year old Tadd Portenier was crushed to death Friday in a hay swather. Officials say Portenier was at a friend's place in Middleton trying to jump start the machine. As he removed the cables, his coat got caught in the swather's drive shaft.


Mine Worker Dies in Collision With Road Equipment

February 11, KAYENTA, AZ - 55 year old Roy Black died in an accident at the Kayenta coal mine when his vehicle collided with a piece of road maintenance equipment Friday afternoon. A spokeswoman for Peabody Energy Corp. couldn't immediately provide a hometown for Black, but says he worked for the mining operation for 30 years. Black's hometown is unknown at this time. The federal Mine Health and Safety Administration is investigating.


Worker Crushed Under Concrete Pipe

February 11, RANCHO CUCAMONGA, CA - 58 year old Peter Lisa was killed Friday when he was crushed under a concrete cylinder pipe at a business in Rancho Cucamonga. Lisa was cutting an 84-inch concrete cylinder pipe when one half of the pipe fell over and pinned him, according to the release.


Rail Worker Dies in Accident

February 8, KANKAKEE, IL - 43 year old Stanley L. Watts, an employee for Norfolk Southern railroad, was crushed to death between two train cars. “Mr. Watts was riding on one car, as part of his job, to go down the tracks to get more cars. He was employed at the rail yard and we are still investigating to determine why he became entrapped between the two cars,” said Kankakee County Coroner Gessner.

February 3, LAPLACE, LA - 32 year old Samuel Moyer, an employee for Arcelor Mittal and a local pastor, died as a result of burns when a piece of material fell from the ceiling of the old Bayou Steel Plant while he was standing on a podium raining down on him. The steam explosion is underway by OSHA. Some changes have been made at the plant but his wife said, "We didn't understand why you would put somebody in that kind of situation to where they were that close to molten steel to just pour over on them".

Store Employee Murdered by Ex Boyfriend

February 10, QUEENS, NY - 32 year old Guimmia Villia, a cashier at Crescent Chemists pharmacy and mother of two, was shot to death inside the pharmacy Thursday. A witness says the suspect entered the pharmacy, handed the victim a bouquet of flowers, and then shot her in the head. The suspect was later found dead in an apartment of a self inflicted gunshot wound.


County Supervisor Killed in Road Accident

February 10, TYLERTOWN, MS - 83 year old Travis Alford, a longtime Walthall County supervisor died Thursday after being hit by a truck while he was trying to clear a tree from a county bridge. Alford was hit about 6 a.m. when the driver of a truck tried to dodge the tree and apparently didn't see Alford in the road.


Worker Dies After Altercation at Work

February 10, CANONSBURG, PA - 34 year old Trevor Casey Compton died of a gunshot wound after being shot by one of his co-workers during an altercation at Rosenbloom Welding. Police report that there was a fight in the welding shop between two co-workers including the alleged shooter and another man. Compton died at a local hospital. The man identified as the shooter was sitting on the floor of the welding shop when police entered the building.


Worker Crushed Under Metal Racks

February 9, PORTLAND, OR - 25 year old Jeromy Judd was killed when a rack of beer kegs collapsed onto him at the Maletis Beverage beer distributing plant. The accident happened inside a cooler where kegs are stored and an investigation is under way, according to a spokeswoman from the Oregon Occupational Safety and Health Administration. Judd reportedly died of head injuries.


Two Killed When Tractor Trailer Overturns

February 9, CUMBERLAND CO, TN - 41 year old Walter L. Burkins Jr. of Memphis, TN and 27 year old Anthony Terrance Anderson were killed when their tractor trailer crashed. Burkins was driving the semi carrying U.S. mail when it swerved three times and ran off the right side of the interstate. It then went into a ditch, hit a tree and overturned, pinning Burkins and Anderson. Anderson was in the sleeper compartment when the crash happened.


Driver Dies in Single Semi Crash

February 9, SALEM, IL - 52 year old Daniel J. Richer of Bristow, OK died after an incident on I-64 Wednesday afternoon in Wayne County. Authorities say Richer was westbound on I-64, when for unknown reasons, his semi-truck tractor-trailer swerved off the roadway to the right and came to rest on the north side of I-64. His death remains under investigation.


Security Guard Hit and Killed by Out of Control Van

February 9, SANTA CLARITA, CA - 52 year old Julio Garcia of Reseda, a security guard, was hit and killed by a van whose driver had blacked out and lost control of the vehicle. Paramedics from the Los Angeles County Fire Department treated Garcia at the scene of the crash before he was transported to Henry Mayo Newhall Memorial Hospital in critical condition where he later died. Garcia was a security guard at the Valencia Industrial Center where the CBS television drama “NCIS” was being filmed.



Gas Pipeline Explosion Kills Contract Worker

February 8, MONT BELVIEU, TX - Rick Shaw of Baytown was killed in a gas pipeline explosion and fire at a Houston-area petroleum plant, officials confirmed Wednesday. "One of the workers that was working with him said he was in the middle of the gas cloud when it happened, so I don't really think he got out," said Shaw's brother, Mark Shaw, of Baytown. "It was a big fire." Shaw was hired by Louisiana based Turner Industries working at the Enterprise Products storage plant in Mont Belvieu.


School District Employee Hit and Killed

February 8, DUBUQUE, IA - 33 year old Lawrence Steven "Junior" Brehm, an employee for the Dubuque Community School District, was killed when he was struck by one of the district’s snowplow trucks. Brehm,was working outside the district’s buildings and grounds department in a parking lot. Brehm was an electrician in the Maintenance Department at DCSD and had been employed since 2005.


Worker Hit and Killed Alongside Roadway

February 8, LOUISVILLE, KY - 53 year old Dannye R. Allen was killed when he was hit by a box truck while working on an environmental cleanup along Interstate 71. Even though it was a safe zone on the highway, police say a box truck in the fast lane came up on a slow moving pick-up and veered right into the work area hitting Allen. Two other workers were hurt. One of them told Allen's family he tried to push them out of harm's way.


Construction Worker Struck by Metal Beam Killed

February 8, GLENVIEW, IL - 43 year old Kenneth Puplava of Portage, IN, was killed Tuesday when he was struck by a metal beam at the site of a new medical office building at Glenbrook Hospital, officials said. The project was being helmed by Chicago-based Pepper Construction Group.



Worker Dies in Trench Collapse

February 8, CUMMING, GA - 20 year old Aaron Banks of Covington died Tuesday afternoon after he was buried in a trench collapse at a new home site off Trammel Road. Bystanders were able to free Banks' head from the dirt but emergency workers were not able to free him until about 40 minutes after he was pronounced dead. It is unclear who he worked for. The accident investigation has been turned over to OSHA.


US Worker Dies in Afghan Blast

February 8, KANDAHAR, AFGANISTAN - U.S. Homeland Security Secretary Janet Napolitano said in a statement released late Tuesday in Washington that David Hillman, a Western New York native and retired customs officer who had worked for the U.S. government for 30 years, died in the blast at the Inland Customs Warehouse in Kandahar. Afghan police said the suicide bomber detonated his explosives inside the city's customs house compound during a visit by NATO forces. It is not clear where Hillman's home is in the United States.


Tractor Trailer Driver Dies in Rollover Accident

February 8, NEEDLES, CA - 55 year old Michael Goodman of the Virgin Islands was killed and his passenger injured after he lost control of the big rig and it overturned along Interstate 40 about 50 miles west of Needles, say California Highway Patrol investigators. "Mr. Goodman lost control of his vehicle and overturned onto the center median," investigators said in a written statement.



Construction Worker Dies in Trench Collapse

February 8, STUDIO CITY, CA - 33 year old Javier Bautista-Cruz died Tuesday when he became trapped up to his neck when a trench he was working in collapsed around him. No other construction workers at the site, near Mulholland Drive and Coldwater Canyon Avenue, saw the collapse happen. Rescue crews used a vacuum to remove the soil on top of him, but it was too late.


Worker Dies When Clothes Become Tangled in Equipment

February 8, ROCHESTER, WI - Kyle Wene of West Allis died Tuesday after his clothes apparently got caught in a piece of machinery at the Rochester plant of Green 46 Recycling. Wene was working on a conveyor belt used to process asphalt shingles for recycling. Sheriff's spokesman Lt. Dan Klatt says that the belt apparently caught Wene's clothing and choked him. His death is being investigated.



Two Construction Workers Fall to Their Death

February 8, UPPER WEST SIDE, NY - 49 year old Brett McEnroe of Dover Plains and 51 year old Roy Powell of New Paltz, both iron workers, died Tuesday morning after they fell from a fifth floor through an open elevator shaft at a church construction site where they were installing steel, authorities said. The city's buildings commissioner, Robert LiMandri, said it appeared the workers did not have proper fall protection, including harnesses, safety netting or a support platform. McEnroe and Powell were working on a planned expansion of the Redeemer Presbyterian Church on Manhattan's Upper West Side.



Three Killed in Deadly School Bus/Semi Crash

February 8, JACKSON, MS - At least a dozen high school students were taken to hospitals and three adults were killed when a tractor-trailer sideswiped a school bus and collided head-on with another school bus from Ackerman High School Tuesday on a highway bridge in north Mississippi. Those who died were school bus driver 37 year old Steven B. Moss of Ackerman, teacher 53 year old Phyllis Andrews Graham of Eupora and the driver of the semi was 54 year old Gary T. Bailey of Mantachie, MS. The buses were carrying 57 seniors from Ackerman High School and were returning from a visit to the University of Mississippi.


Tow Truck Driver Struck and Killed

February 7, EL DORADO HILLS, CA - 27 year old Michael Robert Sanders was struck and killed while helping a motorist on westbound Highway 50 west of El Dorado Hills Boulevard during the Monday evening commute. Sanders was operating the controls on the left, traffic-facing side of the tow truck when a white box truck in the right lane drifted to the right and struck the left rear corner of the bus at or near full speed, according to police. The impact carried the box truck along the left side of the bus and into the tow truck driver, who died at the scene.


Store Owner Killed During Robbery

February 7, BRIDGEWATER, NJ - Roy DeVoe, owner of Roy T. Jewelers, was shot and killed during a robbery attempt before the suspect turned the gun on himself, committing suicide. Authorities on Monday night were not releasing any other details about the 4:45 p.m. incident at jewelery store in the Bridgewater Towne Center on Route 202, said Jack Bennett, a spokesman for the Somerset County Prosecutor's Office.


Worker Killed When Pulled Into Machine

February 7, CENTER, CO - 53 year old Glen Pullins of Moffat died in an industrial incident at Power Zone Equipment in Center. According to witnesses at the scene, Pullins was working on a piece of machinery outside the shop and became entangled in the power take off portion of the machine. This happened very quickly. By the time the machinery was turned off by co-workers, the man had died. Investigators believe Pullins' coat sleeve was caught by the moving parts of the machine and he was pulled into it.


Oil and Gas Worker Dies of Unknown Injuries

February 7, BIG SPRING, TX - 32 year old Anthony Bryan Mora, possibly of Abilene, died Monday at a gas and oil site in Big Spring being run by Canada based Trinidad Drilling. Company officials have declined to comment on Mora's death or any circumstances saying Occupational Safety and Health officials are investigating.




Lumber Company Employee Killed in Industrial Accident

February 7, DeWitt, NY - 35 year old Thomas O. Pelton of Camden died at a DeWitt lumber business Monday. Pelton was changing blades on a cutting machine at B&B Lumber Co. shortly before 5:30 a.m. when another employee started the device, Chief Eugene Conway said early this afternoon. The worker did not realize he had started the machine and was unable to stop it before Pelton was fatally hurt, Conway said.


Worker Dies After Fall Off Forklift

February 6, HAZELWOOD, MO - 53 year old Donald C. Mitchell of St. Louis was fatally injured as he operated a forklift at Supervalu Distribution Center on Sunday. A witness told police he heard a commotion and was hit by falling boxes of soup. He then saw Mitchell being thrown off the forklift. He saw his body hit a rail and his head and back hit the floor. Mitchell was able to sit up right after it happened. He was talking and in pain when he was loaded into an ambulance, but he died of his injuries less than three hours later, police added.



Highway Patrolman Crushed When Cruiser Hit

February 5, OLD WESTBURY, NY - 44 year old Michael Califano, an officer for Nassaw County Highway Patrol was killed when his cruiser was hit from behind during a traffic stop. Califano, with lights running, had pulled over a vehicle for having insufficient lights when he was hit from behind by a flatbed truck. The impact propelled the cruiser under the stopped truck while the flatbed truck mounted the cruiser, trapping Califano inside. Califano later died at the hospital.



OH Officer Dies Due to Work Related Injury

February 5, STOW, OH - 47 year old Jonathan V. Bastock, Stow Patrol Officer and FOP Treasurer, died unexpectedly in the hospital after a surgery for a previous work related injury. Bastock was on injury leave for knee surgery a few weeks ago. The chief said he "tore up his knee on a call" in early January.

February 5, MOBILE, AL - 70 year old Elmer Hultquist, a farmer, was working Saturday to clear trees from his land when his tractor flipped over pinning him underneath. He died at the scene.

February 5, CHICAGO, IL - 60 year old William "Billy" King died while shoveling snow while working on Saturday. King suffered a heart attack while clearing the snow from around trash cans on his northwest side garbage route. King worked for the city for more than a decade.


Worker Dies After Fall Into Scalding Hot Pit

February 4, FOUNTAIN INN, SC - 29 year old Jared Lee Rousey of Greenwood suffered fatal burns early Friday when he fell into a superheated pit at a Laurens County plant, according to the coroner. The accident happened as Rousey was preparing a heat treatment that is used to treat metals which involved dumping salt into a pit that is heated to extreme temperatures, according to Nichols. Rousey was alive after the accident and managed to get himself out of the pit but died later as a result of burns.

February 4, PALATKA, FL - 29 year old Jason D. Wisham, a service technician for AT&T, was killed Friday morning when a van hit him as he was working alongside a road in Bradford County. Semi Driver Drowns After Crashing Into Creek

February 3, OWNINGSVILLE, KY - 56 year old Michael Dean Hall of Stauton, VA died when his tractor-trailer crashed into an eastern Kentucky creek. Hall's body was found Thursday evening in the creek about 100 yards from where the truck went into the water. Police told WLEX-TV in Lexington that Hall was on his way to Detroit with a load of automotive parts.

Worker in Power Truck Killed in Rollover Crash

February 3, ROCKAWAY, NJ - 51 year old Martin Harsch of Dingham's Ferry died as a result of a multi-vehicle crash. Preliminary investigation showed the crash occurred when the driver of a 2002 Subaru lost control of his vehicle, struck the center barrier then rebounded into the rear of the JCP&L bucket truck, police said. Both vehicles veered left, and when the truck hit the center divider, it toppled partially and struck an overhead road sign, dragging the truck onto the concrete barrier, he said. Harsch worked as a chief lineman and has been a Jersey Central Power &Light employee for 27 years.

January 3, CAMERON, PA - 49 year old Kevin Campbell of Washington has died of injuries he sustained at a well site owned by Consul. According to Chief Deputy Kevin Cecil, Campbell worked for an excavation company called 18 Karat, Inc. and was operating a bulldozer when the accident happened. "He was attempting to hook a chain to a vehicle that was stuck and the bulldozer rolled backwards and pinned him against the vehicle," said Cecil.

February 3, NORFOLK, VA - 51 year old Jerry Holton died Thursday morning in an industrial accident in Norfolk involving a city trash truck. Police spokesman Chris Amos said emergency dispatchers received a call just after 9 a.m. of a man "in physical distress" inside a city trash truck. Holton was found in the back of the truck and died at the scene. The accident is being investigated.


Tanker Explosion Kills One

February 3, PICABO, ID - 53 year old Craig Adamson of Carey, owner of Adamson's Inc., a fuel and oil company, was killed on a private ranch southwest of Picabo. Adamson and the driver of another truck were working in tandem to transfer fuel between two trucks, when the fuel trailer the other driver was hauling ignited and exploded. Adamson was on top of the fuel trailer when it exploded and was killed instantly. His co-worker was standing between the trucks and was knocked to the ground by the explosion and was treated at the scene.

January 2, HEREFORD, TX - 31 year old Pedro Mejia was killed when a tank containing cattle feed supplement collapsed on him at 200 Front St., said Deaf Smith County Justice of the Peace Karen Boren. The structure holding the tank collapsed on the tractor trailer that Mejia was loading. He was an employee of Magnum Minerals.

February 2, ROANOKE, VA - 60 year old Gregory Criswell of Vinton suffered some kind of medical problem around 7:00 a.m., while driving Roanoke County Schools bus #117. The bus ran off the road near King Street and Orange Avenue, hit a utility pole, then came to a stop in a pine tree farm. Criswell was also a pastor at Jordantown Wesleyan Church in Bedford County since 2005.


Industrial Accident Claims Life of Plant Worker

February 2, GALLATIN, TN - 42 year old Wiley Sherburne of Castalian Springs died of injuries he sustained Monday. Sherburne and another worker were severely burned after an electrical short triggered a flash fire with some materials within the plant. They were employed as maintenance workers for GKN/Hoeganaes Corp. The accident is under investigation.


School Principal Shot to Death in Office

February 2, PLACERVILLE, CA - 50 year old Sam LaCara, principal of Schnell School, was shot and killed in his office at the school. A custodial employee was arrested in the shooting, which was believed to have stemmed from a dispute between the two men, police said.


Taxi Driver Dies of Natural Causes

February 2, HOUSTON, TX - 59 year old Alvin Irving Jr. was found dead in his Yellow Cab vehicle Wednesday morning. Police say the cab was in the parking lot of a park. The driver had failed to arrive at a scheduled location to pick up a passenger.


Highway Worker Dies After Shift Plowing Snow

February 2, ST. CHARLES, IL - William 'P.T." Scardamaglia, an employee for Kane County Highway Department, died Wednesday after finishing a shift plowing the county roads. Scardamaglia had recently returned from plowing the roads and was refilling his truck with salt when he collapsed, according to Kane County Board Chairman Karen McConnaughay. Scardamaglia’s age and cause of death were not immediately known.


Corrections K-9 Officer Killed in Shoot Out

February 2, GRITNEY, FL - Col. Greg Malloy, a K-9 Officer for the Department of Corrections, was killed in a gun battle with an ex con who was being hunted for killing his parents. The ex con was also killed. "The colonel and other officers put themselves in harm's way to apprehend this dangerous suspect," said Holmes County Sheriff Tim Brown in an e-mail statement. "My heart goes out to all his family, friends and fellow officers."


Construction Worker Dies in Fall

February 2, SPARTANBURG, SC - 27 year old Joseph Daniel Hall died after he fell while doing renovation work at the South Carolina School for the Deaf and Blind in Spartanburg. Hall fell from a ladder while installing an electrical box at the school. He worked for a subcontractor, Bobby Jackson Electrical, doing renovations.


Worker Killed in Gas Well Pipeline Accident

February 1, GALETON, PA - 50 year old Joseph L. Craig, a gas well worker for West Penn Energy Services, was killed Tuesday when he was struck by a truck that was backing up on a gas well pipeline job in West Branch Township, Potter County. According to police, the truck driver did not see Craig and his death is ruled an accident.


Municipal Services Director Falls From Tower

January 31, WESTERN SPRINGS, IL - 41 year old William Nelson of Downers Grove deputy village manager and director of municipal services for the village of Western Springs, died Monday after an apparent fall from a village water tower. Deputy Police Chief Brian Budds said Nelson had been alone when he apparently attempted to climb the water tower. He was a hands-on sort of administrator, Budds said.


Truck Driver Dies in Wreck

January 31, THURBER, TX - 67 year old Jerry Albert Hildreth of Odessa was killed Monday night when his vehicle went through an interstate median into a creek bed near Thurber. It was raining and at 10:48 p.m., Hildreth's vehicle went through the median and into a creek bed where he was pronounced dead, the report said.

January 31, ATLANTA, GA - 45 year old Spencer Pass, an employee for the GA DOT H.E.R.O. (Highway Emergency Response Operators) Unit was struck and killed while helping a stranded motorist in the emergency lane of I-85 near Metropolitan Parkway in S.W. Atlanta Monday morning. He was struck by a passing utility truck towing a flatbed trailer with heavy equipment which first struck Pass's H.E.R.O. vehicle before striking him.

Town Worker Killed in Highway Rollover

January 31, NEEDHAM, MA - 62 year old John A. Barisano of Marshfield was killed this morning when he lost control of his pickup truck on Route 3 northbound in Pembroke, State Police said. He was driving his town-owned truck when it struck and flipped over the guardrail on the left side of the road and rolled over into the median strip, ejecting him, State Police said in a statement. The crash is being investigated.


Oil Field Worker Killed in Accident

January 30, NINE MILE CANYON, UT - 32 year old Cody Earl McMullin of Roosevelt, a water truck driver, had been removing water from a pit at a location in the Wells Draw area of Nine Mile Canyon when he climbed onto the pump jack at the well site for some reason, said Duchesne County Sheriff's Sgt. John Crowley. McMullin fell into the pump jack and suffered fatal crush injuries, Crowley said. The man's body was discovered by other oil-field workers when they arrived at the site Sunday morning.


Two Employees Killed by Rival Shop Owner

January 30, SAN FRANCISCO, CA - Qiong Han Chu and Feng Ping Ou, both 30 years old, were shot inside a souvenir shop at San Francisco's famous Fisherman's Wharf that likely stemmed from a business dispute involving another store, police said Monday. The suspect worked at a nearby souvenir shop, and police believe an ongoing business dispute was to blame for the violence, police spokesman Lt. Troy Dangerfield said. He declined to elaborate.



Prison Guard Strangled by Inmate

January 29, SEATTLE, WA - 34 year old Jayme Lee Biendl of Granite Falls, a corrections officer who had raised concerns about being the sole guard in the chapel of a Washington state prison was strangled there Saturday, and an inmate serving a life sentence is the primary suspect, authorities said Sunday. Biendl led religious programs at the prison and she was alone at the chapel Saturday night and was not carrying a weapon, as is typical for many corrections officers, they added.

January 29, AUGUSTA, GA - 45 year old Antonio "Tony" Jones died after feeling ill while on duty and leaving to drive himself to his doctor. While en route to the doctor he passed out at a traffic light and his vehicle rolled and bumped into another vehicle prompting a call for help.


Teenage Resident Stabs Shelter Worker to Death

January 29, LOWELL, MA - 34 year old Jose Roldan, was stabbed to death Saturday morning by a teenage resident of Lowell Transitional Living Center, the Middlesex district attorney’s office and Lowell police said in a statement. When police responded they found Roldan stabbed and the suspect holding the knife. The teenage suspect has been charged with murder.


Volunteer Firefighter Dies on Duty

January 29, PARKTON, NC - 57 year old James Walters died of an apparent heart attack while on duty Saturday. Walters, a volunteer firefighter for the Parkton Fire Dept., was doing paperwork in the station at night with another firefighter when he collapsed. Crews administered CPR and used a defibrillator on Walters and he was transported to Cape Fear Valley Medical Center, where he later died.


Corrections Officer Killed in Crash

January 29, NAPANOCH, NY - 52 year old Casmiro Pomales of Kerhonkson, NY died in a vehicle accident in the line of duty on Friday while transporting a prisoner. She worked at the Eastern Correctional Facility in Napanoch, Ulster County. According to the state police, another driver lost her focus while adjusting her car’s heater. Her car veered right and sideswiped the DOCS van. Both vehicles were forced off the road and down an embankment, but only the van overturned. The other driver, prisoner and another officer in the van were not seriously injured.


Heavy Fog Possible Contributor to Fatal Triple Semi Crash

January 29, PRINCE GEORGE, VA - 49 year old Lionardo S. Reina of Miama, FL died following a crash that involved three tractor-trailers Saturday morning. Police say heavy fog may have played a role in the wreck that disrupted traffic on Interstate 295 for more than 10 hours. A passenger in Reina's truck was injured as was the driver of one of the other semi's.


Store Owner Died After Fall Down Stairs

January 28, WHITING, ME - 55 year old Judy Stevens, store owner for the Whiting Store, was found Friday morning by an employee of the Whiting Store. Washington County Sheriff Donnie Smith says it is believed Stevens fell and died instantly when it appears Stevens was going up the stairs when she slipped and fell.


Tanker Driver Dies in Fiery Crash

January 28, SALIDA, CO - 78 year old Allen Hubbard of Denver has died in a fiery crash that closed U.S. 50 between Salida and Swissvale for about seven hours. Authorities say preliminary reports indicate he was heading east on U.S. 50 around 9 a.m. Friday when the tanker went off the road on a curve, overturned and exploded.


Worker Crushed to Death

January 28, CLERMONT, FL - 20 year old Delmas Ramsey is dead and another was injured after an industrial accident Friday at De Witt Excavating on State Road-50 near the Lake-Orange County Line in Clermont, Lake County Emergency Medical Services said. Delmas and the injured worker were operating a cement crusher machine when it appears it became clogged and something went wrong. Investigators believe the machine may have been running, trapping the workers inside. One employee was able to escape, but suffered severe injuries to his lower body. But Delmas was stuck inside the crusher and was killed.


Construction Worker Killed Falling From Scaffold

January 27, AUSTIN, TX - 29 year old Elvis Mendez of Honduras but living near Dallas died after falling nearly 20 feet from a construction scaffolding unit at an apartment complex being built on Crow Lane near Stassney Lane and South Congress Avenue in South Austin.


Worker Falls to His Death From Makeshift Lift

January 27, AUSTIN, TX - 35 year old Elvin Ferrufino, who was later identified, died Thursday after falling from a makeshift lift at a housing complex in South Austin. A second worker was taken to the hospital. A witness, who ran to the site after hearing loud noises, said the lift was made of wood and should have been metal.


Double Semi Crash Kills One

January 27, CARLSBAD, NM - 55 year old David Hugaboom was killed in a crash of two tanker trucks head-on. Initial investigation by the state police determined that the 2001 Mack, driven by Hugaboom, was traveling eastbound on the highway and the 2001 Peterbilt, driven by another driver, was traveling westbound. "For reasons unknown at this time, the two trucks collided head-on on the south side of the roadway after the other driver's vehicle was driven left of the center (of the road)," said Hooper. "The force of the collision caused catastrophic damage to both tanker trucks."


UPDATE to Identity of Miner Who Died in Accident

January 26, PINEVILLE, WV - 19 year old John Lester has been identified as the miner who died at Baylor Mining's Jims Branch No. 3A mine. Lester was a red hat miner, which means he was still training, but was able to work some positions without direct supervision. The accident involved a conveyor belt in the underground mine. No other details were immediately available.


Semi Driver Dead in Fiery Crash Named

January 21, MERRITT ISLAND, FL - 42 year old John Larchen Lynch of Lauderhill died when another vehicle hit his fuel tanker causing it to crash. Lynch was westbound of State Road 528 but after being hit by a pickup, the tanker, owned by Pipeline Transportation of Jacksonville, smashed through a concrete wall and landed on the eastbound side. Its trailer disconnected, fell off the bridge next to the Ford and blew up. The tanker was carrying 8,700 gallons of gasoline. Nearly all of the fuel burned, destroying the Beachline overpass.
