The Weekly Toll: Death in the American Workplace

Due to our commitment to remembering the worker's who have been lost in the United States, we continue with a list we received that we did not previously have.  Our hearts go out to all who have suffered a loss.  The next several posts will contain following months.

APRIL 2012

April 2 - Houston, TX - 34 year old Carlos Guillen died of an unknown cause. He was found by others laying on the floor deceased.  He was employed by Metroclean Commercial Building Services.

April 2 - 35 year old Daniel Yonashiro Jr., of Cushing, died on the job when he came into contact with an energized wire and was electrocuted. He was employed by Bloom Oil Field Electric Services.

April 2 - Pembroke Pines, FL - 28 year old Victor Sanchez died of thermal burns when the fuel being poured into a riding mower ignited/exploded.  He was employed by MDL Property Management, Inc.

April 2 - Manning, ND - 58 year old Hiroshiki Soto died of blunt force trauma when he was struck by pipes that slid off a truck, causing him to fall off and under the pipes. He was employed by A&R Construction.

April 3 - Brooklyn, NY - 22 year old Giovanni Santos Garcia died on the job while working for J.D. Construction and Contracting Corp. when the second story of a home collapsed, crushing him.


April 3 - San Antonio, TX - 55 year old Roy Garcia died while working when he fell from a ladder while painting.  He was employed by Spectrum Painting. 



 April 4 - Douglas, MA - 53 year old Martin "Marty" Gamache died when he came into contact with an energized wire and was electrocuted while working on machinery. He was employed with Classic Envelope Inc. 

April 4 - Gonzales, TX - 33 year old Jose Adrian Govea did on the job while working for Express Energy Services Operating, LP.  He was a derrick hand.  He slipped, lost his footing and fell approximately 50 feet to the steel rig floor.


April 5 - Florham Park, NJ - 57 year old Frank Nobile Jr. died at work of a heart attack. He was employed with Latierno & Son Corp.





April 6 - Simsboro, LA - 54 year old Carolyn Durbin was killed at work when she was struck by a forklift. She was employed by St Gobain Containers, Inc. dba Verallia Containers.



April 9 - Prudhow Bay, AK - 53 year old David "Davey" James of Fairbanks, was killed on the job when he became caught between a pipe elevator and wall on a Doyon Drilling, Inc. rig.  




 April 10 - Kansas City, KS - 62 year old Ivica Malic was killed on the job when he was crushed under a tractor axle that fell from an overhead hanger. He was employed by Harlan Global Manufacturing.

April 11 - Big Spring, TX - 57 year old Maria Patricia Cardenas was killed when the truck she was driving for One Source Industrial was struck by a train. The incident occurred at the Alon Refinery in Big Spring. 

April 11 - Andalusia, AL - 49 year old Melinda Ward, of Rose Hill, died while at work when she suffered an aneurism while cleaning. She was employed with Environmental Service Systems, LLC.


April 13 - Newark, DE - 31 year old Nicholas "Nick" Njenga of Middletown died on the job when he became pinned between a hydraulic liftgate and truck. He was employed by Savanna Auto Care, LLC. 



April 14 - Leonia, NJ - 19 year old Denis Duarte was killed on the job while working for D. Barro Landscaping & Construction, LLC.  He was working to smooth a section of ground under a large slab of concrete when the chain holding the slab broke and crushed Duarte.

April 16 - Fort Rucker, AL - 41 year old Anthony Brock was killed on the job when steel beams collapsed onto him. He was employed by Precision Erection Company, Inc.

April 16 - Hammondsport, NY - 51 year old Daniel Martin Sr., of Bath, died when he became trapped under an overturned tractor in a Keuka Lake Vineyards, Ltd.

April 16 - Mandeville, LA - Benito Cerna died while working when he was struck by lightning while walking to his work truck.  He was employed by Marcello Juarez dba Marcello Juarez Concrete.

April 17 - Dallas, TX - 64 year old Ozel Dismuke was killed on the job while working for Trinity Industries, Inc. He fell from a tractor and was run over by the rear axle. 


April 17 - Alva, OK - 25 year old Gaylon Taylor of Granbury, was killed on the job while working for Caprock Energy Services, LLC.  He was struck in the head by a flailing high pressure water hose and fell face down in water, drowning. 






April 18 - Millersview, TX - 77 year old Richard Stovall of Midland, drowned while working underwater on a concrete tower pumping station. Details as to how he became trapped underwater is not clear. He was owner of Stovall's Scuba & Travel in Midland.




April 18 - Galveston, TX - 19 year old Bryan Hunt of Houston, was killed on the job while working for WM Contractor. He was power washing a roof when he stepped on the roof skylight and fell to the ground. He was wearing a harness but had not attached the lanyard. 




April 18 - Catarina, TX - 34 year old Cory Glen Zablosky died on the job when he was struck in the head by an injector leg of a machine. He was employed by Thru Tubing Solutions. 

April 19 - Angleton, TX - Carlos Cuellar was killed on the job when he fell 22 feet through an unprotected roof skylight. He was employed with Juan Alajandro Alvarez, dba Alex's Remodeling.


April 20 - De Pere, WI - 58 year old Raymond Aschenbrenner was killed on the job when he was crushed under a falling boom crane while unloading equipment from a trailer. He was employed by Lunda Construction Company.




April 21 - New Orleans, LA - 44 year old Dan Gordon was killed on the job when he was crushed under a collapsed concrete wall of a business adjacent to his workplace. He was employed by Dan Gordon. 

April 21 - Capron, OK - Martin Kern was killed on the job when he fell from a derrick board. He was not tied off. He was employed by Dan D. Drilling, Inc.


April 23 - Oklahoma City, OK - 46 year old Peter Cleeland was killed on the job while working for Horizon Energy Services, LLC.  He was pinned between a hoist platform and substructure while attempting to pin the hoist in place. 

April 23 - Omaha, NE - 83 year old Robert Karasek died on the job when he was struck by a metal door of a roll-off container that was being offloaded from a truck.  He was employed by Teamwork, Inc.



April 24 - Portsmouth, NH - 26 year old Ben Harris, of Newington, was killed on the job when an over-pressurized keg ruptured into pieces, striking him in the head and chest. He was employed by Craft Brew Alliance, Inc. dba Redhook Ale Brewery. 





April 24 - Princeville, IL - 51 year old Harvey Smith of Chillicothe, died of injuries to his legs when he was struck and run over by his runaway garbage truck. He was employed by Wigand Disposal Company.





April 24 - Pueblo, CO - 58 year old Raymond "Pete" Duran was killed on the job when he was struck by and run over by a five-ton industrial forklift.  He was employed by Evraz Rocky Mountain Steel mill. 





April 25 - Liberty Corner, NJ - 25 year old Nick Rutkowski was killed on the job when he was struck in the head by a falling tree branch.  He was employed by Tamke Tree Experts, Inc.

April 26 - Cuyahoga Falls, OH - 46 year old Win Tun died of a heart attack while working at a window finishing machine. He was employed by Associated Materials LLC dba Alside.

April 28 - Magnolia, TX - Elroy Vargas-Contreras died while working when he fell from a height. He was standing on an unstable ladder on a scaffold. The ladder slipped, causing the ladder, scaffold and worker to fall.

April 28 - Washington DC - 32 year old Valdimir Polejaev was killed on the job while working for DSNK Corporation.  He leaned against a temporary balcony guardrail which gave way, causing him to fall 21 feet. 

April 28 - San Francisco, CA - 25 year old Richard Din died of a deadly bacteria, the same type of meningitis strain that he had been researching at the VAMC hospital.  He was employed by U.S. Dept. of Veteran's Affairs. 


April 30 - Willoughby, OH - 47 year old Drago Glavas of Mentor, was killed on the job while working for Glavas LLC.  He was working in a basket 20 feet from the ground when he touched a live wire.  That caused him to be thrown back in the basket which then came off the forks and fell to the ground. He struck his head on a metal table as he fell.



April 30 - Borger, TX - 50 year old Walter Dale Richardson of Dumas, was killed on the job when he was struck by a load-out chute when a securing pin failed.  He was employed by ConocoPhillips.





April 30 - Larose, LA - 51 year old Hector Hernandez died while working when he was found unconscious at the bottom of stairs leading to an employee housing unit. He was employed by North American Shipbuilding, LLC.

April 30 - Philadelphia, PA - 33 year old Andres Cano was electrocuted when a bucket he was hoisting up made an electrical contact. He was then knocked from the scaffolding. He was employed by Eight Fairmount Builders, LLC.

Robert Karasek died o April 30, 2012 not April 23, 2012 the date of the accident. He walked imto the hospital on his own from the ambulance. He was not treated for trauma and clots killed him in the ICU unit while hooked up to their monitors. They did not give him preventitive
stroke damage drug in the ICU at Lakeside Hospital in Omaha NE. He should have been treated as a trauma patient but was not. The clots could have been prevented from travelling throughout his system. Noone cares but me his wife. The love of my life should not have been swept under the rug and made to suffer. Lakeside covers all inciidences as inthe files there are many. Including an April 31, 2012 entry, as we all know there is no April 31st. Be careful what hospital you chose.

Mrs. Karasek,
I am so very sorry for the loss of your husband. I am also sorry we had the date incorrect, we obtained the information with date from an agency from a Freedom of Information request. I am truly sorry for how your husband was treated!
Again, I apologize for the incorrect date.
