The Weekly Toll: Death in the American Workplace

December 22, 2012 - Rushville, IN - 30 year old Eric W. McKinney was found dead at his place of employment, Intat Precision, Inc.  He was first reported missing when he did not come home after work.  Upon a search of the facility, he was found.  His death involved machinery but no other information is found.

December 27, 2012 - Alexandria, MN - 50 year old Tim Anderson of Evansville was killed on the job while working for Pope/Douglas Solid Waste.  He was crushed by a piece of machinery.

December 28, 2012 - Rockford, IL - 60 year old Terry L. Fogel suffered a fatal heart attack when he was returning to the Rockford, IL DOT maintenance facility. He was a highway maintainer.

December 30, 2012 - Hermiston, OR - 69 year old Freeman Thomason of Post Falls, ID was killed on the job when the vehicle he was a passenger in went out of control in icy road conditions and rolled three times.  Thomason suffered fatal head injuries.  He and the driver were employed with WalMart Associates, Inc.  

January 2, 2013 -Gage Park, IL - 57 year old Michael Kozel was shot to death during a robbery at his muffler shop.  He owner the Independent Muffler Shop for two decades.

January 2, 2013 - Jacksboro, TN - 40 year old Richard L. Haga died on the job from fall injuries when he fell from a mobile scaffold to the concrete floor.  He was employed by Huling Construction and Erections, Inc.

January 3, 2013 - Indianapolis, IN - 57 year old Michael J. Higgs died of natural causes while working for United Parcel Service.  The particulars of how he died are not listed.

January 3, 2013 - Mason City, IA - A medical helicopter crash near Mason City killed three people. They were on their way to another location to pick up a patient.  Those killed in the incident were:
44 year old Shelly Lair-Langenbau of Hanlontown. She was a nurse for Mercy Medical Center North Iowa.  48 year old Russell Phiel of Forest City was a paramedic, and 53 year old Gene Grell, home town unknown, worked for Med Trans Air Medical Transport.

January 3, 2013 - Columbia Tusculum, OH - 20 year old Edward Griffin of Hamilton was killed on the job when he fell from a height while working on a Columbia Tusculum construction site.  It is not clear who he was employed by.

January 4, 2013 - Hebron, KY - 67 year old Jessie "Tom" Lane of Burlington died on the job when he fell approximately 13 feet while installing stairs from a 2nd story landing.  He was kneeling and fell head first to the ground. He was self employed with Tom Lane Contracting.  

January 4, 2013 - Asheville, NC - 64 year old Phillip Allen Pace was killed on the job while working for American Cable & Rigging Suppliers.  He was using a core drilling machine to drill holes in precast concrete when the machine malfunctioned, causing the machine to fall off an 8 foot wall.

January 7, 2013 - Smithville, TX - 38 year old Thomas Blain Stewart of Buffalo Gap, was killed on the job when a tank being loaded onto a truck slid off, striking him.  He was an employee with Smith Pipe Co.

January 8, 2013 - Memphis, TN - 69 year old Gaylon Karnes  was killed on the job when he was struck by and run over by a John Deere front end loader after he had exited his truck.  He was waiting for his truck to be loaded when he was struck.  He was employed with Johnston Trucking, Inc. 

January 9, 2013 - North East, MD - 42 year old Luis E. Tadeo of Bowie, MD and 41 year old Victor D. Jackson of Washington DC were killed on the job when they were struck by a motorist while working in a highway work zone that was clearly marked with orange cones.  The men worked for Pessoa Construction Company.

January 9, 2013 - Kokomo, IN - 54 year old Rex D. Markham died on the job of cardiac causes after he collapsed and hit his head on a piece of office equipment.  He worked in the Advance Manufacturing Engineers building of Chrysler Group.

January 9, 2013 - Bridgeland, UT - 62 year old David Broome of Vernal was killed on the job while working for RJ Taylor-Wyatt LLC.  He was using a pipe bending machine and it began to slip on an icy surface.  He tried to prevent the machine from moving but it fell into a trench, as did Broome, and he was struck on the head by the pipe.  

January 10, 2013 - Wayne, NJ - 35 year old Heidi Cullere of Paterson was killed on the job when she was struck by a raised, asphalt filled, dump-truck bed that malfunctioned and collapsed.  She was employed by H. Williams Paving in Wayne.

January 17, 2013 - North Charleston, SC - 71 year old Howell Jeffcoat died of a coronary blockage after he suffered trauma while at work for County of Orangeburg Public Works.  He was working in a small hut at a recycling facility when a strong storm with rain blew open the door and as it was slamming shut it struck Mr. Jeffcoat.  He was knocked out of the door, suffered broken bones and was discovered on the ground by citizens.  He was taken to the hospital where he died.

January 19, 2013 - Chester, VA - 29 year old Jeffrey Lockhart, Jr. suffered a possible heart attack while working for Integrity Staffing Solutions when he was found on the floor unresponsive.  There was no sign of injury to Mr. Lockart.  

January 21, 2013 - Chicago, IL - 57 year old Theresa Burns was killed on the job, and a co-worker injured, when they were struck by a hit and run motorist while attempting to remove a student from the roadway.  Burns, who was employed by Northern Illinois Academy, went after an unstable teen who sat down in the middle of the road. When she and a co-worker attempted to remove the student, they were struck.

January 21, 2013 - Markham, IL -  55 year old Norman Wallace died of a probably heart attack while working for the Illinois Department of Transportation.  He was feeling sick and laid his head on his desk.  He was later found unresponsive.

January 21, 2013 - Apex, NV - 54 year old Kenneth Kurakis of North Las Vegas was killed in a mining incident while working at the Lhoist North America quarry.  It is unclear how he died.

January 22, 2013 - Pittsburg, PA - 59 year old Harold E. Taylor, Jr. was killed on the job when he was electrocuted while working at Carnegie Mellon University for Eaton Corporation.

January 22, 2013 - Cawtaba, SC - 39 year old Samir Storey died, and his co-worker injured, when they were overcome by noxious fumes while cleaning inside a tank at Resolute Forest Products.  The co-worker, Storey's cousin, said he questioned the safety training they received before performing the work.  They were contract workers for Tradesmen International.

January 22, 2013 - San Francisco, CA - 45 year old Youjun Wang of Milbrae was killed on the job when he was pinned between the limousine he had been driving and another vehicle.  The limo was apparently not in park and began to roll.  Wang was pinned as he was attempting to stop the vehicle.  He was employed with Sherwin Limo Service, Inc. 

January 23, 2013 - Canton, GA - 21 year old Josh Roberts of Woodstock was killed on the job when the upright mast of a forklift he was working on collapsed onto him.  He was employed by Mauldin's Body Shop at Little River Marina.

January 25, 2013 - Memphis, TN - 49 year old Hong Dao died on the job while working for Quality Trailer Services, LLC.  He was a mechanic changing a tire of a car on a trailer.  The truck the trailer was attached to somehow moved and Dao was run over by the tires of the trailer.  

January 27, 2013 - Smyrna, TN - 50 year old Martin O'Connell of Murfreesboro was killed on the job when he was pinned by his truck while working at the Nissan Plant in Smyrna.

January 29, 2013 - Fort Wayne, IN - 61 year old Leroy J. Hammond of New Haven was killed on the job when he fell through an unguarded opening and fell approximately 7 feet to the ground. He was employed with Rea Magnet Wire.  

January 31, 2013 - Renville, MN - 30 year old Erv Knuston of Sacred Heart was killed on the job when he was struck and run over by a payloader while working for Southern Minnesota Beet Sugar Cooperative.
