The Weekly Toll: Death in the American Workplace

As always our hearts go out to the family members of the men & women who lost their lives while working to support their families.

June 12, 2013 -  Swedesboro, NJ - 37 year old Jason Leffler, a well known Joe Gibbs Racing team driver, was killed in a horrific crash of his Sprint car at Bridgeport Speedway in New Jersey. 

August 1, 2013 - Dugout Lake, OR - 60 year old John "Big John" Hammack of Madras was killed when he was struck by the top of a tree which fell while he and another worker were trying to clear an area burned by a wildfire near Dugout Lake.  Hammack was a firefighter working for R & K Water Services of Bonney Lake, WA battling the Incident No. 398 fire in the remote Mt. Washington Wilderness Area.

August 2 - Evansville, IN - 31 year old Stephan Jennings died on the job after being electrocuted and then falling from a second story as he was doing roofing work.  It is unclear who he was employed by.

August 2 - Utica, NY - 56 year old Kelvin Sperry of Herkimer, NY was killed on the job when he was crushed between a dumpster and compactor while working for Oneida-Herkimer Solid Waste Authority.

August 2 - Kenley, NC - 59 year old Mike Sabiston of Troy was killed when he fell from a lift gate of New Day Motorsports hauler as he was helping to unload a car at the infield track before a race at Southern National Motorsports park.  He was a team member of New Day Motorsports.

August 3 - Lumberton, MS - 56 year old Jim Horak, Jr. of Pensacola, FL died when an equipment failure occurred during a tandem skydive near Lumberton with a student, who survived.  Horak was an instructor for Emerald Coast Skydiving Center in Elberta, AL. 

August 3 - Grand Prairie, TX - 59 year old James Anthony Long of Clarksville, TN died in a fiery crash when his semi truck crashed through a guardrail and plummeted to a lower section of the President George Bush Turnpike below.  It is unclear why the truck left the roadway or who his employer was.

August 4 - Abbottsville, PA- 63 year old Kramer Williamson of Palmyra, NY, a Sprint Car race driver, died from injuries he received from a crash during the United Racing Company 358/360 Sprint Car Challenge at Lincoln Speedway.  

August 4 - St. Charles Parish, LA - 41 year old Jeff Watson, an officer for the St. Charles Parish Sheriff's Office, was killed in a car crash as he was responding to a call for assistance from another officer. 

August 5 - Blue Springs, MO - 28 year old Gusteno A. Santamaria of Independence was killed on the job when a four-post vehicle lift tipped over and fell on him as he and other employees were trying to move it.  He was employed by Highline Auto Shop in Blue Springs.

August 5 - Lowville, NY - 55 year old Willard Moser of Deer River was killed on the job when he was struck in the head.  The MJL Crushing employee was trying to free an excavator that was stuck in a piece of machinery when it came loose, striking him in the head. 

August 5 - Stanton, TX - 25 year old David Edward Oliveira of Turlock, CA and 26 year old Christopher Geoffrey Breed of Moscow, ID were killed on the job when they fell from a cable suspended to a helicopter.  Both men, employees for Haverfield Aviation, were working on power lines when the helicopter pilot flew over a finished power line and the power line cut the cable that was holding Oliveira and Breed. 

August 5 - Hettick, IL - 66 year old Ronald D. Releford died at hospital from injuries he received when his farm tractor flipped, pinning him under it.  He lay under the tractor approximately 10 hours before being found by friends who went to find him. 

August 5 - Middletown, OH - 72 year old Charles Groh of Fairfield died of asphyxia when he was buried in grain at an unregulated family owned farm.  OSHA has no jurisdiction because the farm employed less than 10 workers.

August 5 - Casper, WY - 38 year old Jason Thompson of Gillette was killed on the job when he was struck by a dump truck while working at a City of Gillette construction site.  It is unclear who Thompson was working for.  The dump truck driver was arrested for driving impaired.

August 6 - Middletown, OH - 42 year old Tim Taylor of New Carlisle died of asphyxia inside a silo when he became buried under 8,000 pounds of fly ash (used in making cement).  He was employed by Central Ready Mix.   

August 6 - Portland, OR - 19 year old Jesse Trader was killed on the job when the water truck he was driving overturned on an embankment of a rural road.  He was a fire crew member working the Big Windy Complex of fires in Southwest Oregon.  It is unclear who he specifically worked for. 

August 6 - McCracken County, KY - 47 year old Deputy Chad Shaw collapsed and later died of a medical issue while working with school staff as they prepared for the new school year.  He was employed by McCracken County Sheriff's Office.

August 6 - Stamford, CT - 59 year old Francisco Dossantos of Naugatuck was killed on the job when he was struck by a front-end loader as he was walking.  He was employed by O & G Industries.

August 7 - Tyler, TX - Joel DeLeon of Houston died on the job when a Bobcat he was operating backed into a concrete pit full of damp sand.  The machine landed on top of him.  He was a contractor for D.H. Griffin working at Tyler Pipe.

August 7 - Evart, MI - 23 year old Zach Elden Adams was electrocuted as he worked to clear trees after a recent storm.  He was an employee for Trees Inc., contracted to Traverse City Light & Power when he came into contact with a high power electric line.  

August 7 - Waynesboro, VA - 37 year old Payman Biazarikari , only in the United States from Iran for two weeks, died on the job after he fell approximately 160 feet from a tower at nTelos Headquarters.  He was a private contractor working for Summit Tower Construction.

August 7 - Albany, NY - 61 year old Lance Gayton was injured and later died after being struck by a piece of heavy equipment at Youmans Flat garbage transfer station.  He was a New York Parks Department employee working at Bear Mountain State Park.  

August 7 - Milwaukee, WI - 44 year old William "Bill" Melendez, Jr. was shot and killed during a robbery attempt while working his shift at a Family Dollar store on the South side of Milwaukee.

August 8 - Athens, GA - 50 year old Lt. William Morgan collapsed and died of a probable medical event inside the Clarke County Sheriffs Office jail intake facility. 

August 8 - Pharr, TX - 51 year old Sgt. Jorge Luis "JL" Garcia died as the result of a two vehicle crash while en route to a work related meeting.  He was employed with the Pharr Police Department.

August 9 - Beulaville, NC - 54 year old Richard Maready died after being pinned under his farm tractor.  He was attempting to pull metal around a tree stump with a chain when the front end of the tractor came up and rolled over. 

August 9 - Buckeye, AZ - 48 year old Herme Escobar was the target of workplace violence when a co-worker shot and killed him on the farm they worked at.  The co-worker who shot Escobar was arrested on site.

August 11 - Midland, GA - 51 year old Glenn Nunley was volunteering to cut down trees at a local church when he was struck by a tree limb as it fell.

August 12 - Coates, NC - 49 year old John Dailey of Silvester, GA was killed on the job when he fell approximately 200 feet from a cell tower.  He was employed by Transmit PM LLC of Duxbury, MA. 

August 13 - Palatka, FL - 14 year old Blake Bryant was killed on the job when he fell approximately 50 feet from a tree.  Apparently he cut through his safety harness.  He was employed by John Wilkes Tree Service.

August 13 - Minden, NE - 55 year old Robert Honomichl died and a co-worker injured when they were overcome by toxic fumes while working inside a sewer confined space while trying to clear a blockage.  He was employed by Minden City Water & Sewer.

August 14 - Wolf Creek, OR - 60 year old Kathleen Roberta Wilson of Trail was killed on the job when she was struck by an arrow board trailer after the trailer of an 18 wheeler struck the board. She was working the road construction project for Knife River Corporation.  

August 14 - New Prague, MN - 28 year old Robert Fenstermacher of Oologah, OK was killed and a co-worker injured when they fell 50 feet into a hole being dug for a power pole.  The men were tethered to a strap suspending them when it somehow broke, causing the fall.  He was employed by Aldredge Electric of Libertyville, IL on the CAPX2020 transmission line construction project.

August 15 - West Lafayette, OH - 27 year old John E. Roberts of Caldwell, OH died on the job of head injuries after he slipped and fell nearly 50 feet from an oil rig.  The rig is owned by and Roberts was employed with FMS Drilling.

August 15 - Adamsville, TN - 44 year old Joseph S. Jones of Jackson was killed on the job when he lost control of his TN Dept. of Transportation dump truck and it rolled down an embankment and struck a tree.
