The Weekly Toll: Death in the American Workplace

This post is dedicated to all the miners, coal and mineral, who were lost in the second half of 2013. 

COAL MINE DEATHS 2013

July 3 - New Eagle, PA - 87 year old Samuel V. Vignoli held an unknown position for Robert E. Murray's Maple Creek Mining Inc., Maple Creek Preparation Plant at the time of his death. He was operating a skid steer type of machine at Maple Creek Preparation Plant's Ginger Hill #1 Pond when the machine traveled into the compound's waters as he was mowing an embankment.

August 6 - Holmes Mill, KY - 56 year old Lenny D. Gilliam was a Continuous Miner Operator for Arch Coal, Inc., Lone Mountain Processing, Inc., Huff Creek No. 1 mine at the time of his death.  He was killed when a coal/rock outburst occurred, causing the fall of a rock wall, fatally injuring him.

August 16 - Wright, WY - 24 year old Jacob R. Dowdy was a Utility Person for Arch Coal Inc., Thunder Basin Coal Company LLC, Black Thunder mine at the time of his death.  He was killed when an electric shovel lost propel capabilities, rolling backwards into a pickup truck he was operating.

October 4 - Cameron, WV - 62 year old Roger R. King was a Longwall Maintenance Coordinator for Consol Energy, Inc., McElroy Coal Company's McElroy Mine.  He was working with powered haulage when a series of events occurred which caused a piece of equipment to break loose, striking him in the back of the head.

October 5 - Carmi, IL - 47 year old Robert E. Smith was a general laborer driving a battery golf cart for Alliance Resources Partners LP, While County Coal, LLC, Pattiki mine as the time of his death.  He was operating the powered cart when it overturned on top of him. 

October 6 - Point of Rocks, WY - 44 year old Mark C. Stassinos was a heavy equipment operator for MidAmerican Energy Holdings Company; IDACOR, Pacific Minerals dba Bridger Coal Company's Jim Bridger Mine at the time of his death.  He died when the dozer he was operating went over the edge of a highwall he was building. 

October 11 - Petersburg, IN - 59 year old Larry Schwartz was a Shuttle Car Operator for Donald Blankenberger, Five Star Mining, Inc. Prosperity Mine at the time of his death.  He was killed when he was pinned between a shuttle car and a coal rib on the blind side of the shuttle car.

November 4 - Macedonia, IL - 36 year old Dallas D. Travelstead was a Longwall Chief for Coalfield Transport, Inc., M-Class Mining LLC's MC #1 Mine at the time of his death.  He was killed when a solid piece of coal and cap rock rolled into him as he was shoveling coal and loose rock.

November 23 - Beallsville, OH - 32 year old Ryan Lashley was a Shieldman for Robert E. Murray, American Energy Corp's Century Mine at the time of his death.  He was killed when he was struck by a high pressure hydraulic hose or fluid when it became stuck between a piece of equipment and the mine floor.

Metal Miner Deaths 2013

August 5 - Lowville, NY - 55 year old Willard Moser was a Plant Manager for Michael J. Lyndaker; MJL Crushing LLC at the time of his death.  He died when he was struck by a piece of an excavator bucket that dislodged and struck him.

September 16 - Huntsville, MO - 58 year old David A. Gully was a Truck Driver for Summit Materials, LLC; Con-Agg of MO, LLC at Huntsville Quarry at the time of his death.  He died when the truck he was driving traveled through a berm near the quarry and over a highwall.

September 18 - Albany, KY - 56 year old Lonnie E. Ferrill was a Front End Loader operator for Roy Beard; Gaddie-Shamrock LLC at Caldwell Quarry at the time of his death.  He was engulfed by material inside a hopper as he was attempting to remove a rock from the pug mill hopper. 

September 19 - Monroeton, PA - 31 year old Timothy Farr was a Laborer for Jan Tuttle/Charles Gold; T.G. Mountain Stone at Northrup Quarry at the time of his death.  He was operating a truck hauling water tanks on a steep roadway when the truck overturned.

October 17 - Lebec, CA - 52 year old Fernando Rivera was a Journeyman Electrician for Vicat S A; National Cement Co of CA Inc. at Lebec Cement Plant at the time of his death. He died after falling from a ladder as he was working to pull SO cable into a cable tray. The tray mounting bracket broke loose from the wall causing it to strike and knock over the ladder.

November 7 - Lithia Springs, GA - 46 year old Carl J. Clinton was an Equipment Operator for Vulcan Materials Company; Vulcan Construction Materials, L.P. at Lithia Springs Quarry at the time of his death. He was operating a haul truck that went off the road over an embankment and overturned.

November 17 - Ouray, CO - 59 year old Rick L. Williams was a Shift Supervisor for Rory Williams/James Williams; Star Mine Operations, LLC at Revenue Mine at the time of his death. He died of carbon monoxide exposure after entering a mine to look for another miner who did not return after entering the mine where an explosive had been previously detonated. 

November 17 - Ouray, CO - 33 year old Nicholas K. Cappanno was a Powderman Trainee for Rory Williams/James Williams; Star Mine Operations, LLC at Revenue Mine at the time of his death. He died of carbon monoxide exposure after entering a mine where an explosive had been previously detonated. 

November 22 - Salem, KY - 33 year old Bobby Crume was an Electrician Foreman for W. Scott McGeorge; Pine Bluff Sand & Gravel Company at Cumberland River Quarry at the time of his death. He was working in an electrical enclosure with cable for a new pump when co-workers saw him fall to the ground.  He was injured on November 18th and died later at hospital.

December 4 - Junction City, KS - 63 year old Stephen W. Hetzler was a Lead Man for Bayer Construction Company, Inc. at Kansas Falls Quarry at the time of his death. He was struck by fly rock after he detonated a blast at the quarry.

December 6 - Ishpeming, MI - 61 year old Gary A. Hytinen was a Reagent Handler for Ontario Tilden Company/Cliffs TIOP Inc.; Tilden Mining Comapny LC at Tilden Mine at the time of his death.  He died from a fall to the ground as he worked atop a railroad tanker car.

December 10 - Ledbetter, KY - 27 year old Dustin Burnham was a Deckhand for HMT Holdings, Inc; Hunter Sand & Gravel, LLC at Dredge IV (5) at the time of his death.  He drowned after falling into water from a barge as he was working on a dredge and stepped onto the barge.

December 13 - Junction City, GA - 53 year old Paul D. Barnes was a Utility Worker for Brown Brothers Sand Company at Wittichen Stephen Plants at the time of his death.  He died after being engulfed when the bank of a pit he was standing on collapsed under him.
