The Weekly Toll: Death in the American Workplace

We never want to let one worker go unnoticed and so do our best to acknowledge them as we are made aware of another tragedy. Our hearts and thoughts are always with the families left behind. If there is anything we can do please let us know!

March 11, 2014 - Middleton, CT - 67 year old Robert Williams was overcome and died from carbon monoxide fumes while operating a forklift inside a closed up warehouse. More than likely because it was cold outside, the warehouse doors were closed which led to the fumes building up. He was employed by Custom Carpets.

May 23, 2014 - Muscatine, IA - 29 year old Torkor Dahn died on the job of an unknown cause while working for Fabricator's Plus, a metal fabrication business.

May 27, 2014 - Aston, NJ - 20 year old Jacob S. Blonsinski was killed on the job when a dump truck he was operating overturned. It is unclear who he was employed by but he was working behind Kokes Family Builders.

May 28, 2014 - Ashford, WA - 38 year old Matthew Hegeman of Truckee, CA and 28 year old Eitan Green of Seattle, WA were both guides for Alpine Ascents International, Inc. when they died, along with four climbers, while leading a tour on MT Rainier in Washington State. It appears an avalanche occurred, causing their deaths.

May 29, 2014 - Rapid City, SD - 74 year old Alvin Howe died after the dump truck he was operating tipped over. He was raising the bed to unload gravel but the heavy load caused the truck to tip on its side. It is unclear who he was employed by.

May 29, 2014 - Chenango, NY - Christopher G. Skinner was a New York State Trooper on a routine traffic stop when a motorist from Florida intentionally drove into him, killing him instantly. It is unclear what caused the motorist to do what he did.

May 29, 2014 - Black Creek, WI - 72 year old Thomas Raymond Fischer died in an incident on his farm.

May 29, 2014 - Edgefield, SC - 62 year old John W. Varner died on the job after he fell from a forklift he was operating and then was run over by it. He was employed by Tides International Lumber yard.

May 30, 2014 - Fife, WA - 58 year old Robert Hoquist of Tacoma was killed on the job when his forklift rolled off the end of a loading dock and landed on top of him as he fell off. He was working for Cogdall Construction Co, Inc dba Contract Furnishings Mart.

May 30, 2014 - Corrigan, TX - 56 year old Charles Wayne Kovar of Moscow, TX has died after he was injured on 4/26/2014 with burns when a fire broke out at the Georgia-Pacific plywood plant which injured seven.

May 30, 2014 - Windsor, CT - 51 year old Danny King was killed on the job when he was pinned between a trench box and backhoe while working inside a trench. He was employed by King Construction of Bloomfield, owned by his father. 

May 30, 2014 - Jacksonville, FL - 26 year old Jose Lopez of Axton, VA died on the job after falling from the roof of a hanger at Cecil Field. He was attempting to move from one section of the building to another and had untethered his harness.  He was employed by IMC Erectors for Flightstar Aircraft Services.

May 31, 2014 - Cleveland, OH - 51 year old James Eiflander died on the job after falling from the exterior of the Anthony J. Celebreeze Federal Building. He was installing windows on the building when he fell. He was a member of Cleveland Local 17 (Ironworkers) hired for a subcontractor National Enclosure Company of Michigan.

June 1, 2014 - West Pikeland, PA - 67 year old William McGuigan of Ardsley was an equipment operator working in a closed lane of construction for Pennsylvania Turnpike Commission when he was struck by a tractor trailer driver who entered the closed lane and struck he and a coworker. 

June 1, 2014 - Carlisle, PA - 52 year old Jody Rhoads died on the job when the pallet truck she was operating crashed into some shelves at Amazon Fulfillment Center warehouse in Carlisle.

June 1, 2014 - Pompano Beach, FL - 55 year old Robert Elliott was died and his coworker injured after they were struck by lightening while working on the roof of Joey Accardi Jeep Dealership. It is unclear who he was employed by.

June 2, 2014 - Saginaw, MI - 31 year old Andrew Beckman was killed on the job when he was crushed under 3,100 pounds of steel rods that shifted as he was loading them onto a flatbed trailer. He was pinned up against a steel shelf. He was employed by Peerless Steel.

June 2, 2014 - Rockbridge, IL - 68 year old Lee Whitehead died after an incident involving his farm tractor.

June 2, 2014 - Central, SC - 56 year old Jerry Wayne Kelly died on the job after he fell from a ladder while painting in a home from the second story. He was a self employed painter.

June 2, 2014 - Clinton, AR - 51 year old Hubert Keith Moore of Chester and 40 year old Ricardo Trochez of Atkins were killed and 21 others injured when a loaded log truck blew a tire, spilling its load onto those workers as it overturned. They were working an Arkansas State Highway Transport work site but it is unclear who they were employed by.

June 3, 2014 - Shaw, MS - 50 year old Henry Butler of Rosedale was killed on the job when his Mississippi Department of Transportation truck was struck from behind by a tractor trailer driver, causing a fiery crash.

June 3, 2014 - Great Falls, MT - 38 year old Barry Ladd died on the job from blood loss after he stepped into an auger and his leg was severed. He was working for Montana Specialty Mills. 

June 3, 2014 - Middletown, OH - 24 year old Sarah Rhoads of Miamisburg died of injuries she suffered on June 1 when she inadvertently walked into a spinning plane propeller.  She was employed by Start Skydiving of Middletown.

June 4, 2014 - Salt Lake City, UT - 58 year old Ronel Christensen of Burley, ID was killed on the job when his milk tanker overturned on a Utah highway.  It is unclear who he was employed with. 

June 4, 2014 - Queens, NY - 26 year old Thomas Patane of Rossville died on the job after he fell down a five story elevator shaft while working on construction of PS 313 school for the New York School Construction Authority.  It is unclear who specifically he was working for. 

June 5, 2014 - Florence, AL - 39 year old Christopher Lane Williamson died on the job when he was electrocuted while attempting to restore power after storms in the area.  He was employed by Florence City Electric Department.

June 5, 2014 - Hiram, GA - 42 year old Ron Crist, owner of Crist Roofing and Construction, was killed on the job when a skid steer he was operating malfunctioned, causing his death.

June 5, 2014 - Newark, NJ - 50 year old Jorge Giraldo of Elizabeth drowned inside an overturned crane he was operating when it flipped into the river. He was employed by Global Development of Belleville.

June 6, 2014 - Wanamingo, MN - 26 year old William Pohlman was killed when he became pinned under a piece of hay/farm equipment near Dodge Center Airport. He was employed by Durst Brothers Dairy of Mantorville.

June 6, 2014 - Corrigan, TX - 58 year old Kenny Morris of Lufkin has died after being injured in an explosion at the Corrigan Texas Georgia-Pacific plywood plant. He was the second person to die in the incident. Charles Wayne Kovar, 56, of Moscow died on May 30th.

June 6, 2014 - Woodbridge, VA - 23 year old Junior Montero of Hyattsville was shot and killed by the owner of a truck as he was towing it for repossession. After being shot his truck rolled into a water-filled ditch where police found it. The name of the tow company is not known. 

June 7, 2014 - Indianapolis, IN - 45 year old Harry Bigg, a store clerk for Mr. Fuel and Kicks 66 died after being shot in the head on June 4th during a robbery.

June 7, 2014 - Chicago, IL - An unidentified man died on the job when he fell about 35 feet through an opening on a roof he was working on. He was employed by Donaly Roofing and Construction.

June 7, 2014 - Jacksonville, FL - 43 year old Stuart Carson was shot and killed while sitting inside his Gator City Taxi after being robbed.

June 8, 2014 - Las Vegas, NV - 42 year old Alyn Beck and 32 year old Igor Soldo, both Las Vegas Metro officers were murdered in an ambush as they sat in a restaurant having lunch. Both were shot by a husband and wife who were later shot and killed by fellow officers inside a store across the street from the restaurant.  A innocent man was also shot by the wife as he tried to stop the pair inside the store, likely saving other people. His name was Joseph Robert Wilcox.

June 8, 2014 - Union Beach, NJ - 54 year old Robert "Bob" Meyer suffered a fatal heart attack while battling a blaze in the borough.  He was a member of the Union Beach Host Fire #1.

June 8, 2014 - Hoffman Estates, IL - 48 year old Melba R. Farr of Elgin was a cab driver for an unknown company when her taxi was struck from behind by another motorist causing the cab to burst into flames. She was unable to escape.

June 9, 2014 - Prineville, OR - 77 year old Gordon Stroebel was a farmer who lost his life while attempting to load bales of hay onto his truck. The bales shifted and fell from the truck, striking Stroebel.

June 9, 2014 - Moab, UT - 51 year old Jay L. Stephens died on the job when he was crushed between a lift gate and back of a garbage truck. He was at a transfer station when the driver of a garbage truck had just unloaded at a place where a gate was raised. As he moved forward the gate lowered, trapping Stephens.

June 9, 2014 - Meridian, CT - 26 year old Frank Cline of Dover Plains, NY was struck in the head by a tree limb. His coworker had tied the limb as a precaution but it began swinging and struck Cline. They were employed by Tree Inc., of Houston, working for Connecticut Light and Power who are having trees removed after storm damage.

June 10, 2014 - Knoxville, TN - 42 year old Larry Releford of Powell has died due to burns he received in a furnace fire. He was injured, along with five others on May 15th when a fire broke out in a furnace at the Gerdau Ameristeel plant in Knoxville.

June 10, 2014 - Long Grove, IL - 51 year old Brian Pierce of Milwaukee, WI was killed on the job when the tractor trailer semi he was driving crossed a center line and crashed into a dump truck for an unknown reason.  His employer is not listed.

June 10, 2014 - Freeport, ME - 59 year old Scott MacDonald of Limington was working with a landscaping crew to unload materials when the forks of skid steer loader, holding granite steps, suddenly came down on top of him. He was employed by Coastal Landscape, Construction and Snow Services of Cumberland after owning his own business for 30 years.

June 10, 2014 - Weston, FL - 45 year old Ernesto Rodriguez of Hialeah was working in a large water-filled ditch on an underwater pipe when he did not resurface. His coworker became alarmed and went into the ditch to find Rodriguez who later died at hospital. His employer is not listed.

June 10, 2014 - Mint Hill, NC - 46 year old James Kirk Mitchman died on the job when he was buried in a trench collapsed. He had been lowered into a 15 foot deep trench for a sewer line installation to continue digging with a shovel. The trench had not been properly shored. He was employed by One Grade, Inc of Indian Trail.

June 10, 2014 - Brooklyn, NY - 22 year old Gustavo Tapia died after being pinned against a wall by a motorized vehicle. He was using a walk-behind electric forklift, trying to push a Yale forklift with a non-working rear wheel to the store’s rooftop parking lot, when the Yale forklift rolled backwards, pinning Tapia to the wall of the building. He was employed by Moisha's Discount Supermarket.

June 10, 2014 - Indianola, MS - 29 year old Tarares Williams of Itta Bena was killed on the job after falling from an industrial lift. He was working from the top of a scissor lift, or manlift, in the freezer warehouse section of Lewis Grocer Division of Supervalu when he fell about 18 feet.

June 10, 2014 - Las Vegas, NV - 59 year old Ricky J. Robinson died on the job after being pushed out of a third story window when he couldn't get out of the way of a motorized cart that began moving. He was crushed under the machine as it fell down on top of him.  He was working for the MGM Grand Hotel.
