Golf club worker killed on job


ST. LOUIS, SC — A 22-year-old man has died after a trailer fell on top of him at a St. Louis golf course where he worked. The St. Louis Post-Dispatch reports the man was driving a small utility vehicle about 9 a.m. Saturday at Quail Creek Golf Club when he ran into a fifth-wheel trailer that had been lifted on a jack. The trailer fell on the man and crushed him. He died


Road worker struck, killed on I-95 in Md.

WHITE MARSH, Md. - A Maryland Transportation Authority worker died Wednesday after a car hit him while he was picking up debris on southbound I-95. The MDTA says Gregory Anthony Bauer, 37, of Abingdon was pronounced dead at Franklin Square Hospital. State police say Bauer, a facility maintenance technician, was picking up road debris between mile markers 67 and 74 about 2 p.m. when a Mazda lost control and hit the median. Police say the car slid down the median and hit Bauer, throwing him into the northbound shoulder. Thirty-five-year-old Kathryn Eileen Cote of Salisbury, who was driving the Mazda involved in the accident, was taken to Franklin Square Hospital with injuries that were not life-threatening.


Employer Shoots Employee in Kingsport

Johnson City,TN - An Elizabethton man died over the weekend after police say he was shot by his boss. The Sullivan County Sheriff's Department received a call around 9 pm Friday night on a shooting at Kingsport Transfer on Moreland Drive. Investigators say the owner, Samuel Ireson and employee James Guinn were arguing when Ireson shot Guinn in the chest. Guinn was taken to Holston Valley Hospital and died Saturday morning. Investigators say Ireson was released to allow time to determine exactly what led up to the shooting.


Report explains death at Greenheck Fan

Wausau, WI — A man who died Saturday while working on a machine at Greenheck Fan Corp. in Schofield made a similar repair on the same machine a week earlier, a co-worker told police. Michael Damask, 42, of Hatley, died of compression asphyxia or suffocation, Marathon County Medical Examiner John Larson said this afternoon. Damask was killed when part of a hydraulic press dropped on him while he was changing a bearing, Larson said Larson determined the cause of death after conducting an autopsy this morning. The weight of the machine on his body prevented him from breathing or calling for help, Larson said. Damask was employed by Greenheck for approximately two years and worked nine months in the maintenance department. Damask is married and has three children.


Contra Costa Water District Worker Found Dead

San Francisco, CA - A Contra Costa Water District employee was found dead near the Los Vaqueros Reservoir in an unincorporated part of the county Sunday night shortly after he was reported missing, according to the sheriff's office. Sheriff's Sgt. Robert Roberts said authorities are investigating the cause of death, however foul play is not suspected. At about 6:30 p.m., the man, who was working at Los Vaqueros Reservoir, was reported missing by co-workers after he did not radio in for several hours, according to Roberts. A sheriff's ranger found the man next to a vehicle in the foothills of the reservoir shortly after he was reported missing, Roberts said.


Ames company worker dies in Winnebago County site fall

THOMPSON, IA — One man is dead after a fall at a Christensen Farms construction site in Thompson today. According to Christensen Farms vice president Eric Christensen, the man was working on construction of a feed mill for the company. The general contractor on the project is Todd and Sargent Inc. which is headquartered in Ames. Phil Sargent, executive vice president of Todd & Sargent, confirmed that the man who died was an employee of the company. “We are getting a team together right now to lead an investigation into what happened,” Sargent said. “At this point we’re just trying to get a handle on this.” Sargent said Todd and Sargent has contacted the Occupational Safety and Health Administration (OSHA). The structure is 150 feet tall and is about 50 percent complete, Christensen said. He said he expected OSHA officials to be on site this afternoon.


Rhodes, MI, man dies in an industrial accident in Beaverton

GLADWIN, MI -- A funeral is set Saturday for a Rhodes man killed in an industrial incident. Ronald K. Urbaniak, 48, died Friday at William E. Lang Inc. in Beaverton, where he had worked since 1989. Urbaniak suffered mortal injuries when an 18-foot, 3,500-pound dump truck box fell onto him, Gladwin County sheriff's deputies said. One of several chains used to suspend the box slipped, deputies said. Urbaniak and his son, Brennan L. Urbaniak of Rhodes, owned and operated B&R Ponds and Excavating. Besides his son, Urbaniak leaves his wife, Lorrie, and daughter Brielle L. Urbaniak of Bay City.


Workman falls 150 feet to his death from feed mill

Des Moines, IA - A Fairmont, Minnesota, man is dead after falling nearly 150 feet from a feed mill under construction in northern Iowa. The Winnebago County Sheriff's Department says 42-year-old Jose Aparicio Maltos was killed while he was working with concrete Monday morning at the Christensen Farms site near Thompson at the intersection of County Roads R-34 and A-42. The Iowa Occupational Safety and Health Administrator Mary Bryant says Maltos was working on a swinging scaffold when he slipped and fell. Bryant says her agency as well as Winnebago County authorities are continuing their investigation.


Man Dies After Falling From Tower

WAKE FOREST, NC - According to media reports, a worker died after falling from a cellular phone tower last Saturday afternoon. According to reporters on the scene, the man fell from the approximately 150 foot tower while working on it. Wake Forest Police officers responded to the scene.A report on WRAL-TV identified the worker as Charles Wade Lupton, age 34, of Midwest City, Oklahoma. WRAL-TV said that the cause of the fall was still unknown and the incident was being investigated by the federal Occupational Safety & Health Administration.


Man who worked to send money to family killed in robbery

Abdur Rahman had been living and working in the United States for nearly 20 years, sending money to his family in Bangladesh, his impoverished homeland. Customers and coworkers at the U.S. Gas station on Burlington-Mount Holly Road in Westampton knew him as a pious and friendly man who worked long hours manning the pumps overnight. Rahman, 50, was shot and killed Monday night during a holdup at the gas station, the first fatality in a string of robberies in the area. Maureen Hogan, who lives on a farm across from the filling station, near Exit 5 of the New Jersey Turnpike, heard the shots that killed Rahman around 11:15.



Construction worker killed in Colorado Springs

Pueblo, CO - A large, earth mover-type of construction equipment rolled at a construction site in Colorado Springs Tuesday morning, killing one person. He has now been identified as Larry James McCollum. Colorado Springs Fire Department Lt. James Eddy said, "It appears they were grading that area with the compacter and it hit an area that was too much of an angle and it rolled over onto its side." The construction site is at 655 Space Center Dr. Colorado Springs Fire Department Lt. Julie Stone said the victim was pronounced dead at the scene. The 911 call was received at 9:16 am. OSHA is in charge of the investigation into what caused the accident.


Goodwill employee found dead on the job in Tacoma

TACOMA, Wash. -- A worker at the Goodwill Operations Center in Tacoma has died in an accident on the job. Authorities say 27-year-old Nicholas Miller of Sumner was found Tuesday morning beneath a machine used for handling large boxes. Miller was pronounced dead at St. Joseph Medical Center. Goodwill spokesman Matthew Erlich says Miller was a production worker whose duties included sorting books and disposing of trash. The state Department of Labor and Industries is investigating. Spokeswoman Elaine Fischer says there have been worker safety complaints against the Goodwill Operations Center in previous years, but none related to the machine. Miller was developmentally disabled. Erlich says he had worked for Goodwill for more than five years.


Bayside man dies working on high-rise

Bayside, NY - A 25-year-old iron worker from Bayside became the 13th construction industry fatality in New York City this year, when an equipment failure sent him plunging nine floors at an Upper East Side high-rise. Kevin Kelly was installing windows on the 23rd floor of the Laurel, a luxury condo at 400 East 67th Street on Monday, April 14. Shortly before 10:30 a.m., a “ceiling strap” attached to his safety harness pulled loose from the building and he fell to his death on a 14th floor balcony below. Kelly, a graduate of Benjamin N. Cardozo High School, was single and lived with his family on Springfield Boulevard in Bayside.


Man Killed on Job Site After Being Backed Over

Des Moines, IA - A Des Moines truck driver died Tuesday after being backed over at a demolition yard. It happened at DeCarlo Demolition on Northeast 44th Avenue. The Polk County Sheriff's Department says William Thomas Rogers was standing in the yard talking on his cell phone when he was hit. Both Rogers and the driver of the truck worked for Seibert Trucking of Des Moines. Witnesses told authorities that the back-up alarms were working on all six trucks that were in the yard at the time, and that people were trying to warn both Rogers and the truck driver before the accident happened.


Window washer falls to death at American Indian museum

WASHINGTON — A window washer at the Smithsonian's National Museum of the American Indian has fallen at least 50 feet to his death. D.C. Fire Department spokesman Alan Etter says rescuers were called to the museum on the National Mall about 10 a.m. Thursday. It's not clear whether the man's safety harness failed or whether it had been engaged properly when he started work. Authorities have not been able to identify the man. Etter says he appeared to be in his early 30s and worked for a contractor hired by the Smithsonian. The Occupational Safety and Health Administration is investigating.


Man killed when two semis collide head-on

EVERETT, Wash. - A truck driver was killed early Friday when his tractor-trailer rig collided with another one on Highway 527 near Interstate 5 at the Everett Mall. Sgt. Robert Goetz said the crash happened about 4:15 a.m. at the intersection of Everett Mall Way and the highway. Investigators are trying to determine whether one of the trucks ran a stop light. One of the semi-truck drivers, Viktor T. Stelmakh, 57, of Everett, was thrown from his cab in the impact and died at the scene. Police are interviewing the other driver, who was not seriously injured. "We are trying to determine who had a green light and who had a red light," Goetz said. Debris was strewn over a large area, and the crash investigation closed parts of Highway 527 and Everett Mall Way.


Man accidentally shot to death by co-worker identified as Patrick J. Reuland

Salt Lake, UT- Police have identified a man who was killed Wednesday in what they have deemed an accidental shooting by his co-worker. Patrick J. Reuland, 43, was cleaning a foreclosed condo about 6:40 p.m. at a complex near 9200 South and 700 West with his co-worker, who had found two rifles under a bed. She was carrying them along with some other items when the 300 Winchester Mag rifle discharged, police said. The bullet struck Reuland in the stomach. He was pronounced dead at University Hospital. Police on Wednesday still were questioning the woman, who is in her 40s, said Sandy police spokesman Victor Quezada. "We have to make sure we're covering all our bases," Quezada said.


I-75 Crash Kills DOT Worker

Tampa, FL - A Department of Transportation worker was killed about 12:15 a.m. today when a tractor-trailer slammed into his truck on Interstate 75 near Bradenton while he was fixing a light, the Florida Highway Patrol said. Troopers say this is what happened: Douglas R. Smith, 46, of Bradenton, was up in a lift bucket attached to his truck. He was working on a light on Interstate 75 near State Road 64. A Department of Transportation truck with an arrow sign was behind Smith's truck, and the driver of that truck, Hamilton Dixon, 44, of Bradenton, had set up 10 traffic cones to direct traffic around the two vehicles.


Worker dies in fall from canyon bridge

Salt Lake City, UT — A construction worker died after falling off a bridge while working Thursday in Weber Canyon. Antonio Contreras-Martinez, 40, fell more than 50 feet to his death just before 11 a.m., said Weber County Sheriff's Lt. Philip Howell. The man, an employee of Wadsworth Brothers Construction, was working on a bridge over some railroad tracks near Interstate 84, Howell said. Contreras-Martinez of Salt Lake City was constructing a platform underneath the bridge when he fell. "We're not sure if something broke and he fell, or if he lost his balance," Howell said. The man suffered massive head injuries and died at the scene, police said.


Two Killed in Upshur County Drilling Rig Accident

ROCK CAVE, WV - Emergency workers were called to an accident off the Rock Cave-French Creek Road in Upshur County Thursday evening. Firefighters say two workers, in their 20's were killed in a drilling rig accident. State Police are not sure exactly what happened, but say the rig collapsed on the workers. They say emergency workers called in a crane to help move the equipment. The names of the victims have not been released. According to state troopers, the workers were employees of S.W. Jack Drilling Company and were doing some drilling for Keyspan.


Bartender killed after asking man for ID

Sunrise Beach, MO - The Gossip Inn is a neighborhood corner bar. "A family tradition since 1938," says the sign out front. But the small tavern in Kansas City, Kan., had a firm rule posted prominently behind the bar: Nobody gets served a drink without showing an ID. Bartender Lori Reynolds stood fast by that rule about 1:30 a.m. Saturday, and it cost Reynolds her life. A man upset when Reynolds said she couldn't serve him without seeing his ID opened fire, striking her multiple times and spraying six to eight more rounds through the bar. No one else was reported injured. The man fled the bar and was still at large Saturday evening. Reynolds died at a hospital. "She did her job. That's all she was trying to do -- her job," said bar owner Laura Maude. Maude said Reynolds, 44, was a good bartender, liked


Crane operator killed

Yucca Valley, CA — A Yucca Valley man was killed Wednesday night around 8:20 p.m. when a crane he was operating came into contact with a live electric line, according to a coroner’s department news release. Paul Henry, 29, was rushed to a local hospital, where he was pronounced dead at 8:49 p.m. The San Bernardino County Sheriff’s Department’s Adelanto Station, as well as the state Division of Occupational Safety and Health, is investigating the incident.


Texas Hotel Clerk Killed In Robbery

Fort Worth, TX - Fort Worth police say a hotel clerk has been killed in the robbery of the business. Authorities say a customer discovered the body of the Best Western Hotel employee before dawn Monday, after shots were heard earlier. The name of the man who was killed wasn't immediately released. No arrests have been made. Investigators are reviewing video from the motel’s surveillance camera.


Grand Prairie taxi cab driver murder suspect arrested

Dallas, TX - Grand Prairie Police have arrested Eric Christopher Hodges, 31, in connection with the murder April 13 murder of Lois Jean Ivory, 53, of DeSoto. A Yellow Cab taxi driver, Ivory was found dead sitting inside her taxi in the 100 block of Dorris Drive in Grand Prairie shortly after 9 p.m. that night. Her body has been released to the Dallas County Medical Examiners office for autopsy. She suffered at least one gunshot wound. Det. John Brimmer said that Ivory was an independent cab driver and therefore it was unclear if she was robbed, because dispatchers did not keep records of her fares. Hodges was arrested in Alvarado and transported to the Grand Prairie Jail. Detectives will continue the investigation.


Worker dies at county plant

Martinsville,VA - A Springs Industries employee died Monday after injuring his head in an apparent fall onto a cement floor, according to state and local officials. James Sherill Flaneary, 55, of 2873 County Line Road, Patrick Springs, was pronounced dead on arrival at Memorial Hospital in Martinsville on Monday morning, according to Lt. Kimmy Nester of the Henry County Sheriff’s Office. Flaneary was employed by Springs Industries, located at 460 Beaver Creek Drive, Martinsville, according to a release from the sheriff’s office. The company, which is in the Patriot Centre industrial park, produces bed and bathroom linens and other home furnishings. Flaneary had been operating an industrial machine used to manufacture mattress pads, Nester said, and just before 8 a.m., he was found lying on the floor next to the machine.


Man dies in work-related accident

BILLINGS, Mont. - A man died in a work-related accident Monday in an industrial area between Billings and Laurel, authorities said. Chad Fehr is the deputy coroner for Yellowstone County. He didn't release the man's name and said an autopsy is scheduled for Tuesday. The cause of death was not released. The man died at Par Electrical Contractors at about 2:30 p.m. Company officials could not be reached for comment. The Occupational Safety and Health Administration is investigating.


2nd steel worker dies in week

Harrisburg, PA - For the second time in less than a week, an industrial accident has claimed the life of a Dauphin County man. Roger H. Prichard, 58, of Hummelstown, was killed Friday afternoon after he got trapped in between a piece of machinery at the ArcelorMittal Steelton plant, causing massive internal injuries, according to county coroner Graham Hetrick. Prichard died about 4:20 p.m.


Robeson man dies in industrial accident

Reading, PA - A 28-year-old Robeson Township man was killed in an industrial accident at work in Montgomery County, officials said Monday. Jeremy J. Andre died Friday of injuries suffered when he was pinned by his truck in Ches-Mont Disposal Inc., a waste transfer facility in Skippack, east of Potts town, said Dr. Walter Hoffman, Montgomery coroner. Andre, a truck driver for the company, was trying to remove trash that was stuck in the truck’s cargo area Friday about 11 a.m., Hoffman said.


Details of fatal workplace accident near Billings released

Missoula, MT - Authorities have released the name of a man killed in an accident at work near Billings. Larry Atwood, 40, of Joliet died of blunt force trauma after being struck in the head by flying debris. It happened about 2:30 p.m. Monday afternoon at Par Electrical Contractors between Billings and Laurel. Yellowstone County Sheriff's Deputy Chad Fehr says employees were performing a tension test with a 25-to-35 pound piece of equipment when a cable snapped, propelling the object into the air and striking Atwood in the head. The Occupational Safety and Health Administration is investigating.


Truck driver killed wted Presshen pinned by vehicle

PIKEVILLE, KY - Kentucky State Police said a coal truck driver was fatally pinned when he tried to get into his vehicle after it began rolling. The incident occurred Monday night in Pike County after Kenneth Mounts of Pikeville parked the truck on the shoulder of U.S. 119 near Buckley's Creek and stepped out of the cab. The police report said the 42-year-old driver tried to get back into the cab and apply the brakes when the truck began rolling. Mounts apparently fell and was pinned by the truck. The accident remains under investigation.


Arrest in cabbie murder

RALEIGH, NC - Raleigh police have made an arrest in the murder of a cab driver.
Sabiu Tuker, 52, was found stabbed to death around midnight Tuesday in the 1100-block of Brighton Road. Officers found the taxicab crashed into a fence. Tuker radioed for help but it was too late when police arrived. Octavia Miller, a fellow cab driver, heard about the attack this morning. "He always smiled, he was happy. He got along with everyone. It's a shame!" Miller said.


Worker Dies After Falling While Changing Bulb in Courthouse

NY - A city employee died of injuries suffered in a fall while working in the attic of a courthouse in Queens on Tuesday, city officials said. The cause of the fall was unclear, the authorities said. The accident occurred when Henry Chang, a stationary engineer for the Department of Citywide Administrative Services, was changing a light bulb about 11:20 a.m. in a maintenance room at the State Supreme Court building in Long Island City, the police said. Mr. Chang, 58, of New Hyde Park in Nassau County, fell 15 to 20 feet and struck his head on a metal electrical box, causing severe injuries, the authorities said. A co-worker of Mr. Chang’s called for help, and the unconscious engineer was taken to Bellevue Hospital Center, where he underwent surgery, the police said. He was later pronounced dead, the authorities said.


Wells transfer station worker found dead

Portland, ME — The supervisor of the transfer station in Wells was found dead Sunday in one of the facility's buildings. Police say his death has been ruled a suicide. A police officer discovered the body of Mike Pauk, 57, of Wells, at 12:40 p.m. after his vehicle was spotted at the transfer station. Pauk's girlfriend had reported him missing Saturday evening. An audit of the Wells transfer station earlier this month revealed a number of administrative problems that town officials say are being addressed with more accounting supervision. Town Manager Jane Duncan said there is no evidence that problems at the transfer station involved criminal activity. The former public works director in Scarborough is filling in as the supervisor in Wells.


Employee Dies at Kimberly Plant

KIMBERLY - Authorities are investigating the death of a worker at Luvata in Kimberly. Company officials confirmed to fox11online.com, an employee was killed Wednesday. OSHA and Fox Valley Metro Police, along with the Outagamie County coroner's office are also investigating the death. According to the company's website, Luvata is involved in metal fabrication, component manufacturing and related engineering and design services.


Cab driver killed on last day of work

NORTH PHILADELPHIA - A Philadelphia cab driver in North Philadelphia knew the risks of his job and wanted out, but before he had a chance to leave, he ran into the very danger that he was trying to escape on his very last day at work. Byron Elder's brother, 46-year-old Benjamin, was found dead Sunday in the cab he drove. A resident discovered Benjamin slumped over in the taxi he drove for Germantown Cab Company just before 9:30 at 20th & Berks Streets. He'd been shot behind the ear. "My heart is broken," Byron said. "My heart just dropped because me and my brother were very, very close," Robert Elder said. see video


Pool worker dead of apparent electrocution

OCALA, FL - A 28-year-old pool worker was killed Wednesday - apparently by electrocution - as he worked with a power tool on a pool containing water in east Ocala, according to fire and police officials. The name of the independent contractor was not released until his family members could be notified. A homeowner at 4530 E. Fort King St. had checked on the worker at about noon, Ocala police Lt. Butch Green said. When she checked on him again around 2:40 p.m. she found him lying face up in the deep end of the pool, which had a couple of feet of water in it.


Western Hotel waitress shot to death, DA has not yet declared shooting accidental

CALLICOON, NY - Rumors about the tragic death of a waitress at the Western Hotel have been flying around the hamlet since last Sunday morning when the event occurred. The shooting took place in the bar of the hotel at 12:30 a.m. on Sunday, April 13. The victim, Lori Schubeler, 41, was pronounced dead at the scene. “All we heard was that it was an accident,” said a worker at the local convenience store. The same statement was repeated by everyone in the hamlet. Another person said that someone in the hotel was showing an antique rifle that was on the wall of the bar and it went off accidentally. That rumor that was contradicted by Sullivan County District Attorney Steve Lungen. No one has not been charged with anything by Lungen, who announced that Schubeler was shot with a .25-caliber handgun in the upper chest, the bullet piercing her aorta and lung.


Worker Crushed At Landfill

OPELOUSAS, La. - A 22-year-old Opelousas man has died in an industrial accident at the St. Landry Parish Landfill near Beggs. Officials with the St. Landry Parish Sheriff's Office identified the victim as Martel Smith, a two-year employee of the landfill operated by the parish's Solid Waste Disposal District. Officials said Smith was apparently trying to repair a hatchway on a vehicle yesterday when the hatch closed, crushing him. Solid Waste Executive Director Katry Martin declined to provide any other information, saying the accident has been turned over to the sheriff's office for investigation.


Kimberly factory worker electrocuted

KIMBERLY, WI — The electrocution death of a 43-year-old worker at a manufacturing plant has prompted a federal investigation. Dean Effa died Wednesday at Luvata, a company that makes copper wire components. An autopsy showed he died of electrocution, Outagamie County Coroner Ruth Wulgaert said today in a press release. The federal Occupational Safety and Health Administration is investigating Effa’s death as an accident, said officials, who released no other details. Effa, of Seymour, was transported from Luvata to St. Elizabeth Hospital in Appleton, where he was pronounced dead, Wulgaert said Wednesday. The company's spokesman, Appleton attorney Greg Gill, said production at the Kimberly facility and its Appleton plant was halted Wednesday out of respect for Effa, but said he expected production to resume today. He said the company is making Employee Assistance Program counselors available. Gill said the accident happened about 3 a.m., and a coworker and emergency response personnel made "heroic efforts" to save Effa. He said OSHA, police and the coroner's office were notified immediately.


Md. man killed in Spotsy accident

Germantown, MD - Man dies instantly after air compressor falls on head. A Maryland man was killed Tuesday evening in Spotsylvania County when an air compressor he was installing on a truck fell on his head and killed him, police said yesterday. Gerardo Sanchez-Luizaga, 37, of Germantown, Md.--who was originally from Bolivia--was found dead around 7:52 p.m. by co-workers at Our Way Services on Filter Lane, said Lt. Eddie Peck of the Spotsylvania Sheriff's Office. "Co-workers heard a noise and a loud scream," Peck said. But by the time they got to Sanchez-Luizaga, he was dead. Sanchez-Luizaga was an employee at the service company, Peck said.


Bear from movies turns on handler in Calif., killing him

BIG BEAR LAKE, Calif. — When friends Linda Carter and Cherrie Giles booked a three-day retreat in a remote cabin in the San Bernardino Mountains, the proprietor told them not to be startled by the roar of lions and bears from the exotic-animal training center nearby. The women fell asleep to the roars the first night, but on Tuesday they were startled by a different sound — an urgent yell. About 30 minutes later, sirens wailed as paramedics rushed to an animal trainer who had been bitten on the neck by a 700-pound, 7 1/2-foot-tall grizzly bear. Stephan Miller, 39, died at the scene. On Wednesday, friends and neighbors tried to make sense of the attack, which from all accounts involved a well-trained and gentle bear and an experienced animal trainer.


Garnavillo woman killed in Clayton sand mine accident

CLAYTON, IL - A Pattison Sand Co. worker was killed when another employee accidentally backed over her with an end loader this morning, officials have confirmed. Rebecca Dysart, 48, of Garnavillo, was one of three workers walking near where the end loader was being operated about 8 a.m., the Clayton County Sheriff's Office reported. Dysart was somehow caught behind the vehicle as it was backing up. She died at the scene. The end loader was operated by Barry Brinkman, 32, of Cassville, Wis. Pattison workers said the company "will release a statement when it is prepared to do so." The Pattison Sand Co. opened its $25 million-plus silica sand-mining operation on the banks of the Mississippi River south of Clayton last fall. The Sheriff's Office was assisted by the Iowa State Patrol and the Clayton County Medical Examiner. The investigation is continuing.


Man killed when train hits semi in Robertson County

Waco, TX - A 44-year-old truck driver is dead after the semi he was driving collided Thursday with an oncoming Union Pacific train in Robertson County. A Texas Department of Public Safety spokesman today said investigators are still looking into how the accident happened. The spokesman said Samuel Lee Taylor of Willis was westbound in a 2005 Peterbilt semi truck on Hoxie Chapel Road just east of Easterly, when just before 4 p.m. Thursday he collided with a northbound train. Taylor was ejected from the cab of the truck and pronounced dead at the scene. The cab was then destroyed by fire after colliding with the train. The train’s engineer, David Allen Echols, 57, of Gilmer, Texas, and conductor Tony Swanson, 50, of Palestine both complained of minor injuries at the scene, the spokesman said.


Worker Killed At Southaven Amphitheater

Southaven, MS - A man working in the stage area for the Southaven Springfest event was killed when he fell some 40 feet from a catwalk at the Snowden Grove Amphitheater. Southaven Police Capt. Steve Stewart says the accident occurred Thursday. Stewart did not immediately make public the name of the 30-year-old victim and said the accident remains under investigation.


DOT Worker Hit, Killed On I-75

Atlanta, GA - A DOT worker was hit and killed following an accident in Henry County Tuesday night. It happened near Exit 220 on Interstate 75 northbound in a construction zone. The Georgia DOT said the victim was a 30-year-old man from Chamblee who works with US Traffic Technologies. He was helping to prep a work zone, when a barrel apparently got away from him. That's when motorist Leah Mitchell said she spotted it. "He came close to the barrel. It seems as if the wind blew him, or he tripped, or he was moving so fast that he could not stop his self. I was on brakes -- by time I knew it, he was in my car," Mitchell said.


Farmer dies of injuries from tractor accident in his barn

LAPEL, Ind. - A Madison County farmer has died after being accidentally pinned between the front of his tractor and the machine's bucket frame. Madison County Sheriff Ron Richardson said 72-year-old Don L. Conrad was in a barn doing maintenance work on the tractor when the accident occurred Friday afternoon on the farm between Anderson and Lapel. He said it appeared there was a loose hydraulic line and the bucket came down on top of Conrad. The tractor was not moving at the time, Richardson said. Conrad's body was found later by his wife, the sheriff said.


Worker dies on job-site in Calera

CALERA, TX — A worker died after slipping and falling at the town’s new wastewater treatment plant work site. Calera Police Chief Don Hyde said that Ricky Dale Drawbaugh, 41, was pronounced dead after being taken to the Medical Center of Southeastern Oklahoma Wednesday morning. The accident happened at approximately 10:45 at the construction site on Third and Dansby streets. Drawbaugh was found unconscious by his foreman, according to Hyde. He had been cleaning out a trench to allow standing water to flow to a pump. No one witnessed the accident. “Our preliminary investigation shows that there was a lot of mud and water in the area,” Hyde said. “He lost traction, striking his head on a concrete ledge and fell into eight inches of water.” The exact cause of death has not been determined, but it was ruled accidental. The job site was closed Wednesday after the accident, according to Hyde.


Cab driver shot, killed in his car

BOYLE HEIGHTS, CA - A taxi driver who picked up a fare Friday night was found shot to death in his cab moments later, police said. Andres Delamadrid, 37, crashed his cab in the 3400 block of Gleason Avenue in Boyle Heights after being shot in the chest, said LAPD officials. The motive for his killing is under investigation. LAPD Capt. Blake Chow said the car was an illegal, unlicensed taxi.


Worker killed in Elk County mine accident

Pittsburgh, PA - Federal and state mining agencies and other officials are investigating the death of a worker who was killed Friday inside a mine in Horton, Elk County. Randy E. Huey, 43, of Rossiter, was pronounced dead by Elk County Deputy Coroner Robert Sorg at the Rosebud Mining Co.'s Little Toby mine in the village of Brockport, off Route 219. State police said Mr. Huey was killed at 1:30 p.m. after he was pinned between the mine wall and a piece of equipment about two miles inside the mine. The federal Mine Safety and Health Administration, the state Department of Environmental Protection's Bureau of Mine Safety, the coroner's office and state police are investigating.


Construction worker killed near Powers, Palmer Park blvds.

Colorado Springs,CO - A construction worker was killed this morning when a piece of heavy equipment rolled at a project site near the intersection of Powers and Palmer Park boulevards in Colorado Springs. The accident was reported at 9:18 a.m. at 655 Space Center Drive, said Colorado Springs Fire Department spokeswoman Lt. Julie Stone. The victim, a 50-year-old man, was pronounced dead at the scene, Stone said. He was inside an earth compactor when the vehicle overturned, she said. An investigator from the Occupational Health and Safety Administration was en route, Stone said.


Man killed in tragic vineyard accident

Lakeport, CA - A vineyard worker was tragically killed in the early hours of Wednesday morning as he operated an irrigation pump during frost protection at Clover Valley Vineyards in Upper Lake. Upper Lake resident Silvino Eufracio Navarro, known to all by his nickname "Toyahua," was a long-time vineyard worker and much loved member of the community. He came from the town of Nochistlán in the state of Zacatecas, Mexico. He had lived in the community for 22 years. "It was his first day operating the anti-frost machinery," Navarro's wife Celia Duran Eufracio said. The 48-year-old father of three went to the vineyard to check an irrigation pump. He called his foreman at around midnight to report it was in working order, and said he planned to go home. When his wife had not heard from him and he had not returned home, she called his cell phone. Then she and the foreman went to look for him at 7 a.m. There, they found his body submerged in the pond.


Postal Worker Dies Three Weeks After Crash

CHICAGO, IL - Dion Atwood, 36, has died, three weeks after he was injured when a car clipped the mail truck he was driving on the West Side, ejecting him from the vehicle. Atwood was westbound on West Madison Street when he attempted to make a U-turn at 4540 W. Madison St. at 10:22 a.m. on March 29. While making the turn, a westbound Acura clipped him, causing the truck driver to be thrown from the vehicle, according to Harrison District police. The front left bumper of the Acura hit the right rear bumper of the mail truck, police said. Police News Affairs Sgt. Antoinette Ursitti said the Acura hit the postal truck at an intersection in the 4500 block of West Madison Street. Ursitti also said after Atwood fell to the ground, a 1993 Ford Escort struck the postal truck. After the crash, Atwood, of 4701 W. Congress Pkwy., was taken to Mount Sinai Hospital. The Acura driver was cited with driving too fast for conditions. The Police Major Accident Investigation Unit is investigating.


Bradenton man killed in accident

MANATEE, FL - A 46-year-old Bradenton man was killed early Friday on Interstate 75 when a tractor-trailer driver plowed into the back of a repair truck and threw him from an aerial lift bucket. According to a Florida Highway Patrol report, a repair crew in two trucks was stopped in the right northbound lane of the interstate under the exit sign for State Road 64 to change the light bulbs. The truck parked in the rear had a flashing lighted arrow board and safety cones to direct approaching drivers around the two trucks. Douglas Seth of Bradenton was in the aerial lift bucket attached to the first truck fixing the overhead sign light, when the driver of the tractor-trailer, Ernesto Reyes, 51, of Miami, failed to notice the flashing warning arrow and safety cones. The tractor rig smashed into the back of the truck with the warning arrow, pushing it into Seth's truck.


Arrest Made In CMC-Pineville Nurse's Aide's Slaying

PINEVILLE, NC - Police said they have arrested a man accused of killing a nurse's aid in a Pineville hospital parking lot Thursday afternoon. Authorities had been searching for 26-year-old Jesus Rangel for several hours after the hospital employee was found slain late Wednesday. Rangel was arrested at his apartment in Rock Hill around 1:30 p.m. and waived extradition, allowing him to be taken back to North Carolina immediately. Rangel is accused of killing 23-year-old Virginia Sanchez. Officers said a woman driving through the back parking lot of Carolinas Medical Center-Pineville found her co-worker slumped over in her car around 8:30 p.m. Wednesday and called 911. Officers said it appears the woman was strangled, but an autopsy will determine her cause of death. They said she was found in the passenger seat, which indicates there was likely a struggle in the vehicle.


Worker, 48, dies in machinery accident, Plainfield resident was longtime employee of Sabert Corp.

Old Bridge, NJ - A machine operator died in an apparent accident while working at a borough factory last week. Gonzalo Mera, 48, of Plainfield, an employee of Sabert Corp., died around 7:50 p.m. April 9 after suffering a traumatic blow to his head while he was in the process of fixing machinery. "Sabert Corp. and all of its employees are grieving the loss of a valuable and long-term employee," the company said in a statement. The plant, located in the area of Main Street and Crossman Road, is where the 25-year-old company manufactures plastic food-packaging products for food service distribution, supermarkets and large chain stores like Costco. The company is expected to start building its world headquarters and warehousing facilities on 12 acres of property located on the Main Street Extension in Sayreville's waterfront area in the next 60 days.


Construction worker dies after 50-foot fall

Lake City, FL - A construction worker fell 50 feet to his death Thursday morning while working on a bridge over railroad tracks in Weber County. Antonio Contreras Martinez, 40, of Salt Lake City, was working with Wads- worth Brothers Construction to refurbish an overpass above Union Pacific rail lines near Interstate 84 and the Weber and Morgan county line about 10:45 a.m. when he fell from scaffolding suspended beneath the bridge. Martinez suffered severe head injuries and died at the scene, said Weber County sheriff's Lt. Philip Howell. Deputies are investigating the cause of the fall, but "initial indications are [the scaffolding] may have broken," Howell said. Investigators so far have found no sign of foul play, Howell said. Martinez had worked with the company for about 18 months.


Oil field worker dies from injuries

Shreveport, LA - A 28-year-old man who fell 40-90 feet from an oil field rig on Tuesday has died. Everett Cobbs died just before 2 a.m. this morning at LSU Hospital in Shreveport where he was on life support, officials said. Cobbs, and employee of Sandia Drilling Company, was coming down the rig with safety gear on when he fell and landed on the deck of the rig, causing severe injuries, said Cindy Chadwick, Caddo sheriff's spokeswoman. The accident happened off state Highway 1, just south of Ellerbe Road.


Worker At Blountsville Tyson Foods Plant Dies In Accident

BLOUNTSVILLE, Ala. - A contract security guard was killed this week in an industrial accident at the Tyson Foods plant in Blountsville. Blount County Coroner John Mark Vaughn says 70-year-old Allen Hayes of Locust Fork died Tuesday after he was struck by a truck in a loading area. Vaughn said Hayes was airlifted to UAB Hospital but he was pronounced dead on arrival. "We're saddened by the tragic death of a contract security guard at our Blountsville poultry plant," Tyson Foods said in a statement. The statement says Hayes was employed by DSI Security Services. Tyson Foods, DSI and OSHA are all investigating the accident.


Farm Hand Now Dead After Allegedly Killing Boss Then Killing Self

Shreveport, LA - Hempstead County Sheriff's deputies say Delton Pinter, 53 has died of his injuries after shooting his boss and then himself. Farmer Billy Joe Butler, 77, was fatally shot by Pinter who was employed at the Butler Farm as a farm hand. Police say the two had a disagreement Wednesday morning and that's when Pinter went to his car grabbed a gun and shot Butler and then himself. Butler a prominent cattle farmer in Hempstead County was pronounced dead at the scene. Pinter meanwhile was airlifted to a Hot Springs hospital where he later died. Officers say Pinter's wife was standing nearby and saw the whole thing.

Broken Hearted, I cannot recall deleting anyone. I usually leave a loss in and add a comment if it does not belong in the toll (such is added but out of the US) .
