Worker Dies of Toxic Chemical Exposure

June 25, Nitro, TN - the death of a still unidentified worker is being investigated by OSHA as to how a dangerous gas was apparently pumped into a man's respirator at the AC&S Chemical plant.  The man was rushed to the hospital after inhaling dangerous gases and then died from his injuries. The man was wearing a respirator and an incorrect gas was pumped into the apparatus' clean air supply, according to an initial report.

Mail Carrier Dies of Heat Illness

July 24, Kansas City, MO - 57 year old John Watzlawick died a day after complaining he wasn't feeling well. He wasn't allowed to end his shift early. The medical examiner this week confirmed that Watzlawick died of hyperthermia, or heat-related illness. After being found lying unconscious by a passerby, he was transported to an area hospital.

Two Deputies Murdered in Ambush

August 16, LaPlace, LA - 34 year old Brandon Nielson of Destrehan (left) and 27 year old Jeremy Triche of LaPlace, both deputies for St. John the Baptist Parish, were gunned down in an ambush after they responded to reports of a traffic officer being shot multiple times near an isolated parking lot for a Valero refinery. The officers were conducting an interview when someone came out of a trailer with an assault weapon and opened fire. Many arrests have been made in the incident.

Off Duty, But Still Working

August 16, New Harmony, UT - 27 year old Kevin Scott Smith of Cedar City, and a Cedar City firefighter, was killed in an off-duty industrial accident while working for his employer in New Harmony.  Smith and two other men were apparently working on a water pump when their boom truck came in contact with overhead power lines causing an electric shock that killed Smith and injured another.
 
Man Crushed Under Equipment

August 16, Jacksonville, FL - 21 year old Lawrence DaQuan Davis died when he became pinned underneath a piece of heavy equipment at the Bacardi Bottling Corp. plant. It is unclear how or what cause Davis to become trapped by the equipment.

Utility Worker Struck and Killed

August 17, Chester, PA - An unidentified 34 year old subcontractor for the Chester Water Authority was struck by a vehicle as he was exciting a manhole. The man worked for Metra Industries from Little Falls, New Jersey was replacing the waterline in a hole.

Incident at Quarry Kills Worker

August 17, Sloan, NV - 29 year old Anthony Holden of Las Vegas was suffered fatal crushing injuries at a quarry in Sloan. Holden was an employee for Southern Nevada Paving.  No other details are available.  

Inspector Killed in Roof Collapse

August 17, Medley, FL -  58year old Pierre Mezidor  died after the roof of a silo filled with powdery concrete mix collapsed, while he was standing on top of it. Mezidor, an inspector at Titan America in Medley, was about 200 feet up when the silo's roof collapsed underneath him. 



Worker Succumbs to Burn Injuries

August 19, Mount Prospect, IL - 29 year old Luis Macias, one of three workers injured August 9th in a chemical flash fire, has died.  The men were working in the basement of an apartment complex when the explosion happened but there are no details as to what caused the explosion or who the men worked for.

Worker Killed After Being Knocked Out of Truck

August 20, Canarsie, Brooklyn, NY - 52 year old Thackoo Hargobin, an employee for Astoria-based Hephaistos Building Supply, died when he was hit by a school bus while loading his flat-bed truck at Super Tex Products in Queens. Hargobin died at the scene.

Falling Pipes Crush Worker

August 20, Theodore, AL - 55 year old Paul Edward Hodgon was killed when a two large pipes he was unloading fell on him.  Hodgon died at the scene after emergency responders arrived at Hertz Energy & Pump Services.

Plant Worker Falls to His Death

August 20, Heyworth, IL - 48 year old Michael Burleson has died after an injury suffered August 15 at the Cargill, Inc. soybean processing plant.  An official says Burleson was in the elevator area where the soybeans are stored and apparently fell onto a lower level , but it is unclear how it happened.

Carpenter Killed on Job Site

August 20, Washington, CT - 42 year old Christopher Dennis of Waterbury died after he fell from a scoffold at a job site.  He was airlifted to a hospital where he died of his injuries. It is unknown who he was working for.

Road Worker Killed When Truck Flips

August 21, Milford, MI - An unidentified 58 year old Oakland County Road Commission employee was killed when the truck he was driving overturned.  Police say the man was on a job, putting down material on the road when the accident occurred, but it’s not clear why or how the truck flipped over.

"Freak Accident" Kills Logger

August 21, Rowan County, KY - 30 year old Josh Ferguson of Elliotville was killed in what authorities are calling a freak accident.  "As he was cutting the timber, the tree that he cut fell against another tree, caught the top of that tree and basically lifted the bottom of that tree up. It pushed him over and caught him between the dozer blade and the tree, itself," described Sgt. Jim Bowling of the Kentucky State Police.

Another Construction Site Incident

August 21, Paso Robles, CA - 65 year old James Seim, a construction supervisor, died after apparently falling through the roof skylight of a commercial building his company was working on. It is unclear who the contractor was for the construction.   

Man Dies From Electrocution

August 21, Bedford, PA - 41 year old James Dick was electrocuted while relocating power lines. Dick was working from a bucket truck for J.R. Wyles relocating power lines for Adams Electric. Authorities say Dick was shocked by a 7,000 volt live wire.

Construction Worker Crushed by Equipment

August 23, Oklahoma City, OK - 43 year old Mario Mejia-Ramirez died after being pinned between a truck trailer and a piece of excavation equipment. He was working for Sherwood Construction Co. helping to dismantle the old Interstate 40 Crosstown.

Road Worker Struck by Out of Control Car

August 23, El Paso, TX - Jose Anguiano was struck and killed while doing road construction on Interstate 10. Authorities say a van hit a smaller car, causing it to go out of control, hitting Anguiano. He was pinned against a heavy piece of equipment.  

Worker Crushed Under Flipped Mower

August 24, Vine Grove, KY - 47 year old Thomas "Greg" Spence was crushed under a commercial lawn mower. He was mowing on a steep hill when the mower lost traction and rolled over onto Spence. Spence was employed by Whispering Hills Lawn Service.  

Worker Struck by Cement Truck

August 24, Berwick, ME - 55 year old Thomas Palermo died after being struck by a cement truck in the driveway of the Redimix Companies plant. There are no details on how or why Palermo was struck.   

Maintenance Employee Electrocuted 

August 25, San Antonio, TX - 49 year old Ricardo Corpus, a maintenance worker for Sea World San Antonio, died while working on an air conditioning unit at the park. Corpus and a co-worker turned off the machine to work on it but something went wrong and his co-worker found him with his arm inside the machine and him energized. The co-worker pulled Corpus out and called for help. 

Rail Worker Electrocuted

August 27, Queens, NY - 51 year old Ronald Sampson, a Metropolitan Transportation Authority worker, died after he was electrocuted near the Queens Village station of the Long Island Rail Road.  Sampson, a third railman, was electrocuted when he came in contact with the third rail while performing track maintenance work.
